  • To establish the differential diagnosis and functional status in ovarian cystic cows, progesterone(P$_4$) and estrogen(E$_2$) level of cystic follicular fluid, ultrasonography for measuring the cystic diameter and thickness of cystic wall, and histological findings were investigated in cystic ovaries from slaughtered Korean native cows. Ovarian follicles were classified as systic if the diameter was greater than 25 mm by ultrasonography. Ovarian cysts < 3 mm of cystic wall thickness, < 10 ng/ml P$_4$ concentration and >10 ng/ml E$_2$ concentration were classified follicular cyst, ovarian cysts 3 mm of cystic wall thickness, 10 ng/ml P$_4$ concentration and <10 ng/ml E$_2$ concentration were classified luteal cyst, and ovarian cysts 3 mm of cystic wall thickness, < 10 ng/ml P$_4$ concentration and <10 ng/ml E$_2$ concentration were classified non-functional ovarian cyst, respectively. Also ovarian cysts were classified 8 types by anatomical and hisctological findings. Ovarian cysts with corpus luteum were 3 of 73 cows and ovarian cysts without corpus luteum were 70 cows. The incidence rates of 8 various types of ovarian cysts were as follows; 2Aa 56.2%, 2Ba 20.5% and 2Ab 15.1%, respectively. The incidence rates of ovarian cysts without corpus luteum were follicular cyst 76.7% and luteal cyst 19.2%. The thickness of cystic wall were lAb 3.9 mm, 2Ab 3.3 mm and 2Bb 3.2 mm, and the cystic fluid P$_4$ concentrations were above 10.0 ng/ml in lAb, 2Ab and 2Bb, respectively. There was significantly correlations between the thickness of cystic wall and cystic fluid P$_4$ concentration in ovarian cysts(p<0.05). The ovarian cyst was classified follicular cysts, luteal cyst and non-functional ovarian cyst by hormone analysis. The luteal cyst was accuratly dignosed by cystic wall thickness. But follicular cysts was misdiagnosed 13 cows of 56 cystic cows. The 13 cystic cows was determined as had non-fuctional ovarian cysts. The cystic fluid P$_4$ concentration was 3.3 ng/ml in follicular ovarian cysts and 30.1 ng/ml luteinized ovarian cysts. There was significantly positive correlations between thickness of cystic wall and serum P$_4$ concentration in follicular(r$^2$ =0.59, p<0.001) and luteal cysts(r$^2$=0.65, p<0.001). These results indicated that ovarian cysts had various stages of degeneration and luteal cyst was accuratly diagnosed measurement of cystic wall thickness by ultrasonography, but follicular cysts was not diagnosed only cystic diameter and cystic wall thickness. In conclusion, it is suggest that ovarian cysts was diagnosed by combination of clinical sign and anatomical cystic features.

  • Purpose: Ovarian cysts are fluid-filled sacs or pockets within or on the surface of an ovary. Most cysts present little or no discomfort and are noncancerous and harmless. So non-surgical treatments with herbal medicine or acupuncture and moxa therapy are important and suitable in this pathology. But there are few RCT articles about ovarian cyst in Korean Medicine(KM) clinical and research field. Methods: To review treatment method and other research tendency about ovarian cyst in journals related with KM and Traditional Chinese Medicine(TCM) through web-searching. Searching was done through web site #http://omis.ksucc.ac.kr# and pub-med(http://www.ncbi.nlm.nih.gov/sites/entrez) with key words #ovarian cyst#, #TCM#, #herb#, #CAM#, and #acupuncture#. Results: Seven articles were searched in journals related with KM, then six of them were simple case report and the other one was about diagnosis for ovarian cyst and another gynecologic disease such as uterine myoma and infertility through the iris diagnosis. And two articles were searched in journals related with TCM, then one of them was simple case report but the other is RCT article. Conclusion: KM and TCM therapies have some beneficial effect to resolve cyst, but objective power as a evidence is weak in the viewpoint of EBM. So continuous clinical trials such as RCT and multi center trials are needed.

