A Case Report of a Stage IV Non-small Cell Lung Cancer Patient Treated with Modified Yieum-jeon Showing Improvement in Malignant Pleural Effusion and other Respiratory Symptoms (비소세포성 폐암 4기 환자의 악성흉수로 인한 증상이 이음전가미방으로 호전된 1례)

  • Objectives: This is a report of a case of a stage IV non-small-cell lung cancer (NSCLC) patient whose malignant pleural effusion & respiratory symptoms have been relieved with Korean medicinal prescription, modified Yieum-jeon. Methods: A 56-year-old male diagnosed with stage IV NSCLC suffering malignant pleural effusion with complaints of cough, sputum, bilateral leg edema, dysphagia, post neck and left scapular pain was treated with modified Yieum-jeon and acupuncture during 16days. After treatment, blood lab and chest X-ray were used to follow the condition of malignant pleural effusion & pulmonary symptoms. Results: Malignant pleural effusion and pulmonary symptoms were improved with modified Yieum-jeon intake without any related adverse side effects. Conclusions: This study suggests Yieum-jeon may play a positive role in improving the malignant pleural effusion symptoms in advanced lung cancer cases.

A Case Study on Cervical Alignments Affecting by Activator (액티베이터가 경추의 정렬에 미치는 영향에 관한 사례 연구)

  • This study was performed to assess the efficacy of Activator for the patient with chronic neck pain. Mail, 26 years old, is treated with Activator, which was applied for 15 minutes every two days during the 4 weeks. The results of this study are as follows: 1. The treated case compared with the previous case showed that R.O.M particularly increased in flexion and extension. 2. The atlas/skull angle compared with the previous case showed that the angle increased from $0.8^{\circ}$ to $1.5^{\circ}$. In the atlas/skull, normal angle is $5.6^{\circ}$. 3. The angle of cervical curve lordosis compared with the previous case showed that the angle increased from $38.3^{\circ}$ to $48.1^{\circ}$. In the healthy spine, the angle is $42.0^{\circ}$. 4. The anterior curvature compared with the previous case showed that curve radii decreased from 7,220mm to 200mm. In the spine, normal curvature is 165mm. 5. Rush Jackson's angle compared with the previous case showed that the angle increased from $14.8^{\circ}$ to $29.1^{\circ}$. In the healthy spine, normal angle is $39.9^{\circ}$. We can not find significant level for Activator, but it is considered that activator method of chiropractic technique is effective.

The Effect on the Tension Trapezius Muscle of the Height Keyboard Computer (작업대 높이가 승모근의 근긴장도에 미치는 영향)

  • Purpose: Many kinds of musculoskeletal disease and symptom are caused by the longtime computer works. However, trapezius muscle tonus has not been established in regarding to keyboard height during typing. Therefore, this study is to evaluate the relationship between trapezius muscle tonus and the height of keyboard while typing, controling for the postures of neck, Lumbar, cervical vertebra. Methods: The experimental height of keyboard was set at elbow height, 3cm higher, 6cm higher, 9cm higher, than elbow. We studied trapezius tonus with the mean value for 2 minutes by EMG in 15 males and 15 females worker of hospital in seoul, who did not have a history of muscle disease, neurological signs, nerve damage. Results: In this experimental, as the height of the keyboard went up, the trapezius tonus significantly increased with shoulder abduction of brachium. Second, right and left trapezius tonus appeared similar while typing. Third, the best height that release the trapezius tonus the was as high as elbow and 3cm higher than elbow. Conclusion: With these above results, we suggest that the appropriate height of keyboard during typing to release the trapezius tonus most is the height of the elbow and 3cm higher than elbow. The study has important implications for focusing on the height of VDT worktable and complaining of a pain by oneself which are useful to establish a method of prevention of musculoskeletal disorder in work in the future.

Body Types and Its Relation with the Body Type Awareness, and Subjective Symptoms of Female High School Students in an Urban Area (도시지역 여자고등학생들의 체형과 체형에 대한 의식 및 자각증상과의 관계)

  • Purpose : This study investigated the relationship between body type, subjective symptoms and health awareness in female high school students Methods : The subjects were 393 students at the female high school in Taejon City by self-recorded questionnaires in May, 2006. Results : In the classification of body type based on the degree of obesity, low weight group was 20.9%, normal weight group was 58.0%, over weight group was 11.7% and obesity group was 9.4%. In the level of body type wareness, those who consider themselves to be obesity type 13.7%, standard type were 43.8%, and leptosomic type 8.1%, but there was a clear tendency to be leptosomic type that the rate was 76.3%. In the complaint rates of subjective symptoms, “fatigue” was the highest, followed by “neck pain(stiffness)”, “dizziness”, “stomachache” in the descending order. In the complaint rates of subjective symptoms by the degree of obesity, dizziness, irregular menstruation and anorexia were higher according to the lower obesity, but constipation and wearied eyes were the higher the more obesity. Conclusion : The study results showed that the recent female high school students were clear tendency to be leptosomic type, and leptosomic and obesity type were related with subjective symptoms. Considering these findings, it appeared that being excessively thin is related to symptoms and young women's thinness-oriented attitudes are unhealthful.

