자세의 유지성 및 안정성에 관한 해석 (Identification of postural steadiness and stability: state-of-the-art)

  • Since Romberg's test in 1953, the analysis of postural sway during upright stance has been widely used as a tool for evaluating balance and disorders of the postural control system. This review describes the methods that have been used to evaluste the static and dynamic performance of the postural control system. Various identification methods of postural control system based on standing balance are discussed and measures of postural sway are described. The application areas of standign balance research, with an emphasis on postural control evaluations, are also briefly described. This review can be used to gain an understanding of the dynamics of human standing balance.

고령자의 게임기반 시각피드백 훈련에 따른 자세 균형 효과 (Effects of Postural Balance at Game-based Visual Feedback Training of the Elderly)

  • 본 연구에서는 게임 콘텐츠를 적용한 시각 피드백 훈련 프로그램을 이용하여 정상 여성 고령자와 정상 20대 성인의 자세균형 조절의 효과를 분석 하였다. 실험은 힘판 기반의 자세균형 훈련 장치를 이용하여 20대 성인 23명과 60대 고령자 14명은 주 3일, 하루에 15분씩 총 4주의 자세균형 훈련을 받았다. 자세균형 훈련에 대한 평가는 Balance SD(Biodex, medicalscience Inc., USA)의 자세 안정성과 자세 한계성을 분석하였으며, 훈련 전, 중, 후의 각 시점에 대한 유효성을 검증하였다. 실험 결과, 젊은 성인과 고령자 모두 훈련 전, 후에 자세안정성과 자세한계성에 유의한 효과가 있었음을 확인하였다(p<0.05). 이는 자세 균형게임은 자세 균형과 관련된 전 후 좌 우 다양한 방향에서의 무게 이동 능력을 향상시켰음을 알 수 있었다. 게임 기반 시각적 피드백 훈련은 자세균형에 효과적으로 적용가능하며 개인과 다양한 연령층에 맞는 맞춤형 콘텐츠 개발이 필요할 것으로 사료된다.

시각 자극에 의한 신체자세 균형제어에 관한 연구 (Human Postural Balance Control by Visual Stimulation)

  • In this paper, we report the effects of visual stimulation patterns in the postural balance control. We used the motor-driven cloth panel and HMD(Head Mounted Display) to evaluate the effect of visual stimulation patterns in the postural balance control. We also investigated the usefulness of HMD in the postural balance rehabilitation training system from the view of reducing the scale of experimental system. Our results showed that a vertical-strip visual pattern was more effective than the others in the postural balance control. It was also indicated that HMD might be applied to clinical use as a new postural balance training system.

  • 본 연구에서는 자세균형 훈련 효과를 향상시킬 수 있는 게임 콘텐츠를 적용한 시각 피드백 자세균형 훈련 프로그램을 이용하여 정상 20대 성인을 대상으로 자세균형 조절 능력의 향상에 관한 유효성을 검증하고자 한다. 힘판 기반 자세균형 훈련 장치를 이용하여 23명의 피험자들이 일주일에 3일, 하루에 15분씩 4주간의 자세균형 훈련을 받았다. 게임기반의 시각 피드백 자세균형 훈련에 대한 평가는 Balance SD(Biodex, medicalscience Inc., USA)의 자세 균형 평가를 통하여 자세안정성과 자세한계성을 분석하였으며, 실험자의 자세균형 능력의 증진을 통한 유효성을 검증하였다. 그 결과 자세안정성과 자세한계성 모두 훈련 전후로 유의한 차이가 있었음을 확인하였다(p<0.05). 이번 연구 결과는 게임기반의 시각 피드백 자세균형 훈련이 자세균형 능력 향상을 위한 운동에 적용될 수 있음을 의미하며, 향후 다양한 자세균형 훈련의 프로그램 종류, 강도 및 각 질환별 최적 콘텐츠 개발에 대한 정량적인 데이터 수집 및 분석연구가 필요하다.

뇌졸중 환자에서 자세정렬변화가 족저압 및 균형에 미치는 영향 (Effects of Changes in Postural Alignment on Foot Pressure and Balance of Patients with Stroke)

  • Purpose: This study was conducted in order to investigate the exercise limit that may occur depending on changes in postural alignment by examining the significance of postural alignment changes, foot pressure, and balance of patients with stroke. Methods: In this study, 50 patients diagnosed with a stroke were selected as subjects. Imbalance of postural alignment of the trunk, pelvic tilt of trunk rotation of the body, angle of kyphotic curving of the thoracic, and angle of lordotic curving of the lumbar vertebra were measured. Foot pressure was examined by measuring average pressure and weight bearing. Balance was examined by measuring the center of pressure and limit of stability. Results: The significance of postural alignment, foot pressure, and weight bearing of the non-paretic side was examined. In addition, the significance between postural alignment and balance was examined. Conclusion: It is thought that limits of foot pressure and balance in the standing position can be caused by postural alignment. Thus, both a therapeutic intervention program and postural alignment training should be provided together in order to improve the function of patients with stroke.

Balance Exercise Program Using Training Mats Improves the Postural Balance of Elderly Individuals: A Randomized Controlled Trial

  • Purpose: The purpose of this study was to investigate the effect of balance training using a training mat on the postural balance of the elderly individuals. Methods: Thirty-five participants were selected from a falling prevention class and were randomly allocated to two groups; 17 in an exercise group (EG, $72.7{\pm}5.1$ years) and 18 in the control group ($74.9{\pm}4.0$ years). The EG underwent balance training using training mats for 60 minutes a day, 2 days a week, for 4 weeks. Postural balance parameter (timed up and go test, functional reach test, and one leg standing) were measured pre- and post- training. Results: The EG showed significant improvements in all variables that were analyzed. Conclusion: This study confirmed that balance training using a training mat effectively improves the postural balance in elderly people at risk for falling.

