  • Among several diagnostic tools for clonorchiasis (Clonorchis sinensis infection), radiologic examinations are commonly used in clinical practices. During the 2 past decades, many reports regarding imaging findings of clonorchiasis were introduced. The basic imaging finding of clonorchiasis is diffuse dilatation of the peripheral intra-hepatic bile ducts, without dilation of the large intrahepatic or extrahepatic bile ducts. By this finding, however, active clonorchiasis cannot be differentiated from cured infection. Some recent radiologic studies suggested specific findings of active clonorchiasis. Besides direct demonstration of worms, increased periductal echogenicity on sonography and periductal enhancement on dynamic contrast-enhanced CT or MR imaging possibly represent active clonorchiasis. Those images of the liver clonorchiasis are known to be correlated with worm burdens (EPG counts) in their frequency and also severity. The images of cholangiocarcinoma associated with clonorchiasis show both the tumor with obstruction images and diffuse dilatation of the peripheral intrahepatic bile ducts. Radiological images can be a good practical alternative diagnostic method of clonorchiasis.

Prevalence of Clonorchiasis and Related Factors for Inhabitants Near the Sumjin River (섬진강 유역 주민의 간흡충 감염실태 및 관련요인)

  • Purpose: This study was conducted to investigate the prevalence of Clonorchiasis and factors related to it among inhabitants of the area near the Sumjin River. Method: The data were collected from December 16, 2008 to January 10, 2009. Participants were 511 people who responded to a questionnaire. The formalinesther sedimentation technique was used to detect Clonorchiasis. The data were analyzed using descriptive statistics, $x^2$-test with the SPSS WIN 14.0 program. Results: The prevalence of Clonorchiasis by stool examination was 21.3%. There were statistically significant differences in prevalence of Clonorchiasis by gender ($x^2$=5.71, p=0.017), habit of eating raw fish ($x^2$=13.26, p<0.001), and alcohol consumption ($x^2$=10.54, p=0.032). The mean score for knowledge on Clonorchiasis was 10.68 (range 0-17) and there was no significant difference between the Clonorchiasis positive group and Clonorchiasis negative group. There were statistically significant differences in intention to change their habits of eating raw fish between Clonorchiasis positive group and Clonorchiasis negative group ($x^2$=26.10, p<0.001). Clonorchiasis positive group's intention was lower than the others. Conclusion: These results suggest that the prevalence of Clonorchiasis still high among the inhabitants in near the Sumjim River. Therefore, an effective management program for Clonorchiasis should be developed and administered.

Effects of a Clonorchiasis Prevention Education Program for Clonorchiasis Prevention Lecturers (간흡충 예방 강사를 위한 간흡충 예방교육 프로그램의 효과)

  • Purpose: The study was conducted to clarify effects of a clonorchiasis prevention education program for clonorchiasis prevention lecturers. Methods: The research adopted a single group pretest-posttest design to see effects of the educational program to prevent Clonorchis sinensis infection. The subjects of this study were 74 clonorchiasis prevention lecturers from primary health care facilities. The pretest was conducted before the clonorchiasis prevention education program and the post test was done after the 2-day program in August, 2011. Descriptive statistics, t-test, and ANOVA were conducted to analyze the data. Results: The confidence level in Clonorchiasis management activities was improved significantly from $4.1{\pm}0.53$ points before the education to $4.4{\pm}0.46$ points after the education (t=-5.117, p<.001). The knowledge level about prevention of Clonorchis sinensis was improved significantly from $16.1{\pm}2.72$ points before the education to $18.3{\pm}1.14$ points after the education (t=-6.629, p<.001). Conclusion: The results suggest that the education program was effective in improving the confidence and knowledge levels in Clonorchiasis management activities for the clonorchiasis prevention lecturers. Based on the results of this study, continuous research on how the increased knowledge and confidence levels of Clonorchis sinensis prevention affect the prevalence of Clonorchis sinensis infection.

