Interpretation of Excess and Deficiency Syndromes(有餘不足證) Described in "Somun . Jogyongron(素問.調經論)" ("소문(素問).조경론(調經論)"의 유여(有餘).불족증(不足證)에 대(對)한 연구(硏究))

  • The "Somun Jogyongron(素問 調經論)" describes excess and deficiency syndromes. The study suggests that excess syndrome(實證) is caused by vigorous pathogenic fire(火邪)(the spirit(神)), pathogenic dryness(燥邪)(Gi(氣)), pathogenic wind(風邪)(blood(血)), pathogenic dampness(濕邪)(physique(形)) or pathogenic coldness(寒邪)(will(志)). When pathogenic fire is dominant within the body, Gi and blood becomes excessive and come out of the body, but the body cannot take them back, leading to the symptom in which the patient cannot stop laughing. When pathogenic dryness prevails, the lung(肺) cannot function properly. This means that the convergence(收斂) function of the clearing the lung and descending Gi(肅降) is deteriorated, and the patient shows symptoms of dyspnea and cough. Strong pathogenic wind increases the ascencling Gi in the liver(肝氣) and fuel angry emotion when the patient becomes upset. When pathogenic dampness is dominant, spleen(脾) function drops due to lumping effects, and the patient will experience abdominal distention(腹脹), which will disturb urination and defecation. When pathogenic coldness prevails, abdominal distention occurs due to condensating effects, and Yang Gj(陽氣) in the kidney(腎) is disturbed, leading to digestion disorders and eventually water-grain dysentery. Deficiency syndrome is caused by the lack of essential Gi(精氣) in the five viscera(五藏). Deficiency of sprit means the lack of Gi in the heart(心氣), so the patient becomes vulnerable to sadness. Deficiency of Gi means the lack of Gi in the lung(肺氣), so the patient may have breathing disorders. Deficiency of blood means the lack of Gi in the Liver(肝氣), so the patient can be easily scared. Deficiency of physique means the lack of Gi in the spleen(脾氣), making it difficult to use arms and legs. Deficiency of will means the lack of Gi in the kidney(腎氣), so Gowl syndrome(厥證) can ensue.

  • In this study, I investigated physiology and pathology of the kidney and then researched deficiency of the kidney-nourishing the Eum music therapy to apply for clinical use, because symptoms such as chronic fatigue, morbus asthenia, anxiety, dizziness, tinnitus, and amnesia are prevalent in our modern societies. These symptoms are due to deficiency of the kidney. The kidney stores vital essence and has a function to keep activities as motive power. If the kidney is abnormal in storing the essence and holding Gi, various symptoms like dizziness, tinnitus, amnesia are caused by deficiency of the kidney. In deficiency of the kidney-nourishing the Eum music therapy, Water-Gi music which can store the essence is mainly used. In case of patients with deficiency of the kidney, dizziness, tinnitus, and amnesia, we can make use of NO.3, BWV 1068, Air of J.S.Bach and Jinyangjo of Geomungo. If there are flaring-up and fidgetiness due to deficiency of vital essence, Earth-Gi music that can help the vigor of spleen and stomach should be used prior to Water-Gi music. In the concrete, Hahyeondodeuri of Yeongsanhoesang played on Geomungo can be used. The tinnitus is caused by wind-heat and fire in the gallbladder. It is good to nourish the Eum through Water-Gi music after dispelling pathogens through Jajinmori among Gayageum sanjo.

Study of The Indicators of Gi Deficiency Pattern Identification In Stroke Patients (중풍환자의 기허변증지표에 관한 연구)

  • Background and Purpose The purpose of this study was to confirm that what symptoms are adequated indicator in the Gi-Deficiency patients. Methods In the time period July. 2005 to Sep. 2006, 136 patients with a first-ever stroke admitted in the department of Internal Medicine of Daejeon University Oriental Medical Hospital in Daejeon city, Wonkwang Oriental Medical Hospital in Iksan, JeonJu city were included. Patients were hospitalized within 3 months after the onset of stroke. Stroke patients had been interviewed by resident who studied standard operation procedures in Fundamental Study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. Gi-deficiency patients was confirmed by medical specialist diagnosis, resident diagnosis, case report form analysis without a dissenting voice. Results Gi deficiency group included 23 case, Non Gi deficiency group 47 case out of 136 patients. Fatigue, weakness purse, somnolence, low voice, difficulty of uprise, pale face, pale tongue were higher among Gi deficiency group. Gi deficiency and Non Gi deficiency patients do not significantly differ in white coating tongue, light-red tongue, poor appetite, frequent sweating, teeth printed tongue. Conclusions This study was insufficiency because sample size very small. More data from prospective cohort studies will help to Korean Standard Differentiation of the Symptoms and Signs for the stroke.

