Pena Operation as a Redo Procedure for Anorectal Malformation (항문직장기형 재 수술로서 Pena술식)

  • The aim of this study was to evaluate the posterior sagittal anorectoplasty (PSARP) as a re-do operation in patients who failed initial repair of anorectal malformation. Nine patients (4 boys and 5 girls) who had previous failed surgery for anorectal malformation underwent secondary operations through posterior sagittal approach. The main reasons of surgery were constipation (n=3) and persistent anatomical derangement in spite of previous correction surgery (n=6). In addition to constipation, the former group (n=3) had various anatomical defects, and the latter group (n=6), of course, had constipation in some degrees. Patients ranged in age from 2 to 19 years (median 3 years) with only one over the age of 6 years. The primary procedures included PSARP (n=8) and anoplasty (n=1). The rectum was mobilized from surrounding structures through posterior sagittal approach and anatomical defects were corrected. The rectum underwent reconstruction, which involved relocation of the rectum and anus within the limits of the intact muscle complex. Patients underwent follow-up for periods ranging from 6 to 77 months (mean 37 months) after surgery. Anatomical corrections of all the defects were successfully fulfilled in 9 patients. All the patients were satisfied with the functional results after redo-PSARP compared with the preoperative defecatory function. This study suggests that (1) some of the patients with troublesome constipation may have anatomical defects, prominent or hidden, (2) surgeons should suspect the possibility of anatomical defect as the cause of incontinence and (3) preoperative thorough investigation to reveal the anatomical defects should be included in estimating patients with severe incontinence after previous surgery and planning the correction for failed previous surgery as well.

  • Anatomical characteristics on abnormal type of bisected compression woods and light colored wood intervened between these dark colored compression woods on under part and their respective opposite woods on upper parts of cross section in Pinus densiflora S. et Z. stem were compared through optical microscopic investigation. and the obtained results were summarized as follows: 1. Tracheid length increased linearly as the number of rings from the pith increases in all investigated parts, but increasing tendency in tracheid length was similar within bisected two compression woods and their opposite woods. 2. Tracheids were the shortest in bisected two compression woods and tracheid length in intervened wood between these two compression woods was longer than in compression woods but shorter than in the opposite woods. 3. Bisected two compression woods and intervened wood between these two compression woods revealed similar features in tracheid transition from springwood to summerwood, intercellular space, tracheid entanglement, and cross sectional view of tracheid. but spiral check was observed only in these two compression woods, therefore the intervened wood between these two compression woods disclosed anatomical characteristics similar to, not typical of, compression woods.

Surgical Treatment of Chronic Lateral Ankle Instability: Repair versus Reconstruction (만성 족관절 외측 불안정성의 수술적 치료: 봉합술과 재건술의 비교)

  • Surgical treatment to restore stability in the ankle and hindfoot and prevent further degenerative changes may be necessary in cases in which conservative treatment has failed. Anatomical direct repair using native ligament remnants with or without reinforcement of the inferior retinaculum is the so-called gold standard operative strategy for the treatment of lateral ankle instability. Non-anatomical lateral ligament reconstruction typically involves the use of the adjacent peroneus brevis tendon and applies only those with poor-quality ligaments. On the other hand, anatomic reconstruction and anatomic repair provide better functional outcomes after the surgical treatment of chronic ankle instability patients compared to a non-anatomic reconstruction. Anatomical reconstruction using an autograft or allograft applies to patients with insufficient ligament remnants to fashion direct repair, failed previous lateral ankle repair, high body mass index, or generalized ligamentous laxity. These procedures can provide good-to-excellent short-term outcomes. Arthroscopic ligament repair is becoming increasingly popular because it is minimally invasive. Good-to-excellent clinical outcomes have been reported after short and long-term follow-up, despite the relatively large number of complications, including nerve damage, reported following the procedure. Therefore, further investigation will be needed before widespread adoption is advocated.

