A Literature Study on the Application of Spleen Tonifying Sa-Am Acupuncture in the Diagnosis of Cold-Heat & Deficiency-Excess (한열허실변증(寒熱虛實辨證)을 이용한 비정격(脾正格) 적응증 고찰)

  • Objectives & Methods : We investigated the cases for which spleen tonifying Sa-Am acupuncture was applied in ${\ulcorner}$Sa-Am-chim-gu-jeong-jeon(舍巖鍼灸正傳)${\lrcorner}$ and classified them according to the concepts of cold-heat & deficiency-excess for a better understanding of the application of spleen tonifying Sa-Am acupuncture. Results : 1. Tonification of Spleen Sa-Am acupuncture can be used to treat heat syndromes such as deficient fire caused by collapse of the spleen qi, stagnation of evil qi or spleen yin deficiency, by way of tonifying spleen qi, activating the flow of qi and blood or harmonizing spleen yin and yang. 2. Tonification of Spleen Sa-Am acupuncture can be used to treat cold syndromes such as weakness of spleen yang by way of promoting fire to generate earth, strengthening spleen yang and raising the clear yang. 3. Tonification of Spleen Sa-Am acupuncture can be used to treat deficiency syndromes such as malnutrition or poor appetite caused by spleen deficiency by way of tonifying spleen qi. 4. Tonification of Spleen Sa-Am acupuncture can be used to treat excess syndromes such as stagnation of qi and blood, abdominal mass or toxication by way of tonifying spleen qi and promoting the flow of qi and blood.

  • The purpose of this study is to introduce the music therapy as one of the 17 kinds of oriental medicine music therapy and investigate the record in the literature. The spleen is an organ that has the function of digestion, absorption, and supply of nutrition. Damp, anxiety, and improper diet can cause deficiency syndrome of the spleen. The symptoms of deficiency of the spleen are poor appetite, abdominal distension, borborygmus, water-grain dysentery, dyspepsia, and lack of strength in limbs. CV12, CV6, S36, B20, B21, and Sp4 are can be used as acupuncture points that are a great help to the patients who suffer from deficiency of the spleen. This study describes the theory of invigorating the spleen music therapy that has a good possibility as a new method of oriental medical treatment and present the acupuncture points that can be used with invigorating the spleen music therapy. The tone of the spleen is Gung(宮) and the sound of the spleen is related to a song. The music therapy of invigorating the spleen includes beating Earth-Gi(地氣) rhythm with Cheonnyeonmanse, stimulating Spleen Meridian and Stomach Meridian with Gayaguem sanjo jungjungmori, and acupuncture with "Le Cygne" of Camille Saint-Saens.

  • Wandering spleen is a rare condition characterized by migration of the spleen from its normal position due to laxity or absence of the supporting splenic ligaments. Ultrasonography shows the absence of the spleen in left upper quadrant (LUQ) and the presentation in the lower abdomen of a homognous mass whose sonographic appearance is consistent with that of a spleen. Contrast-enhanced CT reveals the absence of the spleen in LUQ, a soft tissue mass resembling spleen in the lower abdomen, and the splenic pedicle of whirled appearance. We present a case of wandering spleen, which could be diagnosed with US and CT by the ectopic location of spleen and the whirling pattern of splenic pedicle.

Correlation of virus replication and spleen index in rock bream iridovirus infected rock bream Oplegnathus fasciatus

  • Rock bream iridovirus (RBIV) is a member of the Megalocytivirus genus that causes severe mortality to rock bream (Oplegnathus fasciatus) with characteristic clinical signs of spleen enlargement. In this study, we assessed spleen size and RBIV copy number patterns in RBIV-infected rock bream to determine lethal and safe levels of virus copy number/spleen index that may define disease progress. We found that rock bream infected with RBIV ($1.1{\times}10^7virus\;copy\;number/100{\mu}l$) and held at 29, 26, 23 or $20^{\circ}C$ exhibited significantly higher levels of spleen size compared to $17^{\circ}C$. In dead condition (100% mortality at $20{\sim}29^{\circ}C$), the spleen index ($spleen\;weight/fish\;weight{\times}100$) and virus copy number were 3.00~5.38 and $10^6{\sim}10^8/{\mu}l$, respectively. Conversely, in survived condition (0% mortality at $17^{\circ}C$), spleen index and virus copy number was as low as not-infected control ($0.34{\sim}1.22/10^0{\sim}10^1/{\mu}l$, respectively). These findings suggest that spleen index can be an indicator of disease severity of RBIV disease.

