Studies of the effects and mechanisms of ginsenoside Re and Rk3 on myelosuppression induced by cyclophosphamide

  • Background: Ginsenoside Re (Re) is one of the major components of Panax ginseng Meyer. Ginsenoside $Rk_3$ ($Rk_3$) is a secondary metabolite of Re. The aim of this study was to investigate and compare the effects and underlying mechanisms of Re and $Rk_3$ on cyclophosphamide-induced myelosuppression. Methods: The mice myelosuppression model was established by intraperitoneal (i.p.) injection of cyclophosphamide. Peripheral blood cells, bone marrow nucleated cells, and colony yield of hematopoietic progenitor cells in vitro were counted. The levels of erythropoietin, thrombopoietin, and granulocyte macrophage colony-stimulating factor in plasma were measured by enzyme-linked immunosorbent assay. Bone marrow cell cycle was performed by flow cytometry. The expression of apoptotic protein bcl-2, bax, and caspase-3 was detected by Western blotting. Results: Both Re and $Rk_3$ could improve peripheral blood cells, bone marrow nucleated cell counts, thymus index, and spleen index. Furthermore, they could enhance the yield of colonies cultured in vitro and make the levels of granulocyte macrophage colony-stimulating factor, erythropoietin, and thrombopoietin normal, reduce the ratio of $G_0/G_1$ phase cells, and increase the proliferation index. Finally, Re and $Rk_3$ could upregulate the expression of bcl-2, whereas they could downregulate the expression of bax and caspase-3. Conclusion: Re and $Rk_3$ could improve the hematopoietic function of myelosuppressed mice. The effect of $Rk_3$ was superior to that of Re at any dose. Regulating the levels of cytokines, promoting cells enter the normal cell cycle, regulating the balance of bcl-2/bax, and inhibiting the expression of caspase-3 may be the effects of Re and $Rk_3$ on myelosuppression.

Effect of Astragalus Membranaceus Extract against Improvement of Myelosuppression and Quality of Life in 5-Fluorouracil Treated Mice (황기 추출물이 5-Fluorouracil을 투여한 생쥐의 골수억제 및 삶의 질에 미치는 영향)

  • Objective : To evaluate the effect of Astragalus Membranaceus Extrac (AME) on myelosuppression, activity and immune modulation in 5-fluorouracil (5-FU) treated mice. Method : We carried out complete blood count, histological analysis of bone marrow, and cell colony forming assay for hematopoietic progenitor to evaluate the effect of AME on myelosuppression and conducted swimming test, survival rate, nitric oxide (NO) assay, 51Cr release assay in natural killer cell, mRNA expression of $IL-1{\beta}$, IL-2, IL-4, IL-6, IL-10, $TNF-{\alpht}$, $IFN-{\gamma}$, $TGF-{\beta}$ and GM-CSF in spleen cells to evaluate the effect of AME on quality of life (QOL). Results : AME improved 5-FU induced myelosuppression and peripheral blood count was recovered effectively, had significant efficacy to protect against chemotherapy induced marrow-destruction and on hematopoiesis compared with the control group, improved increase survival rate and the swimming time, had a stimulatory effect on macrophage activation and NK cell activity, and up-regulated cytokine gene transcription (IL-2, IL-6, $IFN-{\gamma}$) in murine immunologic system. Conclusion : We can conclude that AM is an effective herbal agent for improvement of myelosuppression and QOL in 5-FU treated mice.

Effects of Saenghyul-dan (SHD) on the Myelosuppression Induced by 5-Fluorouracil

  • Objective : This study aimed to investigate the effect of SHD on myelosuppression using an animal model for therapeutic evidence supporting clinical positive results. Methods : After determining the optimal concentration of 5-FU as 300 mg/kg for developing the mouse model, ICR mice or BALB/c mice were administered with SHD, and several blood parameters, including hematopoietic cytokines, colony forming activity and histological findings, were examined to evaluate the effects. Results : SHD restored the WBC and hemoglobin, and showed effects on maintaining body weight, producing GM-CSF and IL-3 and stem cell colony forming activity in accordance with histological relative entirety on bone marrow. Conclusion : SHD is an herbal drug having therapeutic effects on myelosuppression. Thus, it could be prescribed to cancer patients undergoing chemo-therapies or radio-therapies in the process of cancer treatment.

A Case of Azathioprine Induced Severe Myelosuppression and Alopecia Totalis in IgA Nephropathy

  • Azathioprine is commonly used as immunosuppressive therapy for various inflammatory diseases including chronic glomerulonephritis. Myelosuppression is a common side effect of azathioprine, resulting in the need for dose reduction. However, severe pancytopenia or alopecia is not often encountered. Here, we report a case of severe myelosuppression, and alopecia totalis that occurred after azathioprine treatment in a patient with IgA nephropathy. A 10-year-old boy with IgA nephropathy was treated with oral deflazacort and later with azathioprine. After 4 weeks, the patient complained of hair loss, and despite a dose reduction in azathioprine, he developed bone marrow suppression and alopecia totalis in two weeks. The blood indices and alopecia of the patient had returned to normal after azathioprine withdrawal and 3 consecutive doses of granulocyte colony-stimulating factor. We suggest that physicians remain vigilant to the side effects of azathioprine. Unusual hair loss after azathioprine treatment might suggest a defect in the metabolism of the drug, warranting the discontinuation of azathioprine to prevent more severe side effects.

