마이크로니들을 이용한 경피약물전달의 피부 미용학적 접근 (A Study on Transdermal Drug Delivery System with Microneedle System in the Field of Skin Care)

  • The penetration of outside material into skin is not easy. It is since the skin, which is a very hard barrier, protects the body against outside chemical and physical stimulation. Microneedle system which can help improve drug penetration into skin is advancing variously in transdermal drug delivery system (TDDS) in the field of skin care. After inserting microneedle into skin by using electrical or artificial forces, it makes microhole and drug penetration easily and induces natural skin rejuvenation. Diffusion and penetration of drug by optical and electrical force of microneedle is better for fast and effective TDDS. This is more developed than the traditional method such as the manual stamp, roller, and meso gun. The drug absorbed into dermal layer by microneedle helps revive and repair damaged skin. In the future, utilization of microneedle for skin care will progress constantly because of its human-friendly biodegradable materials and the development of the no pain microneedle.

Analysis of Trend of Studies on Microneedle Treatment System (MTS)

  • Objectives: The purpose of this study was to analyze the microneedle therapy system (MTS) and its research methods for the past 10 years in Korea. Methods: Data on microneedle therapy system were collected using NDSL, KISS, RISS, and OASIS electronic databases from January 2010 to August 2021. "microneedle," "derma stamp," "microneedle therapy system" were used as the keywords. The present study, however, excluded data that were 1) unrelated to the microneedle therapy system, 2) from review/meta/protocol studies, and 3) from overseas studies. Data selected through the primary screening process, animal studies, case reports, and clinical data were included in the analysis. However, information data not related to the microneedle therapy system were excluded from the study. Results: Among the MTS-related papers published from January 2010 to August 2021, 7 animal research, 2 clinical trials, and 10 case studies were published. Based on the research topics, there were 8 papers on skin improvement and skin diseases, 7 papers on hair growth and hair loss, 3 papers on stability, and 1 paper on peripheral facial paralysis. Conclusion: Most of the studies related to MTS focused on skin, hair, and stability. The effect of MTS on hair growth and skin improvement has been confirmed, and it has been proven to have significant effects on the treatment of acne, acne scars, and hair loss in clinical practice. No serious side effects were observed during the MTS treatment, and the safety assessment confirmed that it was safe for use.

흉터복원술과 자동 미세침(Auto Microneedle Therapy System, AMTS)을 이용한 여드름 흉터의 치험례 (Case Study of Treating Acne Scars Using Reconstruction of Skin Scars and Auto Microneedle Therapy System)

  • Objective : Acne is very common disease, it can leave permanent scars. However, there are few reports about it in oriental medicine academia. This study performed to evaluate the effect of reconstruction of skin scars and auto microneedle therapy system on acne scars. Methods : Six patients were treated by using reconstruction of skin scars and auto microneedle therapy system. We compared the degree of treatment of acne scars on pre and post case photos and Qualitative Global Acne Scarring Grading System(QGASC). Satisfaction degree were asked from the patients. Results & Conclusion : After comparing case photos and QGASC, observing patients satisfaction, it can be considered that reconstruction of skin scars and auto microneedle therapy system to be valid acne scars treatment.

마이크로 피부침을 이용한 FITC-OVA의 경피흡수 (Transdermal Delivery of FITC-Ovalbumin with Microneedle System)

  • For transdermal delivery of large molecular drugs such as vaccine and protein drugs, novel microneedle treatment device with roll was designed. The roll dimension is 1.43 cm diameter and 2.8 cm perimeter. Total number of microneedle on the roll is 3,360 with $230\;{\mu}m$ height and $740\;{\mu}m$ distance. The pore with $150\;{\mu}m$ depth and $35\;{\mu}m$ diameter on the skin was made by the designed microneedle device. This system could be achieved without pain. The permeation rates of FITC labelled ovalbumin (FITC-OVA, molecular weight: 45,000 g/mol) as a model protein were determined by modified Franz diffusion cells using skins of hairless mice or SD rats which were treated by using microneedle device two or four times. The average penetration fluxes of model protein increased from 674 to $872\;{\mu}g/cm^{2}{\cdot}hr$ as the number of treatment to make pore increased from two to four times. In conclusion, we confirmed the possibility of using the designed microneedle treatment device for transdermal delivery of the large molecular drugs.

