Vitamin D Dependent Rickets Type 1A Caused by CYP27B1 Mutation

  • Vitamin D dependent rickets type 1A (VDDR1A) is an autosomal recessive disorder caused by mutations in CYP27B1. Clinical findings are growth retardation, hypotonia, muscle weakness, hypocalcemic seizures, and radiological features of rickets. We aimed to present the VDDR1A case with a genetic study of CYP27B1. The 14-month-old boy was admitted to the hospital due to a seizure. Serum calcium, phosphorus, alkaline phosphatase, parathyroid hormone (PTH), 25(OH) vitamin D, and 1,25(OH)2 vitamin D values were 5.1 mg/dL, 3.7 mg/dL, 705 IU/L, 429 pg/mL, 24.9 ng/mL, and 8.8 pg/mL, respectively. Radiological study showed cupping and fraying of the distal ulna and radius. The molecular genetic study revealed that the patient had a compound heterozygous mutation, $Phe443Profs^*24$ and c.589+1G>A, in CYP27B1. Genetic analysis of the family members presented that the mother was heterozygous for the mutation c.589+1G>A, and that the father was heterozygous for $Phe443Profs^*24$. The patient was treated with calcium lactate and calcitriol. Until now, six Korean patients with VDDR1A have been studied. Including this case, Korean patients with VDDR1A were found to have only three different mutations in 14 alleles, indicating that the mutation in the CYP27B1 gene is homogeneous in the Korean population.

Enhancement of ergocalciferol (vitamin D) content in mushrooms by UV irradiation (자외선 조사에 의한 버섯의 ergocalciferol(비타민 D) 함량의 증진)

  • Ergocalciferol is known as having vitamin D activity. In this study, the effects of UV irradiation on the increase of egocalciferol content were investigated in 7 kinds of mushrooms, i.e, lily mushroom (Flammulina velutipes), oyster mushroom (Pleurotus ostreatus), young oyster mushroom (Pleurotus ostreatus), king oyster mushroom (Pleurotus eryngii), button mushroom (Agaricus bisporus), shiitake (Lentinula edodes), and wood ear mushroom (Auricularia auricula-judae). Mushrooms which were not exposed to UV light contained negligible amount of ergocalciferol in all kinds of tested mushrooms, but UV irradiation increased their content of ergocalciferol. Of UV A, B and C, UV B light was the most effective to increase ergocalciferol contents. In mushrooms, the increase in ergocalciferol content occurred only in the peel within 1 mm depth from the surface, which was directly exposed to the UV light. Therefore, when fresh whole mushrooms were irradiated with UV light, lily mushroom, the mushroom with a larger surface area compared to volume, such as lily mushroom, was more favorable in producing ergocalciferol. On the other hand when the mushrooms were freeze-dried and cut, the mushrooms with a higher ergosterol, such as king oyster mushroom, shiitake or button mushroom, were more favorable in generating ergocalciferol.

Relation among Mineral (Ca, P, Fe, Na, K,Zn) Intakes, Blood Pressure, and Blood Lipids in Korean Adults (일부 성인남녀의 무기질 (Ca, P, Fe, Na, K, Zn) 섭취와 혈압 및 혈중 지질과의 관련성)

