Analysis of Application of Massive Transfusion Protocol for Trauma Patients at a Single Tertiary Referral Hospital (단일 3차 의료기관에서 외상환자에 대한 대량수혈 프로토콜 적용 분석)

  • Background: Massive hemorrhage due to trauma is one of the major causes of death in trauma patients, and the quick supply of appropriate blood products is critical in order to reduce the mortality rate. We introduced a massive transfusion protocol (MTP) for safe and rapid transfusion of trauma patients. Using records collected since its adoption, we compared the characteristics of MTP applied group (MTP group) and MTP not applied group (non-MTP group) to determine whether there is an indicator for predicting patients to be treated with MTP. Methods: We retrospectively reviewed the electronic medical records and laboratory findings of patients who received massive transfusions in the trauma emergency room of a single tertiary hospital from February to August 2018. We analyzed various laboratory test results, the amount and ratio of the transfused blood products, and the time required for blood products to be released for the MTP group and the non-MTP group. Results: Of the 54 trauma patients who received massive transfusions, 31 were in the MTP group and 22 in the non-MTP group. There was no significant difference in initial vital signs (except blood pressure) and laboratory test results. Also there was no difference in the amount and ratio of blood products, but the time required for blood product release was shorter in the MTP group. Conclusion: There was no significant difference in clinical findings such as initial vital signs and laboratory test results between the MTP and non-MTP groups, but required blood products were prepared and released more quickly for the MTP group.

  • In this paper, a 512b MTP memory IP is designed by using MTP memory cells which are written by the FN (Fowler-Nordheim) tunneling method with only MV (medium voltage) devices of 5V which uses the back-gate bias, that is VNN (negative voltage). The used MTP cell consists of a CG (control gate) capacitor, a TG (tunnel gate) transistor, and a select transistor. To reduce the size of the MTP memory cell, just two PWs (P-wells) are used: one for the TG and the select transistors; and the other for the CG capacitor. In addition, just one DNW (deep N-well) is used for the entire 512b memory cell array. VPP and VNN generators supplying pumping voltages of ${\pm}8V$ which are insensitive to PVT variations since VPP and VNN level detectors are designed by a regulated voltage, V1V (=1V), provided by a BGR voltage generator.

Effects of Orotic Acid and Di-(2-Ethylhexyl)Phthalate on Microsomal Triglyceride Transfer Protein(MTP) Activity and mRNA Levels in Liver and Intestine of Rats (Orotic acid와 Di-(2-ethylhexyl)phthalate 투여 흰쥐의 간장 및 소장 Microsomal Triglyceride Transfer Protein(MTP) 활성과 mRNA 수준에 미치는 영향)

  • Microsomal triglyceride transfer protein(MTP) activity and mRNA level were investigated in the liver and small intestine of rats fed on di-(2-ethylhexyl)phthalate(DEHP) and orotic acid(OA) as serum triglyceride-reducing agents. The concentration of liver triglyceride was significantly increased in the OA group, but that was not increased in the DEHP group compared with the control group. The concentration of serum triglyceride was significantly decreased in the OA and DEHP groups compared with the control group, but this reduction was more pronounced in the OA group. MTP activity and mRNA level in liver were decreased in the OA group compared with the control group, while MTP activity in the small intestine was increased in the OA group compared with the control group. MTP activities and MTP mRNA levels in both liver and small intestine had no influence by the DEHP dietary feeding, despite the triglyceride-lowering action, compared with the control dietary feeding. The activity of liver microsomal phosphatidate phosphohydrolase(PAP), the rate-limiting enzyme in triglyceride synthesis, was increased in the OA group compared with the control group, but that of cytosolic PAP was decreased in the DEHP group compared with the control group. The result suggest that MTP activity and MTP mRNA level are involved in the triglyceride-lowering action of OA, but those are not involved in that of DEHP.

