Forecasting obesity prevalence in Korean adults for the years 2020 and 2030 by the analysis of contributing factors

  • BACKGROUND/OBJECTIVES: There are few studies that forecast the future prevalence of obesity based on the predicted prevalence model including contributing factors. The present study aimed to identify factors associated with obesity and construct forecasting models including significant contributing factors to estimate the 2020 and 2030 prevalence of obesity and abdominal obesity. SUBJECTS/METHODS: Panel data from the Korea National Health and Nutrition Examination Survey and national statistics from the Korean Statistical Information Service were used for the analysis. The study subjects were 17,685 male and 24,899 female adults aged 19 years or older. The outcome variables were the prevalence of obesity (body mass index ${\geq}25kg/m^2$) and abdominal obesity (waist circumference ${\geq}90cm$ for men and ${\geq}85cm$ for women). Stepwise logistic regression analysis was used to select significant variables from potential exposures. RESULTS: The survey year, age, marital status, job status, income status, smoking, alcohol consumption, sleep duration, psychological factors, dietary intake, and fertility rate were found to contribute to the prevalence of obesity and abdominal obesity. Based on the forecasting models including these variables, the 2020 and 2030 estimates for obesity prevalence were 47% and 62% for men and 32% and 37% for women, respectively. CONCLUSIONS: The present study suggested an increased prevalence of obesity and abdominal obesity in 2020 and 2030. Lifestyle factors were found to be significantly associated with the increasing trend in obesity prevalence and, therefore, they may require modification to prevent the rising trend.

  • Background: In Korea, the prevalence of obesity has steadily increased, and the socioeconomic burden of obesity has increased along with it. In 2015, the National Health Insurance Service (NHIS) signed a memorandum of understanding with the Korean Society for the Study of Obesity (KSSO), providing limited open access to its databases so that the status of obesity and obesity management could be investigated. Methods: Using NHIS databases, we analyzed nationwide population-based studies for obesity using the definition of obesity (body mass index ${\geq}25kg/m^2$) in subjects over the age of 20. Age and sex standardization were used for all data. Results: The KSSO released the 'Obesity Fact Sheet 2016' using the 2006-2015 NHIS Health Checkup database. The prevalence of obesity steadily increased from 28.7% in 2006 to 32.4% in 2015, and the prevalence of abdominal obesity also steadily increased from 18.4% in 2009 to 20.8% in 2015. The prevalence of class II obesity steadily increased from 2006 to 2015, such that the total prevalence was 4.8% in 2015 (5.6% in men and 4.0% in women). The highest prevalence of obesity was found in Jeju Island, while the lowest prevalence was found in Daegu City. The highest prevalence of abdominal obesity was also found in Jeju Island, while the lowest prevalence was found in Gwangju City. Conclusion: Based on the Obesity Fact Sheet 2016, a strategy for reducing the prevalence of obesity is needed, especially in Korean men.

한국인의 비만도에 따른 비만관련질환의 유병률 증가 (Increasing Prevalence of Obesity Related Disease for Koreans Associated with Overweight and Obesity)

  • Objective : To develop a boner understanding of the relationship between weight status and the prevalence of obesity related diseases in the Korean population. Methods : The 1998 Korean National Health and Nutrition Survey was used and 10,880 persons who had previously taken health examinations were selected for study. The Korean Society for the Study of Obesity's classification of weight status was used. Hypertension, diabetes mellitus, dyslipidemia, osteoarthritis, chronic heart disease, stroke were included as obesity related disease. A logistic regression model was developed to estimate the prevalence odds ratio by obesity class adjusted for demographic and socioeconomic factors and we converted the odds ratio to a prevalence ratio using the base line prevalence of disease to aid in the interpretation of the ratios. Results : The prevalence of obesity was 26.3% based on the KSSO classification $(BMI\geq25)$. A graded increase in the prevalence ratio was observed with increasing severity of overweight and obesity for all health outcomes with the exception of chronic heart disease in men and stroke in both men and women. With normal weight individuals as the reference, for men who were younger than 50 years, the prevalence ratios were highest for hypertension BMI<23-25: 1.70(95% CI=1.41-2.05), 25$BMI\geq30$: 4.83(95% CI=3.70-5.84). The prevalence ratios for dyslipidemia were as high as hypertension, but were lower than hypertension for diabetes mellitus and osteoarthritis. Prevalence ratios generally were greater in younger adults. The prevalence of having 2 or more obesity related diseases increased with weight status category, except in people who were older than 50 years. Conclusions : Based on results, obesity is an increasingly important health problem in Korea and the disease burden increases according to weight status. For Korean adults, the strongest relationship was seen between weight status and hypertension and dyslipidemia. In older people the impact of excess weight and obesity is stronger than that seen in younger people. Increased efforts in the study of obesity and prevention and treatment of obesity and obesity related disease are required.

