A Study on the Pain, Fatigue Perceived by Rheumatoid Arthritis Patients (류마티스 관절염 환자가 지각하는 통증과 피로)

  • Purpose: The purpose of this study was to investigate the relationship between the level of pain and fatigue in rheumatoid arthritis patients. Method: The subjects for this study were 120 patients registered in S University Hospital, and the period of data collection was from June 20, 2005 to October 15, 2005. Results: The research instruments used in this study were the Graphic Rating Scale of Pain and Fatigue, and the cronbach's ${\alpha}$ of Fatigue scale was .90. In data analysis, SPSSWIN 12.0 program was used for descriptive statistics. The results were as follows. 1) The range of total pain scores was from 10 to 100 and the mean score of the pain in rheumatoid arthritis patients was 53.70. 2) The range of total fatigue scores was from 20 to 58 and the mean score of the fatigue in rheumatoid arthritis patients was 36.04. 3) There was a significant difference in pain according to duration of disease(F=3.934, p<.05). 4) There was a significant difference in fatigue according to duration of disease(F=3.442, p<.05). 5) The level of fatigue was significant correlation with the level of pain in rheumatoid arthritis patients(r=.217, p<.01). Conclusion: The level of fatigue was significant correlation with the level of pain in rheumatoid arthritis patients. Therefore nursing interventions decreasing the pain perceived by rheumatoid arthritis patients are needed to decrease there fatigue levels.

The case-control study of rheumatoid arthritis by sasang typology (사상체질에 따른 류마티스 관절염 환자 대조군 연구)

  • Objective: The purpose of this study is to examine the susceptibility of the patients diagnosed by rheumatoid arthritis to Sasang Typology. Methods: This study are planned to detect the susceptibility of the patients diagnosed by rheumatoid arthritis to Sasang Typology. In order to analyze the susceptibility of rheumatoid arthritis to Sasang Typology, 147 rheumatoid arthritis patients and 147 matched controls are assessed with QSCC II question for Sasang Typology. Then the frequencies of patients by Sasang constitution are compared to those of control, which are statistically analyzed and adjusted by age, sex, smoking status, alchol intake, BMI, and economic status. Results: The demographic characteristics of the study population are similar in sex and age distribution, smoking status, and alcohol intake(p>0.05). 147 rheumatoid arthritis patients and 147 matched controls are assessed with QSCC II question for Sasang constitution. Soeumin is the most susceptible to rheumatoid arthritis compared to Taeumin or Soyangin(Crude OR[95% CI]; 3.82 [2.19-6.64]). It is statistically significant after adjusting age, sex, smoking status, alcohol intake, BMI, and economic status(Adjusted OR[95% CI]; 3.01[1.47-6.15]). Conclusions: It is suggested that the specific Sasang Typology associated with rheumatoid arthritis is used as its susceptibility marker and further as basic data to prevent the risk factors for rheumatoid arthritis.

Physical Activity and Prevalence of Rheumatoid Arthritis in Korean Adults with Prediabetes in the 2009-2013 National Health Cohort Study (당뇨병전기 성인의 신체운동과 류마티스 발생률)

  • Background: Rheumatoid arthritis is highly prevalent in overweight patients with type 2 diabetes mellitus and can be reduced by physical activity via altered proinflammatory parameters. However, the association between the frequency of physical activity and the prevalence of rheumatoid arthritis in prediabetic patients remains unclear and was evaluated in this study. Methods: Utilizing the Korean National Health Insurance Sharing Service database, 58,391 adults, who met the research criteria and underwent a general medical check-up between 2009 and 2013, were selected for this study. To analyze the data, a logistic regression with a proc survey logistic procedure was used. Results: The study revealed that the cumulative rheumatoid arthritis prevalence was lower in prediabetic patients compared to that in the control group (OR, 0.64; 95% CI, 0.483-0.840; p=0.001). More frequent physical activity (≥3 days/week) was significantly associated with a reduced rheumatoid arthritis prevalence in both groups (OR, 0.28; 95% CI, 0.039-0.521; p=0.044 vs. OR, 0.15; 95% CI, 0.063-0.237; p=0.007). Additionally, a 3.8-fold higher risk of rheumatoid arthritis development was observed in prediabetic adults with less frequent baseline physical activity (≤2 days/week). Overall, in prediabetes, the prevalence of rheumatoid arthritis was associated with the frequency of physical activity and not with the fasting plasma glucose levels. Conclusion: More frequent physical activity is associated with a low risk of developing rheumatoid arthritis in prediabetic patients. Thus, further studies are needed to confirm the clinical outcomes of frequent physical activity in rheumatoid arthritis prevention and control.

