고주파 자기장을 이용한 온침 치료시스템의 임상 평가 (Clinical Assessment of Warm Acupuncture Therapy System Using HF Time-varying Magnetic Field)

  • Objectives : The aim of this paper is to develop acupuncture needle heating system by utilizing the new method which uses high frequency(HF) time-varying magnetic field. Also, it is possible to make an assessment for reliability and clinical significance of the system. Methods : We designed the system with solenoid coils for generation of HF time-varying magnetic field that oscillates at 150 kHz. Acupuncture needles are quickly magnetized and heated by 150 kHz. We assessed clinical significance of system, such as body temperature, pressure pulse waveform. Results : Temperature of acupuncture needle increased up to 60 degree. In the result for clinical significance of system, in case of manual acupuncture stimulation(MAS), body temperature change was $0.373^{\circ}C{\pm}0.224$(p<0.05), rate of pulse energy change increased about 7.6%. In case of warm acupuncture stimulation(WAS), body temperature change was $0.645^{\circ}C{\pm}0.281$(p<0.05), rate of pulse energy change remarkably increased about 35.9%. Conclusions : We confirm that the system is able to be applied clinically to various warm acupuncture needle therapy in the area of oriental medicine.

  • The spontaneous regression of herniated cervical discs is not a well-established phenomenon. However, we encountered a case of a spontaneous regression of a severe radiculopathic herniated cervical disc that was treated with acupuncture, pharmacopuncture, and herb medicine. The symptoms were improved within 12 months of treatment. Magnetic resonance imaging (MRI) conducted at that time revealed marked regression of the herniated disc. This case provides an additional example of spontaneous regression of a herniated cervical disc documented by MRI following non-surgical treatment.

  • Spontaneous osteonecrosis of the knee (SONK) is a common form of osteonecrosis of the knee and mainly affects the medial condyle due to localized vascular insufficiency. We report a case of SONK in a 65-year-old woman who had severe knee pain in her left knee whichimpeded her capacityto walk beyond 10 minutes. Bilateral knee X-rays revealed degenerative osteoarthritis of both knees and magnetic resonance imaging revealed R/O SONK in the lateral aspect of the medial femoral condyle, as well as a medial meniscal posterior horn root tear, and a Grade 1 medial collateral ligament injury. She was hospitalized at Jaseng Hospital of Korean Medicine for 21 days and receivedcombination therapy includingacupuncture, pharmacopuncture, and herbal medicine. Patient-reported scales indicated that her pain and physical functional limitations were alleviated. Combination therapy consisting of Korean traditional medicine may bean alternative nonoperative treatment approach for patients with SONK.

An Analytical Comparison in Electoencephalography and Electrocardiography under Pulsed Magnetic Field and Acupuncture Stimulus on Acupoint PC9

  • We have investigated the changes of electroencephalography (EEG) and electrocardiography (ECG) under pulsed magnetic field (PMF) and acupuncture stimulus on acupoint PC9. In order to compare quantitatively the effect of PMF and acupuncture stimulus, the difference of alpha activities are calculated from EEG spectra, and the spectrum curves of ECG were analyzed in the frequency domain of heart rate variability (HRV). The increase of alpha activities after both stimuli could be explained that the impulse of stimulus on PC9 might pass through sensory nerve following meridian and approach the cerebral cortex, causing the central nervous system (CNS) to be activated for pacifying emotion and calming the mind. The decrease in sympathovagal activity of HRV after both stimuli indicates that parasympathetic nerves were activated and the sympathetic nerves were in constrained condition. These findings suggest that PMF could be patient-friendly alternative non-invasive medical treatment for influencing human physiology, in comparison with acupuncture inserting the needle and inducing nervous and anxious state to subject.

Cervical and Lumbar Herniated Nucleus Pulposus Resorption after Acupotomy with Integrative Korean Medicine Treatment: A Case Series of Two Patients

  • The aim of this study was to report on the resorption of herniated nucleus pulposus following acupotomy treatment. Two patients were treated with acupotomy which included integrative Korean Medicine treatment, for either cervical, or lumbar herniated nucleus pulposus. Magnetic Resonance Imaging was used to produce images before and after (several months) treatment. The Numerical Rating Scale scores and Range of Motion were used to assess the patients at admission and discharge. In both cases, substantial resorption of the respective herniated disc was observed in the magnetic resonance images, with a decrease in the Numerical Rating Scale score. Range of Motion improved in 1 case. This study shows that acupotomy treatment may be a clinically effective treatment for herniated nucleus pulposus resorption and pain relief.