Cervical Spondylomyelopathy in Small-Breed Dogs (소형 품종 견에서 경추의 척추척수증)

  • Cervical spondylomyelopathy (CSM) is rarely identified in small-breed dogs. Two neutered female Pekingese dogs (less than 5 kg of body weight) with primary complaints of neck pain and paretic gait were presented. These cases were diagnosed as CSM secondary to vertebral instability through cervical survey radiography, myelography, computed tomography (CT) and CT-myelography. The combinatory treatments of ventral slot decompression, cancellous bone graft and external coaptation were performed. Clinical condition of both dogs remarkably improved and no complications or recurrence occurred following the surgical procedures. In small-breed dogs with CSM, the combination of decompression through ventral slot and stabilization through bone fusion is an effective treatment.

Anti-nociceptive and anti-inflammatory effects of Geranii Herba (현지초(玄之草)의 진통(鎭痛) 및 항염증(抗炎症) 효과)

  • Objectives : The present study investigated the anti-nociceptive and anti-inflammatory properties of the water extract of Geranii Herba (The stem and leaves of Geranium thunbergii Sieb. et Zucc.) in the animal models of pain and inflammation. Methods : We evaluated the anti-nociceptive and anti-inflammatory activities of Geranii Herba extract (GHE) using the writhing test, tail-flick test, carrageenan-induced paw edema and xylene-induced ear edema models. Two dose of GHE (100 and 1000 mg/kg) was administrated orally to the mice. Control group received normal saline and ibuprofen (50 mg/kg) was used as a positive control drug. Results : GHE 1000 mg/kg treated group showed an increased tail-flick response time in the tail-flick test and inhibitory effect on writhing syndrome induced by acetic acid. Treatment with GHE at the same dose inhibited ear edema induced by xylene and foot edema induced by carrageenan toxicity. Conclusion : The results demonstrate that GHE has anti-nociceptive and anti-inflammatory effects in the various models of nociception and inflammation.

  • Giant cell tumors are potentially malignant tumors in vertebrae, affecting frequently difficult to diagnose and are often inoperable. So it will be treated using radiation because of their high recurrence rate and the mechanical compression of spinal cord, but many surgeons described tumors of the vertebra, and the affected vertebral body can be treated using radical or near to total excision, with anteroposterior vertebral fusion or instrumentation of the spine. we report a case of giant cell tumor affecting the third cervical vertebra which caused neck pain and destroyed the vertebra body had treated using radical excison with fusion of posterior arch using instrumentation of the spine together with a literature review.

Effectiveness of Selective Peripheral Denervation for the Treatment of Spasmodic Torticollis

  • Objective : The Bertrand's method of selective peripheral denervation for spasmodic torticollis has already been established as being an effective and safe method. However, such effectiveness has not yet been established in Korea. The authors have performed several cases of selective denervation for the treatment of spasmodic torticollis and we hereby describe the details of the operative method to determine the effectiveness of the method. Methods : Ten patients who had the selective denervation for the spasmodic torticollis from October 1997 to December 2003, were analyzed. There were 4 rotational and 6 combined types [3 rotational plus retrocollis, 3 rotational plus laterocollis]. We performed preoperative electromyograms and neck muscle tonograms in all cases. The technique was used to involve denervation of the ipsilateral posterior paraspinal muscles. Denervation of the sternocleidomastoid muscle was performed on the contralateral side for rotational torticollis and on the ipsilateral side for laterocollis combined type. The surgery was followed by a rigorous physical therapy program. Results : The surgical results were divided into 4 groups labeled excellent, good, fair, and poor. There were 5 excellent patients who showed no detectable abnormal movements, 4 good patients who showed slight residual deviations or slight residual abnormal movements, and 1 fair patient with appreciable residual abnormal movements. Postoperative pain was well tolerated. There was no significant surgical complications. Conclusion : The study confirms that selective peripheral denervation for the treatment of spasmodic torticollis is a very useful and safe surgical technique.

A Survey on the Prevalence of Cumulative Trauma Disorders and Related Factors among Seafares (선원들의 누적외상성질환 발생과 관련요인)

  • The purpose of this study is to provide basic data for the prevention and management of Cumulative Trauma Disorders in Korea seamen. 569 seamen's questionnaire had been acquired at Korea Marine Training and Research Institute. The prevalence in Cumulative Trauma Disorders within recent 12 months was 68.5%. The more working hours(p<0.01), services on the vessel(p<0.01), and the less job satisfaction(p<0.05), the higher prevalence of Cumulative Trauma Disorders. The distribution of Cumulative Trauma Disorders by the part of the body was turned out as Back ; 43.6%, knee ; 23.9%, shoulder ; 19.3%. The officers had more frequency of symptom around the neck than the ratings(p<0.01) and the seamen who work at the engine room were more frequent in elbow than those at deck department(p<0.05). In the duration of pain, 55.4% were less than one week, 20.6% less than 30 days, and 24.0% 30 days and more. the cause of symptom was turned out as 34.5% by excessive hard work and 30.1 % was unknown.

The distribution of musculoskeletal symptoms by the part of the body was turned out as Back ; 43.6%, knee : 23.9%, shoulder : 19.3%. The officers had more frequency of symptom around the neck than the ratings(p<0.01) and the seamen who work at the engine room were more frequent in elbow than those at deck department(p<0.05). In the duration of pain, 55.4% were less than one week, 20.6% less than 30 days, and 24.0% 30 days and more. the cause of symptom was turned out as 34.5% by excessive hard work and 30.1% was unknown. for the treatment of the musculoskeletal symptoms, 40.3% with symptomes did not have any medical treatment, 27.6% was self-treated and 22.7% was treated at hospital. This study shows that musculoskeletal disorders are seamen's important health problem and they can not properly take medical service due to the out of home for a long period as characteristics of occupation.
