Evidence-based Treatment of Acute Lateral Ankle Sprain (근거에 기반한 급성 발목관절 외측 염좌의 치료)

  • Acute lateral ankle sprain, which is the most common musculoskeletal injury, can be treated effectively with appropriate evidence-based initial care using PRICE (protection, rest, ice, compression, and elevation) and functional rehabilitation. Many systemic reviews reporting a high-level of evidence supporting the clinical usefulness and necessity of primary surgical repair for acute lateral ankle sprain have been insufficient. Regardless of the severity of ligament complex injuries, the surgical treatment for acute lateral ankle sprain without concomitant pathologies is not recommended and should be considered only in young professional athletes with complete ligament rupture (grade III) and severe instability.

The Comparative Study on the Bee-Venom Therapy and Common Acupuncture Therapy for the Acute Ankle Sprain (급성 족관절 염좌에 대한 봉독약침치료와 일반 침치료의 비교연구)

  • Objective : The purpose of this study is to compare the effect of Bee-Venom Therapy and common acupuncture therapy for the acute ankle sprain Methods : This study has been carried out for 32 cases of ankle sprain patients who have visited Cheon-an Oriental Medical Hospital from may 1, 2004 to June 30, 2004. We have treated 16 cases of them by Bee-Venom therapy(group I) and the other 16 cases by common acupuncture therapy(group II). And we have compared those two group. Results and Conclusions : 1. There were good effect on the ankle sprain in both two groups. 2. There were no significant difference at the ROM of ankle joint in both two groups. 3. Common acupuncture therapy had more quick effect on the joint of swelling. 4. In the VAS score, Bee-Venom therapy had more quick effect on total improvement of ankle sprain.

The Clinical Effects of Heating-Conduction Acupuncture Therapy(tentatively called) for Anterior Talofibular Ligament Injury Induced by Acute Ankle Sprain (전열침(가칭)이 급성 족관절 염좌로 인한 전거비 인대 손상에 미치는 임상적 효과)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on anterior talofibular ligament injury induced by acute ankle sprain. Methods : From October 31, 2008 to July 21, 2009, the 11 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental medicine hospital, Dae-Jeon university with acute ankle sprain were performed heating-conduction acupuncture therapy on anterior talofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with Pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results : 1. The pain threshold score and the VAS score showed statistical significant improvement after 1st treatment. 2. The pain threshold score and the VAS score showed improvement but had no statistical significance after 2nd treatment. 3. The difference between left and right were not statistical significant in pain threshold score and VAS score. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with anterior talofibular ligament injury induced by acute ankle sprain.

Dose Effects of Bee Venom Pharmacopuncture for the Acute Ankle Sprain in Rats (발목염좌 급성기에 봉독 약침의 용량에 따른 진통 및 회복에 미치는 효과)

  • Objectives : The aim of this study was to observe the dose-dependent effects of bee venom (BV) pharmacopuncture on the serious ankle sprain in rats. Methods : The grade III ankle sprain was produced by surgically damaging the lateral ligaments complex of Sprague-Dawley rats. BV pharmacopuncture with the different doses($5{\mu}g/kg$, $10{\mu}g/kg$, $50{\mu}g/kg$) were treated on the different acupoints(GB34, GB39 and GB42) of the affected hind limb, respectively. By measuring foot weight bearing force ratio(FWBFR), the pain levels by ankle sprain and the pain recovery for 7 days were observed under BV pharmacopuncture on each acupoint. Results : In the normal and ankle sprained rat, the BV single administration decreased FWBFR in a dose - dependent pattern. The higher the BV dose, the higher the pain resulted in the normal and the sprained ankles. Especially, the dose - dependent effects of BV resulted in the most pronounced decrease in FWBFR in GB34. The recovery of FWBFR was shown at the low dose($5{\mu}g/kg$) BV and the effect was most remarkable in the BV of GB34, but the others showed no recovery effect compared with the control group. Conclusions : BV pharmacopuncture does not exhibit analgesic effects in acute phase of ankle sprain. However, the recovery of ankle sprain was more effective than the natural recovery in the case of low dose of BV repeated over time. Considering this, it is presumed that it would be important to select appropriate clinic guidelines for acute phase of ankle sprain.