  • The purpose of this study was to investigate the relation among mineral intakes, blood pressure, and blood lipids for health management of Korean adults over 20 years old. Subjects were recruited from the district where they lived, which included rural (Yoju, n = 137) , coastal (Taean, n = 100) , and urban district (Suwon, n : 117) . hnthropornekical measurement, blood pressure measurement, dietary intake assessment using 24-hour recall method, and blood collection and analysis of blood lipids were conducted. The average age, height, weight, and BMI were 54.1 years, 168.5 cm, 67.3 kg, and 24.5 kg/$m^{2}$ for men and 53.9 years,153.8 cm,59.2 kg, and 25.0 kg/$m^{2}$ for women, respectively. The mean daily intakes of total food and energy were 1219.1 g and 1740.9 kcal for men and 1071.3 g and 1432.6 kcal for women. The mineral intakes of men and women were 448.5 mg and 400.4 mg for calcium,955.3 mg and 803.7 mg for phosphorus, 13.3 mg and 11.1 mg for iron, 4302.8 mg and 3393.3 mg for sodium,2588.0 mg and 2264.9 mg for potassium, and 8.9 mg and 7.3 mg for zinc. The daily intakes of energy, calcium, zinc, vitamin $B_{2}$, and vitamin I were $76.1\%$, $60.1\%$, $73.1\%$, $68.6\%$, and $80.4\%$ of RDAs, respectively. The intakes of energy, calcium, zinc, vitamin $B_{2}$ and I were not meet Korean RDAs. Especially, calcium, zinc, and vitamin $B_{2}$ were less than $75\%$ of RDAs. Blood pressure of the subjects was 128.3/75.5 mmHg for men and 124.1/73.7 mmHg for women. Serum lipids of men and women were 180.2 mg/dL and 184.9 mg/dL for total cholesterol, 160.8 mg/dL and 137.6 mg/dL for triglyceride,41.5 mg/dL and 44.7 mg/dL for HDL-cholesterol,106.6 mg/dL and 112.7 mg/dL for LDL-cholesterol, and 3.5 and 3.3 for atherogenic index. While serum triglyceride and AI of men was significantly higher, HDL-cholesterol was lower than those of women. In the relation among mineral intakes, blood pressure, and serum lipids, we found a significantly negative correlation of intakes of calcium, calcium/phosphorus, calcium density with systolic blood pressure. There were significantly negative correlation between intake of calcium/phosphorus and serum cholesterol, calcium density and serum cholesterol, sodium intake and serum HDL-cholesterol. Based on these results, we concluded that high intakes of calcium, calcium/phosphorus and low sodium intake were associated with favorable blood pressure and serum lipid profiles in adult men and women.   • This study was designed to evaluate the nutritive value of pearl barley. Forty males and the same number of females of albino rats, the Leuise strain, weighing from 52.1g to 59.1g were divided into eight experimental groups, five males and females were composed of one group. A group: casein 15%+starch 75%+oil 8%+vitamin mixture 1%+salt mixture 1% B group: pearl barley 98%+vitamin mixture 1%+salt mixture 1% C group: rice 98%+vitamin mixture 1%+salt mixture 1% D group: barley 98%+vitamin mixture 1%+salt mixture 1% E group: rice 49%+pearl barley 49%+vitamin mixture 1%+salt mixture 1% F group: rice 73.5%+pearl barley 24.5%+vitamin mixture 1% +salt mixture 1% G group: barley 49%+pearl barley 49%+vitamin mixture 1% +salt mixture 1% H group: barley 73.5%+Pearl barley 24.5%+vitamin mixture 1%+salt mixture 1% The rats were kept in an individual cage and were fed 8 different diets for 7 weeks and fed by ad-libitum feeding method. The results of this study were eluciated as followings. 1. B group was significantly lower in weight gain than the other groups. 2. Polished pearl barley showed higher digestion rate in protein and carbohydrate than brown pearl barley but lower in fat. 3. Liver weight was obserbed significant differences between B group and C group. but no significant differences between the others. 4. B group was significantly lower in the total protein content of liver and A/G ratio in plasma than other groups. 5. B group was also the lowest in the hematocrit and hemoglobin contents among the experimental groups. 6. Free amino acid composition in plasma was similar to those in diet.

Seasonal Variation of Nutritional Intake and Quality in Adults in Longevity Areas (고령인구 비율이 높은 지역 성인 및 노인의 계절별 영양소 섭취실태)

