Effect of Sihogayonggolmoryeotanghabcheongsimyeonjaemgagambang on Hypertension in Spontaneous Hypertensive Rats (시호가용골모려탕합청심연자음가감방(柴胡加龍骨牡蠣湯合淸心蓮子飮加減方)이 고혈압백서의 혈압 관련인자에 미치는 영향)

  • This study was done to investigate the effect of SHCG on hypertension in spontaneous hypertensive rats. SHR was sensitized and challenged with Sihogayonggolmoryeotanghabcheongsimyeonjaemgagambang (SHCG) for 4 weeks. The 3 groups have 6 rats, respectively. Experimental group was treated with 56.7 mg/kg of SHCG orally and control group was treated with 56.7 mg/kg of normal saline instead. SHCG significantly showed safety against cytotoxicity on hFCs and toxicity in the liver. SHCG significantly decreased the blood pressure and the heart rate. SHCG significantly decreased the levels of aldosterone. SHCG significantly decreased the levels of dopamine, norepinephrine, and epinephrine. SHCG significantly decreased the levels of potassium and chloride. SHCG significantly decreased the levels of uric acid and creatinine. These results suggest that SHCG might be hopeful in treatment of hypertension.

Effects of Oyaksoonkisan and Kamioyaksoonkisan on Hypertension and Pulse Rate (오약순기산과 가미오약순기산이 고혈압과 심박동수에 미치는 영향)

  • Experiments were performed to determine the effects of the Oyacksunkisan(烏藥順氣散) and Kamioyacksunkisan(加味烏藥順氣散) liquid extract on the hypertension and the pulse rate in Sprague-Dawley rat(SDR) and Spontaneous Hypertensive rats(SHR). The results were obtained as follows ; 1. Blood pressure was significantly decreased firstday and 11th day after administration of Oyacksunkisan extract and pretreatment of phentolamine. 2. Blood pressure and pulse rate were regulated first day after administration of Kamioyacksunkisan and blood pressure was significantly decreased after pretreatment of clonidine and phentolamine. 3. Oyacksunkisan and Kamioyacksunkisan didn't show any significant changes of blood pressure and pulse rate after pretreatment of prdpranolol, hydralazine and verapamil From the above result, it was concluded that Oyacksunkisan(烏藥順氣散) and Kamioyacksunkisan(加味烏藥順氣散) could be applied effectively to the hypertension.

Recurrent Spontaneous Intracerebral Hemorrhage

  • Objective : Recently, the survival rate and prognosis of spontaneous intracerebral hemorrhage[S-ICH] has improved, and their enhanced survival has become associated with a consequent rise in the recurrence of S-ICH. The aim of this study is to improve the prevention of recurrent S-ICH. Methods : Between January 1999 and March 2004, we experienced 48 cases of recurrence. We classified the patients into the two groups; a double ICH group and a triple ICH group. We investigated their brain CTs, MRIs, cerebral angiographies, and medical records, retrospectively. Results : Majority of patients had the intervals at least 12 months, and most of patients underwent conservative treatment. The most common hemorrhage pattern of recurrence was ganglionic-ganglionic [basal ganglia - basal ganglia], and the second attack was contralateral side of the first attack in a large percentage of all patients. Prognosis of patients was worsened in recurrent attack. Nearly all patients had medical history of hypertension, and most patients have taken antihypertensive medication at the arrival of emergency room. Conclusion : In treating hypertension for S-ICH patients, we stress that blood pressure must be thoroughly controlled over a long period of time.

