Analysis of emergy comsumption for Hybrid Gait Orthosis (하이브리드 보행보조장치의 에너지 소모도 비교분석)

  • It is a challenging task to make the paraplegic to walk with out the assist of the caregiver. So, we have developed various type of gait orthosis for paraplegic during the five years lately. The purpose of this study ultimately is energy consumption test of serveral type gait orthosis for developing the high efficiency gait orthosis. From the experimental results, the oxygen consumption rate were 6.9$\pm$3.3ml/kg in RGO gait, 5.3$\pm$1.3ml/kg in PGO, and 6.2$\pm$3ml/kg in HGO gait. The maximum hip flexion angle were 16$^{\circ}$ in RGO , 15$^{\circ}$ in PGO, and 47.5$^{\circ}$. in HGO. As a result It was found that. Hybrid Gait Orthosis need high energy consumption more than PGO for walking, but it is small weight and strengthened muscle.

The Effect of Ankle-Foot Orthosis and Trunk Orthosis on Movement patterns used in a Supine to Stand Rising task (누운자세에서 똑바로 일어서기 운동형태에서 족관절보조기와 체간보조기의 영향)

  • The purpose of this study was to examine the effect of ankle-foot orthosis and lumbosacral orthosis on movement patterns used to rise from the supine position to erect stance. Thirty-two healthy adults participated. Subjects were videotaped while rising from a supine position on a floor mat. Each subject performed 10 trials each of three condition;general condition, right ankle-foot orthosis, lumbosacral orthosis. subjects rose most commonly using a symmetrical push pattern of the upper extremities, a symmetrical squat pattern in the lower extremities, a symmetrical in the trunk under each of three conditions. Changes in the incidence of movement patterns occurred in lower extremities of the ankle-foot orthosis and lumbosacral orthosis condition and trunk of the ankle-foot orthosis condition. From a dynamic pattern theory perspective, ankle motion is a control variable for the supine position to erect standing movement.

  • Objectives : The objective of this study is to report the effect of foot orthosis on by observing 3 clinical case studies. Methods and Results : We investigated for the changes in Cobb's angle and pelvic height by using the standing full X-ray. After application of foot orthosis, Cobb's angle and pelvic height was reduced in cases. We treated 2 cases with only Foot Orthosis, 1 case with Foot Orthosis and Korean Medicine therapy. Conclusions : This study showed that foot orthosis can be effective way on nonstructural scoliosis and statistics study should be followed.

Treatment of Snoring and Obstructive Sleep Apnea with Dental Orthosis (구강내 장치를 이용한 코골이 및 폐쇄성 수면무호흡증의 치료효과)

  • The purpose of this study was to examine the anatomic changes in the upper airway with a dental orthosis. the effectiveness and side effects of orthosis in the treatment of snoring and obstructive sleep apnea. To meet this puppose a dental orthosis, designed to increase the size of the upper airway by advancing the mandible, was used in 42 patients (30 M, 12 F), aged 29 - 69 years, to treat snoring and varying decrees of obstructive sleep apnea. Cephalometric study of anatomic featured was made with and without a dental orthosis, and the evaluation of the effectiveness and side effects of orthosis was done by questionnaires. The obtained results were as follows : 1. All subjects were habitual snorers and 32 patients comp1ained the loudness of snoring as severe as be heard outside of the patient's room. 2. According to the degree of respiratory distirbance index(RDI) and aprea index(Al) from the polysomnograph in 34 patient, mild obstructive sleep apnea patients were 5, moderate 6 and severe 16. 3. Various anatomic changes in the upper airway with denta1 orthosis were as follows : (1) More superioly positioned hyoid bone ( p<0.001) (2) Enlarged oropharyngeal (superior p<0.01, middle p<0.01. inferior p<0.01) and hypopharyngeal (P<0.05) airway space. 4. According to the results of the changes of clinical syptoms after the usage of the dental orthosis acquired from questionnaires, there was significant improvement in the frequently, the loudness and the severity of snoring, cessation of breathing and awakening from the difficulty of breathing during sleep. 5. The effectiveness and side effects of dental orthosis by questionnaires were as follows ; (1) Dental orthosis satisfied almost all the patients (68±20%). (2) Snoring was improved in all the patients (73±19%). (3) Obstructive sleep aphea was improved in all the patients (61 ± 37%) (4) Sleepiness in the daytime was significantly improved (61 ±37%). (5) The sleep quality was significantly improved (61±37%). (6) The discomfort of the dental orthosis was minor (33±18%) and no serious complications were observed. 6. The dental orthosis is an effective treatment for the symptom of snoring, and it can also effectively treat varying degrees of obstructive sleep apnea.