Study on the Endogenous Dampness Caused by Gi Deficiency of the Spleen and Sagunja-tang (비기허(脾氣虛)로 인한 내습(內濕)의 발생과 사군자탕(四君子湯))

  • The spleen is the source of gi, blood, body fluid and plays a vital role in maintaining life. The function of the spleen is to transform food nutrients and water, and to transport them to the heart and the lung. The movement of splenic gi is marked by elevation. The spleen governs the activity of elevating the lucid. The function of transportation and transformation is usually disturbed in the state of Gi deficiency of the spleen. The main clinical manifestations of gi deficiency of the spleen can be divided into as followers: anorexia, loose stool if the digesting and absorbing functions are disturbed; phlegm and edema if dampness and water are retained due to unhealthy water metabolism. Sagunja-tang can be applied for gi deficiency syndrome of the spleen. Ingredient bakchul(Rhizoma Atractylodis Macrocephalae) and bokryeung(Poria) can be used as monarch drug to eliminate dampness and strengthen the spleen.

  • Syndrome of asthenia of spleen gi usually caused by improper diet, overstrain, emotional upsets, followed by syndrome of sinking of splenic gi. There are several pathologic categories of splenic gi deficiency syndrome. These include failure to nourish the body, failure to astringe liquid substances, failure of splenic gi to rise, gi stagnation in which gi can't disperse normally, failure of transportation. In the splenic gi deficient situation, body fluid is usually stagnated because the rest of the water absorbed from the food is transported to every part of the body by the action of splenic gi. In addition, there is abnormal sinking of clear gi, followed by fever due to gi deficiency. Bojungikgi-tang is composed of restoratives which are invigorating splenic gi and herbs which uplift splenic gi. It is mainly applicable to splenic gi deficiency syndrome accompanied by gi stagnation and fluid accumulation.

A Study on the relationship between Shin(腎) with Thyroid (신(腎)과 갑상선(甲狀腺)의 상관성(相關性)에 관(關)한 고찰(考察))

  • So far we oriental medical doctors have referred to Shin(腎) as endocrine system, especially suprarenal gland, sexual gland and autonomic nervous system, thyroid etc. as well as kidney. But the sight on thyroid is weak and the relationship with Shin(腎) hasn't been suggested clearly. The purpose of this study is to find out the relationship between Shin(腎) with thyroid. The following are the results. 1. Ki Gi(氣機) of Shin(腎) is similar to the fuction of thyroid in that they are the base of human metabolism, influencing all the metabolism of human tissue. 2. Shin Yang(腎陽) and thyroid hormone are almost identical in that they are the base of Yang Gi(陽氣), the former as source of heat energy, the latter as energy hormone. 3. Shin(腎) and thyroid hormone are almost the same in that they activate growth of human body. 4. Ki Gi(氣機) of Shin(腎) such as Ju Gol(主骨), Saeng Su(生髓), Tong uh Noi(通於腦) is similar to the effects of thyroid hormone on bones, central nervous system and hair 5. The symptoms of deficiency of Shin Eum(腎陰虛) are almost identical with those of hyperthyroidism, so the process of Yang Hwa Gi(陽化氣) caused by exuberance of Yang due to deficiency of Shin Eum(腎陰虛陽亢) is similar to excessive metabolism caused by hyperthyroidism. 6. The process of Eum Seong Hyung(陰成形) caused by preponderance of Eum due to deficiency of Shin Yang(腎陽虛陰盛) is similar to the lowering of metabolism caused by hypothyroidism. 7. Deficiency of Shin Eum(腎陰虛) is similar to hyperthyroidism, deficiency of Shin Yang(腎陽虛) to hypothyroidism. But there are major difference in edema and the fuction of intestine. To conclude, Ki Gi(氣機) of Shin is similar to the function of throid in many respects. I hope that there will be further studies on the relationship beteen thyroid malfuction with deficiency of Shin Yang(腎陽虛) or of Shin Eum(腎陰虛) in the future.

A Study on Gak-Gi(脚氣) in Comparison with Beriberi (각기(脚氣)에 대한 고찰(考察) -Beriberi와의 비교를 중심으로-)

  • Objectives :The Korean Medical concept Gak-Gi(脚氣) is understood as being identical to Beriberi and Vitamin B1 deficiency, without the necessary investigation that is required in such identification. Therefore this study aims to systematically compare these two concepts for thorough investigation. Methods : Research was carried out in two directions. First, the cause, symptoms, treatment principles of Gak-Gi(脚氣) in the medical texts were analyzed. Second, the process of the Beriberi theory development and its identification with Gak-Gi(脚氣), medical information on Beriberi, and the basis of various diseases that have been linked to Gak-Gi(脚氣) were studied. Results of the two directions were used to compare Gak-Gi(脚氣) and Beriberi. Results : Gak-Gi(脚氣) and Beriberi overlap in many aspects. However, unlike Beriberi which is understood as nutrient deficiency essentially, Gak-Gi(脚氣) can be caused by exterior pathogens such as wind toxin, while its different manifestations and treatment principles cannot be fully explained by nutrient deficiency. Conclusions : Gak-Gi(脚氣) could be understood as a type of syndrome in which causes and symptoms are connected, where symptoms start in the legs, moving upwards. It cannot be identified with Beriberi, which has been interpreted as a single disease.