Effect of Gamikyejakjimo-tang(jiāwèiguìsháozhīm-tāng) ethanol extract on Rheumatoid Arthritis factors (가미계작지모탕(加味桂芍知母湯) 주정 추출물이 류마티스 관절염 인자에 미치는 영향)

  • Objectives : The purpose of this study is to prove the effect of Gamikyejakjimo-tang (ji$\bar{a}$w$\grave{e}$igu$\grave{i}$sh$\acute{a}$ozh$\bar{i}$m-t$\bar{a}$ng, GK) on rheumatoid arthritis. Methods : We checked viability and measured production of IL-$1{\beta}$, IL-6, IL-17, TNF-${\alpha}$ in RAW 264.7 cell after treat by GK. Then we measured rheumatoid arthritis index score of DBA/1 mice with rheumatoid arthritis induced by CIA after GK oral administration, checked IL-$1{\beta}$, IL-6, IL-17, TNF-${\alpha}$ and hs-CRP tests in serum. Also we were observed mRNA expression of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ in spleen by RT-PCR. Results : GK showed cell viability of 100% or higher in all concentration in RAW 264.7 cells. GK inhibited LPS-induced productions of rheumatoid arthritis mediators cytokine in RAW 264.7cells. GK treated group showed improvement from rheumatoid arthritis at decreased the index score. Also, GK treated group decreased level in serum of IL-1b, IL-6, IL-17, TNF-a and hs-CRP tests by 31%, 35%, 20%, 57% and 58% respectively. Finally, GK treated group showed decrease expression of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ mRNA in spleen by 46%, 51%, 25% and 42% respectively. Conclusions : In this study, in-vitro and in-vivo results observed rheumatoid arthritis factors cytokine of IL-$1{\beta}$, IL-6, IL-17 and TNF-${\alpha}$ decrease in RAW 264.7 cells, serum, mRNA expression. Also, GK showed decrease of inflammation figure in hs-CRP tests depending on effect of rheumatoid arthritis. Thus, these results can used as a effective drug of GK for rheumatoid arthritis.

A Comparative Study on Symptoms, Living Activities, Self-efficacy and Family Support between Rheumatoid Arthritis Patients and Fibromyalgia Patients (류마티스관절염과 섬유조직염환자의 증상, 일상생활, 자기효능 및 가족지지 비교 연구)

  • This study was conducted to compare symptoms, living activities, self-efficacy and family support between rheumatoid arthritis patients and fibromyalgia patients. The subjects were consisted of 150 rheumatoid arthritis patients and 125 fibromyalgia patients at H Rheumatism Clinic. Data were collected by means of structured interview with questionnaires. The results were as follows: 1. Fibromyalgia patients have more symptoms than rheumatoid arthritis patients have. But the latter has more difficulties concerning living activities. 2. Self-efficacy score of fibromyalgia arthritis patients was higher than that of rheumatoid arthritis patients, but both the scores were at moderate level. 3. The level of family support of rheumatoid arthritis patients was moderate, similar as that of fibromyalgia patients. As self-efficacy and family support are important factors to relieve symptoms of both diseases, their promotion holds a key post.