A Clinical Study of Bee Venom Acupuncture Therapy on the Treatment of Acute Ankle Sprain (급성기 족관절 염좌 환자의 봉약침 시술 효과에 대한 임상적 연구)

  • Objectives : This study was to evaluate the clinical effectiveness of Bee Venom acupuncture therapy for acute ankle sprain. Methods : From September 1st, 2005 to December 31st, 2005, the 60 patients who had visited Conmaul oriental medical hospital with acute ankle sprain were sample into 2 groups for this study; one group for the bee-venom acupuncture therapy combined with needle acupuncture therapy and the other group for needle acupuncture therapy only at least 3 times respectively. Both group had been treated with same additional infrared lamp, physical therapy and ankle supporter during the whole treatment period. Among 60 patients, 20 participants satisfied the whole inclusion criteria. To estimate the efficacy of treatments, Numerical Rating System(NRS) and Ankle-Hindfoot Scale(AHS) were applied before 1st and 4th treatment. Each score results were analysed and compared by Mann-Whitney test and Wilcoxon signed rank test with the level of 5% significance. Result : The NRS score in bee-venom acupuncture therapy group was increased significantly and the AHS score in bee-venom therapy group was decreased significantly compared to each of single acupuncture therapy group(P<0.01). The score change comparisons between the two groups had no significance before and after the treatment. Conclusion : Both bee-venom acupuncture therapy and single acupuncture therapy were effective to treat the acute ankle sprain. but there were no significant data to prove that bee-venom acupuncture therapy is more effective than single acupuncture therapy.

The Comparative Study on the Effect of Acupuncture Treatment with or without Indirect Moxibustion Treatment for Acute Ankle Sprain (급성 족관절 염좌에 대한 침구병행 치료와 침 단독 치료 효과의 비교 연구)

  • Objectives : The purpose of this study is to compare with the effect of acupuncture treatment with or without indirect moxibustion treatment for acute ankle sprain. Methods : This study was carried out on the 48 patients who had been treated for acute ankle sprain from June 1st to November 30th 2009 in the Dept. of Oriental Rehabilitation, Pohang Oriental Medical Hospital, Daegu Hanny University. The patients were divided into 3 groups ; Group A took near acupuncture point_needling, Group B took remote acupuncture point needling, Group C took remote acupuncture point needling with indirect moxibustion. 3 Groups had been co-treated with rest, ice, compression and elevation(RICE) therapy, physical treatment and administered Ojeok-san(Wuji-san) extract granule. To evaluating the efficiency of each treatment, Ankle-Hindfoot Scale(AHS) and Visual Analogue Scale(VAS) were applied before treatment and after 3rd treatment. Results : In results, the AHS scores were increased and the VAS scores were decreased after 3rd treatment in all group. Also, AHS scores and the VAS scores of intergroup were showed no significant change. Conclusions : In this study, there is no significant effect between only acupuncture treatment and moxibustion co-treatment for acute ankle sprain.

The Effect of Bee Venom Acupuncture(BVA) on acute Ankle Sprain : A Randomized Controlled Trial and double blinding - Pilot study (급성 족관절 염좌에 대한 봉약침 효과-무작위대조시험, 이중맹검)