  • With the development of fetal ultrasonography, detection of fetal ovarian cysts has been increased. Although ovarian cyst formation during the perinatal period is a self limiting process, there is still considerable controversy regarding the best treatment of the fetal ovarian cyst. The purpose of this study is to evaluate the natural history of fetal ovarian cysts and to analyze the result of treatment. From 1995 to 2004, 31 consecutive fetuses with ovarian cysts were followed by ultrasonography during the perinatal period. The fetal ovarian cyst was diagnosed by prenatal ultrasonography between 25weeks and 38 weeks and the mean size of the cysts was 5cm (ranged from 2 to 8cm). At birth, 3 cysts disappeared. In 2 cases, the diagnoses were changed to multicystic kidney disease and intestinal duplication. During following up of 26 cysts, 15 cysts have resolved completely. Seven cysts required oophorectomy because of cyst torsion (n=3), differentiation of tumorous condition (n=2), increased size of cyst (n=1), and large size (8cm) of cyst at birth (n=l). Fetal ovarian cyst should primarily be observed, and only in the limited cases, surgical treatment would be required for the risk of complications such as torsion and differentiation from benign to malignant pathology.

  • This study was performed to determine the significance of a baseline ovarian cyst on the response to controlled ovarian hyperstimulation and the outcome of IVF-ET. One hundred one patients who underwent IVF-ET were enrolled in this study. The outcome of 31 patients, who had an ovarian cyst of >10mm detected at ultrasound examination performed on day 3, was compared with that of 70 patients who underwent a similar protocol and did not have an ovarian cyst. E2 level on the day of hCG administration, the number of follicles, the number of oocytes retrieved, the number of embryo transferred and the pregnancy rate were evaulated. The E2 level on the day of hCG adminstration and the number of mature oocytes retrieved were lower in the group with a baseline cyst. The pregnancy rate also was significantly lower in the group with a cyst (21% versus 38%). Therefore a baseline ovarian cyst on cycle day 3 was associated with a poorer outcome after IVF-ET.

  • The cytologic samples of 26 ovarian cystic lesions from 25 women, aspirated under guide by trans-vaginal ultrasound, were evaluated for clinicopathologic correlation. Clinically 20 women were seeking medical assistance for infertility problems, and trans-vaginal cyst aspiration was done during follow-up of ovulation induction. Among them seven cases were histologically confirmed. Twenty cases of "benign cyst" in cytologic diagnosis were follicullar cyst and two cases of "endometriotic cyst" in cytology were histologically also proven in one case. One false positive diagnosis was given to corpus luteum cyst. It is emphasized that because the cysts are aspirated transvaginally and mature squamous epithelial cells occasionally could be mixed in the sample, attention should be given not to diagnose such cases as mature cystic teratoma. According to this study. ovarian aspiration cytology is useful in the management of cystic ovarian lesions, particularly in young women.

  • To investigate the changes of pituitary gland associated with ovarian cyst in Korean native cattle, pituitaries and ovaries were collected from 54 Korean native cattle at abbatoir. Pituitaries were stained with HerIant pituitary stain method and all the tissues were examined under light microscope. The results obtained were summarized as follows; 1. The delta cells and beta cells in the pars distalis were dull blue and violet in colors respeetively. Basophil size in the follicular phase and pregnant groups was larger than those of luteal phase and ovarian dysfunction groups. 2. The numbers of delta cells in the pars distalis of follicular cyst group were larger than those of remaining groups (p<0.01). 3. The distribution of delta cells in the acidophil zone was greater than that in the basophil zone of the follicular cyst and follicular phase groups. 4. The granulations of delta cells were more intensive in follicular cyst, follicular phase and pregnancy groups than in luteal phase and ovarian dysfunction groups (p<0.01). 5. The numbers of beta cells in follicular phase and ovarian dysfunction groups were larger than those in luteal phase and follicular cyst groups respectively (p<0.01). 6. In all groups, the distribution of beta cells was greater in basophilic zone than in acidophilic zone (p<0.01).