An Epidemiological Study of Clonorchiasis Sinensis and Paragonimiasis Westermani Prevailed among the People of a Rural County (일부농촌주민(一部農村住民)의 간(肝) 및 폐흡충증에 대(對)한 역학적(疫學的) 조사(調査))

  • An epidemiological study of clonorchiasis sinensis and paragonimiasis westermani prevailed among the people of a rural county. The author studied the infection rates of of clonorchiasis sinensis and paragonimiasis westermanl in the inhabitants of Chungsong Gun, Kyungbuk Province, Korea. The examinations were carried out from March 1965 to November 1965 fowards 1.303 inhabitants whose ages were ranging from three months to over sixty years. All the inhabitants were tested intradermally with clonorchis and paragonimus antigen and then confirmed the eggs by M. G. L. technique and sputum test. The results of the examination were summarized as follows : 1. Out of the persons tested, 15.8% were infected with clonorchiasis sinensis. 2. Out of the persons tested with paragonimus antigen, 20.9% were infected with Paragonimus westermani. 3. The infection rate of paragonimiasis was higher than that of clonorchiasis. 4. The infection rate of clonorchiasis in male was 20.8% (147 out of 706), while that in female was 7.1% (58 out of 597). The infection rate of paragonimiasis in male was 21.2% (150 out of 706), while that in female was 20.1% (120 out of 597). 5. The maximum infection rates of clonorchiasis and paragonimiasis were observed among the $45{\sim}49$ age group (65.2% and 43.5%) respectively), however it was gradually decreased in the senile group. 6. By occupation, the maximum infection rate of clonorchiasis was observed in policemen (45.7%) and the maximum carrier of eggs (by sputum test) was observed in the unemployed (paragonimiasis). 7. By education, the maximum infection rate of clonorchiasis was observed in the unepmloyed (28.9%) and the maximum carrier of eggs (by sputum test) was observed in the unemployed (paragnoimiasis).

Relationship between Clonorchis sinensis Infection and Cholangiocarcinoma in Korea

  • This study provides an overview of the current status of clonorchiasis and cholangiocarcinoma (CCA), and their relationship in Korea during 2012-2020. Data were obtained from the Health Insurance Review & Assessment Service of Korea. Cluster, trend, and correlation analyses were performed. Gyeongsangnam-do and Seoul had the highest average number of cases (1,026 and 4,208) and adjusted rate (306 and 424) for clonorchiasis and CCA, respectively. The most likely clusters (MLC) for clonorchiasis and CCA were Busan/Gyeongsangnam-do/Ulsan/Daegu/Gyeongsangbuk-do (Relative Risk; RR=4.55, Likelihood Ratio; LLR=9,131.115) joint cluster and Seoul (RR=2.29, LLR=7,602.472), respectively. The MLC for clonorchiasis was in the southeastern part of Korea, while that for CCA was in the southern part. Clonorchiasis showed a decreasing trend in the southeastern districts, while increased in the southwestern districts. Cities in the central region had a decreasing trend, while the western districts had an increasing trend. In most adults (30-59), infection rate of clonorchiasis showed a significant decrease until 2018, while thereafter increased, although not significant. CCA showed a sharply decreasing tendency. The incidence of clonorchiasis and CCA were positively correlated. In general, the correlation was weak (r=0.39, P<0.001), but it was strongly positive around the 4 river basins (r=0.74, P<0.001). This study might provide an analytic basis for developing an effective system against clonorchiasis and CCA.

Hypersensitive Reaction to Praziquantel in a Clonorchiasis Patient

  • Praziquantel is the drug of choice for clonorchiasis. Since clonorchiasis is endemic in most river basins, praziquantel has been widely used for 30 years in Korea. A 54-year-old Korean woman suffered from hypersensitive reactions, such as nausea, dyspnea, rash, and urticaria after taking the first dose of praziquantel to treat clonorchiasis. She ingested one dose again and the same symptoms appeared, and she was treated at a clinic with anti-histamines. She tried one more dose with anti-histamines but found the same symptoms. Later, she was found to pass eggs of Clonorchis sinensis and medicated with flubendazole. The hypersensitive reaction to praziquantel is rare but occurs. This is the 5th case report in the world.

A Clonorchis sinensis-specific antigen that detects active human clonorchiasis (간흡충 현증감염 특이항원)

