Current Status of Fluoroquinolone Use for Treatment of Tuberculosis in a Tertiary Care Hospital in Korea

  • Background: Fluoroquinolones are considered important substitutes for the treatment of tuberculosis. This study investigates the current status of fluoroquinolone for the treatment of tuberculosis. Methods: In 2009, a retrospective analysis was performed at one tertiary referral center for 953 patients diagnosed with tuberculosis. Results: A total of 226 patients (23.6%), who received fluoroquinolone at any time during treatment for tuberculosis, were enrolled in this study. The most common reasons for fluoroquinolone use were adverse events due to other anti-tuberculosis drugs (52.7%), drug resistance (23.5%), and underlying diseases (16.8%). Moxifloxacin (54.0%, 122/226) was the most commonly administered fluoroquinolone, followed by levofloxacin (36.3%, 82/226) and ofloxacin (9.7%, 22/226). The frequency of total adverse events from fluoroquinolone-containing anti-tuberculosis medication was 22.6%, whereas fluoroquinolone-related adverse events were estimated to be 2.2% (5/226). The most common fluoroquinolone-related adverse events were gastrointestinal problems (3.5%, 8/226). There were no significant differences in the treatment success rate between the fluoroquinolone and fluoroquinolone-$na{\ddot{i}}ve$ groups (78.3% vs. 78.4%, respectively). Conclusion: At our institution, fluoroquinolones are commonly used for the treatment of both multidrug-resistant tuberculosis and susceptible tuberculosis, especially as a substitute for adverse event-related drugs. Considering the low adverse event rates and the comparable treatment success rates, fluoroquinolones seem to be an invaluable drug for the treatment of tuberculosis.

Patterns of rpoC Mutations in Drug-Resistant Mycobacterium tuberculosis Isolated from Patients in South Korea

  • Background: Rifampicin (RFP) is one of the principal first-line drugs used in combination chemotherapies against Mycobacterium tuberculosis, and its use has greatly shortened the duration of chemotherapy for the successful treatment of drug-susceptible tuberculosis. Compensatory mutations have been identified in rpoC that restore the fitness of RFP-resistant M. tuberculosis strains with mutations in rpoB. To investigate rpoC mutation patterns, we analyzed 93 clinical M. tuberculosis isolates from patients in South Korea. Methods: Drug-resistant mycobacterial isolates were cultured to determine their susceptibility to anti-tubercular agents. Mutations in rpoC were identified by sequencing and compared with the relevant wild-type DNA sequence. Results: In total, 93 M. tuberculosis clinical isolates were successfully cultured and tested for drug susceptibilities. They included 75 drug-resistant tuberculosis species, of which 66 were RFP-resistant strains. rpoC mutations were found in 24 of the 66 RFP-resistant isolates (36.4%). Fifteen different types of mutations, including single mutations (22/24, 91.7%) and multiple mutations (2/24, 8.3%), were identified, and 12 of these mutations are reported for the first time in this study. The most frequent mutation involved a substitution at codon 452 (nt 1356) resulting in amino acid change F452L. Conclusion: Fifteen different types of mutations were identified and were predominantly single-nucleotide substitutions (91.7%). Mutations were found only in dual isoniazid- and RFP-resistant isolates of M. tuberculosis. No mutations were identified in any of the drug-susceptible strains.

  • Mycobacterium tuberculosis complex (M. tuberculosis complex) is a causative agent of contagious chronic disease in a wide range of mammalian hosts, mainly cattle, goat, pigs, wildlife, and humans. The definite diagnosis of tuberculosis is made based on culture of M. tuberculosis, but it takes a long time. In the present study, we analyzed whether the detection time of M. tuberculosis could be reduced when cultured in the medium containing the culture-promoting ingredients-107 (CPI-107) using the BacT/Alert 3D system, an automatic culture system. The time to detection (TTD) tended to decrease as the added concentration of CPI-107 increase. In the case of low numbers of M. tuberculosis, it decreased by 21.0% at 1.2 mg/mL of CPI-107 and by 15.9% in the case of high numbers of M. tuberculosis. In the culture using clinically isolated M. tuberculosis strains, the shortening of the culture time by CPI was more evident. In conclusion, the detection time of M. tuberculosis was shortened in the medium added with CPI-107, and this could be used for isolation, culture and drug susceptibility test of M. tuberculosis.

