보건소 모성과 어린이 건강관리사업의 효율적 운영을 위한 조직구축 연구 (Development of Organization System for Health Management Program of Maternal-child in Public Health Centers)

  • Purpose: The purposes of this study are: to evaluate the effectiveness of organizations engaged in MCH programs; and to suggest the executive organizational system of maternal and child health (MCH) services linked to HP services at public health centers. Method: This study analyzes data on the present conditions and evaluates the organizational effectiveness of MCH workers at public health centers nationwide. The organizational effectiveness of MCH organization is assessed by MCH workers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Result: The results are as follows. Of public health centers, 61.9% have a MCH organization for their MCH programs. For the other 38.2%, MCH services are undertaken by HP organization. Executive organizational systems for MCH services at leading MCH health centers are classified into three types: Type I (provides MCH and HP services through MCH organizations); Type II (provides MCH and HP Services through HP organizations) ; and Type III (provides MCH services through MCH organizations and HP services through HP organizations). The evaluation of the organizational effectiveness of MCH organizations shows that Type II is the most effective in terms of teamwork, autonomy, service quality and resource utilization. Conclusion: This study suggests that executive organizations for MCH and HP services for women and children should be established in accordance with lifetime health programs in order for public health centers to utilize the limited MCH and HP resources efficiently.

Do Fraud Investigations Impact Healthcare Expenditures of Medical Institutions?: An Interrupted Time Series Analysis of Healthcare Costs in Korea

  • Background: The aim of our study was to review the findings of health insurance fraud investigations and to evaluate their impacts on medical costs for target and non-target organizations. An interrupted time series study design using generalized estimation equations was used to evaluate changes in cost following fraud investigations. Methods: We used National Health Insurance claims data from 2009 to 2015, which included 20,625 medical institutions (1,614 target organizations and 19,011 non-target organizations). Outcome variable included cost change after fraud investigation. Results: Following the initiation of fraud investigations, we found statistically significant reductions in cost level for target organizations (-1.40%, p<0.001). In addition, a reduction in cost trend change per month was found for both target organizations and non-target organizations after fraud investigation (target organizations, -0.33%; non-target organizations of same region, -0.19%; non-target organizations of other regions, -0.17%). Conclusion: This study suggested that fraud investigations are associated with cost reduction in target organization. We also found similar effects of fraud investigations on health expenditure for non-target organizations located in the same region and in different regions. Our finding suggests that fraud investigations are important in controlling the growth of health expenditure. To maximize the effects of fraud investigation on the growth of health expenditure, more organizations needed to be considered as target organizations.

의료서비스조직 기혼 여성근로자의 직장-가정 갈등, 조직 지원이 조직유효성에 미치는 영향 (The Influence of Work-Family Conflict and Organizational Support on Organizational Effectiveness among Married Working Women of Health Services Organizations)

  • The purpose of this study is to shed light on the effect of work-family conflict on organizational effectiveness, focusing on the moderating effect of organizational support among married working women of health services organizations. Data were collected from 149 married working women of health services organizations located in Busan Metropolitan areas by structured self-administered questionnaire. Main results of this study is as the following: First, based on the type of work-household conflicts, namely time-based conflict(2.82 points), strain-based conflict(2.81 points) and behavior-based conflict(2.69 points), working women mainly experienced time and strain-based conflicts. The level of work-household conflicts was significantly higher among younger groups, highly educated, nurses, and regular workers. Second, the perceived organizational effectiveness found to be an above-average. Especially scores were high in the subcategories of customer orientation(3.84), followed by organizational commitment (3.42) and job satisfaction(3.19). The level of organizational effectiveness was significantly higher among older groups, medical technician and administrative job holders, day-time workers, and higher income groups. Third, the results of the regression analysis on the effects of work-household conflicts on organizational effectiveness showed that strain-based conflicts have a significant negative effect on organizational effectiveness such as job satisfaction and organizational commitment. Fourth, organizational support found to have a strong controlling effect for strain-based conflicts on organizational effectiveness. Above results imply that practical family-supportive policies for lessening the work-household conflicts is crucial for enhancing organizational effectiveness in health services organization.

순회이동보건교육사업의 필요성과 방향 (Importance of Mobile Health Education Teams)

  • For happy life, it is first of all essential to live healthily although wealth is important too. Korean government has enforced National Health Insurance Program and has improved it every year. However, health security still leaves something to be desired in Korea. Although Korean government also enacted National Health Promotion Law in 1995, the national health promotion policy has not been effectively carried out because of the problems of institution related to policy implementation and expenses of organizations. Also, community health services have failed to realize the anticipated results. Especially, health education services which are deeply related to national health consciousness are not well accomplished. Therefore, the average life span of Korean people is 73.5 years which display the level of the developing countries, while it is 74.7 years in the developed countries. Various health education services which Korean government and private organizations are carrying out are not activated. At this time, national health behavior and Quality of life will be greatly enhanced if "Mobile Health Education Teams" play active roles with new image, visiting vulnerable areas to health problems all over the country.e country.

