Trends in Nursing Research on Community Nurse Practitioners as Reported in Korea from 1980 to 2006 (보건진료원 관련 국내 연구논문 분석)

  • Purpose: The purpose of this study was to analyze the trends of research on Community Nurse Practitioners in Korea in order to suggest future directions for research on Community Nurse Practitioners. Method: A total of 109 studies published from 1980 to 2006 were analyzed according to the year of publication, research design, journal type, subject type, and major study concepts. Results: 1) The number of studies related to Community Nurse Practitioners increased by 28.4% from 1985 to 1989, and also by 28.4% from 2000 to 2004. 2) The most frequently used research design in the studies was a descriptive study design (90.8%). 3) There were 43 master's theses on Community Nurse Practitioners, and 16 studies in the Journal of Korean Community Nursing. 4) The most frequent participants were Community Nurse Practitioners (68.9%). 5) The most frequent major study concept was job analysis of Community Nurse Practitioner programs (27.5%). Conclusion: It is suggested that there should be more studies on Community Nurse Practitioner programs in the future and, particularly, experimental studies to exam the effects of interventions on health promotion by Community Nurse Practitioners are considered necessary.

The Role-expectations of Gerontological Nurse Practitioners and Performance of Gerontological Nursing by Nurses in Long Term Care Hospitals and General Hospitals (요양병원과 종합병원 간호사의 노인전문간호사에 대한 역할기대와 노인전문간호 수행 정도 비교)

  • Purpose: The purpose of this study was to compare the role-expectations of gerontological nurse practitioners and performance of gerontological nursing by nurses in long term care hospitals and general hospitals. Methods: Subjects were 200 nurses; 100 nurses from long term care hospitals where as 100 nurses from general hospitals. The subjects completed a questionnaire on general characteristics, role-expectations of gerontological nurse practitioners, and performance of gerontological nursing by nurses. Data were collected from February to March 2013 and analyzed using SPSS/WIN 18.0 version program in order to perform descriptive statistics, independent t-test, and one-way ANOVA. Results: Results indicated that there were significant differences in the role-expectations of gerontological nurse practitioners and performance of gerontological nursing between nurses from long term care hospitals and those from general hospitals. Conclusion: Nurses in general hospitals showed significantly higher role expectations than nurses in long term care hospitals. Therefore it is necessary to spread the knowledge on the roles of gerontological nurse practitioners and the nurse practitioners system to nurses in long term care hospitals.

Effect of Educational Nurse Practitioners at Comprehensive Nursing Care Service Units (간호·간병 통합서비스 병동 교육전담간호사 운영 효과)

  • Purpose: This study aimed to investigate the effect of educational nurse practitioners at comprehensive nursing care service units by comparing the educational satisfaction of general nurses and the occupational stress of preceptor nurses before and after education by educational nurse practitioners. Methods: Participants were 71 general nurses and 71 preceptor nurses working at comprehensive nursing care service units of G University hospital in I City. The level of educational satisfaction of general nurses and occupational stress of preceptor nurses were measured and compared. The differences in educational satisfaction and occupational stress before and after the education program were verified by paired t-test and one-way ANOVA using SPSS/WIN 27.0 statistical program. Differences before and after the education program according to the general characteristics were analyzed with repeated measure analysis of variance after Sapiro-Wilks normality test, and post-hoc analysis was conducted using Scheffétest. Results: It was inferred that there was a significant difference (t=-9.32, p<.001) in the educational satisfaction of general nurses, with an average of 3.72±0.77 before the education and 4.47±0.60 after the education. However, there was no significant difference in occupational stress in the preceptor nurses. Conclusion: It could be concluded that introduction of educational nurse practitioners at comprehensive nursing care service units has a positive effect on the educational satisfaction of general nurses. This result will serve as basic data to implement educational nurse practitioners for general nurses in all medical institutions as well as comprehensive nursing care service units.

Job Descriptions of Hospital Based Home Care Nurse Practitioners in Korea by DACUM Technique (의료기관 가정전문간호사의 직무분석)

  • Purpose: The aim of this study was to develop and to analyze the task of hospital based home care nurse practitioners in Korea. Method: The definition of home care nurse practitioners and job description was developed based on developing a curriculum(DACUM) by 7 panels who have experienced in home care nursing. One hundred fifty four nurses who were working at hospital based on home care were participated. Result: Fourteen kinds of duties were identified : the selection of home care patients; basic home care nursing; advanced home care nursing; patient/family education and counseling; medical decision making and coordination of patient service; management of home care supplies and drugs for patients; management of medical records; management of home care the agency; management of home care personnel; management of the home care supplies for agency; home care public relations; improvement of home care quality; management of long-term care service; and self-improvement. Ninety-six tasks were classified. Conclusion: The abilities for quality improvement and the advanced nursing practice of home care nurses should be empowered.

Role Conflict Experience of Geriatric Nurse Practitioners (노인전문간호사의 역할갈등 경험)

  • Purpose: This study investigated experience of role conflict of geriatric nurse practitioners by qualitative study. Methods: This study is based on the qualitative research using phenomenological methodology. The research participants were 5 geriatric nurse practitioners. The data collection methodologies were in -depth interviews. The interviews were conducted 2 to 4 times where each interview lasted within the range of 30 minutes to 2 hours. The data collection and analysis were carried out simultaneously and the Colaizzi (1978) methodology was adopted for the data analysis. Results: As result of research, from 75 significant statements, 12 meaningful statements, 6 themes, and 3 clusters of themes were extracted. The 3 clusters of themes are 'Intrapersonal role conflict', 'Relationship role conflict', and 'Institutional role conflict'. Conclusion: The finding of this study suggest the legal status guarantee have to be prepared in order to resolve the role conflict of geriatric nurse practitioners.

