Delayed versus Delayed-Immediate Autologous Breast Reconstruction: A Blinded Evaluation of Aesthetic Outcomes

  • Background The technique of delayed-immediate breast reconstruction includes immediate insertion of a tissue expander, post-mastectomy radiation, followed by reconstruction. The aesthetic benefits of delayed-immediate reconstruction compared to delayed reconstruction are postulated but remain unproven. The purpose of this study was to compare aesthetic outcomes in patients following delayed and delayed-immediate autologous breast reconstruction. Methods A retrospective analysis was performed of all patients who underwent delayed or delayed-immediate autologous breast reconstruction by the senior author from 2005 to 2011. Postoperative photographs were used to evaluate aesthetic outcomes: skin quality, scar formation, superior pole contour, inferior pole contour, and overall aesthetic outcome. Ten non-biased reviewers assessed outcomes using a 5-point Likert scale. Fisher's Exact and Wilcoxon-Mann-Whitney tests were used for comparative analysis. Results Patient age and body mass index were similar between delayed (n=20) and delayed-immediate (n=20) cohorts (P>0.05). Skin and scar quality was rated significantly higher in the delayed-immediate cohort (3.74 vs. 3.05, P<0.001 and 3.41 vs. 2.79, P<0.001; respectively). Assessment of contour-related parameters, superior pole and inferior pole, found significantly improved outcomes in the delayed-immediate cohort (3.67 vs. 2.96, P<0.001 and 3.84 vs. 3.06, P<0.001; respectively). Delayed-immediate breast reconstruction had a significantly higher overall score compared to delayed breast reconstructions (3.84 vs. 2.94, P<0.001). Smoking and the time interval from radiation to reconstruction were found to affect aesthetic outcomes (P<0.05). Conclusions Preservation of native mastectomy skin may allow for improved skin/scar quality, breast contour, and overall aesthetic outcomes following a delayed-immediate reconstructive algorithm as compared to delayed breast reconstruction.

Delayed Cranial Nerve Palsy after Microvascular Decompression for Hemifacial Spasm

  • Objective : Microvascular decompression (MVD) for hemifacial spasm (HFS) is a safe and effective treatment with favorable outcomes. The purpose of this study was to evaluate the incidence of delayed cranirve (VI, VII, and VIII) palsy following MVD and its clinical courses. Methods : Between January 1998 and December 2009, 1354 patients underwent MVD for HFS at our institution. Of them, 100 patients (7.4%) experienced delayed facial palsy (DFP), one developed sixth nerve palsy, and one patient had delayed hearing loss. Results : DFP occurred between postoperative day number 2 and 23 (average 11 days). Ninety-two patients (92%) completely recovered; however, House-Brackmann grade II facial weakness remained in eight other patients (8%). The time to recovery averaged 64 days (range, 16 days to 9 months). Delayed isolated sixth nerve palsy recovered spontaneously without any medical or surgical treatment after 8 weeks, while delayed hearing loss did not improve. Conclusion : Delayed cranial nerve (VI, VII, and VIII) palsies can occur following uncomplicated MVD for HFS. DFP is not an unusual complication after MVD, and prognosis is fairly good. Delayed sixth nerve palsy and delayed hearing loss are extremely rare complications after MVD for HFS. We should consider the possibility of development of these complications during the follow up for MVD.

Regulating Delayed ACK Timeout to Construct High Speed Transmission (고속 통신 구현을 위한 Delayed ACK Timeout 값의 유동적인 적용 연구)

  • Delayed ACK is an algorithm implemented to decrease the number of ACK transmissions by delaying an ACK transmission and by waiting for additional ACK instead of transmitting the ACK immediately. By using Delayed ACK in TCP, the congestion of network and the overhead of handling ACKs can be reduced. Waiting time of Delayed ACK is defined as the Delayed ACK timeout, and it is fixed in Window OS basically. However, the fixed value of Delayed ACK timeout is not suitable for dynamic network circumstance, and it may cause unnecessary delay. This paper proposes a regulating Delayed ACK timeout algorithm to reduce the aforementioned unnecessary delay caused by the lengthy default value of the Delayed ACK timeout. We confirm that TCP transmission performance in dynamic network circumstance can be improved using the proposed algorithm.

Trivial Trauma and Non Pathological Delayed Splenic Rupture: A Case Report (경미한 외상에 의한 지연성 비장 손상)

  • Although a majority of patients with splenic rupture present acutely, up to 15% present with a delayed rupture days to weeks following a substantial abdominal injury. The mortality for patients presenting with acute splenic rupture is approximately 1% whereas that associated with delayed rupture approaches 15%. Although many cases of delayed splenic rupture have been reported, the majority of those reports present delayed splenic rupture associated with an underlying systemic disorder such as liver or kidney disease, or another hematologic disorder. We found a delayed splenic rupture case that documented the normal spleens of young healthy soldiers after trivial abdominal trauma, and we have had successful treatment experience with delayed rupture of a normal spleen after trivial trauma. Therefore, we want to review the literature and discuss the phenomenon of delayed rupture of the spleen following trivial trauma.

The Effect of Delayed Compaction on Unconfined Compressive Strength of Lime Soil Mixtures (석회혼합토의 지연다짐이 압축강도에 미치는 영향)

  • In order to investigate the effect of delayed compaction on the strength of the lime soil mixtures, labroatory test with two kind of soils was performed at four levels of lime content, at five levels of water content, and at six love's of delayed times. The results are summarized as follows; 1. Maximum dry density and optimum moisture content decreased with increase of the delayed times. The decreasing rate of those values at the earlier delayed time were large, and those values showed almost constant after about four hours of delayed time. 2. According to the increase of the delayed time, the decreasing rate of maximum dry density and optimum moisture content was large ia S-2 sampl, but was a little in S-1 sample. 3. Unconfined compressive strength of lime soil mixtures decreased with the increase of the delayed time, and the decreasing rate of its strength increased with the increase of the lime content. 4. Water content corresponding to the maximum strength was a little higher than the optimum moisture content along the increase of lime content and delayed time but its value was large in fine soil.

