Working Patterns and Job Satisfaction in Primary Health Practitioners (보건진료전담공무원의 업무분석과 직무만족도)

  • Purpose: The purpose of this study was to identify the frequency and duration of primary health practitioners' work, and their job satisfaction, and to confirm differences in work and job satisfaction by type of primary health care post. Methods: Work frequency, duration of work, and job satisfaction were estimated by 371 primary health practitioners. Chi-square test and t-test were used to identify the differences in working patterns and job satisfaction by type of primary health care post. Results: Primary health practitioners were found to spend more time working with the elderly population than with students, pregnant women, children, people with disabilities, and multicultural families. Those in costal areas were more concerned with students than those working inland. In the latter group of practitioners, more time was spent working with patients with chronic diseases, pregnant women, women, children, multicultural families, and mental health clients. Also, the job satisfaction of inland primary health practitioners was significantly higher than that of costal practitioners. Conclusion: It is necessary to identify the characteristics of primary health practitioners' work, focusing on changes in the medical service environment. Furthermore, it is necessary to provide job training according to type of primary health care post, as practitioners' approaches should differ between posts.

Changes in Work Activities of Community Health Practitioners by Time Period (보건진료원 업무활동의 변화과정 : 문헌사료를 중심으로)

  • Purpose: The purpose of this study was to investigate the changing process of work activities of community health practitioners in medically vulnerable areas. Methods: For the study an historical research method was used, based on literature data and collected literature data related to the work of community health practitioners from 1940 to 2013. Results: There were 45 records according to the literature data about the work activities of the practitioners. Analysis of the process of work activities by time period was based on the historical records: first, the introduction period (1940~1980) saw the "community health practitioners begin their work activities"; second, the development period (1981~1997) involved "provision of comprehensive health and medical service"; third, the stagnation period (1998~2005) characterized by "attempts at new role changes in a crisis"; fourth, the expansion period (2006~2011) "focused on the work of health promotion"; and finally, the stable period (2012~the present) observed "work activities of community health practitioners recognized." Conclusion: Results showed five topics in the process of work activities of community health practitioners by time period. The results offer basic data to investigate work activities of these practitioners and enable discussions about the future of community healthcare.

Job Satisfaction and Related Factors among Community Health Practitioners (보건진료원의 직무만족에 영향을 미치는 요인)

  • Purpose: This study was done to identify factors related to job satisfaction according to socioeconomic status and job related factors among community health practitioners. Method: A total of 91 community health practitioners participated in the survey. The survey was performed in November, 2008. The survey instruments included socioeconomic factors, work related factors and job satisfaction. The statistical analysis included frequencies, percentages, means, standard deviation, t-test, ANOVA, Scheffe test and multiple regression. Results: The mean age was $44.03{\pm}5.21$ years, 35.1% graduated from baccalaureate program. Job satisfactions related to wage level and job description had relatively lower score than other items. Job satisfaction was significantly different by education level, marital status, and motivation for choosing to be a community health practitioner. Conclusion: Characteristics of community health practitioners are changing and the Korean government needs to up-date job descriptions and welfare policies according to the higher quality of public health practitioners.

Looking Back the Past 30 Years: Activities and Achievements of Community Health Practitioners in Rural and Remote Areas in Korea (보건진료원들이 지각하는 보건진료소 30년간의 활동경험과 성과)

  • Purpose: The purpose of this study was to identify community health nursing activities that community health practitioners perceive and their achievements in community by reviewing the community health nursing activities that community health practitioners have done for the last 30 years. Methods: This study was a qualitative study. Thirty one community health practitioners were interviewed using the focus group interview method and data were analyzed using content analysis. Data were collected from April to June in 2011. Results: Community health practitioners perceived themselves as "Community Vitalizers" and 9 categories were identified. They were 'ground to explore new nursing areas', 'assimilation to community', 'ground to establish community diagnosis', 'everyday life health management in the community', 'increased accessibility to medical services for the residents', 'enforced health practices for the residents', 'reinforced self-reliance of community', 'commitment to making a happy village' and 'mental fence of the community.' Conclusion: This study was meaningful in that it explained the unique identity of the community health practitioners and could be used as important basic materials in the process of re-establishment of the roles of Health Offices. Hereafter in-depth study on community competence reinforcement should be made to identify the roles of community health nurses.