Cerebral Arterial Stenosis in Patients with Spontaneous Intracerebral Hemorrhage

  • Objective : Spontaneous intracerebral hemorrhage (ICH) and ischemic stroke share common vascular risk factors such as aging and hypertension. Previous studies suggested that the rate of recurrent ICH and ischemic stroke might be similar after ICH. Presence of cerebral arterial stenosis is a potential risk factor for future ischemic stroke. This study investigated the prevalence and factors associated with cerebral arterial stenosis in Korean patients with spontaneous ICH. Methods : A total of 167 patients with spontaneous ICH were enrolled. Intracranial arterial stenosis (ICAS) and extracranial arterial stenosis (ECAS) were assessed by computed tomography angiography. Presence of ICAS was defined if patients had arterial stenosis in at least one intracranial artery. ECAS was assessed in the extracranial carotid artery. More than 50% luminal stenosis was defined as presence of stenosis. Prevalence and factors associated with presence of ICAS and cerebral arterial stenosis (presence of ICAS and/or ECAS) were investigated by multivariable logistic regression analysis. Results : Thirty-two (19.2%) patients had ICAS, 7.2% had ECAS, and 39 (23.4%) patients had any cerebral arterial stenosis. Frequency of ICAS and ECAS did not differ among ganglionic ICH, lobar ICH, and brainstem ICH. Age was higher in patients with ICAS ($67.6{\pm}11.8$ vs. $58.9{\pm}13.6years$ p=0.004) and cerebral arterial stenosis ($67.9{\pm}11.6$ vs. $59.3{\pm}13.5years$, p<0.001) compared to those without stenosis. Patients with ICAS were older, more frequently had diabetes, had a higher serum glucose level, and had a lower hemoglobin level than those without ICAS. Patients with cerebral arterial stenosis were older, had diabetes and lower hemoglobin level, which was consistent with findings in patients with ICAS. However, patients with cerebral arterial stenosis showed higher prevalence of hypertension and decreased kidney function compared to those without cerebral arterial stenosis. Multivariable logistic regression analyses showed that aging and presence of diabetes independently predicted the presence of ICAS, and aging, diabetes, and hypertension were independently associated with presence of cerebral arterial stenosis. Conclusion : 19.2% of patients with spontaneous ICH had ICAS, but the prevalence of ECAS was relatively lower (7.2%) compared with ICAS. Aging and diabetes were independent factors for the presence of ICAS, whereas aging, hypertension, and diabetes were factors for the cerebral arterial stenosis.

Effects of Kamibangpungtongsungsan on Hypertension in Spontaneous Hypertensive Rats Fed a Hyperlipidemic Diet (고cholesterol 식이 자발성 고혈압 백서(白鼠)에서의 가미방풍통성산(加味防風通聖散)이 고혈압에 미치는 영향)

  • This experiment was performed to determine the effects of Kamibangpungtongsungsan on hypertension in spontaneous hypertensive rats fed a hyperlipidemic diet (H-SHR). The results are summarized as follows : 1. Kamibangpungtongsungsan significantly decreased the blood pressure and pulse of rats in H-SHR. 2. Kamibangpungtongsungsan significantly decreased the levels of Aldosterone in H-SHR. 3. Kamibangpungtongsungsan significantly decreased the levels of dopamine and epinephrine in H-SHR, but did not significantly reduce the levels of norepinephrine, in H-SHR. 4. Kamibangpungtongsungsan did not significantly reduce the levels of electrolytes in H-SHR. 5. Kamibangpungtongsungsan significantly decreased the TNF-$\alpha$ and IL-6 levels in H-SHR but the IL-10 level increase no significant. These results suggest that the Kamibangpungtongsungsan might be usefully applied for the treatment of hypertension with hyperlipidemia.

Gender-specific Association of the ANO1 Genetic Variations with Hypertension

  • Development of hypertension is caused by complex contributions of genetic and environmental factors. In spite of the increased understanding of hypertension, genetic factors that contribute to hypertension largely remain elusive. ANO1 gene encoding a calcium-activated chloride channel has recently been reported to affect spontaneous hypertension in the animal model. In this report, we investigated possible association of the ANO1 gene with hypertension in human with ANO1 variants found in Korean population. Fourteen polymorphisms of ANO1 gene were analyzed to be associated with hypertension. Interestingly, the six polymorphisms that showed statistically significant association were all the male subjects. The highest significant SNP was rs7127129 (OR=1.14, CI: 1.02~1.28, additive P=0.023; OR=1.24, CI: 1.03~1.49, dominant P=0.025), and other five SNPs (rs2509153, rs11235473, rs10751200, rs10898827 and rs10899928) were also statistically associated with hypertension. Consequently, we found that the genetic variants of ANO1 present statistically significant associations with hypertension in human, especially, in male. To the best of our knowledge, this study is the first report describing association of genetic polymorphisms of ANO1 with hypertension in human.