서울지역의 학동기 소아 및 청소년의 비만증 이환율 조사 (The Prevalence of Obesity in Children and Adolescents)

  • The prevalence of childhood and adolescents obesity at ages 6 to 17 in Seoul were investigated on 1992. The number of children and adolescents comprised of 3310 boys and 3270 girls. Obesity was defined as weight that exceeded the standard weighter for heigher and sex by more than 20%(relative weight>120%) The results were as follows : 1) The overall prevalence rate of childhood and adolescent obesity was 14.45% The prevalence rate of boys was 15.83% and that of girls was 13.06% on 1992. The overall difference of prevalence rate between boys and girls in significant(p<0.001) 2) The highest peak age is 11 years old. There were two peaks in the prevalence rate of obesity one is from 9 to 11 years old. the other is from 15 to 17 years old. 3) The prevalence rate of overweight that exceeded the standard weight by more than 20% to 29% was 8.62% by more than 30% to 49% was 5.58% and by more than 50% was 0.25% Our study suggests than the prevalence rate of obesity among the children and adolescent in Seoul, Korea is gradually increasing which will affect the future adult population. We feel it is needed further investigation to determine the causes of increasing rate of obesity and early establishment of preventive programs.

한국 성인의 비타민C 섭취와 비만 유병률의 연관성 - 2016-2017년 국민건강영양조사 자료를 이용하여 - (Association between Vitamin C and the Prevalence of Obesity among Korean Adults - The Korea National Health and Nutrition Examination Survey 2016-2017 -)

  • This study investigates the relationship between vitamin C intake and obesity prevalence among Korean adults. Based on data of the Korean National Health and Nutrition Examination Survey (2016-2017), a total of 1,356 participants were included in this analysis. Increasing the dietary vitamin C intake resulted in a significantly decreased occurrence of BMI obesity prevalence (p for trend=0.0194) and WC obesity prevalence (p for trend=0.0452). Moreover, increasing the dietary vitamin C intake of Korean adults having a high frequency of eating breakfast resulted in significantly decreased BMI obesity prevalence (p for trend=0.0406) and WC obesity prevalence (p for trend=0.0432). Among the Korean adults who ate out frequently, decreased BMI obesity prevalence was determined with increased intakes of dietary vitamin C (p for trend=0.0193) and total vitamin C (food and dietary supplements) (p for trend=0.0429), whereas significant decrease in WC obesity prevalence was observed with increased dietary vitamin C intake (p for trend=0.0484). Our results provide conclusive evidence that consumption of dietary vitamin C is associated with obesity prevalence in Korean adults, according to their habits of eating breakfast and frequency of eating out.

