Extensive gingival necrosis and sequestration of the alveolar bone caused by methimazole-induced neutropenia and three-year follow-up

  • Purpose: Methimazole is an anti-thyroid drug that can cause life-threatening neutropenia in rare situations. The aim of this case report is to describe a set of oral complications associated with methimazole-induced neutropenia and the healing of the gingiva after proper treatment. Methods: A 31-year-old female patient hospitalized for systemic symptoms of sore throat and fever and showing extensive gingival necrosis with pain was referred to the Department of Periodontics from the Department of Endocrinology. Methimazole-induced neutropenia was diagnosed based on blood test results and her medical history. Methimazole was discontinued and a range of treatments was administered, including the injection of granulocyte colony stimulating factor. Results: After systemic treatment, the gingiva began to heal as the neutrophil count increased. Approximately one year later, the gingiva had returned to a normal appearance. Twenty-one months after treatment, sequestra of the alveolar bone that had broken through the gingiva were removed. Periodic supportive periodontal treatment has been continued uneventfully. Conclusions: The oral manifestations of gingival necrosis and ulcerations, in combination with systemic symptoms such as fever and sore throat, are the critical signs presented in the early stages of drug-induced neutropenia. Therefore, dentists need to be aware of these oral complications in order to make an accurate diagnosis and to ensure that prompt medical intervention is provided.

메티마졸 투여 중인 갑상선기능항진증 환자에서 한약의 임상증상 완화효과에 대한 증례보고 (A Case Report on The Relieving Effect of Herbal Medicine on Clinical Symptoms in Patients with Hyperthyroidism Taking Methimazole)

  • 본 증례는 그레이브스병에 의한 갑상선기능항진증으로 진단된 환자에서 메티마졸 단독 투여에도 계속된 갑상선기능항진증의 증상치료를 위하여 한의 변증진단(辨證診斷) 과정을 통하여 심담허겁(心膽虛㥘)과 심화상염(心火上炎) 변증형(辨證形)을 도출하고 가미온담탕(加味溫膽湯)에 청심연자음(淸心蓮子飮)을 합방(合方)한 한약을 메티마졸과 병행투여 한 사례이다. 본 증례에서 한약이 메티마졸의 갑상선 호르몬 억제 효과는 계속 유지하면서 갑상선기능항진증 환자의 증상을 완화한 효과를 보여준 결과는 갑상선기능항진증 치료에 있어서 한약이 베타 차단제를 대체하여 메티마졸과 병행투여할 수 있는 약물로써 가능성이 있다는 것을 보여준다.

Estimation of Human Flavin-containing Monooxygenases Activity(FMO1) in the Baculovirus Expression Vector System by using S-oxidation of Methimazole

  • The flavin-containing monooxygenases (FMOs) (EC 1.14. 13.8) are NADPH-dependent flavoenzymes that catalyze oxidation of soft nucleophilic heteroatom centers in a range of structurally diverse compounds including foods, drugs, pesticides, and other xenobiotics. In humans, FMOl appears to be the predominant form expressed in human fetal liver. cDNA-expressed human FMO and human liver microsomal FMO have been observed to N- and S-oxy-genate nucleophilic nitrogen- and sulfur-containing drugs and chemicals, respectively. In the present study, FMOl can be expressed in the baculovirus expression vector system at level of 2.68 nmol FMOl/mg of membrane protein. This isoform was examined for its capacity to metabolize methimazole to its S-oxide using thiocholine assay. Kinetic studies of its S-oxide by recombinant human FMO1 result in Km of 7.66 $\mu$M and Vmax of 17.79 nmol/min/mg protein.

  • A case is reported of aplastic anemia occuring during Methimazole therapy. Since this seems to be a kind of self-limiting disease having considerably rapid recovery, much more detailed supportive therapy is considered to be crucial rather than any other corticosteroid or the androgen therapy.

Methimazole-Induced Anti-Neutrophil Cytoplasmic Antibody-Associated Vasculitis in the Peripheral Nerves

  • Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis has been reported in Graves' disease patients treated with antithyroid drugs (ATDs), especially propylthiouracil. ATD-induced ANCA-associated vasculitis usually involved the kidneys followed by the respiratory organs and skin. The treatment of ANCA-associated vasculitis induced by ATDs is to stop ATD therapy immediately, which often leads to an overall good prognosis. We report a case of ANCA-associated vasculitis in the peripheral nerves of the lower extremities in a 66-year-old woman who was treated with methimazole (MMI) for Graves' disease. To our knowledge, this is the third case of peripheral nervous system (PNS) involvement of ATD-induced vasculitis and the first case of PNS vasculitis associated with MMI.