  • Ovarian cyst is found in 32% of necropsies in neonates, and can be visualized during gestation by ultrasonography. The clinical evolution of these Cysts is variable, but in most cases the prognosis is favorable. Ovarian torsion, bleeding, rupture, and peritonitis have been described as complications. We report a newborn girl with torsion of ovarian cyst. A cystic mass measuring $41.9{\times}31.9mm$ on left side of abdomen was identified at 32 weeks of gestation by fetal ultrasonography. Surgery was performed after birth.

  • Objective: This study was performed to compare the clinical response to controlled ovarian hyperstimulation (COH) of in vitro fertilization and embryo transfer (IVF-ET) according to the size of baseline ovarian cyst. Method: From February 1992 to March 1999, a retrospective analysis was done of 272 cases who underwent COH using mid-luteal phase long protocol of gonadotropin-releasing hormone agonist (GnRH-a) for IVF-ET. These cases were divided into four group; group 1 (n=63) had cysts with mean diameters between 20.0 and 29.0 mm on their baseline ultrasound on cycle day 3, group 2 (n=57, $30.0{\sim}49.0mm$), group 3 (n=68, >50.0 mm) and control group (n=84). Cases were excluded according to the following criteria; pure male factor infertility, the presence of only one ovary, high CA-125 level and previous endometriosis. Results: There were no statistically significant differences between cases with baseline ovarian cyst <50.0 mm in diameter and control group in any of the parameters. However, cases with baseline ovarian cyst>50.0 mm in mean diameter needed more amount of human menopausal gonadotropin (hMG), showed significantly lower estradiol ($E_2$) level, the number of follicle >15.0 mm on the day of human chorionic gonadotropin (hCG) administration, the number of oocytes retrieved, the number of mature oocytes, and pregnancy rate compared with control group. Conclusion: This study suggests that cases with baseline ovarian cyst <50.0 mm in diameter do not adversely impact on IVF-ET outcome. However, cases with baseline ovarian cyst >50.0 mm in diameter had adverse effects on various parameters. Therefore, to improve the outcome of IVF-ET in these cases, ovarian cyst aspiration prior to initiating COH may be required.

  • To establish the differential diagnosis and treatment method in bovine ovarian cysts, specially ovarian cysts with corpus luteum, serum progesterone (P$_4$) concentration and ultrasonography for measuring the cystic wall thickness and diameter of cyst and corpus luteum were investigated from slaughtered cows with ovarian cysts. Ovarian cysts were classified 8 types by the number of cyst, cystic wall thickness and present of corpus luteum. Ovarian cysts with corpus luteum were 11 (13.6%) of 81 cows and ovarian cysts without corpus luteum were 70 (86.4%) cows. The incidence rates of 8 various types of ovarian cysts were as follows; 2Ba 33.3%, 2Aa 25.9% and 2Bb 14.8%, respectively. The incidence rates of ovarian cysts without corpus luteum were follicular cyst 59.2% and luteal cyst 27.2%. The cystic wall thickness were 2Ab 3.7 mm and 2Bb 3.5 mm, and the serum P4 concentrations were above 2.0 ng/ml in 1Aa, 1Ab, 1Ba, 2Ab and 2Bb, respectively. In ovarian cysts with corpus luteum, the correlation coefficients between corpus luteum area and serum P$_4$ concentration were 0.45. In ovarian cysts without corpus luteum, there was significantly positive correlations between cystic wall thickness and serum P$_4$ concentration ($r^2$= 0.54, p<0.01). These results indicate that PGF$_2$$\alpha$ analogues can be choice for treatment of ovarian cysts with corpus luteum and above 3 mm the cystic wall thickness because serum P$_4$ concentrations were above 2.0 ng/ml in ovarian cysts with corpus luteum and thickened cystic wall. In conclusion, it is suggest that ultrasonography is useful diagnostic tool for diagnosis and selection of treatment remedy in cystic ovaries of bovine.
