A Case study of Soyangin patient with early-stage Parkinson's disease diagnosed as Upper wasting-thirst pattern (소양인(少陽人) 소갈증(消渴證)으로 진단한 초기 파킨슨병 환자의 구갈(口渴), 변비(便秘) 치험례)

  • Objectives The aim of this study was to examine significant improvement of motor and non-motor symptoms after treatment with Sasang constitutional medicine treatment on a Soyangin patient with early-stage Parkinson's disease. Methods The patient was treated with Soyangin's constitutional medications (herb and acupuncture) and scalp acupuncture. The progress was evaluated using the Unified Parkinson Disease Rating Scale (UPDRS) before and after treatment. The UPDRS was used to assess the overall function of patient. Results We observed a significant improvement in patient's motor and non-motor symptoms using UPDRS. The UPDRS score decreased in Part I, Part II, and Part III. Conclusions This study shows that Sasang constitutional medicine can be effective treatment for Soyangin patient with early-stage Parkinson's disease.

  • Background: Parkinson's disease is the second most common degenerative brain disease after Alzheimer's disease. This study reviewed clinical practice of acupuncture and moxibustion treatment for Parkinson's disease to assess the future direction of these treatments. Methods: The literature search used 5 Korean Internet databases. The search terms were "Parkinson's disease." and "Parkinson (Korean)." 36 papers were selected: 27 case reports, 2 respropective studies, 3 uncontrolled clinical trials, 3 randomized controlled trial, 1 non-randomized controlled trial. Results: Filiform needles were used in most of studies. Pharmacopuncture, electro-acupuncture, and auricular acupuncture therapy were also used in combination with Filiform needles. 2. The most commonly used basic acupoints for Parkinson's disease was Saamchimbeop. The most commonly used form of pharmacopuncture involved Bee-venom. Conclusion: It is expected that treatment of Parkinson's disease with a variety of acupuncture and moxibustion therapies will continue to be studied, in addition to treatments that utilize existing Filiform needle treatments in clinical practice.

Effect of a Telephone-administered Cognitive Behavioral Therapy for the Management of Depression, Anxiety, and Chronic Illness Anticipated Stigma in Parkinson's Disease (파킨슨병 환자의 우울, 불안 및 만성질환에 기대되는 낙인 중재를 위한 전화사용 인지행동치료 효과)

  • Purpose: Parkinson's disease (PD) has a high incidence of psychiatric comorbidity, specifically depression and anxiety. This study examined the effectiveness of group cognitive behavioral therapy (CBT) in treating depression and anxiety. Methods: The study included 42 participants, aged between 52 and 77 years, who were diagnosed with idiopathic Parkinson's disease (IPD) and reported suffering from depression and anxiety. Patients were attending a department of outpatient neurology at D University Schools of Medicine in B Metropolitan city. A structured telephone-administered cognitive behavioral therapy was conducted for eight weeks. A repeated measure ANOVA was used to analyze results at pretest, post-test, and four weeks follow up. Results: According to service method, there were significant differences between the two groups in depression, anxiety and stigma was significant difference between time (p<.05). Analysis of the interaction between time and service method revealed significant differences in depression and anxiety (p<.05). Conclusion: The results suggest that telephone-administered cognitive behavioral therapy may be effective and may provide opportunity to adapt to individual needs for treating depression and anxiety in patients with Parkinson's disease.

  • Purpose: Music therapy is a non-pharmacological treatment of the behavioral and psychological symptoms. This study was conducted to contribute to the rehabilitation of patients with idiopathic Parkinson's disease in the areas of functionality, emotionality and sociability. Methods: The study included 35 participants, aged between 52 and 77 years, who were diagnosed with idiopathic Parkinson's disease (IPD) and reported suffering from depression, chronic illness anticipated stigma, and worrying about quality of life. The patients were recruited from the department of outpatient neurology at School of Medicine, D University in B Metropolitan City. Group music therapy was performed for nine weeks. The results were analyzed by repeated measures ANOVA. Results: In terms of stigma, there were significant differences between two groups and time (p<.05). Analysis of the interaction between time and service method revealed significant differences (p<.05). Between time, and the interaction between time and service method revealed significant differences in terms of quality of life (p<.05). Conclusion: The results suggest that group music therapy will be effective on the psychosocial well-being of patients with Parkinson's disease living in assisted and independent living communities.

A case of tremor in Parkinson's disease treated with Korean medicine (파킨슨병으로 유발된 진전이 한약 치료를 통하여 호전된 치험 1례)

  • As society ages, the seriousness of Parkinson's disease in the elderly increases. This study was performed in order to evaluate the effect of Korean medical treatments on Parkinson's symptoms. An elderly patient was diagnosed with Parkinson's disease by clinical symptoms, which were resting tremor, pill-rolling tremor and mild bradykinesia. We treated her disease with Korean medical treatment, including herbal medicine and acupuncture therapy and excluding western medication. For the herbal medicine, Insamyangyeong-tang (人參養榮湯), Bobi-tang (補脾湯), Dokhwaljihwang-tang (獨活地黃湯) and Sipimijihwang-tang (十二味地黃湯) were primarily used. In result, the clinical symptoms, measured by unified Parkinson's disease rating scale (UPDRS) and modified Hoehn and Yahr stage (H-Y stage), of Parkinson's disease in this patient had improved after the Korean medical treatment. This study suggests that Korean medical treatments can be applicable to improve symptoms in patients with Parkinson's disease.