The Consequence of Delayed Diagnosis of an Occult Hip Fracture (잠행 고관절 골절의 진단 지연의 결과)

  • Purpose: Occult hip fracture is not evident on radiographs and the diagnosis is often missed or delayed. This study was undertaken in order to identify the clinical characteristics and complications of patients with a delayed diagnosis of an occult hip fracture. Methods: We retrospectively reviewed patients with occult hip fracture who had normal findings on initial radiographs, the diagnosis was made on additional studies between August 2006 and February 2012. Patients who were diagnosed as having occult hip fractures at the first visit were categorized as non-delayed group and those who were not diagnosed at the first visit were categorized as delayed group. Results: Non-delayed group included 43 patients (86%). In the remaining 7 patients (delayed group), the diagnosis was delayed by a mean of 9.6 days (range 3~19 days). Patients who were diagnosed with an occult fracture on the initial visit presented later than those with a delayed diagnosis (41/43 .vs. 3/7, p=0.002). Other clinical features were no difference between the two groups. Patients in the delayed diagnosis group were more likely to have fracture displacement (4/7 .vs. 0/43)15patients in non-delayed group (34.9%) needed operative treatment, whereas all delayed patients (100%) needed operative treatment. Conclusion: A delayed diagnosis of occult hip fractures was associated with increased rate of displacement and operation. In patients suspected of having occult hip fractures, additional studies should be recommended.

  • In this paper, a time-delayed controller for position control of humanoid robot arms is designed and implemented on a field programmable gate array(FPGA) chip. The time-delayed control algorithm is simple to implement, and robust to reject disturbances. The time-delayed control method uses the one sample time-delayed previous information to cancel out uncertainties in the system. Since the sampling time is so fast with the current hardware technology, the time-delayed controller can be implemented. However, inertia values should be correctly estimated to have the better performance. The position tracking tasks of humanoid robot arms are tested to compare performances of several control algorithms including the time-delayed controller.

Risk factors of delayed diagnosis of acute appendicitis in children: for early detection of acute appendicitis

  • Purpose: This study examined the risk factors of a delayed diagnosis of acute appendicitis in children undergoing an appendectomy. Methods: This retrospective study involved children aged below 18 years, who underwent an appendectomy. After dividing them into a delayed diagnosis group and nondelayed diagnosis group according to the time interval between the initial hospital visit and final diagnosis, the risk factors of delayed diagnosis were identified using logistic regression analysis. Results: Among 712 patients, 105 patients (14.7%) were classified in the delayed diagnosis group; 92 patients (12.9%) were diagnosed using ultrasonography (US), and both US and computed tomography were performed in 38 patients (5.3%). More patients in the delayed diagnosis group underwent US (P=0.03). Spring season and prior local clinic visit were significantly associated with a delayed diagnosis. Fever and diarrhea were more common in the delayed diagnosis group (fever: odds ratio [OR], 1.37; 95% confidence interval [CI], 1.05-1.81; diarrhea: OR, 1.94; 95% CI, 1.08-3.46; P<0.05). These patients showed symptoms for a longer duration (OR, 2.59; 95% CI, 1.78-3.78; P<0.05), and the admission course (OR, 1.26; 95% CI, 1.11-1.44; P<0.05) and C-reactive protein (CRP) levels (OR, 1.47; 95% CI, 1.19-1.82; P<0.05) were associated with the delayed diagnosis. Conclusion: To decrease the rate of delayed diagnoses of acute appendicitis, symptoms such as fever and diarrhea, seasonal variations, admission course, and CRP levels should be considered and children with a longer duration of symptoms should be closely monitored.

  • This paper presents robustness propertis of the Kalman Fiter and the associated LQG/LTR method for linear time-invariant output-delayed systems. It is shown that, even for minimum phase plants, the LQG/LTR method can not recover the target loop transfer function. Instead, an upper bound on the recovery error is obtained using an upper bound of the solution of the Kalman filter Riccati equations. Finally, some dual properties between output-delayed systems and input-delayed systems are exploited.

Effects of Delayed CA Treatment on Fruit Quality of 'Fuji' Apples During Storage (지연 Controlled Atmosphere(CA)저장이 저장 중 '후지' 과실의 품질에 미치는 영향)

  • This study was conducted to investigate the effect of delayed controlled atmosphere (CA) storage on fruit quality and the incidence of storage disorders during CA storage of 'Fuji' apples (Malus x domestica Borkh.). In the first year of experiment, 'Fuji' apples showed 40% of watercore disorder at the commercial harvest time (Oct. 22). Twenty days delayed CA storage caused to develop the incidence of flesh browning while 30 days delayed CA storage did not. In the second year of experiment, watercore was increased with delaying harvest time. As the estabilishment of delayed CA storage was delayed, the incidence of flesh browning increased at 10 to 30 days delayed CA storage but 40 days delayed CA storage did not have any flesh browning incidence. Respiration rate increased with extending the storage duration. Delayed CA storage reduced respiration rate but was not different compared to the rapid CA. Ethylene production rate was lower in delayed CA storage than in cold storage but higher in delayed CA storage, compared to rapid CA storage. Titratable acidity was lower in delayed CA storage as CA storage progressed. Firmness was not different between rapid CA storage and delayed CA storage after 8 months of CA storage. Therefore, the results indicate that delayed CA storage should be a promising approach to reduce the risk development of flesh browning and watercore during CA storage of 'Fuji' apples.