Design of MTP memory IP using vertical PIP capacitor (Vertical PIP 커패시터를 이용한 MTP 메모리 IP 설계)

  • MCU used in applications such as wireless chargers and USB type-C require MTP memory with a small cell size and a small additional process mask. Conventional double poly EEPROM cells are small in size, but additional processing masks of about 3 to 5 sheets are required, and FN tunneling type single poly EEPROM cells have a large cell size. In this paper, a 110nm MTP cell using a vertical PIP capacitor is proposed. The erase operation of the proposed MTP cell uses FN tunneling between FG and EG, and the program operation uses CHEI injection method, which reduces the MTP cell size to 1.09㎛2 by sharing the PW of the MTP cell array. Meanwhile, MTP memory IP required for applications such as USB type-C needs to operate over a wide voltage range of 2.5V to 5.5V. However, the pumping current of the VPP charge pump is the lowest when the VCC voltage is the minimum 2.5V, while the ripple voltage is large when the VCC voltage is 5.5V. Therefore, in this paper, the VPP ripple voltage is reduced to within 0.19V through SPICE simulation because the pumping current is suppressed to 474.6㎂ even when VCC is increased by controlling the number of charge pumps turned on by using the VCC detector circuit.

Overexpression of the Metal Transport Protein1 gene (MTP1) in Arabidopsis Increased tolerance by expression site (금속전달 유전자(MTP1)의 과발현 애기장대에서 발현 위치에 따른 내성 증가 연구)

  • Today's scientists try to remove heavy metals with many new technologies such as phytoremediation. One of the best cutting edge technologies is developing transgenic plants to remove certain heavy metal in soil. I constructed the transformation vector expressing T. goesingense Metal Transport Protein1 gene and TgMTP1: GFP genes. The transgenic plants were selected and confirmed the transformed genes into Arabidopsis thaliana genome. Expression was confirmed in several parts in Arabidopsis cells, tissues and organs. When TgMTP1 overexpressing Arabidopsis thaliana were subjected, transgenic plants showed higher heavy metal tolerance than non-transgenic. For further study I selected the transgenic plant lines with enhanced tolerance against four different heavy metals; Zn, Ni, Co, Cd. The accumulation of these metals in these plants was further analyzed. The TgMTP1 overexpressing Arabidopsis thaliana plant of selected lines are resistant against heavy metals. This plant is characterized by the expression of the MTP1 gene accumulating heavy metal in the vacuole and being simultaneously expressed on the plasma membrane. In conclusion, these plants may be used in plant purification applications, and as a plant with increased tolerance.

The Comparison of Knee Joint Displaying between The Anteroposterior Weight Bearing View and the Metatarsophalangeal View with Osteoarthritis Patients (골관절염 환자의 촬영방법에 대한 고찰 : AP-WB(Weight-bearing AP), MTP(semiflexed) 촬영법의 비교 고찰을 중심으로)

  • Objective : The aim of this study was to compare the knee joint displaying between the anteroposterior weight bearing(AP-WB) View and the metatarsophalangeal(MTP) view for assessing joint space narrowing(JSN) and osteophytes in osteoarthritis patients. Subjects and Materials : Two hundreds of twenty patients(38 men) who came rheumatoid caused by knee pain, had both AP-WB and MTP views taken on a day. Radiographs were evaluated independently by 13 experienced observers(3 orthopedics surgeon, 2 rheumatogist, 3 radiologist, 5 radiological technologist) They assessed JSN and osteophytes using by PACS monitor JSN was scored by the optic evaluation to the nearest at the narrowest point in medial compartments of the tibiofemoral joint in both knees. Osteophytes were graded 0 to 3(bad 0, not bad 1, good 2 and very good 3) according to a standard atlas. All exam was using by Philips(Buckey Diagnostic-TH) X-ray material. Exposure condition was 60 kv, 8 mAs and 100 cm focus to film distance. Results : JSN was scored $1.32{\pm}0.050$ in AP-WB view, $2.51{\pm}0.046$ in MTP view. MTP view of JSN score is higher to AP-WB view significantly(p<0.05). Osteophytes scored $2.14{\pm}0.054$ in AP-WB view, $2.10{\pm}0.054$ in MTP view. There was no difference(p<0.05) between MTP view and AP-WB view in osteophytes. But MTP view was more reproducible than AP-WB view Conclusions : Joint space narrowing is most important factor to diagnosis with knee joint Osteoarthritis patients. This study was summarized as follows; In comparision of JSN, MTP view was more widely displayed than AP-WB view. In comparision of Osteophytes, there was no difference between MTP view and AP-WB view. It was concluded MTP view was more useful method to diagnosis of knee joint Osteoarthritis patients.