  • Objective : The aim of the study was to investigate the therapeutic effect of BVA in the treatment of patients with acute ankle sprain. Design : A prospective randomized double-blind study of BVA was conducted. Setting : The study was done in the Kyungwon University Seoul Hospital from August 1st, 2004 to June 15th, 2005. Patients : 30 patients diagnosed with acute ankle sprain, especially 2nd degree on the Ankle grade pain chart(AGPC) participated in the study, who were divided into two groups (A and B) randomly by a coordinator flipping a coin. Group A and B were relevant to control and BVA group respectively, of which a coordinator never informed any other participant involved. Eventually 13 of 17 in group A and 11 of 13 in Group B finished all the process of the clinical trial. Intervention : In both group A and B, The Procedure of acupuncture treatment was made similar by appearance that four acupoints such as 坵墟(GB40), 中封(LR4), 商丘(SP5), 解谿(ST41) of the injured side were selected and Normal saline aqua-acupuncture(control, as a placebo) or BVA was done and then acupuncture at 坵墟(GB40), 中封(LR4), 商丘(SP5), 解谿(ST41), 足三里(ST36), 陽陵泉(G34) of the affected side was given again. the needles were retained for 20 minutes under the infrared rays. The treatment was given daily for a week. Outcome Measures : Ankle-Hindfoot Scale (AHS) and Visual Analogue Scale(VAS) were followed by three treatments. Statistical Analysis : Analysis regarding variations in AHS and VAS is carried out by applying Mann-Whitney test and Wilcoxon signed rank test sign test with level of significance at 5%. Results : At the end of the treatment, there was significant statistical differences between the two groups in VAS and AHS as well, while at the 3rd day only a VAS showed statistical significance. In each group, both VAS and AHS showed statistical significance along with duration of treatment. Conclusions : BVA was thought to be effective alternatives for relieving symptoms of acute ankle sprain, although further study was needed on the large scale.

With or without Acupuncture Treatment on Peroneus Muscles for the Treatment in Patients with Acute Inversion Sprain of Ankle : A Comparative Trial (급성 족관절 내반염좌의 비골근 자침에 대한 효과의 비교 연구)

  • Objectives : This study was planned to show the influences of acupuncture treatment on peroneus muscles of acute inversion ankle sprain patients. Methods : This study was carried out on the 24 patients who had been treated for their acute inversion ankle sprain from July 1 to December 31, 2010 in the department of oriental rehibilitaion medicine, Dongsuwon oriental medicine hospital. And we divided those 24 patients into 2 groups; group A took near acupuncture point needling treatment on the peroneus muscles along with common treatment, group B took common treatment only. To evaluate the efficiency of each treatment, all patients were examined with visual analogue scale(VAS) and ankle-hindfoot scale(AHS) score before treatment and after the 3rd treatment. Results : On the AHS scores, there's no significant difference between the two groups after the treatment. And on the VAS scores, group A who took near acupuncture point needling treatment on the peroneus muscles along with common treatment showed higher change of VAS than group B who took common treatment only, but it did not show statistical significance in VAS score. Conclusions : In this study, there is no significant effect between common treatment only and near acupuncture point needling treatment on the peroneus muscles along with common treatment for acute ankle sprain.

The Effect of Taping Therapy on the Expression of cFos Protein and Pain Suppression in Acute Ankle Sprain in Rats (흰쥐의 급성 발목삠에서 테이핑요법이 cFos 단백의 발현과 통증억제에 미치는 영향)

  • Objectives : The purpose of this study was to evaluate effects of taping therapy on recovery of behavioral symptoms and neural excitability of the lumbar spinal cord in rat model for ankle sprain. Methods : Adult Sprague-Dawley rats was used and divided into 3 experimental groups: normal group(n=6), ankle sprain(n=6), and ankle sprain with taping treatment(n=6). In order to induce ankle sprain the right ankle joint was injured with 4~5 repetitive over-flexions and over-extensions manually. The severity of joint pain was evaluated by measuring foot weight bearing force ratio(FWBRF) of the hind limb and the injury-induced edema formation by diameter of the joint following ankle sprain. The changes of neural excitability in the lumbar spinal cord was tested by observation of cFos protein expression, a metabolic marker for neural excitation. Results : Severity of ankle injury induced in this experiment coincided with Grade 1 ankle sprain. Compared with ankle sprain group, ankle sprain+taping showed a significant reductions of joint pain as well as of edema formation at the ankle joint following ankle sprain. There was significant upregulation of cFos-immunoreactive neurons in the lumbar spinal cord 24 hours after ankle sprain. In contrast, taping therapy resulted in significant inhibition of cFos-immunoreactive neurons in the lumbar spinal cord. Conclusions : Collectively, these results suggest that taping therapy may be an alternative therapeutic intervention for symptom recovery of the mild ankle sprain.