Compatibility effects of ginseng and Ligustrum lucidum Ait herb pair on hematopoietic recovery in mice with cyclophosphamide-induced myelosuppression and its material basis

  • Background: Ginseng (G) and Ligustrum lucidum Ait (LLA) are core traditional Chinese medicines in treating myelosuppression formula. The present study was designed to profile effect of G and LLA herb pair (G-LLA) on myelosuppressed mice. Methods: The mice myelosuppression model was established by intraperitoneal (i.p.) injection of cyclophosphamide (Cy). Hematopoietic function of bone marrow was measured by hemopoietic progenitor cell culture and peripheral blood count, and serum hemopoietic factors were tested by enzyme-linked immunosorbent assay. Bone marrow cell cycle was performed by flow cytometry. HPLC was used to measure 20 potential chemical components related to myelosuppression, including ginsenoside Rg1, Re, Rb1, Rc, Rb2, Rb3, Rd, Rk3, Rh4, 20 (S)-Rg3, 20 (R)-Rg3, Rk1, Rg5, salidroside, and so on. Results: G, LLA, and G-LLA improved the amount of peripheral blood cells and bone marrow cells of myelosuppressed mice (P < 0.01). They significantly increased the colony quantity of colony-forming unit-granulocyte macrophage, burst-forming unit-erythroid, colony-forming unit-erythroid, and colony-forming unit-megakaryocyte and amount of G2/M and S phase cells (P < 0.01). They also significantly decreased the amount of hematopoiesis-related cytokines (P < 0.01). The content of chemical components in G-LLA changed, and the change of rare saponin was the most obvious. Conclusion: These results show that G-LLA herb pair might produce synergistic or complementary compatibility effects on bone marrow suppression after chemotherapy. It suggests that the substance basis of G-LLA for treating bone marrow suppression may be effective chemical components.

Anti-Myelosuppression Effects of Korean Red Ginseng in SD Rat Injected with 5-fluorouracil

  • Objectives: This study aimed to investigate the preventive effect of red ginseng (RG) on 5-fluorouracil (5-FU)-induced side effects focusing on myelosuppression. Methods: Rats (n = 50) were divided into five groups, nave, control (ip, 5-FU injection of 150 mg/kg), and RG pre-treatment (po, 25, 50 and 100 mg/kg for 5 days before 5-FU injection). On the $7^{th}$ day after 5-FU injection, we evaluated the effects using peripheral hematological parameters, colony-forming assay, cytokine levels and histopathological finding. Results: The peripheral white blood cell and the differential count were dramatically suppressed by 5-FU, while RG (50 and 100 mg/kg) treatment significantly improved total white blood cell, neutrophil, lymphocyte and platelet counts. Also, RG (100 mg/kg) pre-treatment significantly increased the number of CFU-GM colony compared with the control group. RG pre-treatment also ameliorated the histopathological damage in bone marrow, spleen, stomach and small intestine tissue. Conclusions: These results demonstrate that Korean RG has preventive effects against 5-FU-induced myelotoxicity and gastrointestinal damage.

Effect of G-CSF on Myelosuppression and Antitumor Effect of DA-125, a Novel Adfiamycin Derivative (새로운 아드리아마이신유도체 DA-125의 초혈기독성과 항암효과에 미치는 G-CSF의 영향)

  • The present study was designed to evaluate the effects of a recombinant human granulocyte-colony stimulating factor (G-CSF) on leukopenia and tumor growth in mice treated with DA-125, an adri-amycin (ADM) derivative. In normal mice, single intravenous injection of DA-125 produced transient leukopenia accompanied with weight loss and splenic atrophy in a dose-related manner. However, subcutane-ous administration of G-CSF (5$\mu$g/head) for 5 consecutive days after DA-125 resulted in a significantly elevated nadir of leukocyte counts and facilitation of recovery from the leukopenia. To investigate the effect of G-CSF on antitumor effects of DA-125, ADM (12 mg/kg) or DA-125 (40 mg/kg) was administered to Colon-26 murine adenocarcinoma-bearing Balb/c mice with G-CSF. Regardless of treatment with G-CSF, DA-125 and ADM markedly retarded the growth of implanted tumor, though they failed to increase mean survival time of tumor-bearing mice. These results suggest that G-CSF is able to not only ameliorate, but reconstitute DA-125-induced myelosuppression without affecting its antitumor potential.

  • Aim: To examine the efficacy of Hominis Placenta Hydrate (HPH) on the hemopoiesis in a myelosuppression model system. Methods: Mice were injected with 5-Fluorouracil (5-FU) intraperitoneally and were administered with 200 mg/kg and 1000 mg/kg of HPH. Peripheral blood was analyzed at 5, 9, and 13 days. Histopathologic examination of bone marrow was performed at 5 days after 5-FU injection. The expression of cytokine involved in hemopoiesis was examined by using ELISA kit. Results: The hematology data demonstrated that the treatment of all the agents augmented monocytes and leucocytes counts in the peripheral blood WBC and platelet in HPH treated group were significant increased compared with control group. Also, cell numbers of RBC and Hb were restored. In HPH treated group, expression of IL-3, GM-CSF was significant increased But not TPO. Conclusions: Based on the above results, it is suggested that Hominis Placenta Hydrate is an effective remedy for the bone marrow failure and myelosuppression occurring during chemotherapy.

  • Chemokines are small chemotalic cytokines that have a number of biological functions. Some chemokines regulate the proliferation of hematopoietic stem and progenitor cells(HSPC). Leukotactin-l(Lkn-l) is a CC chemokine and is known to reduce colony forming unit(CFU). The N-terminal truncated Leukotactin-l(rtLkn-l), produced by Pichia pastoris, suppressed CFU from 40 to 60%. The rtLkn-l protected CFU from cytotoxic effect of anticancer drug such as Ara-C, doxorubicin, cyclophosphamide and 5-FU by cell cycle arrest.
