우리나라 50세 이상 성인의 골감소증과 골다공증 영향요인 (Influencing Factors on Osteopenia and Osteoporosis in Korean Aged 50 Years and Above)

  • Purpose: The purpose of this study was to identify the influencing factors on osteopenia and osteoporosis in Korean aged 50 years and above. Methods: Data from the Fifth Korean National Health and Nutrition Examination Survey (KNHANES V-1, 2010) were obtained and 2,773 subjects who got bone density test and were classified "normal, osteopenia or osteoporosis" were selected. Data were analyzed $x^2$ test, ANOVA, $Scheff{\acute{e}}$ test and logistic regression using the SAS 9.4. Results: Gender, age, days of flexibility exercise, vitamin D in blood and weight were found to be influencing factors for osteoporosis in Korean aged 50 years and above. Gender. age and smoking were found to be influencing factors for osteopenia in Korean aged 50 years and above. Conclusion: This study suggested that nursing intervention should include exercise promotion, intake vitamin D, smoking cessation and weight control program in order to reduce the prevalence of osteoporosis and osteopenia.

골다공증에 대한 인지측정도구 개발 (Development of an Osteoporosis Awareness Scale for Women)

  • Purpose: This study was conducted to develop an instrument measuring awareness of osteoporosis. Methods: The process included construction of a conceptual framework, generation of 86 preliminary items, verification of content validity, selection of secondary items, verification of construct validity and extraction of final items. The 86 items were reviewed for content validity by two groups of experts and were tested to evaluate inter item correlation coefficient by two groups of adult women. From June to August 2007, data were collected from 383 adult women who lived in Seoul and provinces in Korea. Data were analyzed by performing item analysis, Varimax factor analysis and Cronbach's alpha. Results: There were 31 items in the final instrument categorized into 5 factors. The factors were labeled as "preventive behaviors (10 items)", "risk factors (5 items)", "characteristics of osteoporosis (6 items)", "improving bone health (5 items)", and "bone physiology (5 items)". Cumulative percent of variance was 60.92% and eigen values ranged from 1.20 to 12.44. Cronbach's alpha was .948 and ranged from .804 to .917. Conclusion: Validity and reliability of the scale are confirmed in this study showing its utility for measuring awareness of osteoporosis for women. Utilization of the scale will also contribute to designing an appropriate prevention program for osteoporosis.

  • Purpose: This study aimed to identify the influence of knowledge and subjective health status on health promoting behavior about osteoporosis in industrial workers. Methods: The subjects were 292 industrial workers. Data were collected with structured questionnaires in July 10 to August 20, 2012. The collected data were analyzed with t-test, ANOVA, Pearson's correlation coefficients and hierarchical regression analysis by using the SPSS Win 12.0 statistics. Results: The major findings of this study were as follows; 1) The average scores of knowledge about osteoporosis, subjective health status and health promoting behavior about osteoporosis were 9.26 out of 20, 9.64 out of 14 and 39.77 out of 68. 2) There were significantly positive correlations relationship among knowledge, subjective health status and health promoting behavior. 3) The factors influencing health promoting behavior were age(${\beta}$=.069, p=.032), knowledge(${\beta}$=.026, p=.005), subjective health status(${\beta}$=.058, p<.001). Conclusion: This study suggested that we should develop preventive osteoporosis programs for industrial workers considering these results.

  • Lack of adequate sleep has become increasingly common in our 24/7 modern society. Reduced sleep has significant health consequences including metabolic and cardiovascular disorders, and mental problems including depression. In addition, although the increase in life expectancy has provided a dream of longevity to humans, the occurrence of osteoporosis is a big obstacle to this dream for both male and female. It is known that insomnia and bone health problems, which are very critical conditions in human life, interestingly, share a lot of pathogenesis in recent decades. Nevertheless, due to another side effects of the synthetic drugs being taken for the treatment of insomnia and osteoporosis, patients have substantial anxiety for the safety of drugs with therapeutic expectation. This review examines the pathogenesis shared by sleep and osteoporosis together and herbal medicine, which has recently been shown to be safe and efficacious in the treatment of both diseases other than synthetic drugs. We suggestions for how to treat osteoporosis. These efforts will be the first step toward enabling patients to have comfortable and safe prescriptions through a wide selection of therapeutic agents in the future.