Evaluation of Plantarflexion Torque of the Ankle-Foot Orthosis Using the Artificial Pneumatic Muscle (인공공압근육 엑츄에이터를 이용한 족관절 보조기의 족저굴곡 토크 평가)

  • Ankle-foot orthosis with an artificial pneumatic muscle which is intended for the assistance of plantarfelxion torque was developed. In this study, power pattern of the device in the various pneumatics and the effectiveness of the system were investigated. The pneumatic power was provided by ankle-foot orthosis controlled by user‘s physiological signal, that is, muscular stiffness in soleus muscle. This pneumatic power can assist plantarflexion torque of ankle joint. The subjects performed maximal voluntary isokinetic plantarflexion motion on a biodexdynamometer in different pneumatics, and they completed three conditions: 1) without wearing the orthosis, 2) wearing the orthosis with artificial muscles turned off, 3) wearing the orthosis activated under muscular stiffness control. Through these experiments, we confirmed the effectiveness of the orthosis and muscular stiffness control using the analyzing isokinetic plantarflexion torque. The experimental results showed that isokinetic torques of plantarflexion motion of the ankle joints gradually increased in incremental pneumatic. The effectiveness of the orthosis was -7.26% and the effectiveness of the muscular stiffness control was 17.83% in normalized isokinetic plantarflexion torque. Subjects generated the less isokinetic torques of the ankle joints in wearing the orthosis with artificial muscles turned off, but isokinetic torques were appropriately reinforced in condition of wearing the orthosis activated under muscular stiffness control(17.83%) compared to wearing the orthosis(-7.26%). Therefore, we respect that developed powered orthosis is applied in the elderly that has weak muscular power as the rehabilitation equipment.

The Effect of Low Back Pain and Pelvic Displacement on Foot Orthosis (발 보조기 착용이 요통환자의 골반 변위와 통증 변화에 미치는 영향)

  • The purpose of this study is to investigate the effect of low back pain(LBP) and pelvic displacement on foot orthosis. Before and after experiments were designed to compare the effect. 21 LBP patients who were the subjects diagnosed of applying foot orthosis for 3 weeks after, investigated about pelvic obliquity angle, displacement of ilium, lumbo-sacral angle by x-ray test which is one of pelvic displacement tests, visual analogue scale is used for LBP measurement. The result show the followings; First, Pelvic obliquity angle was significantly reduced after applying foot orthosis compared before using it(p<.05). Second, Displacement of ilium was significantly reduced after applying foot orthosis compared before using it(p<.05). Third, Lumbo-sacral angle was significantly reduced after applying foot orthosis compared before using it(p<.05). Fourth, LBP was significantly reduced after applying foot orthosis compared before using it(p<.05). This study tries to suggest new LBP treatment to reduce pelvic displacement by apply foot orthosis. In conclusion, foot orthosis reduces pelvic obliquity angle, displacement of ilium, lumbo-sacral angle and also decrease LBP. Further more, It needs of biomechanical study which can recognize relation between foot arch and pelvic displacement. This study will serve as a clinically useful data for diagnosis and treatment of LBP and biomechanical analysis of lower limb.