Association of Serum Vitamin D Levels with Bacterial Load in Pulmonary Tuberculosis Patients

  • Background: Vitamin D is known to have diverse effects on various systems in the body. There is evidence to suggest that a link exists between the serum vitamin D status and tuberculosis. The present study was designed to assess the alterations in serum 25-hydroxyvitamin D levels in newly diagnosed sputum acid fast bacilli (AFB) positive pulmonary tuberculosis patients and to study the association, if any, between serum vitamin D levels and different levels of sputum smear positivity. Methods: Serum 25-hydroxyvitamin D levels were estimated in 65 sputum AFB positive pulmonary tuberculosis patients and 65 age and gender-matched healthy controls. Results: The levels of serum 25 hydroxy-vitamin D in tuberculosis patients were not statistically different from the levels of serum 25 hydroxy-vitamin D in healthy controls. However, among patients with pulmonary tuberculosis, there was a significant negative correlation between the levels of serum 25 hydroxy-vitamin D and levels of sputum positivity. Conclusion: Serum vitamin D levels negatively correlates with bacterial load in patients with active pulmonary tuberculosis.

  • 저자들은 폐결핵 치료중 흉통, 발열, 기침 등 폐렴소견과 단순흉부 X 선상 결핵의 악화소견으로 본 병원에 입원한 67세 남자에서 순차적인 단순흉부X선과 기관지 내시경 및 조직생검을 통하여 좌측 주기관지에서 발생한 고등급의 점액표피양 암종을 확인하였으며, 진단후 17개월만에 사망한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • 저자들은 발열, 복통 등의 임상증상을 보이며 복부 CT촬영 결과 다발성의 저음영 병변을 보였으며 초음파를 통한 비장 생검 결과 육아종성 염증소견을 보여 항 결핵제 치료 후에도 호전없어 비장절제를 시행한 비장결핵 1예와 항 결핵제로 치료하고 있는 비장결핵 1예를 각각 경험하였기에 이를 보고하는 바이다.

Notified Incidence of Tuberculosis in Foreign-born Individuals in Jeju Province, Republic of Korea

  • Objectives: In the Republic of Korea (ROK), the notified incidence of tuberculosis in foreign-born individuals (NITFBI) has increased recently, as has the rate of multidrug-resistant (MDR) and rifampicin-resistant (RR) tuberculosis in foreigners staying in the ROK. As Jeju Province in ROK has a no-visa entry policy, control programs for NITFBI should be consolidated. The aim was to evaluate the status of NITFBI, with a focus on the distribution of MDR/RR tuberculosis by nationality. Methods: Data on tuberculosis incidence in individuals born in Jeju Province and in foreign-born individuals were extracted from the Korean Statistical Information Service of Statistics Korea, and the Infectious Disease Surveillance Web Statistics of the Korea Centers for Disease Control and Prevention, respectively. Results: Among all notified incident cases of tuberculosis, the proportion of NITFBI increased from 1.46% in 2011 to 6.84% in 2017. China- and Vietnam-born individuals accounted for the greatest proportion of the 95 cases of NITFBI. Seven cases of MDR/RR tuberculosis were found, all involving patients born in China. Conclusions: In Jeju Province, ROK, NITFBI might become more common in the near future. Countermeasures for controlling active tuberculosis in immigrants born in high-risk nations for tuberculosis should be prepared in Jeju Province, since it is a popular tourist destination.

  • Mycobacterium tuberculosis has infected more than two billion individuals worldwide, of whom 5%-10% have clinically active disease and 90%-95% remain in the latent stage with a reservoir of viable bacteria in the macrophages for extended periods of time. The tubercle bacilli at this stage are usually called dormant, non-viable, and/or non-culturable microorganisms. The patients with latent bacilli will not have clinical pictures and are not infectious. The infections in about 2%-23% of the patients with latent status become reactivated for various reasons such as cancer, human immunodeficiency virus infection, diabetes, and/or aging. Many studies have examined the mechanisms involved in the latent state of Mycobacterium and showed that latency modified the expression of many genes. Therefore, several mechanisms will change in this bacterium. Hence, this study aimed to briefly examine the genes involved in the latent state as well as the changes that are caused by Mycobacterium tuberculosis. The study also evaluated the relationship between the functions of these genes.

