Cognitive Function of the Urban Elderly (도시 노인의 인지기능)

  • Purpose: This study was performed to assess the degree of cognitive function of elderly by MMSE-K performances and of that effect. Method: The subjects were 185 aged over 65 in Daejeon Metropolitan city. Data were collected through personal interview using the questionnaire from 10 to 31, Jan. 2003. The measures were Korean version of Mini-Mental State Examination(MMSE-K). Results: The mean score of MMSE-K was $22.60{\pm}5.39$. The prevalence of cognitive impairment was estimated as 48.6% by MMSE-K ${\leq}23$ and significantly age, gender, and education effect. The subtype score of MMSE-K were significantly lower in female group in each items : orientation in time and place, attention/calculation, language except registration and recall. And the scores were significantly lower in the older group and non-educated group in the all items of MMSE-K. Conclusion: Gender, age, and education showed significant effects on total and subtype MMSE-K score. Cognitive function decline were higher in female, older age group, and non-educated group. Therefore, those three factors are thought to be one of important risk factors for development of dementia, also it is assumed to be affected by other variables than age, gender, education effect.

  • Objectives:Donepezil is a widely used drug for the treatment of patients with Alzheimer's disease(AD). The aim of the present study was to clarify the efficacy and the characteristics of responders to donepezil. Methods:Patients with probable AD(n=80;75.7 years) and small vessel dementia(SVD)(n=18;77.8 years) who received donepezil were retrospectively analyzed using Alzheimer's registry, and three questions were asked:1) Does donepezil therapy improves cognitive symptoms in patients with dementia? 2) If donepezil improves cognitive symptoms, which items of the K-MMSE are improved? 3) What are the characteristics of responder to donepezil medication? Results:1) After donepezil medication, cognitive function measured by the K-MMSE was significantly improved in both types of dementia(AD and SVD), However, statistical differences were not found between these groups. 2) In a clinical trial of donepezil, the patients performed better than before mediation on K-MMSE items assessing orientation, recall, construction, concentration, calculation. 3) In AD, the K-MMSE score before medication was closely related with response of donepezil. Conclusion:This study suggests that donepezil improves various cognitive functions in both types of dementia, and the responsive group had significantly lower K-MMSE scores than the non-responsive group before medication.

A Joint ML and ZF/MMSE Detection Algorithm in Uplink for BS Cooperative System (셀간 협력 통신을 위한 상향링크 환경에서의 ML 및 ZF/MMSE를 결합한 검출 기술)

  • In this paper, we address the issue of joint detection schemes for uplink cellular system when base station cooperation is possible for multi-user detection in multi-cell scenario. The ZF, ML, MMSE and SIC detection are analyzed and evaluated as a conventional scheme. ML attains the optimal performance but the complexity increases exponentially, ZF/MMSE have simple structure but have poor detection performance and SIC has better performance but it has large complexity and potential of the error propagation. However, they need the increased decoder complexity as the number of iteration is increased. We propose a new joint ML and ZF/MMSE detection scheme, which combines the partial ML decoding and ZF/MMSE detection, in order to decrease the decoder complexity. Simulation results show that the proposed scheme attains same or a little bit better BER performance and expect reduced decoder complexity, specially in the case of large number of Base Station are cooperated each other.

An MMSE Based Iterative Soft Decision Interference Cancellation Scheme for Massive MIMO Systems (대규모 다중 입출력 시스템을 위한 MMSE 기반 반복 연판정 간섭 제거 기법)

  • In this letter, an MMSE based iterative soft decision interference cancellation scheme for massive MIMO systems is proposed. To reduce the complexity, the proposed scheme uses the Sherman-Morrison-Woodbury formula to compute the entire MMSE filtering vectors in one iteration by one matrix inverse operation. Simulation results show that the proposed scheme also has a comparable BER to the conventional scheme for massive MIMO systems.

  • The HMM-Net is a neural network architecture that implements the computation of output probabilities of a hidden Markov model (HMM). The architecture is developed for the purpose of combining the discriminant power of neural networks with the time-domain modeling capability of HMMs. Criteria of maximum likehood(ML) and minimization of mean squared error(MMSE) are used for learning HMM-Net classifiers. The criterion MMSE is better than ML when initial learning condition is well established. However Ml is more useful one when the condition is incomplete[3]. Therefore we propose an efficient learning method of HMM-Net classifiers using a hybrid criterion(ML/MMSE). In the method, we begin a learning with ML in order to get a stable start-point. After then, we continue the learning with MMSE to search an optimal or near-optimal solution. Experimental results for the isolated numeric digits from /0/ to /9/, a training and testing time-series pattern set, show that the performance of the proposed method is better than the others in the respects of learning and recognition rates.

