Transthoracic-Extrapleural Approach for Microsurgical Thoracic Discectomy with Video Assistance - Technical Report of Three Cases - (내시경을 이용한 경흉강 늑막외측 접근 흉추 추간판 절제술 3례 - 수술수기 증례보고 -)

  • Microsurgical transthoracic extrapleural approach for thoracic discectomy in three cases were modified and simplified in order to achieve better exposure of ventral extradural space than posterolateral extrapleural(lateral extracavitary) approach and less pulmonary complications than transthoracic transpleural approach. These approaches were optimized by microscope and video-assistance. Surgery was followed by clinical improvement in all patients. The results of treatment seem to support the use of these modified techniques as adequate surgical treatment of thoracic disc herniations without any complications.

The Characteristics of the Moire Topography in Patients of HIVD of Cx-spine (경추 추간판 탈출증 환자의 경근무늬 측정상의 특징)

  • Objectives : This study was designed to find The characteristics of the Moire topography in patients of HIVD of Cx-spine. Methods : The Moire topography using IBS(Intek-plus Body Scanner 2000, Seoul, Korea) were obtained in 688 people of control group and 105 patients of HIVD of Cx-spine. Each values were studied to determine the difference between controls and patients. Results : There were significantly difference between controls and patients in thoracic ratio, lumbar ratio, top of the thoracic depth ratio, top of the lumbar depth ratio, Z axis distance and DZ/DY. Conclusions : There were increase of the thoracic kyphosis and lower position in top of the thoracic kyphosis in patients of HIVD of Cx-spine than controls.

  • Objects : This study was to report a clinical effect of Chuna Treatment for HIVD-cervical spine Failed Back Surgery Syndrome(FBSS) patient. Methods : In order to alleviate both arm tingling, numbness and neck stiffness, the patient was treated by acupuncture therapy, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, Verbal Numerical Rating Scale(VNRS) and Neck Disability Index(NDI) score were used. Results : VNRS and NDI were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with HIVD-cervical spine FBSS patient. Further clinical studies are needed to verify the findings.

  • Objectives: This study aims to investigate the sense of improvement and satisfaction from 7 cases of herniated lumbar disc patients which was treated with spine decompression&chuna. Methods: Each patient has been treated with spine decompression and chuna treatment. The degree of improvement has been evaluated by VAS(Visual Analogue Scale) and ODI(Oswestry Disability Index) score starting from the day of admission, after 1week, and 2weeks. Results and Conclusions: Through the result, spine decompression and chuna treatment proved to have valid effect for Herniated Lumbar Disc. and there needs more clinical studies into synergy between spine decompression and Chuna treatment.

Thoracolumbar Spine Injury (흉요추부 손상)

  • Method of management of the spine injury should be determined, based on the status of neurological injury as well as on the presence of traumatic instability. At the thoracic and lumbar spine, patterns of neurological injury are different from the cervical spine due to their neuro-anatomical characteristics. Especially, at the thoracolumbar junction, neurological injury patterns with their respective prognosis vary from the complete cord injury or conus medullaris syndrome to the cauda equina syndrome according to the injury level. The concept of Holdsworth's instability based on the posterior ligament complex theory has evolved into the current 3-column theory of Denis. Flexion-rotation injury and fracture-dislocation are well known to be unstable that surgical fixation is frequently needed for these injuries. However, there have been some controversies for the stability of burst fractures and their treatment, such as indirect or direct decompression and anterior or posterior approach. In this article, current concepts and management of traumatic instabilities at the thoracic and lumbar spine have been reviewed and summarized.

  • The purpose of this study was designed to find out the effectiveness of vertebral segment instability, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. In this study, the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test(Matthiass Test) by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. The results of the present study were as follows: 1. Instability test of lumbar vertebra segment is 2 type differential angle test between vertebrae segment and loading test of spine(matthiass) by Spinal Mouse. It appeared to improve stability of segments in sagittal plane after applying program. So lumbar spine curve increased lordosis toward anterior and was improved of the lumbar spine flexibility in flexion and extension. Specially, in matthiass test, (-) value was increased between lumbar vertebra segment when was the load on spine. And so applying stability improved after program. 2. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine vertebra segments stabilization. Spine instability patients will have a risk when in lifting a load or working with slight flexion posture during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