Factors Affecting Caregiver Burden in Caregivers of Patients with Parkinson's Disease (파킨슨병 환자 부양자의 부양부담에 영향을 미치는 요인)

  • Purpose: Depression is a common sign of suffering among the patients with Parkinson's disease (PD). Frequent and severe neuropsychiatric symptoms lead to high levels of distress in patients and their caregivers, which results in the high levels of caregiver burden. The aim of this study was to determine the predictors of caregiver burden in caregivers of the patients with Parkinson's disease (PD). Methods: The study included 183 consecutive PD patients and their caregivers. Patients were assessed using the Hoehn and Yahr scale, Mini Mental State Examination (MMSE), Beck Depression Inventory (BDI), and Schwab and England Activities of Daily Living Scale. Caregivers' depressive symptoms were evaluated using the Caregiver Burden Inventor (CBI), BDI, and World Health Organization Quality of Life Scale. Results: All of the patients reported one or more neuropsychiatric symptoms. Patients' and caregivers' depressive symptoms, caregivers' age and education, time for caregiving, and quality of life were significantly associated with the increased caregiver burden. After controlling the level of education as a potential confounding variable, depression in both patients and caregivers, time for caregiving, and quality of life explained 45.6% of the variance in caregiver burden. Conclusion: Substantial attention needs to be given to the early identification of depression in PD patients and their caregivers to improve caregivers' quality of life and burden.

A Case Report on Parkinson's Disease Soyangin Patient with Abdominal Pain and Constipation (만성복통 및 변비를 호소하는 소양인 파킨슨병 환자의 치험1례)

  • Objectives The purpose of this case was to report significant improvement of abdominal pain, constipation and a slight improvement of tremor after treatment by Sasang Constitutional Medicine to the patient with Parkinson's disease.Methods The patient was diagnosed as Soyangin according to the result of Sasang constitutional diagnosis, and treated by Sasang constitutional medications and acupuncture. The progress was evaluated with the Global Assessment Scale (GAS).Results The abdominal pain was reduced from Numeric Rating Scale (NRS)9 to NRS1 and headache was reduced from NRS5 to NRS3. The tremor was reduced to GAS 10. Unified Parkinson Disease Rating Scale (UPDRS) was reduced from 40 to 27. The difference of heat distribution between left and right side was reduced and the abdomen was more warmer on Digital Infrared Thermal Imaging (DITI).Conclusions Sasang constitutional medicine can be a solution for the non motor symptoms caused by Parkinson's disease.

Comparison of the Total Stance Time And the Phase Ratio in Parkinson's Disease Patients And Normal Subjects (파킨슨씨 병 환자와 정상인의 입각기시간과 상비율의 비교)

  • The purpose of this study is to investigate the gait characteristics in Parkinson's disease patients. Specifically, the total stance time and the ratio of each stance phase (heel strike, mid-stance, propulsion) are analyzed from the foot-pressure measurement system which requires low cost and small space compared to the conventional gait analysis system. The gait characteristics were analyzed in 23 Parkinson's disease patients (before and after L-dopa medication), 34 elderly (sixties) normal subjects and 21 young (twenties) normal subjects. Bradykinesia global score (self-developed score of slowness of body movement) of patients before medication was determined to see the relationship between the score and the gait characteristics. The total stance time was greater in the erde. of patients, elderly, youngs (p<0.05). The phase ratio of heel strike and propulsion was smaller and that of mid-stance was greater in the order of patients, elderly, youngs (p<0.05). However, there was no significant difference in the above gait characteristics of patients before and after medication. There was a tendency, though statistically non-significant, that the total stance time is longer and the propulsion phase ratio is shorter in patients with greater Bradikinesia global scale, and this tendency was relieved after medication.

Polyubiquitin-Proteasomal Degradation of Leucine-Rich Repeat Kinase 2 Wildtype and G2019S

  • Parkinson disease (PD) is becoming one of the most neurodegenerative disorder worldwide. The deposited aggregates have been connected in the pathophysiology of PD, which are degraded either by ubiquitin-proteasomal system (UPS) or autophagy-lysosomal pathway (ALP). Leucin-rich repeat kinase 2 (LRRK2), one of the neurodegenerative proteins of PD is also stringently controlled by both UPS and ALP degradation as well. However, the polyubiquitination pattern of LRRK2 aggregates is largely unknown. Here, we found that K63-linked polyubiquitinations of G2019S mutant, most familial variant for PD, is highly enhanced compared to those of wild type LRRK2 (WT). In addition, in the presence of overexpressed p62/SQSTM-1, ubiquitination of LRRK2 WT or D1994A was reduced, whereas G2019S mutant was not diminished significantly. Therefore, we propose that degradation of G2019S via UPS is more involved with K63-linked ubiquitination than K48-linked ubiquitination, and overexpressed p62/SQSTM-1 does not enhance degradative effect on G2019S variant.
