A sensible media simulation system for automobiles is introduced to open up new possibilities for an in-car entertainment system. In this paper, the system architecture is presented, which includes a virtuality-to-reality adaptation scheme. Standard data schemes for context and control information from the International Standard MPEG-V (ISO/IEC 23005) are introduced to explain the details of data formats, which are interchangeable in the system. A sensible media simulator and the implementation of a sensory device are presented to prove the effectiveness of the proposed system. Finally, a correlation between learning styles and sensory effects (that is, wind and vibration effects) is statistically analyzed using the proposed system. The experiment results show that the level of satisfaction with the sensory effects is unaffected overall by the learning styles of the test subjects. Stimulations by vibration effects, however, generate more satisfaction in people with a high tactile perception level or a low visual perception level.
Background Infraorbital nerve dysfunction is commonly reported after zygomaticomaxillary complex fractures. We evaluated sensory changes in four designated areas (eyelid, nose, zygoma, and lip) innervated by the infraorbital nerve. This evaluation was conducted using the static two-point discrimination test and the vibration threshold test. We assessed the diagnostic significance of the blink reflex in patients with infraorbital nerve dysfunction. Methods This study included 18 patients, all of whom complained of some degree of infraorbital nerve dysfunction preoperatively. A visual analog scale, the infraorbital blink reflex, static two-point discrimination, and the vibration threshold were assessed preoperatively, at 1 month postoperatively (T1), and at a final follow-up that took place at least 4 months postoperatively (T4). The results were analyzed using a multilevel generalized linear mixed model. Results Scores on the visual analog scale significantly improved at T1 and T4. The infraorbital blink reflex significantly improved at T4. Visual analog scale scores improved more rapidly than the infraorbital blink reflex. Two-point discrimination significantly improved in all areas at T4, and the vibration perception threshold significantly improved in the eyelid at T4. Conclusions Recovery of the infraorbital blink reflex reflected the recovery of infraorbital nerve dysfunction. We also determined that the lip tended to recover later than the other areas innervated by the infraorbital nerve.

Objective: The aim of this study is to investigate the gender-differences in vibrotactile responses(sensitivity and displeasure) of residual forearm simulated by vibration stimulation in upper limb(trans-radial) amputees. Background: Several studies have reported that vibration stimulation using the haptic vibrator is one the most effective methods for delivering sensation to an amputees. However, few studies have reported the perception to haptic vibratory stimulus, particularly sensitivity and displeasure. Method: We set up a custom-made vibration stimulation system that included 6 actuators(3 medial parts and 3 lateral parts) and a graphical user interface(GUI)-based acquisition system to investigate changes in residual somatosensory sensibility and displeasure in the forearm of upper limb(trans-radial) amputees. Vibration actuators were attached at the 25%-point on the proximal forearm. Stimulation with 32Hz, 64Hz, or 149Hz of frequency was used for the sensitivity tests and with 32~257Hz of frequency was used for the discomfort experiments. The subjective responses were evaluated on a 10 point scale. Results: The results showed that vibrotactile sensory perception in male amputees were higher than that in female amputees. In male amputees, the response at lateral area of forearm was the most sensitive than medial area; but, female amputees showed similar sensitive areas. Subjects did not experience any discomfort during vibrotactile stimuli. Conclusion: Vibrotactile response in the amputees was dependent on gender as well as area stimulated by vibration. Application: The results might contribute to develop the vibrotactile feedback system for the amputees.