An Extra-adrenal Pheochromocytoma Presenting with Spontaneous Intracerebral Hematoma

  • We report a 18-year-old man, who has been taking antihypertensive medication for 1month in a local clinic, presented with a sudden onset headache followed by left blindness. He experienced palpitation and chest discomfort during physical exertion since 2years before admission, but unfortunately has been ignored. Brain CT showed intracerebral hemorrhage in the left temporoparietal area, but cerebral angiogram and magnetic resonance image revealed no vascular anomaly. He was managed conservatively, and headache and visual loss were improved over time. Subsequently, on the evaluation of hypertension, he was diagnosed as having extra-adrenal pheochromocytoma on left paraaortic area from the results of endocrinological evaluations, abdominal CT scan, and $^{131}I$-MIBG scintigraphy.

Effect of gemigliptin on cardiac ischemia/reperfusion and spontaneous hypertensive rat models

  • Diabetes is associated with an increased risk of cardiovascular complications. Dipeptidyl peptidase-4 (DPP-IV) inhibitors are used clinically to reduce high blood glucose levels as an antidiabetic agent. However, the effect of the DPP-IV inhibitor gemigliptin on ischemia/reperfusion (I/R)-induced myocardial injury and hypertension is unknown. In this study, we assessed the effects and mechanisms of gemigliptin in rat models of myocardial I/R injury and spontaneous hypertension. Gemigliptin (20 and 100 mg/kg/d) or vehicle was administered intragastrically to Sprague-Dawley rats for 4 weeks before induction of I/R injury. Gemigliptin exerted a preventive effect on I/R injury by improving hemodynamic function and reducing infarct size compared to the vehicle control group. Moreover, administration of gemigliptin (0.03% and 0.15%) powder in food for 4 weeks reversed hypertrophy and improved diastolic function in spontaneously hypertensive rats. We report here a novel effect of the gemigliptin on I/R injury and hypertension.

The Effect of Gamigehyuldeung-tang on Hypertension (가미계혈등탕(加味鷄血藤湯)이 고혈압(高血壓)에 미치는 영향(影響))

  • Objectives : This study was done to investigate the effect of Gamigehyuldeung-tang on hypertension. Methods : After administering Gamigehyuldeung-tang extract to SHR(Spontaneous Hypertensive Rats) for 5 weeks, changes in blood pressure, pulse rate, aldosterone and catecholamine levels in plasma were examined, and immunohistochemical changes and scanning electron microscopic changes were observed. Results : The following results were obtained; blood presure decreased significantly as well as levels of aldosterone, dopamine and epinephrine in SHR. But levels of norepinephrine were unaffected. No capillary vessel dilation was observed. A decrease in cell damage was seen in microscope investigation. Conclusions : These results support a role for Gamigehyuldeung-tang might be usefully applied in treatment of hypertension.

Analysis of the Risk Factors of Hematoma Enlargement in Patients with Spontaneous Intracerebral Hemorrhage (자발성 뇌실질내 출혈 환자에서 혈종 증가의 위험 인자에 대한 분석)

  • Objective : The purpose of this study was to evaluate the risk factors of hematoma enlargement in patients with spontaneous intracerebral hemorrhage(ICH). Methods : A series of 214 ICH patients diagnosed by brain CT scan in our neurosurgery department from June 1995 to July 1998 were reviewed with clinical status, past medical histories, laboratory findings, CT findings and prognosis. Results : In 27 patients(12.6%), the second CT scan showed an enlarged hematoma. Age, sex, and site of hematoma were not related to hematoma enlargement. A long interval(>6 hours) between the onset and the 1st CT scan strongly reduced the incidence of hematoma enlargement. The incidence of hematoma enlargement significantly increased in patients with previous history of hypertension, cerebral infarction and ICH. This analysis also demonstrated the following independent factors predisposed to hematoma enlargement : initial high systolic blood pressure, high serum total protein, low serum albumin, low serum sodium, prolonged prothrombin time(>14 sec) and activated partial thromboplastin time(>29.5 sec), irregular hematoma shape, and combined intraventricular hemorrhage. Prognosis in the group of hematoma enlargement showed high mortality(48.1%) and poor outcome. Conclusion : Patients with previous history of hypertension, cerebral infarction and ICH, and with high systolic blood pressure, prolonged coagulation time, irregular hematoma shape and intraventricular hemorrhage in CT scan should be observed carefully. And, early surgical therapy of large hematoma and meticulous control of blood pressure may decrease the mortality and morbidity in patients with spontaneous ICH.