골다공증의 한약 치료 효과: 국내 및 국외 동물 실험 및 임상 연구 논문에 대한 문헌 고찰 (Effect of Herbal Medicine on Osteoporosis: A Review of Animal Experiment, Clinical Studies Published in Korea and Overseas)

  • Objectives This study was designed to review the effect of herbal medicine on animal model and patients with osteoporosis. Methods The databases in Korea and overseas were searched with term as osteoporosis, Chinese medicine, Korean medicine, herbal medicine. Among the results, we reviewed studies published in 2011 to 2020. 420 articles were primarily selected and after processing through flowchart, finally 72 studies were selected. Results Above 72 studies, 71 studies showed that the herbal medicine was effective on treating osteoporosis, using macroscopical, osteological, histological, hematological analysis and urinalysis. Conclusions Using herbal medicine for treating osteoporosis is effective in various pathways. But further study should be conducted to clarify its treatment mechanisms and effectiveness.

  • Purpose: The purpose of this study was to examine knowledge, health beliefs and health promoting behavior about osteoporosis in working women in their 20s and 30s. Methods: A survey was conducted among working women in their 20s and 30s who visited the health promotion center of B Medical Center in B Metropolitan City. The data were collected from December 2021 to March 2022 using structured self-reported questionnaires. SPSS/WIN 25.0 program was used to analyze the data. Results: Factors influencing health promoting behavior about osteoporosis in women in their 20s and 30s were in the order of "good" subjective health status (β=.47, p<.001), "moderate" subjective health status (β=.36, p<.001) and knowledge (β=.18, p=.015). These factors explained 12.4% of health promoting behaviors about osteoporosis. Conclusion: The results indicate the need to develop and implement healthcare programs that can improve the health status and provide knowledge to improve health promoting behavior about osteoporosis in women in their 20s and 30s.

건강신념 및 효능기대증진 프로그램이 류마티스 관절염환자의 골다공증 예방행위에 미치는 영향 (The Effect of the Health Belief and Efficacy Expectation Promoting Program on Osteoporosis Preventive Health Behavior in Women with Rheumatoid Arthritis)

  • Osteoporosis has been known as a common complication of rheumatoid arthritis and a major preventable health problem. Lots of studios have demonstrated that changes in life style can help delay or prevent osteoporosis. Therefore nursing intervention related osteoporosis prevention have consisted of education programs aimed at changing dietary and exercise habit. However knowledge gained from education haven't always leaded to behavior change. Therefore it is important to consider other psychological variables in effecting behavior change. Numerous research have found self efficacy and health belief to be an important factor in individual decision making behavior. The purpose of the study was to develop health belief and efficacy expectation promoting program based on Health Belief Model & Self Efficacy Model and to investigate its effects in women with rheumatoid arthritis. For this purpose, one group pretest-post design was used. The subject of the study were 16 women with rheumatoid arthritis in Pusan city and data collection was carried out from April, 1997 to May, 1998. The intervention program was consisted of educating on osteoporosis and enhancing and reinforcing self efficacy by verbal persuasion during the period of 4 weeks. The instruments were used to collect data in this study were Osteoporosis Health Belief Scale, Osteoporosis Self Efficacy Scale, and Osteoporosis Preventive Behavior Scale. Data was analyzed by Wilcoxon signed rank test using SPSS $PC^+$ program. The results are as follows : 1) The behavior should be increased after intervention was supported(Z=-3.5162, p=.0004, diet : Z=-3.2942, p=.0010, exercise). 2) The sub-hypothesis that perceived sensitivity should be increased after intervention was supported (Z=-2.3854, p=.0171). 3) The sub-hypothesis that perceived severity should be increased after intervention was rejected(Z=-1.4327, p=.1520). 4) The sub-hypothesis that perceived benefit should be increased after intervention was supported(Z=-2.6410, p=.0083). 5) The sub-hypothesis that perceived barrier should be decreased after intervention was supported (Z=-2.4138, p=.0158). 6) The sub-hypothesis that efficacy expectation should be increased after intervention was supported(Z=-3.5162, p=.0004). As a conclusion, it was found that health belief and self efficacy promoting program was an effective nursing intervention for preventing osteoporosis of rheumatoid arthritis.