Hemodynamic Interpretation of Various Extraanatomical Bypasses: Clinical & Engineering Views

  • Axillo-bifemoral (Ax-Fem) bypass are now well accepted for bilateral iliac artery occlusion as the second best option. This extra-anatomical (unnatural) bypasses, however, have various hemodynamic liabilities affecting the patency. Hemodynamic conditions of each different type of Ax-Fem bypass were assessed with computer simulation model to determine the hemodynamically more sound type. Simulation models of five different types of Ax-Fem bypass were constructed. Our investigation based on the computer simulation models have shown distinct differences between two most popular Lazy-S type and Inverted-C type on the distribution of flow volume, shear stress and recirculation zone, etc., though both types have shown similar clinical results. Lazy-S type has shown better hemodyanmic status than inverted-C type. The theoretical advantage of "Lazy-S" type has never been adequately proved for its superiority clinically over the inverted-C type. Inverted-C type is now in more favor with clinically better results in spite of many hemodynamic liabilities including retrograde flow to the branching graft. The improvement of over-all long-term patency rate of various extra-anatomical bypasses is still warranted through proper correction of the hemodynamic liability. Even though clinical outcome of the extra-anatomical bypass has been equal regardless of the type of crossover femoral graft configuration, there are distinct differences on the hemodynamic characteristics among various types of configuration. Further hemodynamic study in the pulsatile flow status is warranted to correct hemodynamic defects with proper modification of various hemodynamic factors of each model.

  • A Goryeo-era mother-of-pearl-inlaid incense box in the collection of the National Museum of Korea, a major example of Goryeo dynasty cultural heritage, presents a comprehensive view of the production techniques and aesthetic expressions of its time. This study investigated the type of wood and the techniques used to make the framework of the box, knowledge that was necessary for its restoration. It also examined anatomical characteristics of the shell providing the mother-of-pearl. The incense box was found to be made of cedar, which suggests that there was trade in cedar with Japan. A band was attached to the lower portion of the lid by applying what the author describes as a "wrinkle-bending" technique at the corners. Wooden pegs were used to combine the top and side panels, probably for more efficient use of the thin wooden boards. The investigation of the anatomical features of the mother-of-pearl inlay identified a laminated structure of thin plates. These features are observed in certain shellfish, and further study would be required to accurately identify the type of shell.

The branching patterns and termination points of the facial artery: a cadaveric anatomical study

  • Background: The facial artery is an important blood vessel responsible for supplying the anterior face. Understanding the branching patterns of the facial artery plays a crucial role in various medical specialties such as plastic surgery, dermatology, and oncology. This knowledge contributes to improving the success rate of facial reconstruction and aesthetic procedures. However, debate continues regarding the classification of facial artery branching patterns in the existing literature. Methods: We conducted a comprehensive anatomical study, in which we dissected 102 facial arteries from 52 embalmed and formaldehyde-fixed Vietnamese cadavers at the Anatomy Department, University of Medicine and Pharmacy, Ho Chi Minh City, Vietnam. Results: Our investigation revealed eight distinct termination points and identified 35 combinations of branching patterns, including seven arterial branching patterns. These termination points included the inferior labial artery, superior labial artery, inferior alar artery, lateral nasal artery, angular artery typical, angular artery running along the lower border of the orbicularis oculi muscle, forehead branch, duplex, and short course (hypoplastic). Notably, the branching patterns of the facial artery displayed marked asymmetry between the left and right sides within the same cadaver. Conclusion: The considerable variation observed in the branching pattern and termination points of the facial artery makes it challenging to establish a definitive classification system for this vessel. Therefore, it is imperative to develop an anatomical map summarizing the major measurements and geometric features of the facial artery. Surgeons and medical professionals involved in facial surgery and procedures must consider the detailed anatomy and relative positioning of the facial artery to minimize the risk of unexpected complications.