Review on the Causes of Headache in Hyungsang Medicine (두통(頭痛)의 원인에 따른 형상의학적(形象醫學的) 고찰 -동의보감(東醫寶鑑) 두문(頭門)을 중심으로)

  • The followings are concluded from the treatment of headache in Hyungsang medicine, focussed on 11 kinds of headaches in Donguibogam. Headache is classified into overall headache and migraine according to the affected region. The causes are divided into exogenous affection and internal injury; The former brings on headache due to Wind-Cold and headache due to Damp-Heat. The latter, reversal headache, headache due to adverse rising of phlegm, headache due to regurgitation of Gi, headache due to excessive Heat, headache due to excessive Damp, true headache, and alcoholic headache. Headache due to internal injury generally tends to show deficiency syndrome with external affection. Headache due to exogenous affections is common to those who have big head or white skin and to Bangkwang type, and woman. The primary causes are Wind-Cold and Wind-Heat. When the body is observed in the perspective of eight phases, Damp-Heat is to be produced in the front, and Dry-Damp, in the back. Headache due to Damp-Heat is susceptible to Yangmyeong meridian type whose body develops more in the front and to woman. In the perspective of the upper and the lower, Yangdu(that is, head) is related to Eumdu(that is, glans of penis). Headache is also caused by the problems of Eumdu ,such as deficiency of Essence in man, pathologic change of uterus in women, and San syndrome in lower abdomen. In the case of man, headache is frequently severe and difficult to treat because head is a root for man. Disharmony of Gi and blood between the right and the left brings out migraine and headache due to regurgitation of Gi. Migraine is usually accompanied by symptoms of exogenous affection and often afflicts Gi-type, Shin-type, Soyang meridian type, deer type, and Dam-type. Headache due to regurgitation of Gi is brought by Gi deficiency or blood deficiency so that symptoms of exogenous affection do not show. It is mainly common with old people and those who have sunken eyes induced by deficiency of stomach Gi. In the perspective of the upper, the middle, and the lower, the pathologic change of head, chest and abdomen also bring about headache. The pathologic cause of head is Wind-Heat ,which triggers overall headache, migraine, headache due to Wind-Cold, headache due to excessive Heat, The pathogen of chest is phlegm-Fire and brings out headache due to Damp-Heat and headache due to adverse rising of phlegm. The pathologic factor in abdomen is Cold-Damp and produces headache due to adverse rising of phlegm and headache due to excessive Damp. In case of women, headache is generally caused by phlegm-Fire and retention of undigested food.

  • Through the thought of etiology and pathogenesis of convulsion disease in past document, we concluded as follow. 1. Convusion disease brings about some symptoms such as myotonia, neck stiffness, myospasm of four limbs, and in the ancient times it was called in Gye-Jong, Chu-Pung(抽風), Chi. 2. Etiologies of convulsion disease are external invasion of Pung-Han-Seub(風寒濕) and Ybul-Sa(熱邪), mistreatment, great loss of blood, deficiency of Gi-Hyul(氣血), stagnation of phlegm and blood. 3. There are four pathologic cases which arise convulsion disease. They are muscular denutrition from meridian stagnation by external invasion, muscular denutrition of heat injury, stagnation of phlegm and thrombus in meridian, muscular denutrition with deficiency of Gi-Hyul(氣血). 4. The treatment methods of convulsion disease are divided into three. If caused by external invasion, the methods are San-Han-Hae-Gi(散寒解肌), Hwa-Yung-Jo-Joong(和營調中). If caused by deficiency of Gi-Hyul(氣血), the method is Bo-Gi-Ik-Hyul(補氣益血). If caused by stagnation of phlegm and blood, the methods are Hwal-Hyul-So-Eo(活血消瘀), Do-Dam-Gun-Bi(導痰健脾).

  • Objectives The purpose of this study is to introduce oriental medicine music therapy related to the treatment of ADHD. Methods This study observed ADHD from the oriental medical point of view and tried to treat this disease with the oriental medicine music therapy. Results ADHD is caused by excess of Yang energy, flaming-up of fire, blood deficiency of the heart and the spleen, deficiency of the kidney essence, and instability of emotions. The ADHD patients with the case of excess of Yang energy and flaming-up of fire, patients were played Fire-Gi rhythm as a treatment, and they listened to Metal-Gi music. Whereas, the ADHD patient with blood deficiency of the heart and the spleen, invigoration of vital energy music therapy can be used. Another case of ADHD patients with deficiency of the kidney, Water-Gi rhythm and replenishing vital essence music therapy can be used. According to the patients' seven modes of emotions, the proper music should be chosen. Conclusions Oriental medicine music therapy can be a new type of treatment for ADHD patients.