지역사회 주민을 대상으로 한 천식과 골다공증과의 연관성 (The Association of Asthma and Osteoporosis in the Residents of the Local Community)

  • 본 연구는 천식과 골다공증유병유무와의 관련성을 알아봄으로써 골다공증의 예방 및 관리의 기초 자료로 제공하기 위해 시도되었다. 연구의 기초 자료는 ‘2008년 지역사회건강조사’ 를 이용하였다. 조사 시기는 2008년 9월부터 2008년 11월 까지였고, 충남의 7개 군 및 대전시 서구에 거주하는 19세 이상 주민 6,348명을 대상으로 하였다. 이 중 천식이 있다고 응답한 203명 및 골다공증이 있다고 대답한 465명의 특성을 질병이 없는 군의 특성과 비교하였다. 천식 유병유무와 골다공증 유병유무와는 유의한 상관관계가 있었고, 천식대상자 중에서 천식치료여부와 골다공증 유병유무와도 유의한 상관관계가 있었다. 추가로 좀 더 광범위한 연구 분석과 추적조사가 필요하리라 사료된다.

Serum 25-hydroxy Vitamin D Status is Not Related to Osteopenia/Osteoporosis Risk in Colorectal Cancer Survivors

  • Background: The incidence of colorectal cancer increases with vitamin D deficiency as shown in recently published studies. In addition, prospective investigations have indicated that low vitamin D levels may be associated with increased mortality of colorectal cancer, especially in stage III and IV cases. However, the exact incidence of vitamin D deficiency and the relation between vitamin D deficiency and osteopenia/osteporosis is still not known. The aim of this study is to identify severity of vitamin D deficiency and absolute risk factors of osteopenia/osteoporosis in colorectal cancer survivors. Materials and Methods: A total of 113 colorectal cancer survivors treated with surgery and/or chemotherapy ${\pm}$ radiotherapy were recruited from medical oncology outpatient clinics during routine follow-up visits in 2012-2013. Bone mineral densitometry (BMD) was performed, and serum 25-OH vitamin D levels were also checked on the same day of the questionnaire. The patients was divided into 2 groups, group A with normal BMD and group B with osteopenia/osteoporosis. Results: The median age of the study population was 58 (40-76). Thirty (30.0%) were female, whereas 79 (70.0%) were male. The median follow-up was 48 months (14-120 months). Vitamin D deficiency was found in 109 (96.5%); mild deficiency (20-30 ng/ml) in 19 (16.8%), moderate deficiency (10-20 ng/ml) in 54 (47.8%) and severe deficiency (<10 ng/ml) in 36 (31.9%). Osteopenia was evident in 58 (51.4%) patients whereas osteoporosis was noted in 17 (15.0%). Normal BMD was observed in 38 (33.6%). No apparent effects of type of surgery, presence of stoma, chemotherapy, radiotherapy and TNM stage were found regarding the risk of osteopenia and osteoporosis. Also, the severity of the vitamin D deficiency had no effect in the risk of osteopenia and osteporosis (p=0.93). In female patients, osteopenia/osteoporosis were observed in 79.5% patients as compared to 60.7% of male patients (p=0.04). Conclusions: In our study, vitamin D deficiency and osteopenia/osteoporosis was observed in 96.5% and 66.4% of colorectal cancer survivors, respectively. There is no defined absolute risk factor of osteopenia and osteoporosis in colorectal cancer survivors. To our knowledge, in the literature, our study is the first to evaluateall the risk factors of osteopenia and osteoporosis in colorectal cancer survivors.

AMD3100 improves ovariectomy-induced osteoporosis in mice by facilitating mobilization of hematopoietic stem/progenitor cells

  • Inhibition of an increase of osteoclasts has become the most important treatment for osteoporosis. The CXCR4 antagonist, AMD3100, plays an important role in the mobilization of osteoclast precursors within bone marrow (BM). However, the actual therapeutic impact of AMD3100 in osteoporosis has not yet been ascertained. Here we demonstrate the therapeutic effect of AMD3100 in the treatment of ovariectomy-induced osteoporosis in mice. We found that treatment with AMD3100 resulted in direct induction of release of SDF-1 from BM to blood and mobilization of hematopoietic stem/progenitor cells (HSPCs) in an osteoporosis model. AMD3100 prevented bone density loss after ovariectomy by mobilization of HSPCs, suggesting a therapeutic strategy to reduce the number of osteoclasts on bone surfaces. These findings support the hypothesis that treatment with AMD3100 can result in efficient mobilization of HSPCs into blood through direct blockade of the SDF-1/CXCR4 interaction in BM and can be considered as a potential new therapeutic intervention for osteoporosis.
