A Review of Korean Medicine Treatment for Managing the Thoracolumbar Compression Fractures: A Retrospective Observational Study (흉요추 압박골절 치료에 대한 한의복합치료 고찰: 후향적 관찰 연구)

  • Objectives This study aims to find out effect of Korean medicine treatment on managing thoracolumbar compression fractures through retrospective observational study. Methods Among hospitalized patients at the Department of Korean Medicine Rehabilitation from January 1st, 2018 to February 28th, 2023, a total of 24 inpatients who were diagnosed with thoracolumbar compression fractures and received Korean medicine treatment were included in this study. Numeric rating scale (NRS) was used for pain assessment and clinical variables such as sex, symptoms, age, thoracolumbar injury classification and severity (TLICS) scores were collected. For subgroup analysis to analyze factors affecting treatment response, we divided patients into responders and non-responders according to NRS change. For statistical analysis, we compared before/after hospitalization and analyzed distinct features between two groups. Results Most of the patients were in their 70s and 83.33% were female. Average hospitalization period was 24.54±11.91 days. All patients had back pain as their chief complaint and only 2 patients received surgeries. In TLICS, only 1 patient got score 6, which represented surgery indication. After Korean medicine treatment, NRS of almost every patient got lower significantly at the time of discharge (3.02±1.93) than admission (5.52±1.95). Comparing two groups, responders had lower NRS at the time of discharge and TLICS score of them were lower than non-responders. Conclusions Our results show that Korean medicine treatment for thoracolumbar compression fractures was effective in reducing pain. There were distinct clinical features such as age, past history, surgeries between those with significant improvement in pain scores and those who did not.

  • Purpose: Gastric cancer (GC) is among the most prevalent and fatal cancers worldwide. National cancer screening programs in countries with high incidences of this disease provide medical aid beneficiaries with free-of-charge screening involving upper endoscopy to detect early-stage GC. However, the coronavirus disease 2019 (COVID-19) pandemic has caused major disruptions to routine healthcare access. Thus, this study aimed to assess the impact of COVID-19 on the diagnosis, overall incidence, and stage distribution of GC. Materials and Methods: We identified patients in our hospital cancer registry who were diagnosed with GC between January 2018 and December 2021 and compared the cancer stage at diagnosis before and during the COVID-19 pandemic. Subgroup analyses were conducted according to age and sex. The years 2018 and 2019 were defined as the "before COVID" period, and the years 2020 and 2021 as the "during COVID" period. Results: Overall, 10,875 patients were evaluated; 6,535 and 4,340 patients were diagnosed before and during the COVID-19 period, respectively. The number of diagnoses was lower during the COVID-19 pandemic (189 patients/month vs. 264 patients/month) than before it. Notably, the proportion of patients with stages 3 or 4 GC in 2021 was higher among men and patients aged ≥40 years. Conclusions: During the COVID-19 pandemic, the overall number of GC diagnoses decreased significantly in a single institute. Moreover, GCs were in more advanced stages at the time of diagnosis. Further studies are required to elucidate the relationship between the COVID-19 pandemic and the delay in the detection of GC worldwide.

Occurrence of the Onion Moth, Acrolepiopsis sapporensis, in the Welsh Onion Farms and its Treatment Using 'BtPlus' (대파 재배지 파좀나방(Acrolepiopsis sapporensis) 발생 현황과 '비티플러스' 처리 효과)

  • The onion moth, Acrolepiopsis sapporensis, was monitored in the farms cultivating the welsh onion, Allium fistulosum, using sex pheromone from transplantation to harvest. Two occurrence peaks were observed at early June and late July after the overwintering population. However, the population sizes were varied among different years and the cultivating environments. To effectively control A. sapporensis with microbial pesticides, different Bacillus thuringiensis strains were screened to select B. thuringiensis kurstaki (BtK). To enhance the insecticidal virulence of BtK, the culture broth of Photorhabdus temperata temperata (Ptt) was added to the BtK. This mixture of two entomopathogenic bacteria was called 'BtPlus', which was superior to BtK alone in the insecticidal virulence. The enhanced virulence was explained by the immunosuppressive activity of the secondary metabolites contained in the Ptt extract. The metabolites inhibited both cellular and humoral immune responses of A. sapporensis, resulting in the enhanced virulence of BtK. These results suggest that A. sapporensis occurs in the welsh onion fields and the resulting economic damage would be effectively prevented by BtPlus application.

