Effects of Changes in Reality Shock among New Nurses on Turnover Intention in South Korea

  • Purpose: This study aimed to investigate the changes in reality shock among new nurses and their influencing factors to identify turnover reduction methods. Methods: A total of 146 new nurses from 6 general hospitals participated in the survey. The survey data were collected from August 5 to November 10, 2015. Results: The mean scores for factors related to turn over were 2.75 for nurse practice environment, 2.84 (4point scale) for reality shock, and 3.08 (5point scale) for turnover intention. A significant difference in the impact of reality shock was observed depending on whether the nurses selected their work unit (F=6.24, p=.003) and whether they could take the possibility of holiday on the desired day at will (t=-2.57, p=.013). Changes in reality shock correlated with turnover intention, with the 'increased reality shock' group demonstrating an odds ratio of 2.37 (CI: 1.41~3.98) for turnover intention. Conclusion: The current findings indicate the need for lowering reality shock and turnover intention by considering nurses' work-related characteristics and improving their practice environments. To further study the changes in reality shock new nurses experienced, additional research is warranted while homogenizing the participants in terms of preceptorship duration and timing of independence.

A Study on the Risk Reduction of Distribution Line through Analysis of Electric Shock Accident (감전재해 분석을 통한 배전선로의 위험성 저감에 관한 연구)

  • In this study, we analyze the current status of major disasters in distribution works and propose safety measures through the distribution live-line work method and electric shock risk assessment. The result of analyzing the ratio of electric shocks to the occurrence of industrial accidents in the recent 13 years shows that the death rate is higher than other industries, especially the construction industry occupying most of the disaster, and it is higher than the collapse disaster. We analyze statistic data of 101 victims selected as core words of live work, distribution line, pole and 22.9 kV in the investigation report of major accident of electric shock fatal from 2001 to 2014. The safety measure was established through the risk assessment of the distribution method using the standard model of the risk assessment based on the results of electric shock analysis on the distribution line. In order to prevent the electric shock accident which is recently being discussed, the risk assessment procedure were carried out in the above-mentioned 22.9kV special high voltage live-line operation method. We derived the risk reduction plan for the distribution line from the results of the major accidents statistic and demonstration of the line works.

A Concept Analysis on Reality Shock in Newly Graduated Nurses Using the Hybrid Model (혼종모형을 이용한 신규간호사의 현실충격에 대한 개념분석)

  • Purpose: The purpose of this study was to define and clarify the concept of reality shock in new graduated nurses. Methods: The hybrid model was used to develop the concept of reality shock. The model included a field study. The participants were 9 newly graduated nurses with a nursing career spanning less than a year. Results: The reality shock in newly graduated nurses was identified to have three dimensions and seven attributes. Specifically: 1) the dimension of performance included two attributes (conflict between theory and practice, and being overwhelmed by the workload), 2) the dimension of relationship included three attributes (loss of support, embarrassment from interference, and relational withdrawal), 3) the dimension of expectations included two attributes(value confusions and incongruity in personal life). Conclusion: Newly graduated nurses' reality shock was defined as a state of incongruence in their entire life that the new nurses experienced owing to value confusions that occurred due to the conflicts between theory and practice in an unfamiliar work environment, getting overwhelmed by the workload, and withdrawing establishing relationships with others due to the loss of support and excessive interference. These findings could help develop intervention strategies to decrease reality shock in newly graduated nurses.

The Characteristics of Electric Shock Accidents and Their Countermeasures for Portable Electrical Tools (이동형 전기기기의 감전재해 특성 및 그 대책)

  • According to the data of Korea Occupational Safety and Health Agency, electric shock accidents during recent 11 years exceeded more than 60% in architecture/other and construction work, the countermeasures for safety are required in the harsh environment of a construction site where moving electric machines and equipments are widely used. The establishment of countermeasure for insulation degradation and defect is required, in consideration of increasing trend for accidents caused by defective insulation among low voltage electrical installation each year. The aim of this study is to propose the policy about portable electrical tool standards and/or worker's safety standards for preventing electric shock accidents on safety workings, and is to develop the technology and the safety device to prevent electric shock for accident prevention reduction through experiments. It obtained the followings through this study; statistical data analysis of late about 10 years of electric shock-related industrial accidents analysis and portable electric tools; safety device development of 'device for testing continuous grounding and power shut-down' to prevent electric shock from portable electric tools. Furthermore, developed results and proposal in this study will help to prevent the electric shock accidents from portable electric tools and will be expected the utilization of policy formulation, educational data and field supplement of the safety device, and etc.

Impact Shock Components and Attenuation in Flat Foot Running (편평족 달리기 시 충격 쇼크의 성분과 흡수)

  • Objective : The purpose of this study was to determine the differences in the head and tibial acceleration signal magnitudes, and their powers and shock attenuations between flat-footed and normal-footed running. Methods : Ten flat-footed and ten normal-footed subjects ran barefoot on a treadmill with a force plate at 3.22m/s averaged from their preferred running speed using heel-toe running pattern while the head and tibial acceleration in the vertical axis data was collected. The accelerometers were sampled at 2000 Hz and voltage was set at 100 mv, respectively. The peak magnitudes of the head and tibial acceleration signals in time domain were calculated. The power spectral density(PSD) of each signal in the frequency domain was also calculated. In addition to that, shock attenuation was calculated by a transfer function of the head PSD relative to the tibia PSD. A one-way analysis of variance was used to determine the difference in time and frequency domain acceleration variables between the flat-footed and normal-footed groups running. Results : Peaks of the head and tibial acceleration signals were significantly greater during flat-footed group running than normal-footed group running(p<.05). PSDs of the tibial acceleration signal in the lower and higher frequency range were significantly greater during flat-footed running(p<.05), but PSDs of the head acceleration signal were not statistically different between the two groups. Flat-footed group running resulted in significantly greater shock attenuation for the higher frequency ranges compared with normal-footed group running(p<.05). Conclusion : The difference in impact shock magnitude and frequency content between flat-footed and normal-footed group during running suggested that the body had different ability to control impact shock from acceleration. It might be conjectured that flat-footed running was more vulnerable to potential injury than normal-footed running from an impact shock point of view.