  • AIMS: Korean National Growth Chart was newly developed at 2007. Prevalence of obesity is expected to be changed according to reference data. This study was performed to compare the prevalence of obesity based on the newly developed growth curve with that of previous data. We also investigate the incidence of obesity, overweight, and normal criteria by two kinds of growth curves. Method: We obtained weight, height from 438 middle school students and 1786 high school students. We divided enrolled students into four groups (middle school boys, middle school girls, high school boys, high school girls). The obese, over weight group was defined as BMI(body mass index) more than 95th, 85th${\sim}$94th percentile respectively for age and sex. Obesity by relative weight was defined more than 20% of ideal body weight. We compare the prevalence of obesity in each group by the newly developed Korean National Growth Chart in 2007 and that in 1998. Result: Prevalence of obesity by BMI based on 2007 Korean National Growth Chart and that in 1998 were 8.2-12.9% and 9.0-20.4% respectively. The concordance rate were 99.0%-100% in obese group and 45.0%-91.9% in overweight group according to two kinds of reference data. Prevalence of obesity by relative weight was 11.7-23.0% that was same between 2007 Korean National Growth Chart and that in 1998 except middle school boys group. Conclusion: We found that there is a decrease in prevalence of Obesity by BMI based on 2007 Korean National Growth Chart. It is necessary to evaluate correlation between criteria for obesity and obesity related comorbid conditions for finding proper BMI or relative weight cut off value to prevent obesity and obesity related complications effectively.

소아 비만이 간기능에 주는 영향 (Prevalence of the Increased Liver Enzymes in Obese Children)

  • Purpose: With remarkable increase in the prevalence of childhood obesity, nonalcoholic fatty liver disease increased, The aim of this study is to evaluate the prevalence of the increase liver enzymes, lipid levels and fasting blood glucose level in normal and obese children. Methods: A total of 2206 elementary students were grouped according to obesity index; normal group and obesity group(mild, moderate, severe). Aspartate aminotransferase(AST, SGOT) and alanine aminotrausferase(ALT, SGPT) were measured with tota1 cholesterol, triglyceride and fasting blood glucose. Results: Compared with the 2.7% of ALT > 50 IU/L in normal group, obese groups showed significantly higher prevalence; 6.7% in mild obesity group, 11.8% in moderate group and 15.0% in severe group. The prevalence of hypertriglyceremia was 12,4% in normal weight group, which is significantly lower than obesity group(mild obesity group 24.8%, moderate and severe 32.1% each). Conclusion: The prevalence of nonalcoholic fatty liver increased along with severity of obesity. Of Nutritional assessment, intervention, and preferably prevention are necessary for health promotion elementary students.

Trends in Obesity Prevalence by Occupation Based on Korean National Health and Nutrition Examination Survey From 1998 to 2015

  • Background: It is well known that the prevalence of obesity in Korea is increasing over time, however it is not known how the trends among occupational groups and sex differ in such increasing trends. This study was designed to provide recent trends of obesity among workers in Korea and to identify whether there were differences among occupational groups. Methods: We used data from the Korean National Health and Nutrition Examination Survey, Phases I to VI (19982015), to analyze trends in the prevalence of obesity in adult Korean workers. Obesity was defined as a body mass index of 25 kg/㎡ or higher. Occupations were classified into 3 groups: (a) nonmanual workers, (b) service/sales workers, and (c) manual workers. Results: During the period of the Korean National Health and Nutrition Examination Survey Phases I to VI, the prevalence of obesity in male workers increased in all occupations (31.1% to 39.5% in manual workers, 32.3% to 38.2% in service/sales workers, and 25.3% to 39.7% in manual workers). However, female workers did not show any particular tendency toward obesity, except for a significant decrease in the prevalence rate in service/sales workers (30.8% to 23.9%, p for trend = 0.0048). Conclusion: The trends of obesity prevalence by sex and occupation were different. For male manual workers, the prevalence rate increased steadily during the data period, while it decreased steadily in female sales/service workers.