Efficient Mass-rearing Method of Mythimna loreyi (Lepidoptera: Noctuidea) using Artificial Diets (인공사료를 이용한 뒷흰가는줄무늬방나방(Mythimna loreyi) (나비목: 밤나방과)의 효율적인 대량 사육 방법)

  • A lot of individuals of Mythimna loreyi have been attracted to the sex-pheromone traps of Spodoptera frugiperda during recent few years. However, there is no information about this pest. Thus, an efficient mass-rearing method of M. loreyi is demanded to study this pest. In this study, we compared the effects of artificial diets and rearing methods on its larval development and oviposition to suggest an efficient mass-rearing method of M. loreyi. Between S. frugipera and Agrotis ipsilon artificial diets, A. ipsilon diet showed more rapid larval development with higher survivorship, and decreased pupa weights and oviposition numbers compared to S. frugipera diet. Moreover, a grouping rearing caused more rapid larva development, decreased pupa weight and survivorship compared to an individual rearing. Therefore, for mass-rearing of M. loreyi, it is considered efficient to rear the newly emerged larvae in groups using A. ipsilon artificial diet and then rearing them individually after second or third larval stadium.

North Korean Defectors' depression through the CES-D and the Rorschach test (CES-D와 로샤검사를 통해 본 북한이탈주민의 우울감)

  • This study was performed to investigate the North Korean defectors's depression through the CES-D and a projective test simultaneously. So 40 North Korean defectors and 40 South Koreans participated in this study. The results are the following. First, the North Korean defectors demonstrated more higher CES-D scores than the South Koreans, while there were not significant differences in the Rorschach depression-related indexes, DEPI, CDI, S-CON between two groups. Second, at the analysis of the sex difference effect, women among the North Korean defectors showed the most high CDI index scores. In addition, we analyzed the mean differences between two groups for morbid content, shading response, achromatic response, human movement response, form dimension response, Lamda, and the number of total response. However, there are not significant differences between two groups for those variables, except for form dimension(FD) response. It suggested the probability that the North Korean women defectors could experience more psychological distress underlyingly than the North Korean men defectors. Third, most importantly, the correlation between CES-D and DEPI in the Rorschach test was significantly different in the two groups(the North Korean defectors and the South Koreans). In conclusion, the self-report of the North Korean defectors on depression could be exaggerated. So we should be cautious to interpret the self report results of the North Korean defectors.

Impact of Smoking and Alcohol Consumption on Early-Onset Gastric Cancer Development in Young Koreans: A Population-Based Study

  • Purpose: Although smoking and alcohol consumption are known risk factors for gastric cancer (GC), studies assessing their effects on early-onset GC are limited. In this nationwide, population-based, prospective cohort study, we assessed the effects of smoking and alcohol consumption on early-onset GC in patients aged <50 years. Materials and Methods: We analyzed data of patients aged 20-39 years who underwent cancer and general health screening in the Korean National Health Screening Program between 2009 and 2012. We calculated the adjusted hazard ratios (aHR) and 95% confidence intervals (CI) for GC incidence until December 2020. Results: We enrolled 6,793,699 individuals (men:women=4,077,292:2,716,407) in this cohort. The mean duration of follow-up was 9.4 years. During follow-up, 9,893 cases of GC (men:women=6,304:3,589) were reported. Compared with the aHRs (95% CI) of never-smokers, those of former and current-smokers were 1.121 (1.044-1.205) and 1.282 (1.212-1.355), respectively. Compared with the aHRs (95% CI) of non-consumers, those of low-moderate- and high-risk alcohol consumers were 1.095 (1.046-1.146) and 1.212 (1.113-1.321), respectively. GC risk was the highest in current-smokers and high-risk alcohol consumers (1.447 [1.297-1.615]). Interestingly, alcohol consumption and smoking additively increased the GC risk in men but not in women (Pinteraction=0.002). Conclusion: Smoking and alcohol consumption are significant risk factors for early-onset GC in young Koreans. Further studies are needed to investigate sex-based impact of alcohol consumption and smoking on GC incidence in young individuals.