  • Objectives: This study was performed to suggest the roles and professional competencies of health education specialists to improve the efficacy of health promotion activities in public health organizations. Results and Conclusion: Based on the advanced cases of utilizing health education specialists in international and domestic public health organizations, five key roles of health education specialists were proposed. They included developing and applying behavior change strategies necessary to begin and maintain health behavior practices, analysing the needs of the priority population in a systematic way, organizing multiple health behavior change programs and multilevel intervention programs, and doing research on health determinants and scientific evidence of health promotion programs, In order to improve the quality of health promotion services in public health organizations, professional competencies of health education specialist should be developed and strengthened.

대리(de Facto) 정신보건 서비스체계의 이용에 영향을 미치는 요인에 관한 분석 (Factors Influencing Use of de Facto Mental Health Service Systems in Persons with Chronic Mental Illnesses)

  • The paradigm of care for persons with chronic mental illnesses has recently been shifted in Korea from institutionalization to community integration. Numerous changes in the service environment tend to target clients of mental health care organizations while a considerable proportion of people with similar clinical conditions have been institutionalized in various types of organizations whose auspices are other than mental health care. Under the current circumstances in which the subgroup of people, who tend to be socio-economically disadvantaged, has no viable alternative in the community to using such de facto mental health organizations, their chances of deinstitutionalization may decrease. This study thus aims at providing an increased understanding of the characteristics of the users and their service utilization, which in turn enables us to establish differentiated strategies within the current service context. In this study, a survey is conducted on the residential history of those with chronic mental illnesses both in the community and in institutions. Major characteristics of the users of de facto services are investigated along with the extent of their use of various existing mental and non-mental health services. Included is also an examination of the subjects' clinical and individual factors that may influence their use of de facto services. The study findings demonstrate relative disadvantages in deinstitutionalization in a subgroup of the population with chronic mental illnesses under the current mental health policies, and the author suggests intervening strategies specific to this population.

산업보건서비스기관의 운영 효율성 분석 - 자료포락분석(DEA)기법을 이용하여 - (Evaluation of Managerial Efficiency in Occupational Health Service Organizations Using the Data Envelopment Analysis Method)

  • This study analyzed the managerial efficiency of 11 organizations, the branch centers of a occupational health service organization in Korea, using the Data Envelopment Analysis (DEA) method. The DEA is a good method for evaluating health services since it can handle multiple inputs and outputs simultaneously, and also identify the sources and amount of inefficiency. The author approached this study using two efficient models: the monetary value model and the real value model. The DEA method based on the monetary value model included cost factors, while the real value model excluded cost factors. The input variables used were manpower of physicians, medical technicians, nurses, industrial hygienists and administrators; labor, maintenance, and material expenses. The output variables used were the number of medical examinations, workplace evaluations, group health management services and income from each service. The major results were as follows: First, in the monetary value model, 6 out of 11 organizations (54.6%) showed an efficiency score of 1.0, which means that they have been operating in very efficient ways. However, 5 organizations (46.4%) showed themselves to be relatively inefficient. Second, in the real value model, 7 out of 11 organizations (63.4%) showed an efficiency score of 1.0, which means they have been operating efficiently, while 4 organizations (46.4%) showed themselves to be relatively inefficient. Third, the reliability of DEA method were analyzed by comparing the results of the monetary value model and real value model. The results of 8 out of 11 organizations were same in terms of being efficient or not. Thus, the DEA could be a valid application method for occupational health service organizations. Fourth, the organizations that displayed common inefficiency in both the monetary value model and in the real value model 3, 9, and 10, were also considered to be managed inefficiency from expertise opinion. In summary, this study evaluated the efficiency of occupational health service organizations applying the DEA method with different variables, and found that the results of analysis could be valid in terms of both modeling and expert sense. In the future, the DEA method will be used as a useful tool to identify and evaluate the efficiency of occupational health service organizations through more applications and refinements.

간호 연구와 조직 이론의 통합적 접근: 동향과 함의 (The Application of Organization Theories to Nursing Research: A Critical Review and Implications)

  • This article explores the potentials of the application of organization theories to nursing research on health care organizations in Korea. The critical review of the literature reveals that the important topics on organizations in health services research have been also dealt with by nursing researchers. Notwithstanding this theoretical closeness between the two fields, some issues remain: the paucity of attention to 'level issue', which requires to increase the attempts at 'meso-' or 'macro-' analysis on nurses' behavior in organizations; the need for theoretical frameworks specifying organizational factors relating to nursing processes; the necessity of interdisciplinary approaches to bridge the recent developments of organizational study and nursing research. This article finishes with a few of research agendas for future research to fill these gaps.

Method: This study analyzes me data on the present conditions and evaluates the organizational effectiveness of MCH workers at 23 advanced public health centers. The organizational effectiveness of MCH organization is assessed by both MCH workers and clients who have received MCH services at the three health centers selected from me 23 advanced public health centers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Results: The results are as follows.