Influencing Factor on Social Support perceived by Patients with Rheumatoid Arthritis (류마티스 관절염 환자가 지각하는 사회적 지지와 영향요인에 관한 조사연구)

  • Purpose: The purpose of this study was to investigate the influencing factors on the social support perceived by patients with rheumatoid arthritis. Methods: The subjects for this study were 468 patients registered in 2 general hospitals and the period of data collection was from October 20, 2013 to February 20, 2014. The data were analyzed with t-test, ANOVA and Pearson's correlation coefficients. Results: The mean of item of social support was 3.31 in patients. For social support, there were significant differences by gender and economics in patients with rheumatoid arthritis. Conclusion: Managing social support was important for patients with rheumatoid arthritis to promote health. To enhance social support of patients with rheumatoid arthritis, nursing intervention strategies must be developed for increasing economics or giving information about their activity of daily living.

  • Objectives : Tumor necrosis factor-${\alpha}$(TNF ${\alpha}$) is a proinflammatory cytokine involved in the pathogenesis of rheumatoid arthritis. This study was designed to investigate the relation between TNF-${\alpha}$ gene polymorphism and rheumatoid arthritis in Korean population. Methods : This study was carried out on 103 rheumatoid arthritis patients who fulfilled the American College of Rheumatology 1987 revised criteria for rheumatoid arthritis and 208 healthy control subjects. Blood samples from all subjects were obtained for DNA extraction. The extracted DNA was amplified by polymerse chain reaction(PCR). PCR products were visualized by 2% agarose gel electrophoresis. We investigated the genotyping of TNF-${\alpha}$ by using Pyrosequencing. Results: The genotypes of TNF-${\alpha}$ gene were GG, AG and AA. While the distribution of TNF-${\alpha}$ polymorphism in control subjects was 92.31%, 7.21%, 0.48% respectively, in rheumatoid arthritis patients was 93.20%, 6.80%, 0.00%(GG, AG, AA). There was no statistical significant allelic frequency difference between control and rheumatoid arthritis groups. Conclusions : We concluded that there was no significant association between TNF-${\alpha}$ gene polymorphism and rheumatoid arthritis. However, the findings of this study need to be confirmed in more patients and further studies.

A Study on the Relationship between Mastery and Activity of Daily Life in Rheumatoid Arthritis Patients (류마티스 관절염 환자의 불확실성에 대한 극복력과 일상생활정도의 상관성 조사)

  • Purpose: This study was to investigate the relationship between mastery and activity of daily life in rheumatoid arthritis patients. Method: The subjects for this study were 222 patients registered in H University Hospital Rheumatoid Arthritis Center, and the period of data collection was from August 15, 2003 to September 30, 2003. Reliability coefficients of instruments were found Cronbach's Alpha =.74~.94. In data analysis, SPSSWIN 10.0 program was utilized for descriptive statistics, Pearson's correlation, and multiple regression analysis. Result: The results were as follows. 1) The range of total uncertainty scores was from 33 to 87 and the mean score of the uncertainty in rheumatoid arthritis patients was 63.27. 2) The range of total mastery scores was from 10 to 27 and the mean score of the mastery in rheumatoid arthritis patients was 18.70. 3) The range of total activity of daily life scores was from 22 to 80 and the mean score of the activity of daily life in rheumatoid arthritis patients was 72.56. 4) The level of mastery was significant correlation with activity of daily life(r=.395, p<.01).

Reviewing Research on the Study of Rheumatoid Arthritis Model Treated by Pharmacopuncture in Korean Journals Objective (류마티스 관절염 모델 동물실험에서의 약침치료 연구 동향 - 국내 논문을 중심으로)

  • Objectives This study was designed to collect studies about the Pharmacopuncture used for Rheumatoid arthritis model through analysis on Korean medicine prescriptions of Korean journals objective. Methods We searched 29 studies related Pharmacopuncture treatment of Rheumatoid arthritis model via Korean medicine web database. Results 29 studies were published 2007 to 2015. There were 4 studies have positive control group and negative control group, and 25 studies were design only negative control group. 15 studies were published 2007, 5 studies were published 2008, 1 study was published 2010, 2 studies were published 2011, 5 studies were published 2012, 1 study was published 2013. There were 20 Pharmacopuncture treated for Rheumatoid arthritis mouse or rat model. Most of these studies were effective to treat of Rheumatoid arthritis model. Conclusions These results suggested that Pharmacopuncture is effective treatment to Rheumatoid arthritis model and we need continuously agonize and research more effective therapy method.