Effect of Individualized Exercise Program for Preventing Metabolic Syndrome among IT Company Office Workers (IT 기업 사무직 근로자의 대사증후군 예방을 위한 맞춤형 운동프로그램의 효과)

  • Objectives: Interventions promoting physical exercise and healthy habits in workplaces have been shown to be effective in reducing risk factors for metabolic syndrome. This study was conducted to examine the effects of an individualized conditioning exercise program of IT company office workers with or at higher risk of metabolic syndrome. Methods: A total of 444 IT company office workers with or at higher risk of metabolic syndrome participated in a 3-month conditioning exercise program. Body composition data using bioelectrical impedance analysis and cardiopulmonary data using cardiopulmonary exercise testing from 53 individuals (mean age: 34.8 ± 7.1 years, sex : 21% female, height : 170.4 ± 6.8 cm, weight : 75.2±12.2 kg, body mass index : 25.8±3.3 kg/m2) who have successfully completed pre-test, intervention, and post-test were analyzed. The 12 weeks intervention encompassed: (1) health counseling (2) supervised exercise(endurance-based, aerobic exercise, or circuit training once a week for 50 minutes at heart rate reserve(HRR) of 77-95%) (3) self-directed exercise and biweekly health screening checks. Results: The results indicated a significant decrease in body weight, body fat mass and body mass index, respectively. Moreover, VO2peak, AT VO2 and AT Time significantly improved, respectively. Resting blood pressure(SBP/DBP) showed positive changes but were not statistically significant. We observed the correlation between characteristics of participants and rate of changes in cardiopulmonary outcomes of participants, there are no significant correlation. These results indicate positive changes in body composition and cardiorespiratory fitness parameters following individualized conditioning exercise program. Conclusions: Individualized workplace exercise program for preventing metabolic syndrome can lead to improvements in body composition and cardiorespiratory fitness.

Reproductive Ecology and Spawning of Hoplobrotula armata in the Coastal Waters of Jejudo Island, Korea (제주 연안에서 채집된 붉은메기(Hoplobrotula armata)의 산란생태)

  • This study investigated the reproductive ecology and spawning of Hoplobrotula armata on the coast of Jejudo Island. Samples were collected from February to December 2019 and February to December 2020, with a total of 2,634 samples. The results of the investigation showed that total length (TL) ranged from 22.0 cm to 68.8 cm, and the body weight (BW) ranged from 66.5 g to 3,553.9 g. According to the development process of gonads and the gonadosomatic index (GSI), the spawning period of H. armata was from July to October. The relationship between TL and BW was BW = 0.0024TL3.3278 in females and BW = 0.0035TL3.2162 in males. The sex ratios of surveyed female to male H. armata was 1:0.71, with more females. At 50%, 75%, and 97.5%, the group maturities of H. armata were as follows: Females, 39.9 cm, 42.7 cm, and 49.4 cm; males, 37.6 cm, 40.4 cm, and 46.9 cm, respectively. According to monthly GSI and gametocyte (ovarian, testis) development stage observations of H. armata on the coast of Jejudo Island, they spawn once a year and are presumed to be summer spawners.

