A Personal Sound Amplification Product Compared to a Basic Hearing Aid for Speech Intelligibility in Adults with Mild-to-Moderate Sensorineural Hearing Loss

  • Background and Objectives: This study aimed to compare functional hearing with the use of a personal sound amplification product (PSAP) or a basic hearing aid (HA) among sensorineural hearing impaired listeners. Subjects and Methods: Nineteen participants with mild-to-moderate sensorineural hearing loss (SNHL) (26-55 dB HL; pure-tone average, 0.5-4 kHz) were prospectively included. No participants had prior experience with HAs or PSAPs. Audiograms, speech intelligibility in both quiet and noisy environments, speech quality, and preference were assessed in three different listening conditions: unaided, with the HA, and with the PSAP. Results: The use of PSAP was associated with significant improvement in pure-tone thresholds at 1, 2, and 4 kHz compared to the unaided condition (all p<0.01). In the quiet environment, speech intelligibility was significantly improved after wearing a PSAP compared to the unaided condition (p<0.001), and this improvement was better than the result obtained with the HA. The PSAP also demonstrated similar improvement in the most comfortable levels compared to those obtained with the HA (p<0.05). However, there was no significant improvement of speech intelligibility in a noisy environment when wearing the PSAP (p=0.160). There was no significant difference in the reported speech quality produced by either device or in participant preference for the PSAP or HA. Conclusions: The current result suggests that PSAPs provide considerable benefits to speech intelligibility in a quiet environment and can be a good alternative to compensate for mild-to-moderate SNHL.

Post-stroke fatigue, depression, emotional incontinence, and anger-proneness (뇌졸중 후 후유증: 피로, 우울, 감정조절 장애, 분노 조절 장애를 중심으로)

  • Stroke patients often develop post stroke sequelae when they survive. Post stroke fatigue and emotional disturbances including depression are common along with motor dysfunction. However, medical personnel have paid relatively little attention to emotional changes and the presence of fatigue following strokes. Post-stroke fatigue was common, occurring in 57% of the patients in our series. The post-stroke fatigue appears to be related to the pre-stroke fatigue, physical disability and post stroke depression (PSD) although the relation to the lesion location remains elusive. The prevalence of post-stroke emotional disturbance has been reported to range from 12% to 64%. The wide variation in the frequency of post stroke depression may be related to methodological heterogeneity in items such as the criteria for depression, the timing of assessment, and the study population. Emotional incontinence, which is characterized by inappropriate or excessive laughing or crying is also common. The incidence of and factor related to this post-stroke emotional incontinence (PSEI) also remains unclear. We reported that out-patients with single, unilateral stroke, 18% had PSD and 34% had PSEI. Although both PSD and PSEI were related to motor dysfunction and location (anterior vs. posterior cortex) of the lesion, the latter was a stronger determinant for PSD. PSEI was more closely associated with subcortical strokes than was PSD. Another manifestation of post stroke patients is the occurrence of post stroke anger proneness (PSAP). They may become easily irritated, impulsive, less generous, and prone to be angry or aggressive at others. We also have reported the PSAP which seems to be closely associated with the presence of PSEI. The lesion distribution appears to be also similar. Both PSEI and PSAP respond well to serotonin reuptake inhibitors suggesting that these symptomsmay be possibly related to the alteration of serotonin after brain injury. Likewise, PSAP also produces a great deal of frustration and embarrassment among patients and caregivers. In summary, emotional disturbances such as depression, emotional incontinence, anger-proneness and fatigue are fairly common but under-recognized sequelae of stroke. These emotional disturbances decrease the quality of life of the patients and caregivers, and may adversely affect the overall prognosis. Therefore, these problems must be appropriately recognized and alleviated. Finding strategies to relieve the symptoms is imperative by understanding the causative factors in individual patient.

  • Personal sound amplifiers(PSAPs) provide accessible and affordable healthcare to individuals with hearing disability. Many studies are in progress for affordable PSAP development, but still people do not have a best fitting profile for the PSAP depending on their hearing test. As a result, they do not have a personalized and profiled music and sound, which are very helpful for those who has hearing problems. In this paper, we propose a device and mobile system to provide music with an equalizer value according to the hearing condition of an individual to prevent the hearing loss. In order to overcome the limit of frequency band of the equalizer in a smart phone, we developed bluetooth controlled equalizer based on the fitting profiles.