Geographical Distribution of Physician Manpower under the Influence of Public Health Physician (의사인력의 지역간 분포양상 및 공중보건의사의 영향)

  • The purpose of this research is to assess the geographical distribution of physicians and dentists and the degree of maldistribution of the physician. Data were obtained form the Korean Medical Association's report on physicians registry and census for 1990. To assess the degree of disparity in the rural-urban distribution of physician manpower and to identify changes in the distribution pattern, the Gini index of concentration was used. Major findings are as follows; 1. Urban-rural disparity in the distribution of physician manpower exists in all categories of manpower, i.e. physician, dentist, oriental medical doctor, general practitioner, medical specialist, practitioner, public health physician and public health dentist. Urban area which had 74.4% of nation's population, accounted for over 90% of all physician manpower. 2. In terms of the ration of physician manpower per 10, 000 population, in urban area, they were 8.2 physicians, 2.7 general practitioners, 5.5 specialists, 3.0 practitioners, 1.8 dentists and 1.3 oriental medical doctors. In rural area, the ratios were 1.4 physicians, 0.6 general practitioners, 0.9 specialists, 1.0 practitioners, 0.4 dentists and 0.4 oriental medical doctors. 3. Gini indicies computed to measure inequality of physician manpower distribution were 0. 3675 for physicians, 0.3372 for general practitioners, 0.3338 for specialists, 0.2263 for practitioners, 0.3132 for dentists and 0.3293 for oriental medical doctors. 4. Inspite of increase in the number of physician manpower, urban concentration of physician manpower intensified from 1980 to 1990. However, the Gini index for all physician manpower fell by 18.3~36.7% from 1980 to 1990, indicating more even distribution. 5. In rural area, the public health physicians and dentists had increased the ratios of physicians, general practitioners, practitioners and dentists per 10, 000 population remarkebly, and had decreased the Gini indicies of physicians, general practitioners, practitioners and dentists. Thus, public health physicians and dentists contributed to improve the distribution of physician manpower in rural area. Based on the results of this study, long-term and rational manpower policies should be developed to solve the problem of geographical maldistribution of physician manpower as well as short-term policy for inducing physicians to the rural areas.

The COVID-19 Correspondence Work Experience of Community Health Practitioners (보건진료 전담공무원의 코로나19 대응경험)

  • Purpose: This study aims to describe and understand the meaning and nature of community health practitioners' coping with COVID-19. Methods: In-depth interviews were conducted with 12 community health practitioners from August to October 2021, to describe and understand the nature of their coping with COVID-19. The collected data were examined and described based on Colaizzi's phenomenological method. Results: 16 theme clusters and five categories were derived from the community health practitioners' experience of coping with COVID-19. The categories derived were: a war that began without notice, a variety of correspondence tasks assigned, struggling to fulfill given roles, correspondence tasks becoming more systematic, and a fight that has not yet ended. Conclusion: Community health practitioners handled various response tasks related to COVID-19, and faithfully fulfilled their professional roles while performing the primary task of medical care. Hence, attention is required on issues related to the establishment of the national emergency healthcare system and improvement of professional competence of community health practitioners even after the COVID-19 pandemic is over. Furthermore, it is necessary to make ceaseless efforts to address those issues and, in order to do so, social interest and institutional support are needed.

Cognitive Discrepancies of the Mutual Development Strategies of Western and Oriental Medicine in Korea - A Comparison between Western and Oriental Medical Practitioners and Health Professionals - (양.한방 상호보완 방안에 관한 인식도 연구 - 의사, 한의사 및 보건의료전문가간 비교를 중심으로 -)