Effects of Massive Transfusion Protocol Implementation in Trauma Patients at a Level I Trauma Center

  • Purpose: This study was conducted to investigate whether rapid and efficient administration of blood products was achieved and whether clinical outcomes were improved by applying a massive transfusion protocol (MTP). Methods: From January 2016 to September 2019, the medical records of trauma patients who received at least 10 units of packed red blood cells (PRBC) at Pusan National University Hospital (level I trauma center) were retrospectively reviewed. The patients treated from January 2016 to January 2018 were designated as the non-MTP group, and those treated from February 2018 to September 2019 were designated as the MTP group. Results: During the study period, 370 patients received massive transfusions. The non-MTP and MTP groups comprised 84 and 55 patients, respectively. No significant between-group differences were found in the units of PRBC (23.2 vs. 25.3, respectively; p=0.46), fresh frozen plasma (FFP) (21.1 vs. 24.4, respectively; p=0.40), and platelets (PLT) (15.4 vs. 17.0, respectively; p=0.54) administered in the first 24 hours. No statistically significant differences between the non-MTP and MTP groups were found in the FFP-to-PRBC ratio (0.9 vs. 0.94, respectively; p=0.44) and or the PLT-to-PRBC ratio (0.72 vs. 0.72, respectively; p=0.21). However, the total number of cryoprecipitate units was significantly higher in the MTP group than in the non-MTP group (7.4 vs. 15.3 units, respectively; p=0.003) and the ratio of cryoprecipitate to PRBC in the MTP group was significantly higher than in the non-MTP group (0.31 vs. 0.62, respectively; p=0.021). The time to transfusion was significantly reduced after MTP implementation (41.0 vs. 14.9 minutes, respectively; p=0.003). Conclusions: Although no significant differences were found in the clinical outcomes of patients who had undergone severe trauma, rapid and balanced transfusion was achieved after implementing the MTP.

Studies on nickel uptake in transgenic Arabidopsis thaliana introduced with TgMTP1 gene encoding metal tolerance protein (TgMTP1 과발현 애기장대에서 Nickel 흡수 연구)

  • To enhance phytoremediation, which removes heavy metal from soil, transgenic plants were applied to contaminated soil. We constructed a transformation vector expressing both $TgMTP_1$ (T. goesingense metal tolerance protein):HA and TgMTP:GFP genes. Transgenic plants were generated using an Agrobacterium-mediated transformation system that expressed the two vectors. Screening and analysis confirmed the incorporation of foreign genes into the Arabidopsis thaliana genome. Callus was induced in the 116 T3 line. These transgenic plants and calli were used for further analyses on the accumulation of Ni. The 116 T3-line plants and calli from selected lines were resistant to heavy metals and accumulated Ni in their leaves. The expression level of TgMTP RNA was equal in all leaves, but protein stability increased in the leaves with Ni treatment. According to these results, we suggest that $TgMTP_1$-overexpressing plants may be useful for phytoremediation of soil.

A development of MTP e-portfolio evaluation model for the college students of science and engineering school (이공계 대학 학부생을 위한 MTP(Muti-grade Tutoring Project) e포트폴리오 평가 모형 개발)

  • MTP(Multi-graded Tutoring Project) is a designed curriculum needed to perform a high quality project to improve teamwork and major skills for college student of science and engineering. Each student can experience teaching activities as a tutor and learning activities as a tutee in the MTP for three years. Through this curriculum, students are expected to obtain the capabilities to set up and search the goal of given projects, to make human network among multi-graded students and professionals which could be critical for finding careers, and to perform graduation projects in a more systematic and co-operated way, resulting in highly-qualified outputs. In this paper, MTP e-portfolio evaluation model is suggested. This model evaluates the individual-level performance operating in MTP curriculum. The evaluation model is described with three dimensional structure composed of participator, changes in learning, and learning processes. The resultant model could be utilized as tools for evaluating the curriculum set up for the multi-graded project-based teaching environment.
