Clinical Analysis of Recurrent Bell's Palsy in One University Hospital (한 대학병원에서 분석한 재발 벨마비 환자의 임상양상)

  • Background: Bell's palsy (BP) is the most common cause of unilateral lower motor facial palsy. Recurrent paralysis of the facial nerve is unusual and reported in only 7-8%. Methods: A total of 394 consecutive patients with acute BP patients were enrolled at Daegu Catholic University Hospital from July 2005 to September 2012. We classified the patients into two groups-single BP and recurrent BP-and compared them by patient characteristics, clinical features, MRI findings, electrophysiologic findings and prognosis. The degree of BP was graded according to the House and Brackmann facial nerve grading system. Results: Recurrent BP was observed in 31 (7.9%) patients. The number of recurrence was varied from 2 to 5. The recurrent BP (9.7%) had more incidence of family history and MRI enhancement than those of single BP (2.2%, p=0.047). The single BP (63.4%) had better recovery than recurrent BP (45.2%, p=0.045). Conclusions: The recurrent BP had more incidence of family history, MRI enhancement and poor prognosis than the single BP.

Neuroprotective Effects of Guh-Poong-Chung-Sim-Hwan on Focal Cerebral Ischemia in Rats

  • This study was designed to investigate the neuroprotective effects of Guh-Poong-Chung-Sim-Hwan(GCH) on ischemia induced by middle cerebral artery occlusion(MCAO) in Sprague-Dawley rats. The effects of GCH administration on the size of the brain infarct and the functional status of the rats after ischemia were examined, as well as the expression of COX-2 in acute phase. The recovery of motor functions for 7 days and the brain infarct were examined to find out the delayed effects of daily GCH-administration as well. In conclusion, we found that GCH reduced both functional deficits and brain damage in the MCAO rat model of stroke. In addition, high doses of GCH reduced COX-2 expression in the penumbra. It is well known that herbal medication including GCH is very safe for humans. Accordingly, our results support the clinical use of this GKM for the treatment of stroke and offer the possibility that a potent neuroprotective agent could be developed from Korean herbal medicines.

Effects of Scalp Acupuncture on Short-term NIHSS and MBI in Stroke Patients (두침이 중풍환자의 단기간 NIHSS와 MBI에 미치는 영향)

  • Objective: The aim of this study was to investigate the effect of scalp acupuncture on recovery from motor disorders in stroke patients. Methods: Twenty-two patients with post-stroke hemiplegia were randomized into two groups. Ten patients (test group) were treated by 2 methods: scalp acupuncture and general acupuncture. The other twelve patients (control group) were treated only with general acupuncture. The activity of daily living was measured with a National Institutes of Health stroke scale (NIHSS) and Modified Barthel Index (MBI). The therapy was performed once a day for 2 weeks. Results: In terms of NIHSS score, the test group showed statistically meaningful decrease after 2 weeks treatment, while the control group showed statistically meaningful decrease after 1 week (p<0.05). And in terms of score of MBI, the test group did not show statistically meaningful increase but the control group showed statistically meaningful increase after 1 week (p<0.05). There was no statistically meaningful difference after 1 and 2 weeks treatment between the groups. Conclusions: These results support that the test group has almost same the effectiveness compared with the control group in improvement of the activity of daily living of post-stroke hemiplegic patients.

  • Brachial neuritis is a rare disorder of unknown etiology that affects the lower motor neurons of the brachial plexus. The clinical course is characterized by acute onset of severe pain followed by weakness and gradual recovery. Among diagnostic tests, electromyography may be useful. The brachial neuritis has been confused with other painful shoulder conditions. The awareness of this disorder helps prevent unwarranted diagnostic studies & treatment. The authors report a case of brachial neuritis.

Effectiveness of Combined Korean Medicine on Traumatic Spinal Cord Injury: A Case Report

  • Objectives: This study aims to evaluate the clinical effectiveness of a Korean medicine treatment on a traumatic spinal cord injury. Metohds : A 44-year-old male patient who had suffered a traumatic spinal cord injury was treated for two months. The effectiveness of complex Korean medicine treatments was measured using the Numeric Rating Scale (NRS) and the Manual Muscle Test (MMT). Result : We found that the NRS of pain caused by numbness was decreased from 7 to 0-1, and the overall motor measured by the MMT was also improved. The patient's walking state was changed from wheelchair ambulation to walker ambulation. Conclusion : Based on this finding, complex Korean medicine treatments may be effective in treating patients' pain, controlling paresthesia, and aiding the recovery function of activities in daily living. However, because of the limitation of our study, further high-quality research should be conducted.

