Neo-ostium Formation in Anomalous Origin of the Left Coronary Artery

  • Anomalous origin of a coronary aortic artery is a rare cardiac anomaly. Although it can cause angina, syncope, and palpitations, most patients are asymptomatic. This anomaly requires surgical treatment or intervention because it is associated with sudden death. Several surgical techniques, such as coronary reimplantation, coronary artery bypass grafting (CABG), unroofing, and neo-ostium formation, have been proposed as treatments. We report a case surgically treated with neo-ostium formation in anomalous origin of the left coronary artery from the right coronary sinus.

  • Atrial septal defect is one of the most frequently encountered congenital heart disease. Up to December 31, 1976, 1682 cardiac patients received cardiac catheterization in the cardiac department of Yonsei university medical college. Out of the 1682 cardiac patients 723 cases had congenital heart disease and only 116 cases had congetial atrial septal defect. This amounted to 16.04% of all those with congenital heart disease. 58 cases of congenital atrial septal defect operated in the chest surgery department were presented. Of these 58 cases of atrial septal defect, 27 cases were male and 31 cases were female. Their ages ranged from 5 years to 54 years. The systolic pressure of the main pulmonary artery of 40 out of the 58 cases of atrial septal defect was below 40% of that of the systemic blood pressure: in 6 cases, the range of the systolic pressure of the main pulmonary artery was 50-90mmHg; in 12 cases, the range of the systolic pressure of the main pulmonary artery was 40-50mmHg. Average age of these was 30. 1 years. This study tends to show that Korean patients with atrial septal defect even though younger have a slight higher systolic pressure of the main pulmonary artery than Western patients have. The pulmonary blood is 1.5-2.5 times of systemic blood flow in 52 cases out of 58 cases of atrial septal defect.In only one of the 58 cases of atrial septal defect, the Rp was found to be as high as 45% of Rs. All other cases were below this level.51 cases had ostium secundum defect, 4 out of these cases had ostium secundum defect combined with mitral incompetence and 6 out of them had double ostium secundum defect. The remaining 7 cases had ostium primum defect. Their atrial defects were repaired under direct vision utilizing extracorporeal circulation, by hemodilution technic combined with moderate hypothermia. 44 cases [2nd atrial septal defect] were repaired by direct sutures while 14 cases, including the 7 cases ostium primum defects needed patches [1 pericardium and 13 teflon patch]. In 4 cases there were single defects while showed two defects. However the associated septal defect was so small that it could be closed by direct sutures. The size of the defect ranged between 6.0cm2and 10.0cm2 in 19 cases[33.7%]: the smallest being 0. 5cm2 and the largest 24cm2. The surgical mortality was 2 cases [3.4%]. These one case with ostium primum defect, could not be resuscitated on operation table. The cause of death in this case was myocardial failure and MI. The other, a case of ostium primum defect had a second operation on the first operative day due to massive bleeding from LV vent-line insertion site.The patient died on 26th post-operative day due to sepsis.

Comparison of two techniques for transpharyngeal endoscopic auditory tube diverticulotomy in the horse

  • Auditory tube diverticula, also known as guttural pouches, are naturally occurring dilations of the auditory tube in horses that communicate with the nasopharynx through a small ostium. Infection and select other conditions can result in inflammation and narrowing of the nasopharyngeal ostium, which prevents drainage of fluid or egress of air and can lead to persistent infection or guttural pouch tympany. Auditory tube diverticulotomy allows continuous egress from the auditory tube diverticula and is a feature of disease treatment in horses, in which medical treatment alone is not successful. Transpharyngeal endoscopic auditory tube diverticulotomy was performed using a diode laser either at a single dorsal pharyngeal recess location or bilaterally caudal to the nasopharyngeal ostium in 10 horse head specimens. Both methods resulted in clear communication between the nasopharynx and auditory tube diverticula. Diverticulotomy performed in the dorsal pharyngeal recess required less laser energy and activation time and had a shorter surgical duration than diverticulotomy performed caudal to the nasopharyngeal ostium. Further study related to the clinical application of both techniques is warranted.

Incomplete atrioventricular canal associated with tricuspid valve cleft and multiple ASD: report of one case (삼첨판균열과 다발성 심방중격결손을 합병한 Incomplete Atrioventricular Canal 의 치험)

  • Atrioventricular canal is divided into incomplete, intermediate and complete types. In ostium primum ASD [incomplete type] mitral valve cleft is almost always present, but ostium primum ASD with tricuspid valve cleft is a rare congenital anomaly. The patient was a 7 year old female whose complains were palpitation, exertional dyspnea and growth retardation. The chest films showed moderate cardiomegaly [C-T ratio, 61%]. EKG, Echocardiography, cardiac catheterization and left ventriculography were performed. Open heart surgery was done under the impression of incomplete atrioventricular canal. At the time of operation, ostium primum ASD [2x2.5 cm in diameter], secundum type ASD [lxl.5 cm in diameter] and cleft in the septal leaflet of the tricuspid valve were noted. But mitral valve was normal without cleft and VSD was not noted. Each anomalous portion was repaired. The patient made an uneventful recovery and we report this case, review and discuss the literatures.