폐 및 폐외결핵환자에서의 T 림프구 매개성 면역기능의 변화에 관한 연구 (T-cell Mediated Immunity in Pulmonary and Extrapulmonary Tuberculosis)

  • 연구배경 : 결핵의 감염에서는 세포성면역이 중요하며 그 중에서도 T림프구가 중요한 역할을 하는 것으로 알려져 있고 조력 T림프구와 억제 T림프구의 기능의 불균형이 결핵의 발병에 있어 중요한 역할을 할 것으로 생각되고 있다. 동일한 결핵균의 감염시 일부 환자에서는 결핵의 병변이 폐에 국한되는 반면, 일부의 환자들에서는 폐의 결핵병변의 유무와 관계없이 폐외장기의 결핵이 발생되고 이러한 폐외결핵의 경우 항결핵화학요법에 잘 반응하지 않는 경우를 종종 경험할 수 있을뿐만 아니라 그 유병율의 감소도 폐결핵의 경우와는 달리 현저하지 못하여 폐결핵환자와 페외결핵환자군간의 면역기능의 차이가 의심된다. 방법 : 폐결핵환자와 폐외결핵환자군에서의 T림프구 매개성 세포성면역기능의 차이와 면역기능의 생체내검사와 생체외검사의 상관성을 규명하고자 T림프구 및 아형의 수적변화를 유세포분석법(flow cytometry)을 이용하여 측정하였고 PPD피부반응검사 및 림프아구형성을 측정하여 비교하였다. 결과 : 1) 총 림프구수는 결핵환자군에서 대조군에 비하여 유의하게 감소되어 있었으나 페결핵환자군과 폐외결핵환자군간의 차이는 없었다. 2) PPD 피부반응검사와 백혈구수는 3군간에 유의한 차이가 없었다. 3) $T_3$, $T_4$, $T_8$(+)인 세포의 백분율과 절대수는 3군간에 유의한 차이가 없었으며 $T_4/T_8$의 비도 3군간에 유의한 차이가 없었다. 4) HLA-DR(+)인 세포의 백분율과 절대수는 대조군에 비하여 결핵환자군에서 유의하게 증가되어 있었으며 $IL_2$ 수용체(+)인 세포의 백분율과 절대수도 결핵환자군에서 유의하게 증가되어 있었으나 폐결핵환자군과 폐외결핵환자군에는 유의한 차이가 없었다. 5) Concanavalin-A, Phytohemagglutinin 및 PPD 자극에 대한 림프아구형성은 3군간에 유의한 차이가 없었다. 6) $T_4$(+)인 림프구의 백분율 및 절대수와 PPD 피부반응검사의 크기사이에는 유의한 상관관계가 있었다. 결론 : 이상의 결과에서 폐결핵환자와 페외결핵환자군간에 T림프구성 매개성 세포성면역기능의 변화를 발견할 수 없었다. 그러나 본 연구만으로 세포성 면역기능의 차이를 모두 관찰하였다고 할 수는 없기 때문에 이에 대하여는 추후 연구가 필요하리라고 사료된다.

인후두결핵 1례 (A Case of Tuberculosis of the Pharynx and Larynx)

  • Despite the incidence of tuberculosis has been greatly reduced with chemotherpy, it is still a common disease in Korea. The pharyngeal and laryngeal tuberculosis usually result from direct contamination of the laryngeal or pharyngeal mucosa by sputum heavily laden with Mycobacterium tuberculosis, secondary infection from the lungs via lymphatic or hematogenous routes, or a primary affection from inhaled tubercle bacilli. Recently the authors experienced a pharyngolaryngeal tuberculosis and report this case with brief literatures review.