제주도 고등학교 학생들의 비만 정도, 심혈관 위험인자, 소아시 비만과의 관련성 (Relationship of Adolescent Obesity and Cardiovascular Risk Factor and Childhood Obesity in High School Student in Jeju Island)

  • Background: Recently, adolescent obesity was increased and tended to become obese adults, facing increased risk for diabetes, cardiovascular disease. Even before reaching adulthood, adolescent obesity already was experiencing hypertension, dyslipidemia, and diabetes mellitus. The purpose of this study was to find prevalence of obesity of high school students in Jeju island and association between obesity and cardiovascular risk factors and association between childhood obesity and adolescent obesity. Method: The study subjects were 6,064 students of twenty-two high school in Jeju island and health survey data was gathered from high school health examination data and hospital health examination data from Aug. 1999 to Dec. 1999. Adolescent obesity was evaluated by ideal body weight method in 1998 and body mass index. Cardiovascular risk factors were checked total cholesterol, systolic and diastolic blood pressure, fasting blood sugar. In 1,534 students, obesity at primary school is checked by primary and middle school health record. Results: Prevalence of obesity in Jeju island high school students is 12.6% in male students, 13.3% in female students by ideal body weight method and 8.7% in male students, 8.5% in female students by body mass index. Prevalence of obesity of male students in city area and district area of Jeju island were each 12.9%, 10.3% and that of male students in city area was significantly higher. Prevalence of obesity of female students in city area and district area of Jeju island were each 12.9%, 14.2% and not significantly different. Prevalence of obesity of male students in general and commercial school group were each 13.9%, 10.1% and that of general school group was significantly higher. In female students, Prevalence of obesity general and commercial school group was not significantly different. Total cholesterol, systolic and diastolic blood pressure were significantly associated with degree of obesity in male and female students(p<0.05) and fasting blood sugar was only significantly associated with degree of obesity in male students(p<0.05). In 1,514 students checked by health record, Odds ratio of obesity at primary and middle school that at high school were 12.96∼22.77 and was significantly associated. Conclusion: Prevalence of obesity of high school students in Jeju island is very high and adolescent obesity was significantly associated with cardiovascular risk factors. Mostly childhood obesity tended to become adolescent obesity and program of control of obesity at childhood and adolescence is required.

강원도 춘천 지역 개의 비만 유병률과 위험요인에 대한 단면연구 (A Cross-sectional Study on the Prevalence of Canine Obesity and Associated Risk Factors in Chuncheon, Kangwon Province)

  • As with humans, overweight or obesity is a major health concern in the companion animal population. A cross-sectional questionnaire-based survey of dog owners attending primary small animal veterinary practices from different areas in Chuncheon, Kangwon province, Korea was undertaken to explore the relationships between socioeconomic and other relevant risk factors associated with canine obesity. In addition, the author was to estimate the prevalence of obesity compared to published literatures for dogs. Owners were asked about dog age, neuter status, feeding habits, dog exercise, household income and owner age. The body condition score (BCS) of the dogs was also assessed. Multivariable binary logistic regression was used to assess the association between BCS and potential risk factors controlling for confounding variables, using odds ratios (OR), 95% confidence intervals (CIs). A total of 275 dogs (136 males and 139 females) aged 1-12 years (mean age $5.6{\pm}3.7$ years) were surveyed in 2013. Of these, 46.9% of dogs (n = 129) were classed as an ideal body shape (BCS = 3), 30.9% (n = 85) were overweight (BCS = 4), 8.4% (n = 23) were obese (BCS = 5) and 13.8% (n = 38) were underweight (BCS = 1 or 2). Neutered males and spayed females had the highest prevalence of obesity (43.4% and 33.9%); intact females had the lowest prevalence of obesity (31.6%). In univariable model, risk factors associated with canine obesity are multifactorial and include owner income, owner age, age of dog, neuter status, frequency of feeding per day, frequency of snacks and consultation with veterinarian on dog's weight. In final multivariable logistic regression model, dogs whose owners reported no consultation with veterinarians for weight management were significantly more likely to be obesity than ideal (OR = 7.6, 95% CI, 4.2-13.8; p < 0.0001). This study showed a high prevalence of obesity in domestic companion dogs. Since this was a cross-sectional study with small samples, the association of canine obesity with risk factors warrants more research. To the author's best knowledge, this is the first Korean study on dog body condition and obesity.