Helical tomotherapy for spine oligometastases from gastrointestinal malignancies

  • Purpose: This study evaluated the treatment effectiveness and proper radiation dose of helical tomotherapy (HT) in spine oligometastases from gastrointestinal cancers. Materials and Methods: From 2006 to 2010, 20 gastrointestinal cancer patients were treated with HT for spine oligometastases (31 spine lesions). The gross tumor volume (GTV) was the tumor evident from magnetic resonance imaging images fused with simulation computed tomography images. Clinical target volume (CTV) encompassed involved vertebral bodies or dorsal elements. We assumed that the planning target volume was equal to the CTV. We assessed local control rate after HT for 31 spine metastases. Pain response was scored by using a numeric pain intensity scale (NPIS, from 0 to 10). Results: Spine metastatic lesions were treated with median dose of 40 Gy (range, 24 to 51 Gy) and median 5 Gy per fraction (range, 2.5 to 8 Gy) to GTV with median 8 fractions (range, 3 to 20 fraction). Median biologically equivalent dose (BED, ${\alpha}/{\beta}$ = 10 Gy) was 52 $Gy_{10}$ (range, 37.5 to 76.8 $Gy_{10}$) to GTV. Six month local control rate for spine metastasis was 90.3%. Overall infield failure rate was 15% and outfield failure rate was 75%. Most patients showed pain relief after HT (93.8%). Median local recurrence free survival was 3 months. BED over 57 $Gy_{10}$ and oligometastases were identified as prognostic factors associated with improved local progression free survival (p = 0.012, P = 0.041). Conclusion: HT was capable of delivering higher BED to metastatic lesions in close proximity of the spinal cord. Spine metastases from gastrointestinal tumors were sensitive to high dose radiation, and BED (${\alpha}/{\beta}$ = 10 Gy) higher than 57 $Gy_{10}$ could improve local control.

Case Report of Sacrum Malposition Analysed by Relative Research of Test (검사법의 비교연구로 살펴본 천골변위 증례보고)

  • Objectives : The purpose of this study is to find out the relation between Orthopedic Test of sacroiliac joint and two Sacrum Malposition Tests. Methods : We investigated 56 cases of patients who complained of unilateral sacroiliac joint pain and were diagnosed as sacrum malposition. We used Gaenslen's Test, Sacroiliac Resisted Abduction Test and Patric's Test as Orthopedic Test for pain area, while used Thompson's Sacrum Test and Greenman's 4 Point Test for diagnosis of sacrum malposition. Results : P-value was 0.511 between Thompson's Sacrum Test and Greenman's 4 Point Test within negative Orthopedic Test, and was 0.523 within positive Orthopedic Test(p>0.05) Conclusions : The results suggested that the Thompson's Sacrum Test was not statistically correlation with Greenman's 4 Point Test within negative Orthopedic Test, and was not correlated within positive Orthopedic Test.

Finding of Relation Between 4th Lumbar Rotational Malposition and Scoliosis (요추 4번의 회전변위와 척추측만증과의 상관성에 관한 연구)

  • Objectives : The purpose of this study is to find out the relation between 4th lumbar rotational malposition and scoliosis. Methods : We investigated 22 cases of patients who were diagnosed as scoliosis. We used AP & Lateral view X-ray for patients. And we analysed the relation between 4th lumbar rotational malposition and scoliosis. Results : P-value was 0.436 between 4th lumbar rotational amount and lordotic angle, and was 0.758 between 4th lumbar rotational amount and wedge angle(p>0.05). And p-value was 0.022 between 4th lumbar rotational amount and scoliotic apex rotational amount(p<0.05), but was 0.286 between 4th lumbar rotational amount and Cobb's angle(p>0.05). Conclusions : The results suggest that 4th lumbar rotational malposition was statistically correlation with scoliotic apex rotational malposition, was not Cobb's angle, 4th lumbar lordotic & wedge angle.

A Clinical Study on Effect of Chuna Treatment for Neck Pain Patients Caused by Traffic Accident (자동차사고로 발생한 경항통 환자의 추나요법의 효과에 대한 임상적 연구)

  • Objectives : The purpose of this study is to investigate the effect of Chuna treatment for neck pain patients caused by traffic accident. Methods : 64 patients were divided into two groups, experimental group and control group, of 32 patients each. Control group was treated with general korean medical treatment three times per week for 2 weeks. Experimental group was treated with Chuna treatment and general korean medical treatment three times per week for 2 weeks. We evaluated the treatment effect of each group with the visual analog scale(VAS) and neck disability index(NDI). Results : 1. In both two groups, VAS were decreased significantly in statistics after treatment. 2. In both two groups, NDI were decreased significantly in statistics after treatment. 3. There were significant reduction of the VAS in experimental group than control group, but, there were not significant reduction of the NDI in experimental group than control group. Conclusions : We suggest that Chuna treatment could be recommended as a useful therapy to neck pain patients caused by traffic accident.
