Prognostic Factors of Idiopathic Facial Palsy: A Retrospective Study

  • Objectives : The purpose of this study was to evaluate the clinical prognostic factors affecting facial palsy in 98 idiopathic facial palsy patients who were hospitalized and treated in 2015, using retrospective statistical analysis. Methods : We investigated patients with idiopathic facial nerve palsy, admitted to a Korean medical hospital in 2015, and examined patients' variables and therapeutic variables. For analysis of clinical data, an independent sample t-test, analysis of variance (ANOVA), and simple regression analysis were performed using IBM SPSS version 24.0. Results : 1. The initial degree of facial palsy showed statistical significance with age. The older the age, the more severe the initial palsy. 2. Following treatment degree of facial palsy was statistically significant with age, hypertension, and fasting blood sugar (FBS). The higher the value, the slower the recovery from facial palsy. There was a statistical significance with the number of treatments in a Korean medical hospital. The more frequent the treatment, the faster the facial palsy recovery. 3. Degree of facial palsy after 12 months was statistically significant with age, hypertension, diabetes, FBS, and the initial severity of facial palsy. The higher the value, the slower the facial palsy recovery. 4. Sex, left or right sided palsy, alcohol consumption, smoking, history of facial palsy, season of onset, total number of treatments and bio chemistry (BC), complete blood cell count (CBC), urinalysis (UA) factors had no statistical significance with prognosis of facial palsy. Conclusion : Age, season of onset, hypertension, diabetes, FBS, initial severity of facial palsy, and the number of treatments at a Korean medical hospital showed statistical significance. The number of treatments at the Korean medical hospital positively correlated with facial palsy prognosis, and the others variables showed a negative correlation with facial palsy prognosis.

자하거(紫河車) 약침치료를 이용한 양측 동시성 Bell's palsy 환자 1례에 대한 증례보고 (A Clinical Study on 1 Case of Patient with Bilateral Simultaneous Bell's Palsy Treated by Hominis Placenta Herbal-Acupuncture)

  • Objective : This study was carried out to investigate the progress of bilateral simultaneous facial palsy and the effect of Hominis Placenta herbal-acupunture and the other oriental medical therapies. Method : We used two methods to research the progress of disease. 1. Diagnosis - Facial muscle test, Taste test, Hearing test, Photographies, Lab-finding 2. Treatment - Acupuncture, Herbal-acupuncture, Electroacupuncture, Herb-med Edema rate, Pain endurance, WBC, Hemoglobin, Platelet, Total protein, Albumin, Globulin, RA factor, CRP Results : The onset of Rt. facial palsy was earlier than Lt. facial palsy 3days. The reaction on the treatment of Rt. facial palsy was more dull than Lt. facial palsy. In terms of treatment period, Rt. facial palsy was very longer than Lt. facial palsy. Conclusion : According to the above results, we discoveried that Hominis Placenta herbal-acupunture and the other oriental medical therapies had good influence on the bilateral simultaneous facial palsy. In the future, we should endeavor to know influence between Rt. and Lt. face in case of bilateral simultaneous Bell's palsy.

Bell's palsy의 유발인자로서의 한랭 노출에 대한 소고(小考) (Is it appropriate to exclude cold exposure from the risk factors of Bell's palsy?)

  • Objectives : To express an opinion on the controversy about the cold exposure as one of the risk factors of Bell's palsy. Results & Conclusions : It is widely assumed that there is a causal relationship between Bell's palsy and herpes family virus. Regarding cold exposure as one of the risk factors of Bell's palsy, however, some physicians do not accept that cold exposure could be one of the risk factors of Bell's palsy. There is no evidence supporting the hypothesis that the virus causes Bell's palsy, and it has yet to be confirmed. As there are some experimental and clinical reports suggesting that Bell‘s palsy is related to the cold exposure, we cannot exclude the possibility that the cold exposure may be one of the important risk factors of Bell's palsy. It would be necessary to undertake further studies to determine this.

