Major Reforms and Issues of the Medical Licensing Examination Systems in Korea (의사면허 필기시험 제도의 성과와 과제)

  • Since the establishment of the national medical licensing examination board in 1992, the medical licensing examination system has changed enormously and this has had a number of impacts on examination services. All those reforms were aimed at improving the relevance and reliability of the test. Several attempts of the testing system have appeared in the new examination service, and which have also brought about the changes in the medical school curriculum such as introducing integrated courses instead of traditional subjects, using test scores as a reference to the post-graduation selection test. Some examples of changes in the examination system are as follows: 1) choosing three integrated test subjects and outlines of their reference content instead of 15 academic subjects, 2) adjusting the ratio of multiple choice question items to focus more on the problem solving level, 3) introduction of 'one-best answer' single set and 'extended matching type items, 4) item construction based on real clinical cases and real clinical materials. Recently, a clinical skill test system has been introduced to measure examinees' basic clinical skills competencies. Despite continuing efforts, the examination system still has many issues remaining to be solved. These problems include the differential weighting of test items, appropriate threshold for passing, and practicality of pre-testing to stabilize the passing rate and avoid the hazards of newness and undesirably difficult test items.

Implementation Level and Factors of Parents about Dental Examination in Elementary School Children (초등학생들의 학교구강검진결과에 대한 학부모의 이행수준과 관련요인)

  • This study evaluated the interest of parents of elementary school children on the examination with their memory of results, their consistency and their compliance with results sheets to improve effects of School dental health management project. This study recruited 1,334 second-, third-, fifth- and sixth-grade students at two elementary schools located in Daegu metropolitan city performing School Dental Examination Project in 2008 and investigated whether the students visited dental hospitals following results of the examination, how the parents remembered the results and how their memory was same with real results based on results of the examination, the parents' memory of results. The parents of second- and third-grade students showed good memory of malocclusion and number of Caries and higher consistency for dental caries. The parents of fifth- and sixth-grade students showed good memory of dental caries. Visiting rates of dental hospitals accordance with grade were 75.7% due to dental caries in second- and third-grade students and 60.5% due to malocclusion in fifth- and sixth-grade students. Among students with abnormal results of the examination, 77.1% of the children with dental caries of parents having same memory with real results visited the hospitals while 52.7% of those of parents having different memory with them did.

The University Examination And Course Timetabling Problem With Integer Programming

  • In this paper, we study the university timetabling problem, which consists of two subproblems, the university course timetabling problem and the examination timetabling problem. Given a set of classrooms, students, teachers, and lectures, the problem is to assign a number of courses (and examinations) to suitable timeslots and classrooms while satisfying the given set of constraints. We discuss the modeling and solution approaches to construct course and examination timetables for one of the largest Korean university. By using binary integer programming formulations, we describe these two complex real-world problems. Then, we propose a solution method, called NOGOOD, to solve the examination timetabling model. The computation results show that NOGOOD finds the optimal examination schedule for the given instance. Although we consider a specific instance of the university timetabling problem, the methods we use can be applicable to modeling and solving other timetabling problems.

Real Time Vision System for the Test of Steam Generator in Nuclear Power Plants Based on Fuzzy Membership Function (퍼지 소속 함수에 기초한 원전 증기발생기 검사용 실시간 비젼시스템)

  • In this paper it is proposed a new approach to the development of the automatic vision system to examine and repair the steam generator tubes at remote distance. In nuclear power plants workers are reluctant of works in steam generator because of the high radiation environment and limited working space. It is strongly recommended that the examination and maintenance works be done by an automatic system for the protection of the operator from the radiation exposure. Digital signal processors are used in implementing real time recognition and examination of steam generator tubes in the preposed vision system, Performance of proposed digital vision system is illustrated by experiment for similar steam generator model.

Detection of Plasmodium vivax by Nested PCR and Real-Time PCR

  • Malaria is endemic in the Cukurova region while it is sporadic in other regions of Turkey. Therefore, the laboratory and clinical diagnosis of malaria is important for the treatment of malaria. In this study, 92 blood samples that were taken from the suspected malaria patients for routine diagnosis in a period of 10 years between 1999 and 2009 were analyzed. All of these blood samples were examined by microscopic examinations using Giemsa-stained thick blood films, nested PCR, and real-time PCR. The sensitivity-specificity and positive-negative predictive values for these diagnostic tests were then calculated. It was found that the positive predictive values of microscopic examination of thick blood films, nested PCR, and real-time PCR were 47.8%, 56.5%, and 60.9% for malaria, respectively. The real-time PCR was found to have a specificity of 75% and sensitivity of 100%, while specificity and sensitivity of nested PCR was found 81.2% and 97.7% according to the microscopic examination of thick blood films, respectively.