뇌졸중 환자의 탄력-비탄력 발목 테이핑 적용이 자세균형과 보행능력에 미치는 일시적 효과 (Immediate Effect of Elastic and Non-Elastic Ankle Taping on Postural Balance and Gait Ability in Subject with Stroke )

  • Background: The purpose of this study was to investigate the immediate effects of ankle elastic and non-elastic taping on postural balance and gait ability in subject with stroke. Design: Cross-sectional study Methods: Twenty-seven subjects with stroke participated in this study. The subjects performed to stand quietly for 30s on the balance platform and walking test with three different ankle taping conditions. The sway length, sway area and sway velocity of center of gravity (COG) displacement was measured to assess the postural balance and the timed up and go test, 10m walking test, 6 minutes walking test was measured to assess the gait ability. Repeated measured ANOVA was used to compare the postural balance parameters and gait ability according to three different ankle taping conditions. Results: Postural balance with non-elastic ankle taping was significantly improved compared to no ankle taping and elastic ankle taping condition(p<0.05). On the other hand, gait ability with elastic ankle taping was significantly increased compared to no ankle taping and non-elastic ankle taping condition(p<0.05). Conclusion: These findings suggest that an elastic ankle taping could effect to improve the gait ability, whereas a non-elastic ankle taping could effect to improve the postural balance in subject with stroke.

The Circadian Effects on Postural Stability in Young Adults

  • Purpose: Few studies have addressed the effect of diurnal circadian rhythms on postural stability, and thus the aim of the present study was to examine circadian effects on static and dynamic postural stability in young adults. Methods: Twenty-four subjects (9 men, 11 women: age=$22.20{\pm}1.77$, height=$167.20{\pm}10.47$, weight=$59.85{\pm}10.66$) from a university community volunteered for this study. Static and dynamic balance testing, which recorded using a Good Balance system (Good Balance, Metitur Ltd, Finland) was conducted at 9:00, 13:00, and 17:00 hours on two consecutive days, and the sequencing of static and dynamic balance tests were randomized. Results were analyzed using the non-parametric one-way repeated Friedman test in SPSS version 18.0 (SPSS Inc., Chicago, IL, USA), and variable found to be significant were subjected to Wilcoxon post hoc testing. Results: Static and dynamic balance showed significant difference at the three times assessments (test at 9:00, 13:00, and 17:00) during circadian. In the post hoc test of static (anteroposterior distance, mediolateral distance and COP (center of pressure) velocity) and dynamic balance (performance time), 13:00 was the longer and faster than 9:00. Conclusion: These results indicated that control of postural balance is influenced by diurnal circadian rhythms, and confirm that control of postural balance is more effective and better performance in the 09:00 hours than 13:00 hours or 17:00 hours.

Transient Effects of Calf Muscle Fatigue and Visual Control on Postural Balance During Single Leg Standing

  • PURPOSE: Muscle fatigue is a cause to change proprioception. The purpose of this study was to investigate the effects of calf muscle fatigue and visual control on postural balance during single-legged standing in healthy adults. METHODS: Nineteen healthy adults (male) were participated in this study (mean age: 24.36 years; mean height: 171.32 cm; mean weight: 64.58 kg). The postural balance (sway length, sway area, sway velocity of COG displacement) was measured by Balance Trainer System (BT4) in before and after calf muscle fatigue feeling in single legged stance. In this study, repetitive single-legged heel rise test was used to induce fatigue of the calf muscle. Paired t- test was used to compare the postural balance between before and after calf muscle fatigue. Data of subjects were analyzed using SPSS 22.0 (SPSS Inc., Chicago, IL, USA). Level of significance was set to .05. RESULTS: The sway length, sway area, sway velocity of COG (center of gravity) displacement after calf muscle fatigue feeling was significantly increased compared to before calf muscle fatigue feeling during single leg standing both eye open and close conditions (p<.05). CONCLUSION: This study suggested that calf muscle fatigue feeling has affected on postural balance when standing one leg both eye open and close conditions and postural control was disturbed by muscle fatigue and visual feedback in single leg standing.

유도된 다리길이 차이가 안정성한계와 정적 자세균형에 미치는 영향 (Effect of Induced Leg Length Discrepancy on the Limitation of Stability and Static Postural Balance)

  • Purpose: Leg length discrepancy (LLD) is one of the risk factors for postural imbalance. This study aimed to investigate the effect of induced leg length discrepancy on the limitation of stability (LOS) and static postural balance. Methods: Thirteen adults (males, 7; females 6) participated in this study. The LOS and static postural balance [sway length, sway area, and sway velocity of center of gravity (COG) displacement] were measured by the balance trainer system. The subjects were asked to move the COG for the anterior, posterior, and left and right directions maximally and to keep standing on the platform with and without induced LLD for 30 s in the open and closed eyes conditions, respectively. The LLD was artificially induced to 2 cm using insole. Wilcoxon test was used to compare the LOS and the static postural balance between with and without induced LLD. Results: The anterior and posterior LOS significantly decreased in induced LLD (p<0.05), and the left and right LOS were not significantly different between with and without LLD (p>0.05). Sway length, sway area, and sway velocity of the COG displacement significantly decreased in induced LLD (p<0.05). Conclusion: This study suggests that induced LLD could decease the antero-posterior LOS and increased the static postural balance. Therefore, the LLD could disturb the postural balance.