성별 및 연령에 따른 안면신경마비 출현 양상에 관한 연구 (A Study on the Position and the Pattern of the Facial Palsy According to Sex and Age)

  • Facial palsy is a general nerve disease which is occurred in a lateral incomplete or complete facial palsy. But it makes man restrict a social activity and so it is very important that we know the prognosis factors of a patient for medical care. We want to investigate the position and the patterns of the facial palsy according to sex and age. In this study, we performed the excitability test of the facial nerve on 103 patients (male 45 and female 58) which diagnosed as the Bell's palsy having an acute facial palsy, and the rate of the facial palsy was 42% (43 case) in right oculi and 58% (60 case) in left oculi. The facial palsy was appeared widely from young to elderly patients and especially, the rate of the facial palsy was high in age groups from 40 to 50.

양측성 안면마비에서 적외선 체열 영상 촬영으로 평가한 성상신경절 차단의 치료 효과 -증례 보고- (Changes of Infrared Thermographic Findings in Bilateral Bell's Palsy Treated with Stellate Ganglion Block -A case report-)

  • Facial nerve palsy is not a serious disease, but it can be both upsetting and disabling for patients. More than half of the lesions of facial nerve palsy fall into the category termed Bell's palsy. It is very rare to find a representative case of bilateral Bell's palsy. Here we report the changes of infrared thermographic findings in bilateral Bell's palsy treated with a stellate ganglion block (SGB). A 45-year-old female patient who had a right facial palsy which developed 2 weeks before. Steroid administration and acupuncture was not effective and so she was referred to pain clinic. The right facial palsy was cured after 19th right SGB. Twenty eight days after the onset of the right facial palsy, left facial palsy also developed and cured completely with a left SGB. Serial infrared thermograms were performed. The hypothermias on the affected side improved symmetrically by the end of the treatment.

동측 안면마비와 측방주시마비를 일으키는 Foville Syndrome & Foville-Millard-Gubler Syndrome 치험 각 1례 (Two Clinical Case Reports of Ipsilateral Facial Palsy and Conjugate Gaze Palsy caused by Foville Syndrome & Foville-Millard-Gubler Syndrome)

  • Objective : Foville syndrome & Foville-Millard-Gubler syndrome is a disease characterized by facial palsy, ipsilateral conjugate gaze palsy and ipsilateral internal strabismus. There has never been a clinical report of this disease in oriental medicine. Since gradual improvement has been experienced with oriental medical treatments based on facial palsy and paralytic strabismus in this case, it is worthwhile to introduce this disorder and report the case referring to clinical record. Methods : 75-year-old female patient and 45-year-old male patient suffering right facial palsy, right conjugate gaze palsy and right internal strabismus were treated with acupuncture, herbal medicine, pharmacopuncture and electroacupuncture. Result : Concurrence of right facial palsy, right conjugate gaze palsy and right internal strabismus was diagnosed with Foville syndrome or Foville-Millard-Gubler syndrome which is generally caused by the impairment of PPRF, abducens and facial nerves on the affected side and showed improvement with oriental medical treatments. Conclusion : More accurate diagnosis, more aggressive treatments and further researches are all required on this disease afterwards.

임신(姙娠) 중 구안괘사 환자 3례에 대한 임상적 고찰 (Clinical Studies on 3 Cases of Bell's Palsy during Pregnancy)

  • Purpose: Bell's palsy is a peripheral facial palsy of unknown etiology. During pregnancy, there is an increased incidence of Bell's palsy. Due to the nature of pregnancy, it is unlikely to treat Bell's palsy during pregnancy. The purpose of this study is to report on 3 patients with Bell's palsy during pregnancy, who improved by oriental medical treatment. Methods: The patients who had Bell's palsy during pregnancy were treated with oriental medical treatment such as herbal medication and acupuncture therapy. House-Brackmann facial nerve grading system was used to estimate the efficacy of these treatments. Results: After the oriental medical treatment, the clinical symptoms of Bell's palsy during pregnancy were improved. Conclusion: This case study shows that the oriental medical therapy is effective in treating Bell's palsy during pregnancy. Further systematic studies and specific guidelines of Bell's palsy during pregnancy would be of much value.

