Orthodontic-prosthodontic interdisciplinary treatment for a patient with multiple missing posterior teeth and unilateral scissors bite (다수의 구치 상실과 편측성 가위교합을 갖는 환자의 보철 교정 협진 치료)

  • This clinical report describes an orthodontic-prosthodontic interdisciplinary treatment for a patient with multiple missing teeth and unilateral scissors bite. A 47-year-old female presented with multiple missing posterior teeth, anterior large overjet, deep bite, and posterior scissors bite on the right premolar area. Periodontal therapy was performed and followed by orthodontic treatment. The maxillary anterior teeth were initially aligned, then two implants were placed for the left mandibular molars to increase occlusal vertical dimension. The scissors bite between the right maxillary and mandibular premolars were corrected using the miniscrews as an anchorage. Other implants were placed for the right maxillary and mandibular molars after the occlusal planes and occlusal relationship were harmonized. The patient adapted well to altered vertical dimension without any specific problems including peri-implant marginal bone loss. Interdisciplinary approach resolve the complex orthodontic-prosthodontic problems and concluded in successful results.

  • Successful treatment outcome of esthetic dentistry often requires team approach including orthodontist, surgeon and restorative dentist. Clinician should consider various treatment options to restore missing teeth, especially in anterior region. In this article, interdisciplinary treatment of restoring congenitally missing lateral incisor and unrestorable molars will be presented.

Analysis of Risk Factors for Complication after Hip Fracture Surgery in the Elderly According to Geriatric Interdisciplinary Team Care and Orthopedic Care (노년내과와 정형외과의 협진 여부에 따른 노인의 고관절 골절수술 후 합병증 발생 위험 요인분석)

  • Purpose: The study was to analyze clinical outcomes and risk factors of for complications associated with the hip fracture surgery in the elderly before and after interdisciplinary treatment. Methods: A retrospective method was used to investigate the general and therapeutic characteristics, frequency of complications and clinical outcomes. The subjects of the study were 553 patients who underwent hip fracture surgery from January, 2009 to December, 2014. Results: The interdisciplinary group was older and less likely to walk independently even before the fracture than a usual care group. The incidence of complications was higher in the interdisciplinary group than the usual care group. The prevalence of complications in both groups was 66.5%. Multivariate logistic regression analysis showsed that the risk factors for complications of hip fracture surgery were as follows: advanced age, stroke, Parkinson disease, time interval from emergency room to operation, pre & post ambulatory status, American Society of Anesthesiologists (ASA) classification quality of postop intensive care unit (ICU) care and foley indwelling. Conclusion: This study has implications in that it recognized the necessity for interdisciplinary treatment and provided the basic base data for nursing intervention of the elderly patients who underwent hip fracture surgery.

Preparation of isotropic pitch precursor for pitch-based carbon fiber through the co-carbonization of ethylene bottom oil and polyvinyl chloride

  • For the first time, polyvinyl chloride (PVC) was used as an easily-handled chlorine source for preparation of isotropic pitch-based carbon fiber (IPCF) incorporating ethylene bottom oil (EO) as a raw material. Pitch precursors were prepared by the chlorination-dehydrochlorination triggered by chlorine radicals originated from PVC; aromatization and poly-condensation reactions occurred by polyene-type radicals from PVC. Radical production and co-carbonization were facilitated by pretreatments of EO through vacuum distillation, bromination, and additional heat treatment. Pitches were prepared by the co-carbonization of pretreated EO and EO containing 20 wt% PVC, and had higher yields and better spinnability than those by simple distillation.

  • Adjunctive orthodontic therapy may be required to create appropriate space, to address misalignment and tooth size discrepancy for better function and esthetics. The desired interproximal alveolar contour and gingival embrasure form can be developed during treatment. Various applications of temporary anchorage devices for pre-prosthetic tooth movement allowed clinicians to achieve high efficiency, shorter treatment time, and less discomfort of patient. Biomechanical considerations for the periodontal status of the affected teeth are required to successfully control the vertical and horizontal space. Hence, the interdisciplinary approaches have an essential role in maximizing the favorable treatment outcome. In particular, pivotal Clinical decisions such as whether to open or close the space should be made by consensus of the involved dentists. This article presents the orthodontic treatment approaches for prosthodontic works including mesiodistal and vertical space regaining specially for cases of unrestored teeth over an extended period of time.

