작업치료 임상에서 뇌졸중 환자의 상지기능 향상을 위한 가상현실 치료의 유용성에 관한 고찰 (Feasibility of Virtual Reality for Enhancement of Upper Extremity Function Post Stroke)

  • 본 연구에서는 가상현실 치료가 뇌졸중 환자의 뇌 가소성을 동반한 상지기능 향상에 미치는 영향을 알아보고 강도 높은 가상현실 훈련이 뇌졸중 환자의 상지기능 향상을 위한 집중치료로써 임상적으로 유용한 훈련인지 알아보고자 하였다. 뇌졸중 환자에게 있어 사용-의존성 즉 운동 강도와 반복은 마비 측 사지의 운동 기능향상에 중요한 치료적 요소이다. 최근에는 상지의 지속적 사용을 통한 뇌-가소성에 기반한 변화를 유도할 수 있는 치료방법으로 가상현실 치료가 대두되었다. 가상현실 치료는 재활 임상환경에서 운동기능 향상을 위한 훈련강도와 반복을 제공할 수 있는 기술적 방법으로 채택되기 시작하였다. 특히 뇌졸중 환자의 상지 기능을 향상시키기 위한 치료적 유용성 측면에서는 강도 높은 반복적 훈련이 가능하다는 것과 게임 같은 형식으로 높은 동기부여가 가능하다는 것, 실제 수행을 통한 다중감각적 피드백 제공, 상호작용이 가능한 과제지향적 치료가 가능하다는 장점을 가지고 있다. 임상 환경에서 작업치료와 더불어 부가적으로 가상현실 치료를 실시하는 것은 뇌졸중 환자의 상지 기능회복을 더욱 촉진할 것이다.

  • The current source inverter-fed induction motor(CSIM) drive is widely used in industry because of its four quadrant operation, fuseless protection, fuseless protection, and ruggedness. the CSIM drive system, however, has shortcomins such as slow response and dynamic stability to load torque disturbance and reference speed change. Such a disadvantages can be compensated considerably by means of introducing additional angle angle control loop. The angle control method is dependent upon the inverter type. In this paper, simultaneus recovery and commutation inverter(SRCI) which is developed recently is considered.

  • Energy recovery in the regenerative region is very important when SRM(Switched Reluctance Motor) is used in traction drive. This is because that to reduce energy loss during mechanical braking and/or to have a high efficiency drive during braking. To control excitation voltage in motor operation and regenerative voltage in the generator operation in the SRM, multi-level voltage control is effective. This paper suggests multi-level inverter which is useful for motoring and regenerative operation in SRM.

  • This paper describes the various characteristics of voltage sags which can affect the functions of three-phase induction motors that are mostly used in the power distribution systems. These assorted characteristics include motor speed losses, voltage recovery, motor reacceleration, and transient characteristics. An experimental study on the induction motor behaviors was also carried out to confirm these impacts. In addition sequential voltage sags with short durations were considered and investigated. The results show that the occurrence of the second voltage sag after the first one may affects the induction motor adversely.

침 치료가 신경 재생 및 회복에 미치는 영향에 대한 연구 동향: PubMed를 중심으로 (Current Research Trend on Acupuncture Treatment for Nerve Regeneration and Recovery: Based on the Data of PubMed)

  • Objectives : The purpose of this study is to explore the current research trend on acupuncture treatment for nerve regeneration and recovery effect. Methods : We investigated the researches so far, on acupuncture treatment for the nerve regeneration and recovery via searching Pubmed from 2005 up to October 2014. Data were extracted from the included studies regarding the authors, countries, type of nerve injury, type of acupuncture, treatment period, acupuncture points, assessment tool and results. Results and Conclusions : Twenty-four research papers were included in the review. Outcomes were measured by immunohistochemical results, motor behavior scores, and electrocphysiological results. All but one study favored acupuncture and electroacupuncture treatment for nerve regeneration and recovery regardless of type of nerve injury and acupuncture modality. Acupuncture treatment may have a potential for nerve regeneration and recovery and further research is required.

뇌경색 환자의 운동장애에 대한 주파수별 전침치료효과의 비교 (A Comparative Study of Motor Recovery from Stroke between High and Low Frequency Electrical Acupoint Stimulation)

  • Objectives : Electrical acupoint stimulation (EAS) has been used to treat motor dysfunction of stroke patients with reportedly effective results. The purpose of this study was to evaluate the efficacy of EAS with different frequencies in treating motor dysfunction of ischemic stroke patients. Methods : The subjects of this study were sixty-two ischemic stroke patients with motor dysfunction at Kyunghee Oriental Medicine Hospital who were hospitalized for one week to one month from onset. They were treated with 2Hz or 120Hz EAS for two weeks, and motor evoked potentials (MEP) were measured before and after EAS treatment. To compare the effect of 2Hz EAS with 120Hz, we measured latency, central motor conduction time (CMCT) and amplitude of MEP before and after EAS treatment. Results : After two weeks of treatment, we compared MEP data of the affected side between the 2Hz group and the 120Hz group. The 2Hz group showed more significant improvement than the 120Hz group in latency, CMCT and amplitude (P=0.008, 0.002, 0.002). In case of the affected side MEP data divided by normal side MEP data, the 2Hz group also showed improvement on latency, CMCT and amplitude with significant differences (P=0.003, 0.000, 0.008). Conclusions : These results suggest that low frequency EAS activates the central motor conduction system better than high frequency EAS, and it means that EAS with low frequency is more helpful for motor recovery after ischemic stroke than that with high frequency.

Even in Patients with a Small Hemorrhagic Volume, Stereotactic-Guided Evacuation of Spontaneous Intracerebral Hemorrhage Improves Functional Outcome

  • Objective: The decision to adopt a conservative or surgical modality for a relatively small volume of spontaneous intracerebral hemorrhage (SICH) is difficult and often controversial, especially when consciousness is tolerable. The authors examined the results of stereotactic-guided evacuation of SICH for relatively small volumes with respect to functional outcome. Methods: This prospective study was performed on 387 patients with SICH who underwent stereotactic-guided evacuation (n = 204, group A) or conservative treatment (n = 183, group B) during the past 8 years. The primary end-point was recovery of functional status, which was estimated using the Modified Barthel Index (MBI) and the modified Rankin Scale (mRS). Results: All patients had a Glasgow coma scale (GCS) score of $\geq$ 13 and unilateral hemiparesis of less than motor power grade 3. Group demographic characteristics and initial neurological statuses were similar. In all cases, the volume of SICH involved was < 30 cm$^3$ and location was limited to basal ganglia and thalamus. At 6-month follow-ups, MBI was 90.9 in group A and 62.4 in group B (p < 005), and MRS was 1.2 in group A and 3.0 in group B (p < 0.05). Better motor function and stereotactic-guided evacuation had a significant effect on a functional recovery in regression analyses. Conclusion: Even in patients with a small volume of SICH, stereotactic-guided evacuation improved functional recovery in activities in daily life than conservative treatment did.
