  • Tubulointerstitial nephritis and uveitis (TINU) is a rare syndrome of unknown etiology involving the kidney and the eye. This is the first case reput of TINU in Korea. The diagnosis of TINU was confirmed in a 15-year-old girl on the basis of pathologically proven tubulointerstitial nephritis and bilateral uveitis. Although the renal symptoms disappeared spontaneously, uveitis showed only partial response to topical steroid treatment and persisted fir more than 11 months. TINU should be included in the differential diagnosis of tubulointerstitial nephritis of unknown etiology.

음곡에 시술한 비해약침이 Lipopolysaccharide로 유도된 흰쥐의 신장염에 미치는 영향 (The Effect of Tokoro Rhizoma Pharmacopuncture at $KI_{10}$ in Lipopolysaccharide Induced Acute Nephritis in Rats)

  • Objectives : This study was designed to evaluate the effects of Tokoro Rhizoma pharmat $KI_{10}$ in nephritis induced by lipopolysaccharide(LPS) in rat. Methods : Rats were divided into 4 groups and 3 groups(LPS, saline, Tokoro Rhizoma pharmacopuncture(TR-P) group) were injected LPS to induce nephritis. TR-P group was treated with TR at $KI_{10}$ three times for a week, saline group with normal saline. To evaluate the effects of TR at $KI_{10}$ on nephritis in rats, white blood cell(WBC), neutrophil in blood, creatinine, cytokine-induced neutrophil chemoattractant-1(CINC-1) in serum and urinary volume, creatinine in urine, renal myeloperoxidase(MPO) were measured and renal tissue was analyzed. Results : TR-P group significantly reduced WBC, neutrophil in blood, creatinine, CINC-1 in serum, creatinine in urine and renal MPO than LPS group. TR-P group increased urinary volume but, not significant. Conclusion : TR at $KI_{10}$ has a therapeutic effect on nephritis in LPS stimulated rat. Therefore, it is suggested that TR at $KI_{10}$ may be an useful therapeutics for nephritis in clinical field after further researches.

  • Objectives : This study was designed to evaluate the effects of Desmodii herb pharmacopuncture (DH-HA) at $KI_{10}$(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rat. Methods : Rats were injected LPS to induce nephritis. DH-HA group was treated with DH-HA at $KI_{10}$ three times for a week, NP group with 26 gauge needle, saline group with normal saline. To evaluate the effects of DH-HA at $KI_{10}$ on nephritis in rats, RBC, WBC, neutrophil in blood, TNF-a, CINC-1 in serum, creatinine and total protein in urine, renal TNF-a, renal MPO were measured and renal tissue was analyzed. Results : WBC in blood, CINC-1 in serum, renal MPO significantly reduced in DH-HA group. Conclusions : DH-HA at $KI_{10}$ is effective for nephritis in LPS-induced rats. Therefore, DH-HA at $KI_{10}$ may be useful for nephritis in clinical field.

  • 헤노흐-쇤라인 자반증의 전반적인 예후는 양호하나 심한 신염의 경우 말기신부전으로 진행될 위험이 높다. 최근 연구는 심한 헤노흐-쇤라인 자반증 신염을 가진 소아에서 조기 치료의 중요성을 강조하고 있으나 심한 자반증 신염의 치료는 맹검 대조 연구가 드물어 여전히 논쟁의 여지가 있다. 그럼에도, 정맥 고용량 메틸프레드니솔론 충격요법, 면역억제/세포독성 약제, 섬유소용해 치료, 항응고제, 항혈소판제, 혈장교환술같은 여러 강력한 치료가 심한 자반증 신염을 가진 소아에서 사용되어 왔다. 이 종설에서는 심한 자반증 신염을 가진 소아의 치료를 중점적으로 기술하였다.

백서신장(白鼠腎臟)의 병변(病變) (Lesions in the Kidney of Albino Rats)

  • Nineteen cases of Albino rat having naturally occurred renal lesions were observed during the routine examination of a total of 240 cases. The lesions observed were focal suppurative nephritis (6 cases), acute nonsuppurative nephritis (3 cases), chronic interstitial nephritis (4 cases), nematode infection in pelvic epithelium (5 cases, in which one case was coincident with focal suppurative nephritis) and unilateral hydronephrosis (2 cases). Gross and microscopic findings for the lesions were described and discussed.

Methods : Rats with nephritis induced by LPS, were treated with JM-HA at $KI_{10}$ 3 times a week. The rats in the NP group and the saline group were treated with a needle prick and a saline injection respectively. To evaluate the effects of JM-HA at $KI_{10}$ on nephritis in rats, WBC, neutrophils in blood, BUN, creatinine, TNF-${\alpha}$ in serum, creatinine, total protein in urine and renal MPO were measured. Results : JM-HA at $KI_{10}$ significantly inhibited the increase of WBC and neutrophils in blood, BUN, creatinine, TNF-${\alpha}$ in serum, and MPO in kidney of LPS-stimulated rats. Conclusion : JM-HA at $KI_{10}$ has therapeutic effects on nephritis in LPS-stimulated rats. Therefore, it is suggested that JM-HA at $KI_{10}$ may be a useful therapy in clinical field after further researches.