  • The purpose of this study was to estimate seasonal variation of nutritional intake and quality in adults in longevity areas. Dietary survey was given to 469 subjects over age 20 living Bukjeju-gun, Yecheon-gun, and Sunchang-gun of Korea, using 24-hour recall method every 4 seasons over one-year period. The mean daily intakes (%RDA) of 4 seasons were 1313.3 ㎉ (72.0%) for energy, 47.3 g (82.2%) for protein, 20.2 g for fat, 228.0 g for carbohydrate, 12.8 g for dietary fiber. The differences were hardly significant among the seasons. Mean daily intakes of most of the vitamins and minerals were lower than RDA except vitamin C and folate, especially %RDAs of Ca, vitamin D, vitamin B$_2$, vitamin E were less than 60% of RDA. In general, nutrient intake were high in spring compared to other seasons except vitamin C which was high in fall and winter. Mean daily intakes of cholesterol were 151.7 mg and 124.3 mg in males and females, respectively. The differences was significant between the two sexes but not among the seasons. PUFA : MUFA : SFA ratio of the subject was 1.0 : 1.3 : 1.2. Average CPF ratio of energy intake was 72.7 : 14.4 : 12.9, and energy intake ratio from carbohydrate was low in spring, in contrast energy intake ratios from protein was significantly high in spring. Mean adequacy ratio (MAR), an index of overall nutritional quality was 0.64 for female and 0.71 for male. The indices of nutritional quality (INQ) were over 1 for most of nutrients except 0.73 of Ca, 0.87 of vitamin A, 0.69 of vitamin B$_2$, and 0.65 of vitamin E. Both MAR and INQ were significantly different among sex and seasons, values were higher in males than in females and were higher in spring with the exception of vitamin C. In conclusion, subjects in longevity areas did not consume enough nutrients quantitatively as well as qualitatively, especially Ca, vitamin A, vitamin B$_2$, and vitamin E. Also mean daily intakes of most of vitamins and minerals were insufficient in females, and were significantly different among season. Therefore we must consider a counterplan to augment nutrition intake for them. In addition, it seems to be essential to micro nutrients to the food composition database to estimate dietary intakes more accurately.

A Study on the Dietary Behaviors and the Nutrient Intake of Middle School and High School Girls in Iksan (익산지역 여중생과 여고생의 식행동과 영양 섭취에 관한 연구)

  • This study was carried out to compare the dietary behaviors and the nutrient intake of a middle school and a high school girls in Iksan. The subjects consisted of 132 middle school girls and 122 high school girls in Iksan. The proportion of subjects eating meals irregularly was higher in the high school girls (98.6%) than in the middle school girls(78.7%). The proportion of subjects skipping a meal also was higher in the high school girls(61.5%) than in the middle school girls(53.8%) and most of them (65.8%) skipped breakfast though they thought breakfast was the most important meal of a day (87.4%). The subjects tended to overeat at lunch (38.6%), and at dinner (55.9%). The main reasons of skipping a meal were 1. They had no time (49.3%), and 2. They had poor appetite (24.7%) Mean daily energy intake was 2198.5Kcal in the middle school girls and 2150.5Kcal in the high school girls and mean daily intake of protein, iron, vitamin $B_1$, vitamin $B_2$ and niacin was significantly higher (p<0.05, p<0.01) in the middle school girls than in the high school girls. Mean daily calcium, iron, vitamin $B_1$, vitamin A and C intake of middle school girls were lower than those of RDA and all nutrients intake of high school girls except for phosphorus was lower than those of RDA. The index of nutritional quality (INQ) was under 1.0 for calcium, iron, vitamin A, $B_1$ and C. Mean blood level of cholesterol, triglyceride, Hb, Hct, GOT, GPT were 170.9mg/dl, 78.9mg/dl, 13.1g/dl, 39.5%, 18.8mg/d and 15.7mg/dl, respectively.

Changes of the blood chemistry, lipid and protein components in blood and liver tissue of the rat after oral combined administration of caffeine, iron and vitamin E (Caffeine, 철분 및 vitamin E 혼합투여시 rat의 혈액과 간조직내에서 혈액화학성분과 지질 및 단백질 구성성분의 변화)