  • Real-time techniques are motivated by a number of factors including the potential for direct acquisition of diagnostic quality images, facilitation of patient-specific imaging parameters, and reduced examination time. Real-time MRI includes not only a rapid pulse sequence but also high speed image reconstruction and easy interactivity. The frame rate of the real-time technique used should be matched to the physiological timeframes under study. Principal applications thusfar have been in localization, fluoroscopic triggering, guidance of other processes, and potentially in the generation of diagnostic images of moving structures.

An Analysis of The Science Inquiry Items in Scholastic Ability Test for College (대학수학능력시험에서 과학탐구 영역의 분석)

  • The Ministry of Education established the plan to adopt a new college enterance examination system, so called Scholastic Ability Test for college, from 1994. The National Board of Educational Evaluation had carried out 7 experimental trials for the new examination system. Eventhough, the governmental officials advertised the purpose and characteristics of the new examination system, many students and parents did not understand the essential point very well. In this study the researcher tried to analyze the 7 trial examination items and the first Scholastic Ability Test for college carried out 1993. In this study, the researcher tried to find out all the items appeared in the 7 trials. In the analysis, all the individual items from the 1st trial to the 7th trial were used. The research results showed that the items were evenly distributed in all the four areas(Physics, Chemistry, Biology and Earth Science) of science content; however, they were heavily concentrated on data or information analysis categoris in the inquiry domain. In the domain of context, laboratory context was the major item context. The scores were also analyzed. The results showed that no significant difference between male and female students. However, science track students showed higher achievement scores than the non-science track student. The examination result showed high correlation with the tests routinely have carried out in schools. The over all analysis showed the new examination system showed very different characteristics to the conventional tests. Therefore, for the student to get successful result in the Scholastic Ability Test, they should change the way of study. They should try to understand scientific concepts rather than to memorize them. They should invest their time to carry out real experiments rather than to focus on the princples and laws of science. One of the most important thing would be for the student to apply what they learned to real life situations.

Delphi Study on Introduction of Practical Skills Test in National Examination for Nursing Licensure (간호사 국가고시의 문제점과 개선방안에 대한 전문가 집단 연구)

  • Purpose: The purpose of this study was to investigate the plan to introduce a practical skills test as part of the national examination for nursing licensure. Method: The delphi method was used. The first set of data was collected by open questionnaire from universities and hospitals nationally. The second and third were collected by structured questionnaire. Results: The biggest problem for clinical Practice in nursing education was reported as the lack of opportunities for direct nursing practice. The biggest performance problem in job duties for novice nurses was reported as the lack of integrated problem solving skills in a clinical situation. The biggest problem with the current national examination was reported as the use memorizing solutions to test questions. It was suggested that the best plan to resolve problems with the national examination would be to develop test questions representative of the clinical setting and increase personnel in clinical education. About the introduction of a practical skills test as part of the national examination, 56.9% were in agreement and 38.5% disagreed. Conclusion: The current national examination has many problems when it comes to testing clinical competency. So, a practical skills test must be deferred to a future time. There are also many difficulties in presenting a real situation, therefore further research is needed in preparation for the introduction of a practical skills test.

Analysis of Factors Delaying on Waiting Time for Medical Examination of Outpatient on a Hospital (일 병원의 외래진료대기시간 지연요인 분석)

  • Background : To shorten processing time for variety of medical affairs of the patient at the outpatient clinic of a big hospital is very important to qualify medical care of the patient. Therefore, patient's waiting time for medical examination is often utilized as a strong tool to evaluate patient satisfaction with a medical care provided. We performed this study to investigate factors delaying related with waiting time for medical examination. Methods : The data were collected from June 26 to July 30, 1999. A total 275 case of medical treatment and 5,634 patients who visited outpatient clinics of a tertiary hospital were subjected to evaluate the waiting time. The data were analyzed using frequency, t-test, ANOVA, $X^2$-test by SPSS Windows 7.5 program. Results : The mean patient's waiting time objectively evaluated ($30.9{\pm}33.9$ min) was longer than that subjectively by patient evaluated ($25.1{\pm}26.2$ min). Patient waiting time objectively evaluated was influenced by the starting time of medical examination, consultation hours, patients arriving time etc, as expected. The time discrepancy between two evaluations was influenced by several causative factors. Regarding the degree of patients accepted waiting time with the medical examination is 20 min. Conclusion : The results show that, besides the starting time of medical examination, consultation hours and patients arriving time, influence the patient's subjective evaluation of waiting time for medical examination and his satisfaction related with the service in the big hospital. In order to improve patient satisfaction related with waiting time for medical examination, it will be effective examination rather than to shorten the real processing time within the consultation room.