Clinical analysis of ostium secundum atrial septal defect (이차공형 심방중격결손증의 임상적 고찰)

  • Thirty seven patients with ostium secundum atrial septal defect, operated from January, 1976 to September, 1984 at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital, were given clinical assessment. The following results were obtained. 1.Ostium secundum atrial septal defect was comprised of 18% of congenital heart disease. Their mean age was 15.7\ulcorner.42. Sex ratio [male:female] was 1:1.1. 2.Most frequent clinical symptom was dyspnea on exertion occurred in 26 patients [76.5%]. Only one patient had no symptom [2.9%]. 3.Pre-operative EKG findings revealed RVH in 61.8%, ICRBBB in 29.4%, and RAD in 41.2%. 4.Mean value of systolic pulmonary arterial pressure in patients over 20 years old was 37.8\ulcorner4.4mmHg and it was 28.1\ulcorner10.2mmHg in patients under 20 years old, but the difference between two groups was not statistically significant. 5. In large defect group [>3cm in diameter], Qp/Qs was significantly increased than small defect group [<3cm in diameter], but systolic pulmonary arterial pressure and Rp/Rs were not different between two groups. 6. Overall operative mortality was 5.4%.

Cor Triatriatum Sinistrum with an Ostium Primum Atrial Septal Defect in a Siamese Cat

  • An approximately 8-month-old, 2.61 kg, male Siamese kitten was referred with primary complaints of a 1-week history of respiratory distress, exercise intolerance and dyspnea. Diagnostic studies identified III/VI systolic murmur in the cardiac auscultation, right ventricular enlargement patterns in the electrocardiogram, pleural effusion and right-sided cardiomegaly in the thoracic radiography, and right marked ventricular dilatation, right atrial enlargement, atrial septal defect and abnormal left atrium divided by fibromuscular membrane. Based on these findings, the case was diagnosed as cor triatriatum sinistrum complicated with an ostium primum atrial septal defect. The cat was rescued with furosemide, nitroglycerine, oxygen supplement and fluid removal from pleural cavity.

  • Occlusion of a coronary artery ostium and especially occlusion of the right by an aortic cusp is a rare condition. We experienced an adult patient with occlusion of the right coronary ostium that was due to fusion of the right coronary cusp to the aortic wall along with underlying rheumatic aortic valve stenosis. During the operation, the adherent right coronary cusp was excised. After confirming that the right coronary ostium was patent, the other cusps were removed, and this followed by replacement of the aortic valve with a mechanical valve. The postoperative course was uneventful.

  • Purpose: The objective of this study was to evaluate the amount of height available for a maxillary sinus augmentation procedure without blocking the ostium and jeopardizing the drainage of the ostiomeatal complex using cone-beam computed tomography (CBCT) imaging. Materials and Methods: A total of 200 sinonasal complexes comprising 100 dentate and 100 edentulous scans were retrospectively assessed using CBCT. Invivo 5.0, a CBCT reconstruction program, was used for image evaluation. The coronal section demonstrating the ostiomeatal complex was selected as a reference view to perform measurements of the sinus. The measurements were done by 2 evaluators in separate sessions. Comparative analyses of measurements were performed between dentate and edentulous patients and between male and female patients. Results: The safe height to which the sinus can be elevated without compromising the integrity of the ostiomeatal complex was calculated for each sinus. In the presence of significant mucosal thickening, the height available for augmentation was calculated by subtracting the height of mucosal thickening from the sinus floor to the location of the ostium. In this study, the available height was approximately 27.05 mm for dentate and 23.40 mm for edentulous patients. The inter-operator reliability was excellent for all the parameters evaluated. Conclusion: This retrospective study with a limited number of patients from a single university-based site shows that CBCT is valuable in evaluating the location and patency of the ostium for planning sinus augmentation procedures for dental implant placement.

A Persistent Left Superior Vena Cava Draining into Left Atrium Associated with ASD, Absence of the Coronary Sinus Ostium and PDA Report of One Case (관상정맥동불형성, 좌상공정맥좌심방환류, 동맥관개존을 동반한 심방중격결손의 체험예)

  • A persistent left superior vena cava draining into the left atrium associated with atresia of the coronary sinus-ostium, ASD, and PDA is a rare congenital anomaly. The patient was a 4 year-old female whose complaints were frequent URI and exertional dyspnea. The congenital heart anomaly was suspected at 2 months of her age. Chest films showed cardiomegaly [C-T ratio, 75%]. EKG, Echocardiography, cardiac catheterization and angiocardiography were performed. Open heart surgery was done under impression of LV-RA shunt, bilateral superior vena cavae, and ASD. At the time of operation, huge LA and RA, inferior vena caval defect of a secundum type ASD [1.5 x 3cm in diameter], absence of innominate vein, atresia of the coronary sinus-ostium, and persistent LSVC draining into LA were noted. Direct suture closure of ASD and ligation of LSVC were done. The patient`s postoperative course was somewhat eventful: systolic murmur at apex remained. Four months after the operation, congestive heart failure attacked a few times. PDA that was overlooked at the time of open heart surgery was detected through postoperative cardiac catheterization in.4 months later. Emergent operation for closure of PDA was performed on the day of recatheterization. After that, patient`s heart failure was easily controlled without any notable problem.