  • Objectives: To investigate the different effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck on the radial pulse in healthy subjects. Methods: Sixty healthy volunteers (30 males and 30 females) participated in this study. The participants were randomly divided into three groups: control (C), Sa-Am acupuncture with Spleen Jung-gyuck (SP+) and Sa-Am acupuncture with Spleen Seung-gyuck (SP-). The radial pulse was measured using a multi-step tonometry system before, and 0, 30 and 60 minutes after acupuncture. Results: The heart rate adjusted radial augmentation index (RAI/HR) and high-tensioned pulse area (W area) significantly increased in the SP- group compared to the C and SP+ groups in males. The systolic pulse period (T4) increased significantly in the SP- group compared to the C and SP+ groups in females. The RAI/HR had positive correlations with W area and T4. Conclusion: The effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck are different. Sa-Am acupuncture with Spleen Seung-gyuck induces acute increases of radial pulse parameters related to arterial stiffness in healthy subjects.

Induction of apoptosis in mouse spleen cells by Ginsenoside Rp1 (마우스 비장세포에서 Ginsenoside Rp1의 세포자멸사 유도)

  • Ginsenoside Rp1 is one of ginseng saponins with chemotherapeutic activity. In this study, we investigated the effects of Rp1 on spleen cells. Spleen is a major immune organ consisted of crucial immune cells, such as T lymphocytes, B lymphocytes, natural killer cells, and some antigen-presenting cells. Although the anti-tumor potential of Rp1 was studied, the effects of Rp1 on immune cells have not investigated yet. A viability assay using 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide (MTT), flow cytometric analysis, Western blot analysis were used to detect cellular changes on Rp1-treated spleen cells. MTT assay showed that Rp1 decreased the viability of spleen cells. To further investigate the effects of Rp1 on activated spleen cells, we treated lipopolysaccharide (LPS) as a representative inflammatory agent and Rp1 on spleen cells in a combination. The surface expression levels of activation markers for lymphocytes, CD25 and CD69 were measured. Apoptotic analysis revealed the cytotoxic effects of Rp1 on both na$\ddot{i}$ve and activated cells, and the expression pattern of some apoptosis-related proteins was correlated to apoptotic events of cells. Taken together, ginsenoside Rp1 increases the cellular death of spleen cells and also inhibits the LPS-induced activation of spleen cells.

  • Torsion of the wandering spleen is a rare condition especially in children. Unfortunately, splenectomy was the advocated treatment of choice for wandering spleen in many children up to 1985 in spite of the well-known possibility of overwhelming postsplenectomy sepsis. Because of the conclusive evidence of the vital function of the spleen in infection, indication of splenectomy have been revised and more attention has been paid to preserving the spleen where possible. We are presented a girl with splenic torsion, seen as a migratory abdominal mass and intermittent vague abdominal pain. Splenopexy by fixation of the spleen to the diaphragm and posterolateral peritoneum and the use of Avitene in the splenic bed was successfully performed. Complete fixation of the spleen was confirmed by ultrasound a year after this procedure was carried out. The clinical presentation, etiology, diagnostic procedures and management are discussed also.

Treatment Option for High Grade Spleen Injury and Predictive Factors for Non-operative Management

  • Purpose: Purpose: The prognostic factors of non-operative management (NOM) in high-grade spleen injuries have been extensively studied, but factors that would help treatment decisions are lacking. We compared the characteristics of the patients to identify the factors affecting treatment choices. Methods: This is a review of 207 blunt spleen injury patients from January 2004 to December 2013. We compared clinical features and mortality between surgery and NOM, and used multivariate regression analysis to find the factor most strongly associated with prognosis. Results: Of the 207 patients, 107 had high-grade spleen injury patents (grade III or above). Of these, 42 patients underwent surgery and 65 patients underwent NOM. The mortality was 7% following surgery, 3% with NOM. The amount of packed red blood cells transfused in the first 24 hours and spleen injury grade were associated with management type, and mortality was highly associated with activated partial thromboplastin time (aPTT) and spleen injury grade. Conclusions: The grade of spleen injury was associated with management and mortality, so correctly assessing the spleen injury grade is important.

  • The spleen is the source of gi, blood, body fluid and plays a vital role in maintaining life. The function of the spleen is to transform food nutrients and water, and to transport them to the heart and the lung. The movement of splenic gi is marked by elevation. The spleen governs the activity of elevating the lucid. The function of transportation and transformation is usually disturbed in the state of Gi deficiency of the spleen. The main clinical manifestations of gi deficiency of the spleen can be divided into as followers: anorexia, loose stool if the digesting and absorbing functions are disturbed; phlegm and edema if dampness and water are retained due to unhealthy water metabolism. Sagunja-tang can be applied for gi deficiency syndrome of the spleen. Ingredient bakchul(Rhizoma Atractylodis Macrocephalae) and bokryeung(Poria) can be used as monarch drug to eliminate dampness and strengthen the spleen.