  • Microsomes isolated from Spodoptera frugiperda (Sf)9 cells infected wi th human FM01 recombinant baculovirus catalyzed the NADPH- and 02-dependent oxidation of methimazole, thiourea, and phenylthiourea. However there was no detectable activity with 1,3-diphenylthiourea or larger thiocarbamides. Microsomes from control Sf9 cells were devoid of methimazole or thiourea S-oxygenase activity. (omitted)

Methimazole-disulfide as an Anti-Thyroid Drug Metabolite Catalyzed the Highly Regioselective Conversion of Epoxides to Halohydrins with Elemental Halogens

  • The regioselective ring opening of epoxides using elemental iodine and bromine in the presence of methimazole (MMI, a anti-thyroid drug) and its metabolite methimazole-disulfide as new catalysts are studied. MMI easily converted in vitro to MMI-disulfide without any double activation presented in vivo. FT-Raman and UV spectroscopies are used to study the interaction of iodine with these catalysts. The results indicate that both catalysts are efficient in polyiodide formation, but MMI-disulfide can catalyze this reaction in higher yield and regioselectivity. The complex [(MMI-disulfide)I]+.I3- is considered to be formed initially which could be bulkier by addition of excess of iodine in the course of the reaction. These bulky nucleophiles have a fundamental role in the high regioselectivity by attacking the less sterically hindered epoxide carbon. In this study we suggest that MMI is readily converted to MMI-disulfide by interaction with iodine or activated iodine in thyroid gland, and this process is responsible for high anti-thyroid activity of MMI.

사람 Flavin-containing Monooxygenase 3의 Thiocarbamide 화합물의 기질 크기에 따른 효소활성에 관한 연구 (Effect of Substrate Size on Activities of Thiocarbamides with the Human Flavin-containing Monooxygenase 3)

  • FMOs (Flavin-containing monooxygenases, EC1.14.13.8)는 다양한 종류의 식품, 약물이나 기타 외래 유래물질 (xenobiotics)를 산화시키는 NADPH와 $O_2$ 의존성 약물대사효소이다. 현재까지 5종의 subfamily가 존재하는 것으로 보고되어 있으며 그 중 잘 알려진 FMO3는 대표적 인 subfamily로서 주로 간에 존재한다 사람FMO에 관한 연구는 최근 들어 활성화되기 시작했으며 질소, 황이나 인 등을 포함하는 친핵성 (nucleophilic) 화합물이 대표적인 기질로 보고되어 있다. 본 연구에서는 thiocarbamide를 포함하고 있는 화합물에 대한 사람의 FMO3의 기질 특이성을 알아보고자 하였다. 사람 FMO3를 baculovirus system을 이용하여 대량으로 발현시킨 후 그 microsomal FMO3를 분리하여 thiocholine assay를 시행하였다. 그 결과 methimazole, thiourea, and phenylthiourea는 낮은 $K_{m}$ (4-10$\mu$M)간을 갖는 반면, 이보다 기질의 크기가 큰 1 ,3-diphenylthiourea, 1, 3-bis (3, 4-dichlorophenyl)-2-thiourea, 1, 1-dibenzyl-3-phenol-2-thiourea에서는 효소활성이 나타나지 않았다. 이는 사람 fM01과 비교하여 볼 때 큰 차이는 없었으며, 다른 pig, guinea pig, rat, rabbit에서 보다 받아들일 수 있는 기질의 크기가 더 제한적임을 알 수 있었다.

고양이에서 만성 신부전에 병발한 갑상선 기능 항진증의 진단과 치료 (Diagnosis and Management of Feline Hyperthyroidism Complicated with Chronic Kidney Disease)

  • 14년령의 잡종 암컷 고양이가 다식증, 과잉행동 및 체중감소를 주증으로 내원 하였다. 신체검사에서 빈맥과 고혈압이 확인되었으며, 실험실 검사를 통하여 요소 질소 증 및 갑상선 호르몬 (TT4, fT4)의 상승이 확인 되었다. 환자는 확진을 위하여 T3 억압 시험이 시행 되었으며, 검사 결과 외부에서 T3가 공급된 전후의 갑상선 호르몬 수치에 변화가 없는 것이 확인 되었다. 따라서, 환축은 만성 신부전에 병발한 갑상선 기능 항진증으로 진단되었으며, 메티마졸을 이용한 약물 관리가 실시 되었다. 결론적으로 본 증례의 경우 고양이에서 발생한 만성 신부전에 병발한 갑상선 기능 항진증의 임상증상과 진단, 그리고 치료 반응에 대한 국내 첫 증례보고이다.