Relationship among Emotional Labor, Burnout, Social Support and Turnover Intention of Nurse Practitioners in terms of Interdisciplinary Perspectives (융복합 관점에서 임상간호사들의 감정노동, 직무소진, 사회적 지지, 이직의도의 관계)

  • The great attention has been shown the importance of human resource management in medical institutions. At this point, studies have stressed employees' psychological factors for maximizing their job performance and minimizing their turnover intention and burnout. This study, therefore, empirically examines the mediating effect of social support on the relationship between emotional labor, burnout, and turnover intention of medical institutions, focusing on nurse practitioners to contribute to hospital management for developing valuable strategies in the human resource sector of medical employees. The empirical results demonstrate: 1) the relationship between emotional labor and burnout of nurse practitioners is mediated by social support; and 2) the relationship between emotional labor and turnover intention of them is mediated by social support. This research results show that more understanding of nurse practitioners' psychological factors in response to decrease their burnout and turnover intention for developing human resource management of nurse organizations. Furthermore, the study could suggest efficient management ways of hospital through interdisciplinary human resource management in terms of business administration and psychology.

Developing the Joint Employment System of Occupational Nurse Practitioners for the Minor Enterprises in Korea (part 1) (보건관리자 공동채용 모형 개발 (I) -산업간호사 의견을 중심으로-)

  • The occupational health services for the minor enterprises are poor comparing with that of large enterprises in Korea. In 1997, Korean government announced officially the law on joint employment system of occupational health practitioners to solve the problems. This study is to develop the model on the joint employment system of occupational nurse practitioners for minor enterprises in Korea based on the opinions of occupational health nurses (OH Ns). The data were collected by questionnares from October to November in 1997. The number of subjects was 210 occupational health nurses who had participated the mandatory nursing education program provided by the Korean Occupational Health Nurses Association(KOHNA). The response rate was 47.1%. The contents of questionnaires were general characteristics, occupational & job characteristics of OHNs, and the views on the joint employment system. The SAS-PC program was used for the statistical analysis. The results were as followed, (1) The proper number of industies was 3 and the appropriate number of workers was 448 for one joint employed occupational nurse practitioner (J EONP). (2) 44.7% of the OHNs wanted that the Ministry of Labor monitored the way and content of contraction b/w JEONP and minor emterprises, 82.6% of the OHNs wanted that KOHNA managed the employment informations. (3) The OHNs of occupational health agencies showed more positive attitude than the OHNs of private industries on the joint employment system. (4) 88.3% of the OHNs wanted the education training for JEONP. Especially in occupational hygiene and safety control. (5) The OHNs expected the role expansion of Occupational Health Nursing by JEONP system. At the same time they worried the adverse effects. For the successful execution of this system, it is necessary the cooperation among the Ministry of Labor, KOHNA, the minor enterprises, and the OHNs.

The Effects of Personal Coping Resources and Perceived Organizational Supports on Job Stress among Hospital-Based Home Care Nurse Practitioners (HCNPs) (의료기관 가정전문간호사의 개인대처자원과 조직지원인식이 직무스트레스에 미치는 영향)

  • Purpose: The aims of this study were to describe the levels of personal coping resources, perceived organizational supports, and job stress, and to examine factors that affect job stress in hospital-based home care nurse practitioners. Methods: A cross-sectional survey was conducted. Data were collected from 170 subjects with a structured questionnaire from April to July, 2016 and analyzed using descriptive statistics, t-tests, One-way ANOVA, Pearson's correlation, and multiple regressions. Results: The means for personal coping resources, perceived organizational supports, and job stress were $3.7{\pm}0.43$, $3.4{\pm}0.55$, and $3.4{\pm}0.55$ out of 5, respectively. Personal coping resources and perceived organizational supports were significantly correlated. However, both had no associations with job stress. In the multiple regression analysis, the experiences of home care nursing and hospital location were found to be predictors of job stress. Both personal coping resources and perceived organizational supports were not statistically significant predictors of job stress. Conclusions: It is necessary to carry out organization-based educational programs and support systems aimed at enhancing personal abilities to cope with stress at work. Additionally, further studies are needed to identify other hospital-related characteristics that can lead to job stress in home care nurse practitioners.

Curriculum Analysis of Gerontological Nurse Practitioner Programs (노인전문간호사 교과과정 분석)

  • Purpose: The purposes of this article were to analyze curricula of gerontological nurse practitioner(GNP) programs in the United States, to compare the curricula with Korean GNP programs, and to provide suggestions for better GNP programs in Korea. Methods: Top GNP programs in the United States were identified from the US NEWS and 12 universities were included in the analyses. Class distribution, credit hours, and clinical hours were analyzed and suggestions for Korean GNP programs were made. Results: Average credit hours for class lectures in the US GNP programs were 47 and emphases on physiology, pharmacology, physical assessment, and disease management were identified. Most US GNP programs(75%) provided health concerns for both middle aged and older adults. Not all US GNP programs included 'theory' or 'the introduction to GNP' classes, while these are required classes in the Korean GNP program. The mean clinical hours in the US GNP Programs were 537 which are much higher than those in the Korean GNP program. Conclusion: Based on the analyses, we can conclude that Korean GNP programs are lacking in many ways. Further evaluation and curricula modifications are required to settle down the program better and to have the graduates prepared more as internationally competent nurse practitioners.