  • This study is conducted to compare how the Western.Oriental medical practitioners and health professionals perceive reciprocal development strategies respectively of Western.Oriental Medicine in Korea. A total of 3,273 persons were questioned by direct distribution, e-mail, and mail with a self-developed questionnaire. Of those questioned, 362 responded (the response rate of 11.1%), and of them n persons (Western medical practitioners 206, Oriental medical practitioners 90, and health professionals 64) were analyzed with SPSS for Windows. The results were as follows: The need for legislation on the mutual development system for Western.Oriental Medicine was recognized by 66.9% (218 persons) of respondents. Among them, largest group who agreed with this need was the health professionals. Western medical practitioners chose the "the difference of approach methods on the diseases between Western.Oriental Medicine" as their response, and Oriental medical practitioners & health professionals selected "the indifference or bias of Western medical practitioners"as the reasons for the inactivity in developing a mutual system of Western.Oriental Medicine. Therefore, Western medical practitioners and health professionals selected the category of "the reformation of educational system", while Oriental medical practitioners selected the category of "the activation of joint research on Western.Oriental medical care" as the most important condition for setting a precedent. Also, Western medical practitioners preferred "cooperative health care system for Western medical care supported by Oriental medical care", but Oriental medical practitioners and health professionals preferred “cooperative health care system of Western.Oriental medicine on equal terms" In conclusion, Western '||'&'||' Oriental medical practitioners have to make every effort to close the gap between differing views through mutual understanding and respect if joint research of Western.Oriental medical care is to become a reality. The government should continuously enforce the health policy on development of a legal and systematic infrastructure for mutual development strategy of Western.Oriental Medicine in Korea.strategy of Western.Oriental Medicine in Korea.

Improvement for Work Status of Community Health Practitioners in Island Areas (전남 도서지역 보건진료원 업무환경 개선방안)

  • Purpose: The purpose of this study was to assess the work status of community health practitioners in island areas. Methods: Participants in this study were 59 community health practitioners in primary health care posts located on islands in J Province. Data were collected during December, 2011 and analyzed using SPSS 17.0 program. Results: Characteristics of the majority of community health practitioners in the island areas was that they were over 50 years old, married, and had 10 years job experience, The population under their jurisdiction was less than 200 and they had an average of 6-10 patients/day and worked less than 10 hours/day. Health education and practice were the most common of task, and they had various types of paperwork to complete. They visited senior citizens living alone, managed medications, gathered health data for research, waited for night treatment of emergency patients, carried out health promotion programs, and visited seriously ill patients at home after work hours. Conclusion: Results of the study indicate that in order to improve the role and function of the primary health care posts and to boost morale, the practitioners' opinions should be actively accepted by the community health practitioner association and competent government agencies.

Roles of Primary Health Practitioner and Activities to Increase the Job Performance (보건진료원 업무현황 및 업무향상활동)

  • Purpose: The purpose of the study was to analyze the present status of community health practitioner activities and efforts to improve the job performance. Method: This study employed descriptive exploratory design. The sample consisted of 1,892 community health practitioners which was 90 % of population of community health practitioners. The data was analyzed by using SPSS Windows 10.0. Result: The most popular activities of community health practitioners were women's health, chronic degenerative disease management, elderly health, and outpatient care of primary health care. The activities that community health practitioners want to strengthen were outpatient care, disease prevention, rehabilitation, health promotion, and counseling. The efforts to improve the job performance were consult to other health care professionals, discussion with patient and families to choose effective treatment options. Community health practitioners knew that they were exposed to malpractice and hould try to make many efforts to improve their performance. Conclusion: The roles and activities of community health practitioners should be changed to the shift of health care environment and systems.

Trends in Nursing Research on Community Nurse Practitioners as Reported in Korea from 1980 to 2006 (보건진료원 관련 국내 연구논문 분석)

  • Purpose: The purpose of this study was to analyze the trends of research on Community Nurse Practitioners in Korea in order to suggest future directions for research on Community Nurse Practitioners. Method: A total of 109 studies published from 1980 to 2006 were analyzed according to the year of publication, research design, journal type, subject type, and major study concepts. Results: 1) The number of studies related to Community Nurse Practitioners increased by 28.4% from 1985 to 1989, and also by 28.4% from 2000 to 2004. 2) The most frequently used research design in the studies was a descriptive study design (90.8%). 3) There were 43 master's theses on Community Nurse Practitioners, and 16 studies in the Journal of Korean Community Nursing. 4) The most frequent participants were Community Nurse Practitioners (68.9%). 5) The most frequent major study concept was job analysis of Community Nurse Practitioner programs (27.5%). Conclusion: It is suggested that there should be more studies on Community Nurse Practitioner programs in the future and, particularly, experimental studies to exam the effects of interventions on health promotion by Community Nurse Practitioners are considered necessary.