적외선 체열진단법을 이용한 Bell's palsy의 임상적 예후 진단 연구 (Clinical predictive diagnostic study on prognosis of Bell's palsy with the Digital Infrared Thermal Image)

  • The Background and Purpose : Most diagnostic method for the facial palsy were invasive and complex. And we don't know very well prognosis for the recovery of facial palsy in the first stage after the onset. But the Digital Infrared Thermal Image(DITI) isn't invasive and complex diagnostic method for the facial palsy. So we should study on the clinical prognostic diagnosis of Bell's palsy among facial palsy with the DITI. Objective and Methods : This study researched into the clinical statistics for 89 case who are in Bell's palsy, and they are treated with oriental medical care at the Woosuk university during 2 years form November 1998 to October 2000. All objectives have the Grade 6(Zero state) of Bell's palsy in first week after the onset. It takes a patient's facial temperature after the onset. Group A is taken from 1 day to 4 days after the onset. Group B is taken from 5 day to 8 days after the onset. And group C is taken from 9 day to 12 days after the onset. Results and Conclusions : The Digital Infrared thermal image technique showed the more high temperature, the more rapid cure and short treatment period on TE23, B2, S3, S6 in abnormal site of Bell's palsy. But it showed the more low temperature, the more rapid cure and short treatment period on TE17 of abnormal site of Bell's palsy. As a conclusion, we could think that the prognostic diagnosis of Bell's palsy closely related with the thermal difference normal and abnormal site of Bell's palsy that were took picture after the onset.

밀러 피셔 증후군에서 보이는 지연성 안면마비의 임상양상과 전기생리학적 소견 (Clinical and Electrophysiological Characteristics of Delayed Facial Palsy in Miller-Fisher Syndrome)

  • Background: Miller-Fisher syndrome (MFS) is characterized by the clinical triad of ophthalmoplegia, ataxia, and areflexia, and is considered a variant form of Guillain-Barre syndrome. Although some cases of delayed-onset facial palsy in MFS have been reported, the characteristics of this facial palsy are poorly described in the literature. Methods: Between 2007 and 2010, six patients with MFS were seen at our hospital. Delayed facial palsy, defined as a facial palsy that developed while the other symptoms of MFS began to improve following intravenous immunoglobulin treatment, was confirmed in four patients. The clinical and electrophysiological characteristics of delayed facial palsy in MFS, as observed in these patients, are described here. Results: Four patients with delayed-onset facial palsy were included. Delayed facial palsy developed 8-16 days after initial symptom onset (5-9 days after treatment). Unilateral facial palsy occurred in three patients and asymmetric facial diplegia in one patient. The House-Brackmann score of facial palsy was grade III in one patient, IV in two patients, and V in one patient. None of the patients complained of posterior auricular pain. Facial nerve conduction studies revealed normal amplitude in all four patients. The blink reflex showed abnormal prolongation in two patients and the absence of action potential formation in two patients. Facial palsy resolved completely in all four patients within 3 months. Conclusions: Delayed facial palsy is a frequent symptom in MFS and resolves completely without additional treatment. Thus, standard treatment and patient reassurance are sufficient in most cases.

발병초기 벨마비 환자의 진단에서 순목 반사(blink reflex)의 유용성 (The Usefulness of Blink Reflex in the Evaluation of Early Stage of Bell's Palsy)

  • Background: Bell's palsy is the most common disease of cranial nerve. While most electrodiagnostic tests can detect the abnormality of facial nerve several days later in Bell's palsy, blink reflex usually reveals the abnormality earlier than other tests. Therefore, we investigated the diagnostic usefulness of blink reflex in the early stage of Bell's palsy. Methods: We performed a prospective investigation in patients with facial palsy. We enrolled patients with Bell's palsy who were evaluated within 7 days of symptom onset and excluded patients with secondary causes of facial palsy. We analyzed the findings of blink reflex according to age, sex, evaluation time, and severity of facial palsy. Results: Of 320 consecutive patients with facial palsy, a total of 204 patients were enrolled. Blink reflex was normal for 10 patients and abnormal for 194 patients. The time interval between the symptom onset and the evaluation time was not associated with the result of blink reflex, but House-Brackmann grade was significantly related to the result of blink reflex (P<0.001). Patients with mild Bell's palsy often showed normal blink reflex. Conclusions: Our study shows that blink reflex is useful diagnostic test regardless of evaluation timing in the early stage of Bell's palsy, although it could be normal in patients with mild Bell's palsy.