The dimensions of width and thickness for in-plane microneedle are $600{\mu}m$, $500{\mu}m$, respectively. A delivery system based on guidelines for traditional injection molding was designed for four-cavities molding system. To investigate the effects of processing conditions in the mirconeedle fabrication, injection molding analysis using commercial code was performed. It was shown that the total injection time has a significant effect on the fabrication of in-plane microneedles.

  • Silicon-micromachined microneedle for a biofluid diagnosis system is developed. To fabricate microneedles, two sets of processes are used. One is making buried microchannels in silicon wafer using silicon isotropic etch with a SF6 plasma and then trench-refilling. The other is releasing the body of the microneedle by deep silicon etch. The microneedle has a 4 mm-length and about 12 $\mu\textrm{m}$ diameter buried microchannel, a 1.5 mm$\times$l.5 mm-area reservoir, and about 180 $\mu\textrm{m}$thickness body. Preliminary results indicate that microneedles are capable of flowing fluidic samples. The microneedle with a buried microchannel is expected to be integrated with in vitro diagnosis systems and microfluidic devices.

절개침과 미세피부침, 약초필링을 이용한 위축성 여드름 흉터 치료 7례 임상보고 (A Seven Cases of Treating Atrophic Acne Scar with Subcision, Microneedle Therapy System and Herbal Peeling Therapy)

  • Objectives : The purpose of this study is to present the effect of subcision, microneedle therapy system and herbal peeling therapy which is treatment of Korean medicine to atrophic acne scar patients. Methods : We administered subcision, microneedle therapy system and herbal peeling therapy to atrophic acne scar patients and tested the results by Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Results : After the treatment, all of the patients showed improvements in Qualitative Global Acne Scarring Grading System and Korean Acne Grading System as well. Qualitative Global Acne Scarring Grading System of patients changed from grade 2.71±0.71 to grade 1.14±0.86 along with the change of Korean Acne Grading System from grade 1.57±0.57 to grade 1.00±0.00. Conclusions : Subcision, microneedle therapy system and herbal peeling therapy may be effective in the treatment of moderate to severe acne scars, and could be applied in clinical practice.

  • 최근 몇 년 동안 탈모로 인해 우울증을 경험하는 인구가 증가하고 탈모 관련 치료법도 임상의학이나 외과적인 수술법과 같이 전문화되고 있다. 탈모 방지와 발모를 촉진하기 위한 시술법과 소재들이 있으나 그 효능, 효과가 뛰어나지 못하고 일부는 부작용을 동반하는 경우가 있다. 본 총설의 마이크로니들 시술법은 약물의 흡수를 촉진시켜주는 매우 작고 섬세한 니들을 이용한 시술법이다. 이 시술을 하는 동안 마이크로니들이 두피 내부로 약물의 흡수를 도와주는 마이크로 홀을 만들어 준다. 마이크로니들 시술 약물의 주요성분으로 대두 발효물질을 분석하였으며 대두 발효 소재는 탈모증에 항산화, 항안드로겐, 항혈전의 기능을 가진다. 대두 발효물의 흡수를 증가시켜주는 마이크로니들의 우수한 약물전달시스템으로 인하여 대두 발효물이 마이크로니들과 함께 사용될 때 복잡한 탈모기전에 더욱 효과적이다. 이 시술법은 탈모증을 방지하고 관리하며 개선시켜줄 수 있는 매우 유용한 두피케어법이며 일반적인 두피관리법 보다 더 빠른 효과를 나타낼 수 있어 차별화된다. 대두 발효물을 이용한 마이크로니들 시술법은 두피케어를 위한 진일보한 기술이다.

자동 미세침(AMTS)을 이용한 여드름 치험례 (A clinical report on the acne treatment with AMTS)

  • Background & Objective : Microneedle therapy system(MTS) is popular these days. However, there are few reports about it in oriental medicine academia. This study is performed to evaluate the effect of MTS on acne patients. Methods : This study was conducted to evaluate the effect of MTS. Seven patients were treated for 7 weeks. Follow-up was performed after the treatment with Janus facial analysis system, skindex-29 and Digital Infrared Thermographic Imaging machine. Satisfaction degree and inconvenience were asked from the patients. Results and Conclusions : Microneedle therapy system(MTS) was good for acne like pore, wrinkle, spot(polarised), sebum, porphyrin and skin tone. Especially it had excellent effects on sebum and porphyrin.