DEVELOPMENT AND TESTING OF MEDIUM CAPACITY GRAIN FLOUR SEPARATOR

  • A power operated 90.5 hp electric motor) grain flour separator was designed and developed for separation of grain (wheat, corn, chickpea and soybean) flour into various fractions based on the size of the particles of the product. The separator agitating mechanism, feed control, cylindrical separator unit and an eccentric mechanism. The machine was tested for wheat ( variety ; Sujata) flour separation into four fractions, viz ; semolina, Gr-I and II, flour (coarse) and white (fine) flour. Wheat samples (6.8% m.c., db) were first pearled by CIAE pearler for 15.8% bran removal . The pearled wheat grains were then milled for semolina by a burre mill. The product and machine characteristics were determined at different capacities varying from 24 kg/h to 143 kg/h. It was found that 76 kg/h capacity gave reasonably best results in terms of purity and recovery of semolina vis-a-vis the market product. The energy requirement of the machine at no-load was found to be 230 W and at load c nditions, it varied between 36.3-6.4 KJ per kg of fead seperation. The macine could be used by small flour millers small/medium size traders and retailers and other processors for making available various flour products of different particle size in the market for ready use of the consumers.

A Case Report of Upper Extremities Rehablitation of 2 Cervical Spinal Cord Injured Patients by Functional Electrical Stimulation with Korean Medical Interventions (경추부 척수손상 사지마비 환자의 한방치료와 기능적 전기 자극요법을 통한 상지 재활 치험 2례)

  • Two patients, diagnosed as cervical spinal cord injury, were hospitalized and treated by Korean medical interventions with Functional Electrical Stimulation (FES). The range of motion of the shoulder joint, manual muscle test were adopted to measure the resulting motor recovery after treatment. And the Korean version of Barthel index (K-MBI) was adopted as a method of measuring the improvement of quality of life after treatment. The range of motions and muscle strengths of the shoulder joints were improved. The activities of daily living were slightly improved in case 2, but showed no improvement in case 1. FES was effective for upper extremities rehabilitation of spinal cord injured patients. Further studies are needed to set up an Korean medical protocol for spinal cord injury rehabilitation.

DEVELOPMENT OF AN OPERATION STRATEGY FOR A HYBRID SAFETY INJECTION TANK WITH AN ACTIVE SYSTEM

  • A hybrid safety injection tank (H-SIT) can enhance the capability of an advanced power reactor plus (APR+) during a station black out (SBO) that is accompanied by a severe accident. It may a useful alternative to an electric motor. The operations strategy of the H-SIT has to be investigated to achieve maximum utilization of its function. In this study, the master logic diagram (i.e., an analysis for identifying the differences between an H-SIT and a safety injection pump) and an accident case classification were used to determine the parameters of the H-SIT operation. The conditions that require the use of an H-SIT were determined using a decision-making process. The proper timing for using an H-SIT was also analyzed by using the Multi-dimensional Analysis of Reactor Safety (MARS) 1.3 code (Korea Atomic Energy Research Institute, Daejeon, South Korea). The operation strategy analysis indicates that a H-SIT can mitigate five types of failure: (1) failure of the safety injection pump, (2) failure of the passive auxiliary feedwater system, (3) failure of the depressurization system, (4) failure of the shutdown cooling pump (SCP), and (5) failure of the recirculation system. The results of the MARS code demonstrate that the time allowed for recovery can be extended when using an H-SIT, compared with the same situation in which an H-SIT is not used. Based on the results, the use of an H-SIT is recommended, especially after the pilot-operated safety relief valve (POSRV) is opened.

The Effect of Rectus Abdominis Functional Massage on Forward head posture and Pain in Patients with Chronic Neck Pain (복직근 기능적 마사지가 만성 목통증 환자의 머리전방자세와 통증에 미치는 영향)

  • Background: The purpose of this study was to determine the effects of cervical deep muscle flexion exercise (CCFE) on craniovertebral angle, pain, and neck disability for patients with chronic neck pain Methods: The subjects of this study were randomly divided into three groups of 30 patients with chronic neck pain: rectus abodominis functional massage (n=10), cervical deep muscle flexion exercises group (n=10), and the control group(n=10). To assess visual analog scale (VAS) was used to test the neck pain, To assess neck posture was used to craniovertebral angle, VAS was used to test the neck pain, neck disability index (NDI) was used to test the neck dysfunction. All measurements were performed before and after each intervention was applied 3 times a week for 4 weeks. Results: In the results of all measurements, 2 groups except for the control group showed a significant change in the recovery of posture, neck pain, neck disability index (p<.05). Conclusions: Our results of this study showed that applying cervical deep muscle flexion exercise and rectus abodominis functional massage to patients with chronic neck pain improved cervical posture, neck pain, neck disability.