  • This study was conducted to identify the effects of caffeine or combinations of caffeine and iron or vitamin E on the lipid and protein components or blood chemistry levels of the serum as well as the total homogenate, mitochondrial and microsomal fraction of the rat(Sprague-Dawley, female) liver. Chronic test were conducted to determine those effects. The chronic test was conducted by dividing rats into 5 groups according to the type of drugs and dosages administrated as follows; the control(group A), and group B was given 25mg/kg caffeine orally once daily for 30 days, group C was given 50mg/kg caffeine orally once daily for 30 days, group D was given 25mg/kg caffeine and orally ferric chloride once daily for 30 days and group E was given 25mg/kg caffeine and 25mg/kg vitamin E once daily for 30 days. The concentrations of glucose, urea nitrogen, uric acid, creatinine, total protein, albumin, A/G ratio, triglyceride, total cholesterol, HDL-cholesterol, free fatty acid, phospholipid as well as the activities of alanine aminotransferase(ALT), aspartate aminotransferase(AST) and alkaline phosphatase(ALP) were measured in the serum of each experimental groups. The concentrations of the carbonyl group and malondiaidehyde(MDA) and the patterns of the SDS-PAGE(Sodium Dodecyl Sulfate - Polyacrylamide Gel Electrophoresis) and fatty acid compositions in free fatty acids and phospholipids were analyzed to determine the oxidative damages and metabolic changes on the lipid and protein components in the serum, and total homogenate, mitochondrial and microsomal fractions of the rat liver. The results obtained from this study were summarized as follows; 1. Body weights of groups B, C, D and E were significantly decreased(p < 0.01) in comparison with that of the control in the chronic test. 2. The concentrations of serum glucose in groups B(124.5mg/dl), C(130.1mg/dl), D(122.1mg/dl), E(119.3mg/dl) were significantly higher(p < 0.01) in comparison to that of the control(101.5mg/dl). But, there were no significant differences in the concentrations of urea nitrogen, uric acid, creatinine, total protein, albumin and A/G ratio in comparison to that of the control. 3. The concentrations of total cholesterol and HDL-cholesterol in serum of groups B(69.6, 53.4mg/dl), C(73.0, 56.3mg/dl), D(68.9, 51.1mg/dl) and E(68.2, 51.3mg/dl) were significantly higher(p < 0.01) in comparison to that of the control(52.6, 38.8mg/dl). On the other hand, the concentrations of triglyceride in serum of groups B(45.0mg/dl), C(40.4mg/dl), D(33.8mg/dl) and E(47.2mg/dl) were significantly lower(p < 0.01) in comparison to that of the control(66.2mg/dl). There were no significant differences in the activities of ALT, AST and ALP in comparison to that of the control. 4. The concentrations of free fatty acid and phospholipid in serum of groups B(45.7, 154.4mg/dl), C(50.0, 167.2mg/dl), D(52.5, 148.4mg/dl) and E(41.1, 159.2mg/dl) were higher(p < 0.01) in comparison to that of the control(35.2, 125.3mg/dl). And the concentrations of the carbonyl group and malondialdehyde in serum of group D(1.82, 0.52nM/mg protein) were significantly higher(p < 0.01) in comparison to the control(1.53nM/mg protein). 5. The concentrations of carbonyl group in total homogenate, mitochondrial and microsomal fraction of group D(1.45, 0.94, 1.67nM/mg protein) were significantly higher (p < 0.01) in comparison to the control(1.16, 0.66, 1.27nM/mg protein). And the concentrations of malondialdehyde in the total homogenate, mitochondrial and microsomal fraction of group D(6.70, 6.10, 1.36nM/mg protein) were significantly higher(p < 0.01) in comparison to the control(5.17, 3.64, 0.68nM/mg protein). 6. As the analytical results of the fatty acid compositions of free fatty acid in serum, the proportions of stearic acid and arachidonic acid of groups B(16.52, 12.62%), C(17.52, 15.18%), D(19.73, 13.47%) and E(17.62, 13.28%) were significantly higher(p < 0.01) in comparison to the control(14.75, 7.88%), but the proportions of oleic acid and linoleic acid of groups B(12.97, 32.59%), C(10.88, 31.23%), D(12.37, 30.66%) and E(11.95, 32.41%) were significantly lower(p < 0.01) in comparison to the control(16.44, 35.12%). Otherwise, as the results of the fatty acid compositions of phospholipid in serum, the proportions of stearic acid and arachidonic acid of groups B(39.37, 16.39%), C(40.63, 17.83%), D(42.73, 15.39%) and E(39.16, 15.70%) were significantly higher(p < 0.01) in comparison to the control(37.74, 14.24%), but the proportions of oleic acid and linoleic acid of groups B(4.03, 14.38%), C(3.54, 12.38%), D(4.52, 11.68%) and E(4.29, 13.64%) were significantly lower(p < 0.01) in comparison to the control(5.53, 16.14%). 7. As the analytical results of the fatty acid compositions of free fatty acid in total homogenate, mitochondrial and microsomal fraction of liver, the proportions of oleic acid of groups B(7.8**, 8.73**, 6.88%) and C(6.89**, 7.75**, 6.58%) were lower(**:p < 0.01) in comparison to the control(8.67, 10.08, 7.81%), but the proportions of arachidonic acid of group C(22.62, 19.79, 23.71%) were significantly higher(p < 0.01) in comparison to the control(20.93, 18.47, 22.24%). And the proportions of palmitic acid of group D(25.95**, 26.16, 26.34**%) were significantly higher(**:p < 0.01) in comparison to the control(24.43, 25.42, 23.34%). In addition, the proportions of linoleic acid of group D(23.43, 25.02, 23.95%) were also significantly higher(p < 0.01) in comparison to the control(22.17, 23.75, 21.26%). The proportions of stearic acid of group D(19.87, 19.76**%) in mitochondrial and microsomal fraction were lower(**:p < 0.01) in comparison to the control(21.01, 24.18%), and the proportions of stearic acid of group E(16.71*, 19.65**%) in mitochondrial and microsomal fraction were significantly lower(**:p < 0.01, *:p < 0.05) in comparison to the control(21.01, 24.18%), and the proportions of linoleic acid of group E(25.04, 29.20, 26.48%) in total homogenate, mitochondria and microsome were significantly higher(p < 0.01) in comparison to the control(22.17, 23.75, 21.26%). 8. As the results of the fatty acid compositions of phospholipid in total homogenate, mitochondrial and microsomal fraction of liver, the proportions of palmitic acid of group D(17.58**, 18.78*, 18.23%**) were significantly higher(**:p < 0.01, *:p < 0.05) in comparison to the control(16.28, 17.22, 16.38%), and the proportions of stearic acid of group D(36.41, 37.23, 39.53%) were also significantly higher(p < 0.01) in comparison to the control(34.18, 34.16, 36.04%). But the proportions of oleic acid(3.41*, 3.11**, 3.12**%) and linoleic acid (18.03**, 15.79**, 14.74**%) of group D were significantly lower(**:p < 0.01, *:p < 0.05) in comparison to the control(oleic : 3.63, 3.72, 3.79%, linoleic : 20.03, 18.71, 18.48%). 9. In order to determine the oxidative damages to the protein in serum, mitochondrial and microsomal fraction of the rat liver, the patterns of the SDS-PAGE were identified, but the results of SDS-PAGE were not significantly different between the control and experimental groups.