Investigation of candidated genes for molecular characterization of DongGyeong dog populations (Gyeongju)

  • This study was carried out to investigate the characters of short-tailed dogs (DongGyeong dogs) with anatomical insights, molecular genetics in Gyeongju. The present study was conducted to further characterize of short-tailed dog population in Gyeongju. The short-tailed dog was analyzed in the distribution of 55 individual. The anatomical insights were by x-ray. For discovery of specific genes expressions were measured by Hot-start PCR analysis. Anatomy survey, the number of vertebral typical consists of more than 20. 88.9% of short-tailed dog populations consists of 3-8 vertebrates. The 54 individuals of the 47 observe the vestigial tailed of the sacrum. No detected sacrococcygeal vertebrae degradation individuals were malformation defects. The 3 genes were DEGs (differentially expressed genes) in Dong-Gyeong dogs. We succeeded in finding 3 novel DongGyeong dogs specific genes by using Hot-start PCR analysis, this study suggests that these novel genes may play role (s) in DongGyeong dogs.

Anatomical Observation of Vitrified and Glaucous Leaf from Rehmannia glutinosa Plant Produced in Vitro (지황 기내배양시 투명화된 잎과 정상잎간의 조직학적 관찰)

  • Addition of growth inhibitors such as ancymidol, ABA, chloromequat, and pachlobutrazol into MS medium had no effect to preventing vitrification in cultures of Rehmania glutinosa. Anatomical investigation revealed that vitrified thick leaf tissue in vitro had larger intercellular space with poor development of sponge and pallisade tissue compared to those of in vitro grown glaucous and field grown plants. In vitro grown glaucous leaf had smaller and round type stomata showing distinguishable guard and subsidiary cell than those of reestablished plantlets into soil whereas abnormal stomata and poor development of epicuticular wax on the surface of leaf was observed in verified plantlet.

Individual Differences in Regional Gray Matter Volumes According to the Cognitive Style of Young Adults

  • Extant research has proposed that the Object-Spatial-Verbal cognitive style can elucidate individual differences in the preference for modality-specific information. However, no studies have yet ascertained whether this type of information processing evinces structural correlations in the brain. Therefore, the current study used voxel-based morphometry (VBM) analyses to investigate individual differences in gray matter volumes based on the Object-Spatial-Verbal cognitive style. For this purpose, ninety healthy young adults were recruited to participate in the study. They were administered the Korean version of the Object-Spatial-Verbal cognitive style questionnaire, and their anatomical brain images were scanned. The VBM results demonstrated that the participants' verbal scores were positively correlated with regional gray matter volumes (rGMVs) in the right superior temporal sulcus/superior temporal gyrus, the bilateral parahippocampal gyrus/fusiform gyrus, and the left inferior temporal gyrus. In addition, the rGMVs in these regions were negatively correlated with the relative spatial preference scores obtained by individual participants. The findings of the investigation provide anatomical evidence that the verbal cognitive style could be decidedly relevant to higher-level language processing, but not to basic language processing.

A Study of Skin Biophysical Parameters and Biomarkers related to the Anatomical Site and Age in Korean Women (한국 여성의 피부 부위 및 연령에 따른 피부 측정 인자와 생물 인자 연구)

  • The skin is one of the largest organs in our body and participates in many of the human organism's physiological and pathological events. Skin function were known for self-maintenance and self-repair, mechanical and chemical stress protection, protection against UV and environmental pathogenic micro-organisms, production of vitamin D, and social and psychological function through the physical aspect. The aim of this study was to evaluate the variation of biophysical parameters and to find relation with skin biomarkers in different anatomical site and age in Korean women. About 70 healthy volunteers in age range 20 to 49 were participated in this test. Test areas were the forearms and the cheek. Investigation to determine biophysical parameters on human skin, was carried out using various non-invasive methods. For analysis to skin biomarkers, we studied to examine various biomarkers for the quantitative determination of cortisol, fibronectin, keratin-1, 10, and 11, involucrin, and keratin-6 in human face and forearm. And we measured to skin biophysical parameters for skin anatomical site and age difference with non-invasive methods. As results of measuring site, some parameters were have following significant difference, stratum corneum hydration, trans epidermal water loss and skin color (L and a value). As results of age difference, skin colors were had only significant difference with age. For cortisol, keratin-6, fibronectin, keratin-1, 10, 11 contents, there were no significant difference in age and site. However, involucrin level in the cheeks were the highest for age group 30 ~ 39 compared to other age groups. These results suggest that in individual skin condition may explain detailed skin state variation.