A Research on the Recognition of the Effect of Constitution-Acupuncture Based on the Expert Constitution-Acupuncture Oriental Medical Doctors (팔체질침 전문 한의사의 체질침 효과에 대한 인식조사)

  • Objectives: The purpose of this research is to obtain basic data about the effects of Constitution-Acupuncture (CA) based on Eight-Constitution Medicine (ECM) by the expert CA Oriental Medical Doctors. Methods: The resources were collected from the internet survey method. 35 samples were collected. The SPSS 13.0 for windows and Answer Tree 3.0 was used for statistical analysis. Results: The results from this research are as follow. 1. The motivation that apply ECM to clinic are 'Because ECM is effective for diagnosis, treatment and prevention against disease' 48.6% and 'Because CA therapy is effective' 25.7%. 2. The advantage that CA therapy compare to the other acupuncture therapy are 'Management and prevention against disease are available by ECM theory to a person who exact constitution is confirmed' 82.9%, 'CA therapy is superior in treatment of hard symptoms to treat by the other acupuncture therapy' 80%, 'Treatment effect is good' 77.1%, 'Needle manipulation of CA is simple comparatively' 57.1%, 'It is definite whether treatment effect is good or not' 54.3% and CA therapy is superior all in diseases of twelve category. 3. When the effects of CA is good, patients expressed much 'Eyesight lightened', 'Took deep sleep at night', 'Fatigue decreased', 'Body is light', 'Feelings became refreshing' as well as 'Symptoms were improved' and also physicians confirmed as those reactions whether treatment is effective. Conclusions: The research suggest that CA is effective therapy in clinic, and so further study is needed for CA therapy.

Esthetic restorations of maxillary anterior teeth with orthodontic treatment and porcelain laminate veneers : a case report

  • If orthodontists and restorative dentists establish the interdisciplinary approach to esthetic dentistry, the esthetic and functional outcome of their combined efforts will be greatly enhanced. This article describes satisfying esthetic results obtained by the distribution of space for restoration by orthodontic treatment and porcelain laminate veneers in uneven space between maxillary anterior teeth. It is proposed that the use of orthodontic treatment for re-distribution of the space and the use of porcelain laminate veneers to alter crown anatomy provide maximum esthetic and functional correction for patients with irregular interdental spacing.

  • Interdisciplinary treatment of Class III malocclusion with congenital missing of unilateral maxillary canine and anterior crossbite is discussed focusing on a problem-oriented treatment planning, treatment progress, and treatment result. Maxillary mini-implant provided anchorage for distalization of the maxillary right porsterior dentition. Mandibular mini-implants were used to distalize the whole mandibular dentition. Total treatment time was 17 months to achieve a successful treatment goal. Stable occlusion was maintained after 12 months of retention.

  • The elevation of present medical service using advanced science contributes to offer medical examination and treatment of good quality under the subject of patient satisfaction as well as patient impression. The dental service field charging some part of public health promotion and public welfare promotion also intends to improve the dental examination and treatment quality utilizing advanced science. The object of this research is the development of dental patient education system (DPES) using multimedia of new technology. DPES can be employed for these object, harmonious operation of dental examination and treatment, treatment quality improvement, disease notice to patient under the subject of patient satisfaction and patient impression, explanation of some selective treatment course, the verification of treatment necessities and treatment validation, and the improvement of treatment effect through the oral cavity sanitation education in the sense of prevention. This research also serves the development environment, the step to collect educational document, visual information and to structure those, and the examination of DPES efficiency in clinic.

Transcriptomic Analysis of Genes Modulated by Cyclo($\small{L}$-Phenylalanine-$\small{L}$-Proline) in Vibrio vulnificus

  • Diketopiperazine is produced by various organisms, including bacteria, fungi, and animals, and has been suggested as a novel signal molecule involved in the modulation of genes with various biological functions. Vibrio vulnificus, which causes septicemia in humans, produces cyclo($\small{L}$-phenylalanine-$\small{L}$-proline) (cFP). To understand the biological roles of cFP, the effect of the compound on the expression of the total mRNA in V. vulnificus was assessed by next-generation sequencing. Based on the transcriptomic analysis, we classified the cFP-regulated genes into functional categories and clustered them according to the expression patterns resulted from treatment with cFP. From a total of 4,673 genes, excepting the genes encoding tRNA in V. vulnificus, 356 genes were up-regulated and 602 genes were down-regulated with an RPKM (reads per kilobase per million) value above 3. The genes most highly induced by cFP comprised those associated with the transport and metabolism of inorganic molecules, particularly iron. The genes negatively regulated by cFP included those associated with energy production and conversion, as well as carbohydrate metabolism. Noticeably, numerous genes related with biofilm formation were modulated by cFP. We demonstrated that cFP interferes significantly with the biofilm formation of V. vulnificus.
