한국인의 상악동 외골증 평가 (Evaluation of antral exostoses in a Korean population)

  • Purpose: This study was performed to survey the incidence and imaging characteristics of the antral exostoses in a Korean population. Materials and methods: The cone-beam computed tomographic images of 5,268 patients were reviewed for the presence of antral exostoses. Incidence, location, shape, structure and dimension were evaluated. Gender and side difference were analyzed by chi-squared test. Results: Ninety eight antral exostoses were found in 78 (1.5%) out of 5,268 patients. Gender and sides showed no statistically significant differences. Antral exostoses was often found in the lateral wall and floor of the maxillary sinus, accounting for 58.2% and 38.8% of the exostoses, respectively. Most exostoses had broad base and were composed of spongy bone surrounded by compact bone. The mean dimension was 9.1(W)*4.9(D)*5.3(H) mm. Conclusion: Antral exostoses is not uncommon in a Korean population. The identification of antral exostoses is clinically significant to avoid unnecessary examination and treatment. It also needs to be examined carefully in the implant planning or sinus lift procedure.

다발성 외골증을 가진 치주염 환자의 삭제형 골수술에 대한 증례 보고 (Resective osseous surgery for multiple bony exostoses of the periodontal patient: A case report)

  • Bony exostosis, one of the osseous deformities, could act as a contributing factor of periodontal disease since it makes proper tooth brushing difficult and causes traumatic ulcer. The purpose of this case report is to show improvement of periodontal health by removal of bony exostoses and creation of physiologic osseous form. A 58 year-old female patient with recurrent ulcer and pain on bony exostoses located on left maxillary palatal area and both mandibular buccal areas was treated. Exostoses were removed and physiologically positive osseous form was created following vertical grooving, radicular blending, flattening interproximal bone and gradualizing marginal bone. The patient showed no further recurring traumatic ulcer and bone exposure. Moreover, periodontal pocket was eliminated and food impaction was decreased. In conclusion, periodontal health could be achieved through removal of multiple bony exostoses and development of ideal osseous form.

검은독수리 (Milvus migrans)에서 발생한 Multiple Exostoses (Multiple Exostoses in a Blak Kite (Milvus migrans))

  • 탈진증상을 보이는 검은 독수리 (Milvus migrans)가 경상남도 야생동물구조센터에 진료의뢰되었다. 신체검사상에서 우측날개의 말단부에서 3개의 크기가 다양하고 원형의 경결한 종괴가 확인되었으며, 방사선 검사상에서도 이 종괴들은 골밀도를 보이고 있음이 확인되었다. 본 환자는 불량한 예후를 사유로 안락사 조치되었다. 안락사후 실시한 부검에서 종괴들은 다발성 외골종으로 판명되었다.

유전성 다발성 외골종 환자에서 상지 변형 (The Alignment and Deformity of the Upper Extremity in Hereditary Multiple Exostoses)

  • 목적: 유전성 다발성 외골종 환자에서 상지에 발생한 외골종 병변의 분포와 이와 관련하여 상지의 축성 배열의 변화에 대해 조사하였다. 대상 및 방법: 2001년부터 2009년까지 유전성 다발성 외골종으로 진단받은 38명, 76예를 대상으로 하였다. 외골종 침범 수에 따라 Group A (2부위 이하), Group B (3부위 이상)로 분류하였으며, 각 그룹간 운반각 및 VAS (Visual Analogue Scale), 일상 활동의 제한 정도, 외관상 만족도를 비교 평가하였다. 결과: 외골종으로 진단받은 38명 환자 중 상지에 발생한 외골종은 23명, 43예에서 관찰되었다. 발생 부위로는 상완골 근위부 33예(30%), 척골 원위부 31예(28.2%), 요골 원위부 24예(21.8%) 이었다. Group A, B 각각 운반각은 $10.7^{\circ}$, $13.8^{\circ}$, VAS는 1.3점, 3.5점, 일상생활 능력의 제한은 8점 중 7.3점, 6.6점을 보였으며 외관상 만족한 경우는 13예, 10예였다. 결론: 유전성 다발성 외골종 환자 중 상지 변형은 65%에서 동반되었다. 침범 부위 수가 증가할 수록 운반각 및 VAS는 증가하였고, 일상생활 능력과 만족도는 감소하였다.

