Management of Proximal Iliac Artery Injury during Lumbar Discectomy with Stent Graft

  • Iatrogenic vascular injuries during lumbar disc surgery may occur rarely but they are serious complications, which can be fatal without appropriate management Prompt diagnosis and management of these complications are imperative to prevent a desperate outcome. A 72-year-Old female with proximal left common iliac artery iatrogenic injury during lumbar discectomy was successfully treated by percutaneous deployment of a stent graft in an emergency setting. Postprocedural angiogram demonstrated complete exclusion of the iliac artery laceration. The patient became hemodynamically stable. Two weeks later she complained of vascular claudication. Follow-up angiography revealed decreased arterial flow in the opposite common iliac artery. An additional kissing stent was inserted into the right common iliac artery and the symptoms of vascular claudication disappeared. Endovascular stenting offers a safe and effective method for the treatment of an iatrogenic arterial laceration, particularly in a critical condition. But, the contralateral iliac arterial flow should be kept intact in case of proximal iliac artery injury. Otherwise, additional treatments may be needed.

  • We experienced a case of application of Gianturco Self-Expendable Metal vascular stent for S V.C. syndrome at the postoperative stae of pneumonectomy for Lung cancer[Squamous cell carcinoma, stage IIIa] Placement was performed under fluoroscopic guidance. Clinical problem for patient was resolved satisfactorily Long-term follow-up is required to determine restenosis, complication, and recurrence of cancer, etc.

효율적인 약물 방출 스텐트 제조를 위한 고분자 코팅물질 개발 (Development of Polymeric Coating Material for Effective Drug-eluting Stent)

  • 효율적 비혈관용 약물방출 스텐트 제조를 위해 풀루란 아세테이트(pullulan acetate, PA)가 테프론(poly-tetrafluorethylene; PTFE)으로 피막된 스텐트(PTFE-stent)의 코팅재료로 연구되었다. 파크리탁셀 함유 PA가 코팅된 PTFE-stent의 표면, 약물 방출 거동, 세포독성이 측정되었으며, 동물실험을 통해 이의 가능성이 검토되었다. 전자현미경으로 표면을 관찰한 결과 표면이 PTFE 피막에 비해 훨씬 매끄러웠고, 약물은 80일 동안 서방적 방출 거동을 보였다. PA와 함께 코팅된 파크리탁셀의 안정성을 annexin V 결합 염색법을 통하여 측정한 결과 apoptosis의 비율이 천연 파크리탁셀과 유사한 것으로 보아 봉입된 파크리탁셀의 변성이 없음을 알 수 있었다. 소동물 실험에서는 파크리탁셀이 봉입된 PA-PTFE가 고형암의 성장을 억제하였다. 위의 결과로 보아 PA는 효율적 비혈관계 약물방출 스텐트 개발에 매우 유용한 물질이라고 기대된다.

Endovascular Treatment for Common Iliac Artery Injury Complicating Lumbar Disc Surgery : Limited Usefulness of Temporary Balloon Occlusion

  • Vascular injury during lumbar disc surgery is a rare but potentially life-threatening complication. It has been managed by open vascular surgical repair. With recent technologic advance, endovascular treatment became one of effective treatment modalities. We present a case of a 32-year-old woman who suffered with common iliac artery injury during lumbar disc surgery that was treated successfully by endovascular repair with temporary balloon occlusion and subsequent insertion of a covered stent. Temporary balloon occlusion for 1.5 hours could stop bleeding, but growing pseudoaneurysm was identified at the injury site during the following 13 days. It seems that the temporary balloon occlusion can stall bleeding from arterial injury for considerable time duration, but cannot be a single treatment modality and requires subsequent insertion of a covered stent.

전자빔 처리를 통한 발수성 금속 스텐트 제작 및 평가 (Fabrication and evaluation of hydrophobic metal stent using electron beam equipment)

  • The objective of this study was to fabricate a novel hydrophobic stent for reducing restenosis by employing electron beam equipment. The stent was fabricated from a CoCr alloy tube by using a femtosecond laser and was treated with argon plasma. Subsequently, the stent's surface specification changed from hydrophilic to hydrophobic. Application of the electron beam offers several advantages such as a short processing time, whole surface reforming, and enhancement of material properties. As the surface of the stent was rendered hydrophobic, it can provide equivalent or enhanced mechanical properties and greater functionality with a higher radial force at the extended stent in a blood vessel. The obtained results corresponding to the mechanical properties indicate that the contact angle increased to approximately 130°, and the radial force increased to approximately 3 N. Furthermore, cell culture experiments were conducted for verifying whether cells were cultured on the surface-modified CoCr surface. Based on the obtained results, it is believed that an effective reduction in the restenosis of inserted vascular stents is possible.

  • Penetrated injury of common carotid artery (CCA) is rare and extremely lethal. Carotid artery injury tends to bleed actively and potentially occlude the trachea. It can cause fatal neurological complications. An accurate diagnosis and adequate treatment are very needed to the successful outcome of the penetrating vascular injury in zone 1, 2, and 3 of the neck. Open surgical treatment is more invasive and complicated than endovascular treatment. We experienced a case with penetrating injury in neck zone 2. Here, we report the case successfully treated with endovascular stent graft technique.

Amaurosis Fugax Associated with Stenosis of the Intracranial Internal Carotid Artery : Successful Restoration of Ophthalmic Artery Flow by Stent Placement

  • Two patients presented with amaurosis fugax[AMF]. despite treatment with antiplatelet drugs and anticoagulants due to previous transient ischemic attacks. Angiography demonstrated severe stenosis in the cavernous and petrous internal carotid artery[ICA] respectively, with reduced flow in the ophthalmic artery[OA]. Endovascular stent placement in both patients resulted in normalization of the vessel lumen of the stenotic vessel segments. In addition, complete restoration of OA flow was noted immediately after stenting. Both patients showed no further episode of AMF over a follow-up period of 38 and 23 months respectively. Our clinical and angiographic findings suggest that hemodynamic Insufficiency in retinal vasculature caused by a stenosis of the cavernous or petrous ICA can be treated successfully by endovascular stent.

  • Vascular stenting has a great attention as a treatment for coronary arteries diseases as compared with percutaneous balloon angioplasty. In-stent restenosis and thrombosis are side effects resulting from using bare metal stent (BMS). Employing platelet therapy allowed to reduce the rate of thrombosis, however, the rate of restenosis remains a major problem. In 2002, drug-eluting stents (DESs) were introduced as an effort to reduce the restenosis. The commercially available DESs continue to suffer from coating defects that might lead to a series of adverse effects. Most importantly, multiple concerns remain regarding the polymer coating integrity on metal surfaces or the relation of polymer irregularities to longterm adverse events.