  • A Czonorchis sinensis-specific antigen in excretory-secretory product of C. sinensis (CsE) was assessed in human clonorchiasis by immunoblot. Thirty and 7 kDa antigens of CsE2, one of four different batches of CsEs reacted strongly with infection sera from clonorchiasis patients; however, the antigens reacted weakly with 6-month post- treatment sera from praziquantel-cured cases, but were still highly detected by the sera from praziquantel∼failed patients, indicating that the 30 and 7 kDa antigens can detect antibodies during an active infection. The 30 kDa antigen showed some cross reactions with sera from patients with Pcragonimus westemani and Metcfonimw vokogcujci, while the 7 kDa antigen did not, suggesting that the 7 kDa antigen has high specificity. The 30 kDa antigen reacted with some past clonorchiasis sera, whereas the 7 kDa antigen did not, supporting that antibodies to the 7 kDa antigen are not present in sera from past clonorchiasis patients. In an endemic area, 92% (23/25) of active clonorchiasis patients and 91% (10/11) of mixed infection patients with C. sinensis and M. Wokosawai had IgG antibodies to the 7 kDa antigen, while 40% (6/15) of past clonorchiasis individuals and 43% (3/7) of metagonimiasis patients cross-reacted to the antigen. These data suggest that the 7 kDa antigen in an excretory-secretory antigen may serve as a marker of an active clonorchiasis with reliable specificities in past clonorchiasis, paragonimiasis and metagonimiasis.

Thin Layer Immnunoassay (TIA) for circulating antibody detection in clonorchiasis (간흡충에 대한 항체검출을 위한 Thin Layer Immounoassay (TIA))

  • Thin layer immunoassay was carried out to demonstrate antibodies against Clcnorchis sinensis in sera from clonorchiasis patients. Saline extract of adult worm was used as antigen. TIA technique was performed as described earlier by Elwing et at. (1976), but agarose was used instead of agar. The antibody titres of sera in 60 clonorchiasis casts were higher than that of 10 healthy and 10 amoebiasis cases, but not different comparing with that of 10 paragonimiasis cases. Antibody litres in clonorchiasis gave no differences according to the age, sex, EPG in feces, eosinophilia degree of blood, level of alkaline phosphatase and transaminase (SGOT, SGPT) in sera. It is suggested that, after evaluation, the TIA might supplement or be used as an alternative to other immunodiagnostic tests already in use for the diagnosis of clonorchiasis.

A Case of Clonorchiasis with Clinical Presentation of Eosinophilic Pneumonia (호산구성 폐렴으로 발현된 간흡충증 1예)

  • Although clonorchiasis is one of the most common parasitic infections in Korea, it is unusual that the disease presents peripheral eosinophilia and pulmonary infiltrations.(eosinophilic pneumonia) A case of clonorchiasis manifested as diffuse nodular pulmonary infiltrations was presented. The patient had a mild to moderate pain on the right upper quadrant of the abdomen, cough, dyspnea, and unknown cause of marked eosinophilia (up to 71.4% of total white blood cell count). The causal organism, clonorchis sinensis was found by the identification of parasite ova in the stool We confirmed eosinophilic pneumonia with bronchoalveolar larvage analysis and transbronchial lung biopsy. With corticosteroid and praziquantel treatment, clinical symtoms and pulmonary infiltrations on the chest roentgenograms had rapidly improved. We report a case of eosinophilic pneumonia related to clonorchiasis and review the pertinent literature.

A Seroepidemiological Survey for Human Clonorchiasis on Soonchang-gun Near the Sumjin River in Korea (섬진강 유역 순창 지역 주민의 간흡충증 혈청역학적 조사)

  • Aims: This study was carried out to find the prevalence of human clonorchiasis and to know epidemiological features in a Clonorchis sinensis-endemic area in Korea. Methods and materials: The EHSA was applied for the serodiagnosis of clonorchiasis. A total of 2,591 inhabitants at Soonchang-gun county adjacent to the Sumjin River were screened through the assay. The questionnaire survey was performed for several epidemiological points related to C. sinensis infection. Data from 95 inhabitants were processed for the statistical analysis. Results: The prevalence of human clonorchiasis in Soonchang-gun was 16.1% in average from 33.6% to 7.0% according to the villages of the survey. In the riverside villages to the Sumjin River the prevalences were higher than those in other villages located far from the river. The odd ratio (OR) between men and women was 2.76, indicating that the clonorchiasis was 2.76 times more prevalent in man than woman. The ORs were 2.14 in alcoholic group, 2.40 in the group of raw-eating of fresh-water fishes, 2.44 in the people who thought they were healthy, 5.23 in the people who knew well about the clonorchiasis, and 3.32 in the people who had again raw-eating of the fishes following medication. Conclusions: These results suggested that human clonorchiasis was still highly endemic in riverside area of the Sumjin River and some predisposing factors such as raw-eating of fresh-water fishes were significantly related to the human clonorchiasis.