Influence of denture wearing on a mini-mental state examination(MMSE-K) in the elderly (노인들의 의치 사용이 MMSE-K 점수에 미치는 영향)

  • Objectives: The purpose of the study was to investigate the association between denture wearing and dementia using a Mini-Mental State Examination (MMSE-K) in the community dwelling elderly. Methods: The study subjects were 184 community dwelling elderly using Korean version of mini-mental state examination(MMSE-K). The variables included the general characteristics of the subjects, denture earing, number of mastication tooth, subjective mastication ability, and MMSE(dementia). Denture wearing was divided into two groups of the natural dentition including the fixed prosthesis and removable denture including the complete and/or partial denture. The data were analyzed by ${\chi}^2$ test, t-test, and binary logistic regression using SPSS version 22.0 program. Logistic regression was used to analyze the association of MMSE-K score and denture wearing, and 95% confidence interval was calculated. The subjective mastication ability was measured by Likert 5 points scale. Dementia was measured by MMSE-K. Results: There was a significant association between MMSE-K score and denture wearing. The odds ratio(OR) of denture waering was 6.01(95% CI: 2.824-12.784). After adjusting the age, gender, residence and education, OR was 5.53(95% CI: 2.364-12.935). Conclusions: This study showed a significant association between MMSE-K score and denture wearing in the Korean elderly.

Modified MMSE Estimator based on Non-Linearly Spaced Pilots for OFDM Systems

  • This paper proposes a Modified Minimum Mean Square Error (M-MMSE) estimator for an Orthogonal Frequency Division Multiplexing (OFDM) System over fast fading Rayleigh channel. The proposed M-MMSE estimator considered the effects of the efficient placement of pilots based on the channel energy distribution. The pilot symbols were placed in a non-linear manner according to the density of the channel energy. Comparative analysis of the MMSE estimator for a comb-type pilot arrangement and M-MMSE estimator for the proposed pilot insertion scheme revealed significant performance improvement of the M-MMSE estimator over the MMSE estimator.

Correlations between Cognitive Function and Functional Ability in Strokes using MMSE and FIM

  • The Purpose of this study was to investigate correlations between the Mini-Mental State Examination (MMSE) and the Functional Independence Measure (FIM) in strokes. The data collected retrospectively from fifty-five stroke rehabilitation patients. The MMSE and FIM at the time of admission and discharge were obtained from patients' medical records. Firstly, Pearson correlation coefficients of the MMSE score at the time of admission revealed $0.286{\sim}0.747$ with FIM at the time of discharge (p<0.05). Also, the change of MMSE score significantly correlated with the change of total FIM scores in strokes (r=0.409, p<0.05). Because the MMSE scores at the time of admission and FIM at the time of discharge are correlated, the MMSE scores can be used to predict the FIM at the time of discharge and establish a rehabilitation program.

Status of Physical and Mental Function and, Its Related Factors Among the Elderly People Using from Long-Term Care Insurance Service (장기요양서비스 이용 노인들의 신체적 및 정신적 기능과 관련 요인)

  • This study was performed to determine the levels of physical and mental functioning and to reveal its association with related factors in the elderly people received long-term care insurance services. The interviews were performed, during the period from February 16, to March 31, 2011, to 350 elderlies admitted long-term care insurance dwelling in Daejeon city and Chungchung provinces. As a results, the levels of IADL and MMSE-K in the elderly received home care insurance services was significantly higher than that of the elderly received facility care insurance services. But the levels of ADL and CES-D was not significant. Concerning correlation of ADL, IADL, CES-D and MMSE-K, the level of ADL was positively correlated with the level of IADL and MMSE-K, whereas negatively correlated with the level of CES-D. IADL was positively correlated with the level of MMSE-K, whereas negatively correlated with the level of CES-D. As a results of multiple regression analysis, the factors of influence with the level of ADL and IADL were selected the variables of amnesia, regular exercise and MMSE-K. The factors of influence the level of CES-D and MMSE-K were selected the variables of subjective health status, amnesia and IADL. AS a above results, the levels of physical and mental functioning in the elderly people received long-term care insurance services implies closely related with the health status and health-related behaviors.

  • In this paper, we analyze the performance of the block linear MMSE equalization for OFDM systems in doubly selective channels by computer simulations. The block linear MMSE equalization shows somewhat unusual BER characteristics in that the BER curve drops at first as SNR increases but then rises up as SNR increases further beyond some point. In this paper, we investigate the BER characteristics of the block linear MMSE equalization by analyzing the condition number of the coefficient matrix in the linear system involved in the equalization process, and propose a new method to avoid the BER performance degradation at high SNR.