Pedobarographic Analysis in Functional Foot Orthosis (기능성 족부 보조기 착용에 따른 족저압 변화)

  • Purpose: The purpose of this study was to evaluate pressure distribution on the foot with the use of custom made foot orthosis and evaluate influential factors in young people using custom orthosis. Materials and Methods: 22 individuals comprised of young males and females were evaluated by radiograph, pedobarograph, and satisfaction rate and VAS score. The data was analyzed statistically to find influential factors for satisfaction after wearing the foot orthosis. Results: Around 50% of participants were satisfied in wearing the custom made foot orthosis. Initial VAS score of satisfaction of $36.2{\pm}19.7$ improved to $73.1{\pm}15.6$ after application of foot orthosis. There was a statistically significant difference. Talo-second metatarsal angle on AP radiograph after orthosis application was significantly related to satisfaction. On analysis of pedobarograph data, total contact area was increased and weight distribution was transferred medially on ambulation with the orthosis applied. Conclusion: Before designing the foot orthosis, individual foot factors such as foot anatomy and foot pressure distribution should be evaluated for foot comfort and better patient satisfaction.

  • Walking training is one of the most important rehabilitation processes with paralysis patient. Walking training by using an orthosis can help advancing a patient's independent level. However, existing orthoses have some serious demerit of mechanical problem that the knee joint is locked in the state where it is completely extended, which increases energy consumption and fatigue. For this reason, it is suggested, for more practical orthosis, that the knee joint should be placed and it should have capability of suspending patient's weight. In this paper, 1-DOF walking orthosis which compensates the demerit of the existing orthosis and secures patient's mobility has been proposed. New orthosis has been designed under the following two premises. First, the knee joint of the orthosis was designed fold in order for the orthosis to move in a walking pattern similar to that of a normal person. Second, the knee joint was designed to extend during the swing phase and lock safely during the stance phase.

Effects of elastic band orthosis (aider) on balance and gait in chronic stroke patients

  • Objective: The objective of this study was to investigate the feasibility and effects of balance training using a newly developed elastic band orthosis (aider) for improvement of mobility and balance in chronic stroke patients. Design: Cross-sectional study. Methods: Ten patients with chronic hemiplegia participated in this study. There were six males and four females; two patients had right hemiplegia and eight had left hemiplegia. This study investigated the effect of the elastic band orthosis on balance and gait ability compared with bare foot condition. Gait parameters were measured using the opto-gait system for analysis of the spatial and temporal parameters of walking in stroke patients. In addition, balance ability in stroke patients was evaluated using the Timed Up and Go (TUG) and Berg Balance Scale (BBS). Results: This study investigated the effect of the elastic band orthosis on balance compared with bare foot condition. The TUG and BBS showed significant improvement with use of the elastic band orthosis (p<0.05). Use of the Elastic band orthosis resulted in significantly improved velocity, cadence, less-affected step length, less-affected stride length, and less-affected single limb support in stroke patients (p<0.05). Conclusions: We demonstrated a significant improvement in dynamic balance and gait ability in chronic stroke patients using the elastic band orthosis. This orthosis may aid in prevention of spastic foot drop, leading to improvement of walking ability.

Study of a New Reciprocating Gait Orthosis for a Spinal Cord Injury Patient (척수마비환자 재활훈련용 왕복보행보조기에 관한 연구)

  • This paper presents a design and a control of a New Reciprocating Gait Orthosis and dynamic walking simulation for this system. The New Reciprocating Gait Orthosis is distinguished from other one by which has a very light-weight and a new RGO type with servo motors. The gait of a New Reciprocating Gait Orthosis depends on the constrains of mechanical kinematics and initial posture. The stability of dynamic walking is investigated by ZMP(Zero Moment Point) of the New Reciprocating Gait Orthosis. It is designed according to a human wear type and is able to accomodate itself to human environments. The joints of each leg are adopted with a good kinematic characteristics. To test of the analysis of joint kinematic properties, we did the strain stress analysis of dynamic PLS and the study of FEM with a dynamic PLS. It will be expect that the spinal card injury patients are able to train effectively with a Reciprocating Gait Orthosis. The New Reciprocating Gait Orthosis was able to keep smooth walling by the orthotic servo motors and hybrid system, make a sequence of flexion and extension of the joint during the walking. Also, the New Reciprocating Gait Orthosis turned out to be a satisfactory orthosis for walling training, for the spinal cord injury patient.