The Development of the Analysis Program for the Electric Shock Accidents Occurred in the TN and TT System (TN 및 TT 계통에서의 감전사고 분석 프로그램 개발)

  • According to the statistics from the Ministry of labor, 70~80 people died by work-related electric shock accidents in the workplace and this number is about 10~20 times higher than those of the foreign countries such as Japan and the United Kingdom in frequencies and rates per 100,000. The electrocution deaths had decreased from several hundred in 1990s to 67 in 2002, since then there has been little change over the last 5 years. To reduce this number of the electrocution death, it is necessary to analyze the sources of the accidents and to implement the suitable measures for preventing the similar accidents from occurring. But there is not any consensus standard method or procedures for accident investigation yet. And the results solely depend on the ability of the investigator. Because of the demand of eliminating technical barrier for free trading, the IEC 60364 is adopted as a regulation for low voltage electrical installations in Korea. The facilities designed and constructed by IEC 60364 are increasing. The facilities by IEC 60364 have different features in comparison with the facilities by Korea eletro technical regulation, and these features combined with parameters of electric shock accident can greatly affect the consequences of the electric accidents. The accident analysis program for electric shock injuries in Terra Neutral(TN) and Terra Terra(TT) system was developed in this paper, which was based on KOSHA Electric Shock Analysis Program(ESAP).

Effects of Health Shocks on Employment and Income (건강 충격의 고용과 소득 효과 분석)

  • Using matching and difference-in-differences estimation method, this study estimates causal effects of health shocks on employment and income of full-time workers aged 40-55. Acute hospital admissions lower significantly the employment probability and earnings. The changes in employment and earnings persist up to three years after the health shock. The economic impacts of health shocks vary by socioeconomic status and job characteristics among individuals. Irregular workers are more likely to leave their jobs after health shocks than regular workers. Among irregular workers, the probability of leaving labor market after health shock decreases with the size of the firm.

A Study for Changes of Blood Pressure and Heart Rate by Acupressure to Acupoints in Rat Model of Hemorrhagic Shock (출혈성 쇼크 흰쥐 모델에서 경혈 지압이 혈압과 심박수 변화에 미치는 영향)

  • Objectives : In order to find a possible non-invasive manipulation tool for maintenance of the cardiovascular functions in hemorrhagic shock, this study was aimed at evaluating effects of acupoints acupressure on the changes in blood pressure and heart rate from an animal model of hemorrhagic shock. Methods : In adult Sprague-Dawley rats, hemorrhagic shock was induced by a withdrawal of arterial blood from the femoral artery with volume of 0.8 ml per 100 g of body weight using peristaltic syringe pump. We applied the acupressure with a pressure oscillator to tail as a control and 2 different acupoints of sobu(HT8), youngchun(KI1) under 3 different conditions : 1) normal arterial blood pressure without bleeding, 2) at the beginning of bleeding, and finally 3) hemorrhagic shock. Results : Under normal arterial blood pressure without hemorrhage, there was a significant increase in systolic and diastolic blood pressures by the acupressure to the tail, HT8 and especially KI1 for 30 sec compared with before acupressure. Under hemorrhagic shock condition, the tail acupressure had minimal changes in cardiovascular parameters. Either the HT8 or KI1 acupressure resulted in a significant increase in arterial pressure but did not heart rate. At the beginning of bleeding, tail acupressure failed to change the reduction of arterial pressure and heart rate. However, there was a significant increase in blood pressure and heart rate following either the HT8 or especially KI1 acupressure. Conclusions : HT8 and KI1 acupressure affected cardiovascular signs but tail acupressure did not in rat model of hemorrhagic shock. These experimental data suggest that a acupressure with a pressure oscillator to HT8 or KI1 can be one of alternative emergency manipulations to ameliorate compromised cardiovascular functions under hemorrhagic shock condition.

A Study on the In-depth Analysis of Cause and Characteristic for Recent Electric Shock Accident (최근 감전재해의 발생특성과 재해원인 분석연구)

  • The electric shock victims were predominantly male, and more than half of the victims were found in companies with less than five employees in terms of the scale of company, and also more than half of victims had less than one month of experience for work experience. Considering the category of industry, more than half of the victims were engaged in construction, and considering the subcategory of industries, more than 60% of electric shock victims were found in building and other construction works. This study intended to conduct an in-depth analysis on the cause of electric shock accidents to provide basic data for policy proposing taking into considering the characteristics such as the electric shock path, low voltage, grounding and protection of workers. Also this study conducted an in-depth analysis of electric shock accidents for the past eleven years taking into account the necessity of a long-term plan.

A Study on the Decreasing Method of Electric Shock Accident in the Construction Industry (건설업에서의 감전재해 감소 방안을 위한 연구)

  • Regarding the characteristics of fatal electric shock accident during the past 10 years, fatalities occurred more frequently in July & August, and AM 10-12 PM & 14-16. Also, the electric shock accident is occurred more frequently in the age group from 30 to 40. Especially, the fatal electric shock accident in construction industry covered more than half of total industry. Therefore, it is strongly requested for the protection technology of an electric shock from the low voltage electric path, grounding method and workers. The aim of this study is to propose the policy about the equipment performance standards and/or worker's safety standards to revise the standards for preventing electric shock accidents on safety workings.