  • An emergency service over Voice over IP (VoIP) network is an essential condition, like the existing telecommunication services. To support for the emergency services, standardization works have been performed. The National Emergency Number Association (NENA) has been developing the framework and procedures for an emergency service for Non-IP based network, rather than protocols. In contrast, the Internet Engineering Task Force (IETF) has been only focused on end-to-end IP-based emergency calls. The NENA architecture is incompatible with the IETF protocols. To solve the problem, we design and implement a SIP-based VoIP emergency system by adopting the NENA architecture and by applying IETF protocols, for both IP-based Pubic Safety Answering Point (PSAP) and PSTN-based PSAP. It is implemented and tested under UNIX environment.

WiBro has been standardized as international standard called Mobile WiMAX, and has been deployed in many countries. Emergency Services in WiBro networks are infrastructure services and will be very essential to induce subscribers from other types of networks. This paper proposes a network architecture for supporting emergency services for WiBro mobile VoIP networks. The proposed architecture is based on WMF's architecture for international compatibilities, and reflects national considerations on interfacing PSAP for domestic compatibilities.

  • As the development of the various IP-based services, it is expected that Internet telephony service will gradually replace the traditional PSTN-based telephony service. But there are many issues resolved to spread the Internet telephony service. One of them is to support the emergency services in the Internet telephony. In the case of USA, it has been regulated that 911 services should be supported in the Internet telephony services using VoIP on the similar performance level to PSTN 911 service. According to the regulation, basic VoIP 911 calls should be routed to the general access line of LEA without the location information or the callback number, but the enhanced VoIP 911 calls with the location information and callback number should be routed on the dedicated 911 network and destined to the local 911 distribution center such as PSAP. But, in the current VoIP-based Internet telephony network, the emergency call service has not been handled as one of the special services as well at has a worse performance in comparison to it on PSTN. Moreover, the service has a critical problem that it can not be destined to the nearest PSAP because of the insufficient information about the location information and the call back number. In this paper, we suggest the SIP-based emergency call service mechanism in order to resolve the problems above mentioned. This suggested mechanism is implemented to show its effectiveness and efficiency.

  • During the past 5 years, domestic VoIP service has grown rapidly absorbing more than 8 million subscribers. More recently, the increase of enterprise or institutional subscribers become more dominant than individual subscribers. Supporting emergency services such as 119 in these scaled up VoIP services is very essential. In current domestic VoIP networks, the VoIP terminal's location information which are provided to the PSAP with the arriving emergency call depends on the information which are entered at the subscription time or updated by the subscribers when the terminal is moved to other place. This manual provision of location information which requires the user's intervention can result in serious failure in saving people in emergency situations, in case that users are negligent in entering the information or users enter the wrong information. In this paper, we propose an enhanced mechanism in which the terminal's location information is automatically offered by the network for emergency services in ethernet-based enterprise VoIP networks.

  • 통신 네트워크 발전 추세에 따라 데이터와 음성회선을 통합하고 PSTN을 통한 전화 서비스보다 저렴한 통신비용과 다양한 부가서비스 구현 등의 여러 장점을 가진 VoIP서비스로 전환되고 있다. 하지만 VoIP서비스의 긴급서비스에서 위치식별 대한 문제점이 제기되었다. VoIP는 인터넷망을 이용하으로 부여된 발신전화번호 정보를 송출하지 않아 발신자의 위치 및 해당지역 PSAP의 식별이 불가능하다. 이에 따라 건물 내 LAN 환경에서 VoIP 단말기와 연결하는 실내의 모든 이더넷 포트에 대한 정보를 관리하여 이를 지정된 VoIP 단말기와 연계시킴으로써 위치를 확인하는 방법 등이 제안되었으나 단말기 이동시마다 위치정보를 수정해야 하는 문제가 제기되었다. 따라서 본 논문에서는 VoIP서비스에 서 RFID를 이용하여 발신자 단말기 위치를 식별하는 긴급서비스 지원 모델을 제안하고, 이를 위해 가입자 위치정보 획득 및 관리, 호 라우팅 등에 관한 구체적인 방안을 제시하였다. 그 결과 VoIP 망에서 단말기 이동에 대한 위치 확인 문제도 해결이 가능하다.