The effect of perineural injection therapy on neuropathic pain: a retrospective study

  • Background: Among the various pain-related diseases that can be encountered at the clinic, there is a neuropathic pain that is difficult to treat. Numerous methods have been proposed to treat neuropathic pain, such as taking medication, nerve block with lidocaine, or neurolysis with alcohol or phenol. Recently, a method of perineural injection using dextrose instead of lidocaine was proposed. This study was designed to compare the effects of perineural injection therapy (PIT) with buffered 5% dextrose or 0.5% lidocaine on neuropathic pain. Methods: The data were collected from the database of pain clinic from August 1st, 2019 to December 31st, 2022 without any personal information. The inclusion criteria were patients diagnosed with postherpetic neuralgia (PHN), trigeminal neuralgia (TN), complex regional pain syndrome (CRPS), or peripheral neuropathy (PN), and patients who had undergone PIT with buffered 5% dextrose (Dextrose group) or 0.5% lidocaine (Lidocaine group) for pain control. The data of patients, namely sex, age, and pain score (numerical rating scale, NRS) were collected before PIT. The data of NRS, side effects, and satisfaction grade (excellent, good, fair, or poor) were collected one week after each of the four PIT, and two weeks after the last PIT. Results: Overall, 112 subjects were enrolled. The Dextrose group included 89 and Lidocaine group included 23 patients. Because the number of patients in the Lidocaine group was too small to allow statistical analysis, the trend in Lidocaine group was just observed in each disease. There were no significant side effects except for a few bruise cases on the site of injection in all groups. The NRS in most Dextrose groups except CRPS were reduced significantly; however, the Lidocaine group showed a trend of pain reduction only in PHN. The Dextrose group except CRPS showed increased satisfaction two weeks after the final PIT. Conclusion: From the results, it is suggested that PIT with buffered 5% dextrose may have a good effect for neuropathic pain without any side effect except for patients with CRPS. This may offer a window into a new tool that practitioners can employ in their quest to help patients with neuropathic pain.

The effects of synbiotics-glyconutrients on growth performance, nutrient digestibility, gas emission, meat quality, and fatty acid profile of finishing pigs

  • Glyconutrients help in the body's cell communication. Glyconutrients and synbiotics are promising options for improving immune function. Therefore, we hypothesized that combining synbiotics and glyconutrients will enhance pig nutrient utilization. 150 pigs (Landrace × Yorkshire × Duroc), initially weighing 58.85 ± 3.30 kg of live body weight (BW) were utilized to determine the effects of synbiotics-glyconutrients (SGN) on the pigs' performance, feed efficiency, gas emission, pork traits, and composition of fatty acids. The pigs were matched by BW and sex and chosen at random to 1 of 3 diet treatments: control = Basal diet; TRT1 = Basal diet + SGN 0.15%; TRT2 = Basal diet + SGN 0.30%%. The trials were conducted in two phases (weeks 1-5 and weeks 5-10). The average daily gain was increased in pigs fed a basal diet with SGN (p = 0.036) in weeks 5-10. However, the apparent total tract digestibility of dry matter, nitrogen, and gross energy did not differ among the treatments (p > 0.05). Dietary treatments had no effect on NH3, H2S, methyl mercaptans, acetic acids, and CO2 emissions (p > 0.05). Improvement in drip loss on day 7 (p = 0.053) and tendency in the cooking loss were observed (p = 0.070) in a group fed basal diets and SGN at 0.30% inclusion level. The group supplemented with 0.30% of SGN had higher levels of palmitoleic acid (C16:1), margaric acid (C17:0), omega-3 fatty acid, omega-6 fatty acid, and ω-6: ω-3 ratio (p = 0.034, 0.020, 0.025, 0.007, and 0.003, respectively) in the fat of finishing pigs. Furthermore, group supplemented with 0.30% of SGN improved margaric acid (C17:0), linoleic acid (C18:2n6c), arachidic acid (C20:0), omega 6 fatty acid, omega-6 to omega-3 ratio, unsaturated fatty acid, and monounsaturated fatty acid (p = 0.037, 0.05, 0.0142, 0.036, 0.033, 0.020, and 0.045, respectively) in the lean tissues of finishing pigs compared to pigs fed with the control diets. In conclusion, the combination of probiotics, prebiotics, and glyconutrients led to higher average daily gain, improved the quality of pork, and more favorable fatty acid composition. Therefore, these results contributed to a better understanding of the potential of SGN combinations as a feed additive for pigs.