Osteochondroma of the Rib Mimicking a Mediastinal Mass: Unexpected Menifestation in Hereditary Multiple Exostoses

  • Osteochondroma is a common bone tumor but a rare tumor in the rib. It is often asymptomatic and observed incidentally. This is a case report of a 49-year-old woman with an osteochondroma mimicking a mediastinal mass in hereditary multiple exostoses. The chest X-ray and computed tomography (CT) scans revealed the bony density feature of the mass. Surgical excision confirmed that the lesion was an osteochondroma.

German shepherd에서 발생한 기관 골.연골형성증 (Cases of Tracheopathia Osteochondroplastica in German shepherd)

  • Based on pathological findings, 5 German shepherds, revealed a mean age of 7.6 years, showing no clinical signs were diagnosed as tracheopathia osteochondroplastica(TOC). Grossly, multiple small-sizecd nodules, appeared as cobble-stones, on the cartilage rings situated in the anterior trachea and the mucosal surface of the epiglottis were showed. Numerous tiny nodules were scattered on the pleural surface of the lung. The vascular walls of the heart were irregular and coarse apearance with calcification. Histopathologically, nodules in the trachea represented an irregular expansion of the underlying tracheal ring with protrusion into the submucosa and consisted of proliferated and calcified cartilage and bone with marrow spaces containing numbers of hematopoietic cells. Pulmonary calcinosis and calcification of the vascular walls of the heart were observed. Our observations suggested that TOC arises form eccondroses and exostoses of the tracheal cartilage rings.

  • Hereditary multiple exostosis (HME) is an autosomal dominant disorder manifested by the presence of multiple osteochondromas. Although the lesions are benign in nature, exostoses are often associated with characteristic progressive skeletal deformity and displaying clinical symptoms such as mechanical irritation or impingement. We present the successful arthroscopic resection in a 24-year-old HME male with impingement syndrome and long head tendon tear of the biceps caused by osteochondroma arising from the distal clavicle.

  • 외이도 골종(osteoma)은 임상적으로 고실륜측두골 인상부 봉합선에서 발생하는 분리성, 경성 종양으로 이것은 양성이지만 크기가 서서히 증대되어 외이도를 폐쇄하여 청력장애를 초래한다. 이것과 유사한 것으로 외골종(exostoses)도 있지만 이것은 다발성, 양측 대칭성으로 발생하는 데 이것은 고실륜을 포함하고 광범위하게 골이 융기한다. 저자들은 최근 희귀한 상기 외이도 골종을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

구개 융기의 치험례 (A Case Report of Symptomatic Torus Palatinus)

  • Purpose: Torus palatinus is a bony prominence at the middle of the hard palate. The size varies from barely discernible to very large, from flat to lobular. This oral exostosis is not a disease or a sign of disease, but if large, may be a problem. So, we present the clinical and histopathologic features and applied therapy and provide a comprehensive review of the rare case of the symptomatic exostoses. Methods: A 37-year-old woman had slowly growing exophytic nodular mass of the bone that arises the midline suture of the hard palate. The patient was concerned about discomfort associated with movement of her tongue and about frequent irritation of the palatal mucosa during mastification of the hard food. The patient had a large, unilobulated torus palatinus. It extended from the area adjacent to the canine to a point beyond the junction with the soft palate. The mass was oblong in shape, measuring about 3 cm long, 2 cm wide, and 0.8 cm in height. Results: Before surgical intervention a CT was obtained for the sake of estimating the thickness of the bone between the exostoses and the maxillary antrum and floor of the nose. The surgical procedure was performed with the patient under general anesthesia. Removal of the exostosis was performed after midline mucoperiosteal incision with osteotome and diamond burr. Histologic finding revealed decalcified dense bony tissue, the presence of lacunae, and normal osteocytes. Conclusion: Surgical removal is recommended when one or more of the following condition exist: interference with the construction of prosthesis, interference with oral function, irritation or pathology of the overlying tissue, inability of the patient to maintain proper oral hygiene, and fear of malignancy or other psychologic trauma. We report a rare case of the torus arising in hard palate with symptoms.

Mechanical irritation by protruding bone: A possible cause of breast implant rupture

  • Although breast implants have been in clinical use for almost 6 decades and have undergone considerable development during this time, implant rupture is still a dreaded long-term complication. Some obvious external factors, such as trauma, can lead to implant rupture, but many studies have reported a high rate of "spontaneous" implant rupture. Herein, we present two cases with the aim of raising awareness of a new possible cause of "spontaneous" implant rupture: mechanical irritation by bony protrusions.