Effects of an in vitro vitamin D treatment on the inflammatory responses in visceral adipose tissue from Ldlr-/- mice

  • BACKGROUND/OBJECTIVES: Atherosclerosis is associated with increased inflammation in the visceral adipose tissue (VAT). Vitamin D has been reported to modulate the inflammatory responses of stromal vascular cells (SVCs) and adipocytes in adipose tissue, but the role of vitamin D in atherosclerosis biology is unclear. This study examined the effects of in vitro 1,25-dihydroxyvitamin D3 (1,25[OH]2D3) treatment on the inflammatory responses of SVCs and adipocytes from atherosclerotic mice. MATERIALS/METHODS: C57BL/6J (B6) mice were divided randomly into 2 groups and fed a 10% kcal fat control diet (control group, CON) or 41% kcal fat, 0.21% cholesterol (high fat + cholesterol, HFC) diet (obese group, OB), and B6.129S7-Ldlrtm1Her/J (Ldlr-/-) mice were fed a HFC diet (obese with atherosclerosis group, OBA) for 16 weeks. SVCs and adipocytes isolated from VAT were pre-incubated with 1,25(OH)2D3 for 24 h and stimulated with lipopolysaccarides for the next 24 h. Proinflammatory cytokine production by adipocytes and SVCs, the immune cell population in SVCs, and the expression of the genes involved in the inflammatory signaling pathway in SVCs were determined. RESULTS: The numbers of total macrophages and SVCs per mouse were higher in OB and OBA groups than the CON group. The in vitro 1,25(OH)2D3 treatment significantly reduced macrophages/SVCs (%) in the OBA group. Consistent with this change, the production of interleukin-6 and monocyte chemoattractant protein 1 (MCP-1) by SVCs from the OBA group was decreased by 1,25(OH)2D3 treatment. The 1,25(OH)2D3 treatment significantly reduced the toll-like receptor 4 and dual-specificity protein phosphatase 1 (also known as mitogen-activated protein kinase phosphatase 1) mRNA levels in SVCs and MCP-1 production by adipocytes from all 3 groups. CONCLUSIONS: These findings suggest that vitamin D can attribute to the inhibition of the inflammatory response in VAT from atherosclerotic mice by reducing proinflammatory cytokine production.