Research involving discomfort or pain related to haptic vibratory stimulation the for prosthesis users of myoelectrical hand is very lacking. Our objective of this study was to evaluate the displeasure and sensitivity of areas in forearm using vibration stimulation system between upper limb amputees and non-amputees. Twenty transradial amputees and forty non-amputees (20 youth, 20 elderly) were involved. We set up custom-made vibration stimulation system including eight actuators (4 medial parts and 4 lateral parts) and GUI-based acquisition system, to investigate changes of residual somatosensory sensibility and displeasure at proximal 25% of forearm. Eight vibration actuators were attached to the circumference of proximal 25% point of forearm at regular intervals. Sensitivity tests were used to stimulate the 120Hz and discomfort experiment was used to 37 ~ 223Hz. The subjective responses were evaluated by 10 point scale. The results showed that both groups were similar in sensitive areas. Response at around of radius was most sensitive than other areas in all subjects. Elderly group do not appear discomfort of vibrotactile; however, youth group and amputee presented discomfort of vibrotactile. Prosthesis with a vibrotactile feedback system should be developed considering the sensitivity. Furthermore, Future studies should investigate the scope of application of that principle.
Objective : This paper provides a narrative review of the research literature on the neurophysiological and neurochemical mechanisms of local vibration while studying the treatment principles and mechanisms of Whidam's vibrator Sugi therapy. Methods : Searches related to vibration therapy research were conducted in PUBMED using "Vibration", "Whole Body Vibration", "Localized Vibration", and "Focal Vibration". The Conditions were limited to review and systematic review. Results : Roberto Casale's paper was selected as an inquiry task and reviewed critically and narratively by referring to other papers. The stimulation process of local vibration (LV) was broadly classified into receptor transmission (pain reception phase), ascending sensory pathway to the spinal cord (segmental phase), and action of the cortex and subcortical structures (systemic control phase) according to the pain pathway. In addition, the role of C-tactile mechanoreceptors, changes in neurotransmitters and neurohormones, LV stimulation below perception threshold (lower threshold), pain control and kinesiologic illusions were specially addressed. In addition, the expression and function of Piezo Channels were added to supplement the human pain and tactile sensing mechanism. Conclusions : LV exerts pain control mechanisms through different interactions that can interfere with pain transmission and pain perception. Since LV provides sufficient neurophysiological reasons for clinical application, it is necessary to expand the use of Whidam's vibrator Sugi therapy to a wider range of clinical applications.

Background: Augmented somatosensory feedback stimulates the mechanoreceptor to deliver information on bodily position, improving the postural control. The various types of such feedback include ankle-foot orthoses (AFOs) and vibration. The optimal feedback to mitigate postural sway remains unclear, as does the effect of augmented somatosensory feedback on muscle co-contraction. Objects: We compared postural sway and ankle muscle co-contraction without feedback (control) and with either of two forms of somatosensory feedback (AFOs and vibration). Methods: We recruited 15 healthy subjects and tested them under three feedback conditions (control, AFOs, vibration) with two sensory conditions (eyes open, or eyes closed and the head tilted back), in random order. Postural sway was measured using a force platform; the mean sway area of the 95% confidence ellipse (AREA) and the mean velocity of the center-of-pressure displacement (VEL) were assessed. Co-contraction of the tibialis anterior and gastrocnemius muscles was measured using electromyography and converted into a co-contraction index (CI). Results: We found significant main effects of the three feedback states on postural sway (AREA, VEL) and the CI. The two sensory conditions exerted significant main effects on postural sway (AREA and VEL). AFOs reduced postural sway to a level significantly lower than that of the control (p = 0.014, p < 0.001) or that afforded by vibration (p = 0.024, p < 0.001). In terms of CI amelioration, the AFOs condition was significantly better than the control (p = 0.004). Vibration did not significantly improve either postural sway or the CI compared to the control condition. There was no significant interaction effect between the three feedback conditions and the two sensory conditions. Conclusion: Lower-extremity devices such as AFOs enhance somatosensory perception, improving postural control and decreasing the CI during static standing.