  • Objectives : This study aimed to evaluate the effects of Stephaniae tetrandrae Radix Herbal-acupuncture(SR-HA) at KI10(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Male SD rats (n=32) were allocated to four groups; normal, LPS, saline, and SR-HA. The rats in the LPS, saline, and SR-HA groups were injected intra-peritoneally with LPS (2 mg/kg) to induce nephritis. The rats in the saline and SR-HA groups were treated with saline injection and SR-HA respectively at KI10 three times a week. To evaluate the effects of SR-HA at KI10 on nephritis in rats, the authors evaluated the numbers of WBC and neutrophils in blood, and the levels of creatinine, TNF-${\alpha}$, CINC-1 in serum, creatinine, total protein in urine and renal MPO. Results : SR-HA at KI10 significantly inhibited the increase of WBC and neutrophils in blood, TNF-${\alpha}$, CINC-1 in serum, creatine, total protein in urine and MPO in kidney of LPS-stimulated rats. Conclusions : SR-HA at KI10 may have therapeutic effects on nephritis in LPS-stimulated rats.

  • Objectives : This study aimed to evaluate the effects of Malvae Semen herbal-acupuncture (MS-HA) at$KI_{10}$($Eumgok$) on nephritis induced by lipopolysaccharide (LPS) in rat. Methods : Rats with nephritis induced by LPS, were treated with Malvae Semen herbal-acupuncture(MS-HA) injection at $KI_{10}$. Two control groups, N.P group and Saline group, were treated with 26 gauge needle at $KI_{10}$, 3 times a week. In Saline group, normal saline was injected at $KI_{10}$. To evaluate the effects of Malvae Semen Herbal-acupuncture at $KI_{10}$ on nephritis in rats, WBC, Neutrophils in blood, BUN, Creatinine, TNF-a, CINC-1 in serum, urinal volume, creatinine, total protein in urine and renal TNF-a, renal tissue were analyzed. Results : MS-HA at $KI_{10}$ significantly reduced WBC and neutrophil in blood, TNF-${\alpha}$ in serum of LPS-stimulated rats. MS-HA at $KI_{10}$ significantly reduced total protein level in urine. MS-HA at $KI_{10}$ reduced accumulation of neutrophil in renal tissue of LPS-stimulated rats. Conclusions : Taken together, MS-HA at $KI_{10}$ has a therapeutic effect on nephritis in LPS-stimulated rat. Therefore, it is suggested that MS-HA at $KI_{10}$ may be an useful therapeutics for nephritis in clinical field.

가토(家兎)를 이용(利用)한 방사선신염(放射線腎炎)의 실험적(實驗的) 연구(硏究) (An Experimental Study for Radiation Nephritis in Rabbits)

  • Experimental radiation nephritis was produced in 15 rabbits by X-irradiation. About $2,000{\gamma}$ (tissue doses) were given to both kidneys of a rabbit in 5 days. Other tissues and organs except both kidneys were protected with 2mm thickened lead plates. 5 weeks after the last irradiation, blood pictures, blood pressures, B.U.N., serum creatinine, Ca, Mg, Fe levels and serum erythropoietin activity of the irradiated rabbits were studied. After finishing above studies, rabbits were sacrificed and both kidneys were removed and examined histopathologically. Same laboratory and pathological studies were performed in 6 control rabbits. In this study, the author obtained following results. 1) Both kidneys of rabbitis with experimental raditation nephritis showed marked histopathological changes, i.e.: renal tubules showed diffuse cloudy swelling, impacted intraluminal hyaline casts and focal precipitations of lime salts on the tubular epithelium. Diffuse interstitial fatty necrosis and various degrees of fibrotic infiltrations on the interstitium were also seen in association with focal lymphocytic infiltrations. Hyaline degenerations were observed on the glomeruli and small vessels. 2) Experimental radiation nephritis rabbits showed marked lowering in R.B.C. counts, decreased hemoglobin levels, low hematocrit values and leucopenia in comparison with those of control rabbits. (P<0.01). (Table 1 & 2). 3) Mild proteinuria were observed in experimental radiation nephritis in rabbits. 4) The levels of B.U.N. and serum creatinine increased in experimental radiation nephritis. (P<0.01). (Table 1,3 & 4) 5) The levels of serum Ca and Mg Showed no statistical difference in comparison with those of control rabbits. (P>0.05) (Table 3 & 4). 6) No statistical correlations were observable between the levels of B.U.N. and Hb. values. (${\gamma}=-0.223$) No close correlations (${\gamma}=-0.328$) were noticed between the levels of B.U.N. and serum iron levels. 7) Erythropoietin activity (R.B.C. $^{59}Fe$ Incorporation) was measured by the modified Fried method. No change in its activity was noticed in radiation nephritis group comparing with that of the control group. (P>0.05). (Table 1,3 & 5). 8) Carotid artery blood pressures showed also no difference.   • Objectives : This study was designed to evaluate the effects of Akebiae Lignum herbal acupuncture(AL-HA) at $KI_{10}$ in acute nephritis induced by lipopolysaccharide(LPS) in rat. Methods : Rats were divided into 5 groups and 4 groups were injected LPS to induce acute nephritis. Normal group was normal SD rat, LPS group was injected LPS, AL-HA group was treated with AL-HA at $KI_{10}$ three times for a week, needle prick(NP) group with 26 gauge needle and saline group with normal saline. To evaluate the effects of AL-HA at $KI_{10}$ on acute nephritis in rats, WBC, neutrophil in blood, BUN, TNF-${\alpha}$, CINC-1 in serum and urinary volume, total protein in urine, renal MPO were measured and renal tissue was analyzed. Results : AL-HA group significantly reduced WBC, neutrophil in blood, BUN in serum, total protein in urine and renal MPO. And AL-HA group reduced concentration of neutrophil on glomerulus than LPS group in histological analysis. Conclusions : AL-HA at $KI_{10}$ has a therapeutic effect on acute nephritis in LPS stimulated rat. Therefore, it is suggested that AL-HA at $KI_{10}$ may be an useful therapeutics for acute nephritis in clinical field after further researches.