Dietary Quality Evaluation by Menu Analysis of Korea Antarctic Expedition (남극 세종과학기지 대원들의 메뉴분석에 의한 식사의 질 평가)

  • The purpose of this study is to evaluate dietary quality of Korea Antarctic expedition by menu analysis. Basic menu pattern, intake of dish and dish group, DDS (dietary diversity score), daily nutrients supply, and NAR (nutrient adequacy ratio) & MAR (mean adequacy ratio) were analyzed using 1 year menu list for the 10th Korea Antarctic expedition. Most frequently served basic menu patterns were ${\ulcorner}$Rice + Soup + 2 Side dish + Kimchi (53.5%)${\lrcorner}$ and ${\ulcorner}$Rice + Stew + 2 Side dish + Kimchi (13.4%)${\lrcorner}$. In the analysis of dish group, excluding Rice and Kimchi, ${\ulcorner}$Grilled foods${\lrcorner}$ and ${\ulcorner}$Pan-fried foods${\lrcorner}$ were served more than 25% per month. Most frequently served dishes were "pan-fried rolled egg", "grilled seaweed", "kimchi soup", "fruits cocktail, canned" and "salt-fermented squid". The kinds of served dishes were very restrictive. The average score of DDS showed 2.88 for summer and 2.97 for winter. Dairy group was almost not served. Fruit & Vegetable groups were also served a little as canned product. The energy ratio of Carbohydrate : Fat : Protein was 56.5 : 23.9 : 19.2, and 56.9 : 24.5 : 18.3, for summer and winter, respectively. Both seasons had higher ratio of carbohydrate and lower ratio of fat compared to the recommended ratio (44 : 40 : 16) in polar area. Ca : P ratio was very poor, 0.40 in both seasons. NAR scores of Ca, vitamin A, vitamin $B_2$ and vitamin C were also very low, ranged from 0.6 to 0.7. Consequently, a well-planned menu supplying adequate amount of dairy, fruit and vegetable is necessarily required including Ca, vitamin D, vitamin A, vitamin $B_2$ and vitamin C intake, and some nutritionally well-educated members are urgently needed to join in the expedition.

Cereal intake status and nutritional status of adults: results from the Korean National Health and Nutrition Examination Survey, 2013 ~ 2016 (한국 성인 남녀에서 시리얼 섭취 여부에 따른 영양섭취상태 평가 : 2013 ~ 2016 국민건강영양조사 자료를 이용하여)

  • Purpose: This study was performed to evaluate the cereal intake status and explore its relationship with the nutritional status of the Korean adults from 2013 ~ 2016, Korean National Health and Nutrition Examination Survey (KNHANES). Methods: We analyzed dData from the combined 2013 ~ 2016 Korean National Health and Nutrition Examination Survey were analyzed. The analysis included 12,199 adults aged from 19 to 64 years (male, n = 5,127, female, n = 7,072). In this study, according to cereal intake frequency (From food frequency questionnaire), we classified tThe subjects were classified according into the "non-cereal intake (NCI)" group (male, n = 4,290, female, n = 5,578), and "cereal intake (CI)" group (male, n = 837, female, n = 1,494) according to cereal intake frequency (From food frequency questionnaire). Results: Typically, 79.6% of adults did not consume cereals (male 81.6%, female 77.3%);, 13.4% consumed cereals 1-3 times a month (male 12.2%, female 14.8%) and 7.1% consumed cereals more than once a week (male 6.2%, female 7.9%). In the case of both males and females, the nutrient density (nutrient intake per 1,000 kcal) and Index of Nutritional Quality (INQ) of protein, calcium, phosphorous, potassium, vitamin A, vitamin $B_1$, vitamin $B_2$, niacin, and vitamin C in the Non-Cereal group were significantly lower than those of the CI group (respectively, p < 0.01). Conclusion: Our These results reveal highlight the low frequency of cereal intake in Korean adults and the better nutritional status in adults who consume cereals. It is hypothesized that tThe data of the present study would is expected to serve as basic data to improve the nutritional status of Korean adults.