Purpose: Sensory changes in the upper limb are complications of a mastectomy with immediate breast reconstruction with the treatment of breast cancer. The purpose of this study is to clarify whether immediate breast reconstruction worsens the sensory changes. Methods: From March 2004 to December 2005, 20 patients who had a mastectomy with immediate breast reconstruction(reconstruction group) were compared with 23 patients who had a mastectomy alone(control group). All patients had stage I or II breast cancer. The sensory changes were assessed in a blind manner by one examiner that used light touch sensation, static two-point discrimination, pain, vibration, hot and cold temperature perception. The sensory changes were identified along the sensory dermatome for diagnosing the damaged nerves. The following factors and their relationship with the sensory changes were analyzed : age, complications, and the mastectomy method. Results: There was no statistical difference in the static two-point discrimination, pain, vibration, hot and cold temperature perception between the two groups. However, the ability to recognize light touch was significantly better(p=0.045) in the reconstruction group than in the control group. The main site of sensory change was the proximal and medial portion of the upper limb in both groups. At these sites, the mean value of Semmes-Weinstein monofilament was $1.01g/mm^2$(reconstruction group 0.82, control group 1.17) and 2-point discrimination was 51.74(converted to perfect score of 100; reconstruction group 42.50, control group 59.78). The total rate of early complications was found to be significantly lower(p=0.006) in the reconstruction group than in the control group. Conclusion: These findings suggest that an immediate breast reconstructive procedure following a mastectomy is as safe as or safer than a mastectomy alone with respect to postoperative sensory changes of the ipsilateral upper limb.
Evaluation of clinical usefulness of current perception threshold test and vibration sense perception threshold test in diagnosing the diabetic poly-neuropathy patients is one of the diagnosis methods for diabetic poly-neuropathy. Up to the present, some diagnostic methods were used for diabetic poly neuropathy patients. For example, there are neuropathy impairment score test of lower limbs, nerve conduction test, cooling detection threshold test, heat-pain threshold test and so on. However, most of the above tests require very expensive cost and take a lot of time in test. In this paper, a new apparatus estimating vibration sense ability is introduced. For this purpose, the VCM(voice coil motor) stimulating patient's peripheral nerve and current amplifier were manufactured. Also, softwares sensing and driving the vibration detection threshold test in order to measure the quantitative vibration sensory levels in diabetic poly-neuropathy patients were developed.

This study was designed to investigate the feasibility of utilizing an adaptation for selective elicitation of tactile sensations by means of transcutaneous electrical stimulation. We conducted the first experiment to investigate how the stimulation frequency affected the adaptation. Twenty healthy subjects participated in the second experiment to confirm our proposal that the perception intensity of the low-frequency vibration can be enhanced after a high-frequency adaptation, and vice versa. It was found that (1) a low-frequency stimulation did not adapt the nerve afferents responsible for the high-frequency vibration, (2) a high-frequency stimulation affected the nerve afferents responsible for the low-frequency vibration, but adapted to the pressure sensation more intensely, and (3) more than 62% of the subjects reported a more clear selective sensation after the adaptation had lessened or depressed the unwanted sensation. The observations showed that adaptation of the nerve afferent could be utilized for selective elicitation of tactile sensations.

This study aims at identifying the effect of preferred visual and auditory stimuli on the road traffic noise perception through the series of psychoacoustic experiments. The first experiment was designed to find the addition effect of a sound reproducing system in a square where lay adjacent to roads with road traffic noise, provide appropriate music corresponding to the varying conditions of weather in the given space. In result, it was found that the rating around the square improved toward more positive adjectives. The second experiment was designed to assess the visual effect of twelve roadsides with different landscapes on the road traffic noise perception. As a result, approx. 3 to 5 dB(A) of psychological reduction was seen in places where natural landscape was preferred, compared to the others, although the noise levels were similar. The third experiment was designed to evaluate the effect of visual screen from adjacent roads on road traffic noise perception by means of ME method in a laboratory. In result, the effect of psychological reduction was observed at 65dB(A) or lower. Especially, complete screening from adjacent roads led to 5 to 10% of loudness reduction effect, compared to non-screening cases. Finally, the fourth experiment was designed to evaluate the effect of visual and auditory information with ME method and 7-point SD rating scale in a laboratory. In result, up to 10% of loudness reduction and about 2dB(A) of noise perceptional reduction were seen at 65dB(A) or lower.