  • 통증을 치료하기 위한 방법으로 약물요법, 신경차단법, 외과적 수술 등이 있으며, 대체물리요법으로 침구치료, 초음파, 전기 치료, 자기장 치료 등이 응용되고 있다. 이중 전기치료 요법이 많이 이용되고 있으나, 피부나 피하조직의 임피던스에 큰 영향을 받으며, 주로 경피자극이 주된 방법으로 효과가 매우 확률론적이다. 본 연구에서는 집속형 자기장을 이용하여 경혈이나 치료점에 대한 국소 부위를 비침습적으로 자극 할 수 있는 새로운 시스템을 개발하였다. 수기자극과 유사한 자극을 유도할 수 있는 새로운 자기장 침 전극(자기장 자극의 효율성을 고려하여 새로운 형태)을 설계하였다. 개발된 시스템의 임상적 유효성 및 신뢰성을 평가하기 위하여 수기자극과 자기장 자극에 대한 경락전위변화를 측정하여 비교하였다. 실험 결과 자기장 자극 시 수기자극을 할 때 나타나는 충전 방전 형태의 전위변화와 유사한 경락전위변화가 측정되었으며, 자극방식, 세기, 빈도를 조절함에 따라 다양한 반응특성이 나타남을 확인하였다. 또한, 전자기장 자극에 의해 경락전위변화를 유도할 수 있어 이를 이용한 새로운 경혈자극 및 통증치료기로써의 가능성을 확인하였다.

Modern acupuncture-like stimulation methods: a literature review

  • Acupuncture therapy has been proved to be effective for diverse diseases, symptoms, and conditions in numerous clinical trials. The growing popularity of acupuncture therapy has triggered the development of modern acupuncture-like stimulation devices (ASDs), which are equivalent or superior to manual acupuncture with respect to safety, decreased risk of infection, and facilitation of clinical trials. Here, we aim to summarize the research on modern ASDs, with a focus on featured devices undergoing active research and their effectiveness and target symptoms, along with annual publication rates. We searched the popular electronic databases Medline, PubMed, the Cochrane Library, and Web of Science, and analyzed English-language studies on humans. Thereby, a total of 728 studies were identified, of which 195 studies met our inclusion criteria. Electrical stimulators were found to be the earliest and most widely studied devices (133 articles), followed by laser (44 articles), magnetic (16 articles), and ultrasound (2 articles) stimulators. A total of 114 studies used randomized controlled trials, and 109 studies reported therapeutic benefits. The majority of the studies (32%) focused on analgesia and pain-relief effects, followed by effects on brain activity (16%). All types of the reviewed ASDs were associated with increasing annual publication trends; specifically, the annual growth in publications regarding noninvasive stimulation methods was more rapid than that regarding invasive methods. Based on this observation, we anticipate that the noninvasive or minimally invasive ASDs will become more popular in acupuncture therapy.

요추 추간판 탈출증 환자의 보존적 치료 후 변화된 자기공명영상에 따른 임상적 고찰 (Clinical Study for Patients with Lumbar Disc Herniation on Change of Magnetic Resonanse Imaging after Conservative Treatment)

  • Objectives : To analyze the therapeutic outcomes of back pain modalities in patients with disc herniation according to the change of Magnetic Resonanse Imaging(MRI) whom were treated with herbal medicine, chuna, acupuncture, bee-venom acupuncture. Methods : Clinical outcomes of 35 patients diagnosed with lumbar disc herniation by MRI examination, treated conservatively, were analyzed according to MRI follow-up change. Patients underwent MRI examaintion at baseline and after 24th week of treatment. Patients are divided into three groups ; improved, unchanged, worsened. 35 patients with lumbar disc herniation were treated with chuna therapy, acupuncture, bee-venom acupuncture once a week and took herbal medicine after a meal two times daily. The patients' clinical outcomes were assessed at baseline, 12th week, 24th week by Visual analogue scale(VAS), Oswestry disability index(ODI). Results : 1. MRI follow-up exams on regression of disc herniation resulted on 42.86% of cases by conservative treatment. 2. VAS(low back pain and sciatica) and ODI of disability of daily activities showed significant decrease in "improved" and "unchanged" groups compared to "worsened" group(P<0.05). 3. VAS(low back pain and sciatica) and ODI change after treatment did not show any relationship with MRI follow-up change(P<0.05). Conclusions : This study suggests that "improved" and "unchanged" groups compared to "worsened" group on MRI follow-up in patients with lumbar disc herniation were more effective when treated with herbal medicine, chuna therapy, acupuncture, bee-venom acupuncture. MRI follow-up change does not affect clinical changes in patients with lumbar disc herniation.

  • Objectives : The purpose of this study is to investigate the clinical application of acupuncture treatment to a cervical herniated intervertebral disc patient with hyperventilation syndrome developed after magnetic resonance imaging(MRI). Methods : A patient with cervical herniated intervertebral disc got hyperventilation syndrome after taking magnetic resonance imaging. To relieve symptoms, we took paperbag breathing first aid, but it had no effect. So we used acupuncture therapy with strong stimulation. To measure the outcome of the patient's improvement, we observed change of vital sign and verbal numerical rating scale(VNRS). Results : After 8 minutes of acupuncture treatment, the patient had significant improvement in change of vital sign and verbal numerical rating scale(VNRS). Conclusions : In emergency situation, acupuncture treatment as a first aid has a positive effect to control hyperventilation syndrome.

  • Purpose: To investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. Materials and methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was peformed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34.Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture.
